Granule-laden perivascular cells observed in rabbits with experimental cerebral atherosclerosis.

Granule-laden perivascular cells distributed around the fine vessels in brain of hypertensive rabbits fed a cholesterol diet were examined by electron microscopy. Perivenule granule-laden cells contained secondary lysosomes, residual bodies, Golgi vesicles, fusion vesicles, and vacuoles. In periarteriole granule-laden cells, secondary lysosomes and vacuoles were not prominent. Findings indicated that the granule-laden cell may be a histiocyte that appears during the vascular reaction to hypertension and hypercholesterolemia. This vascular reaction may occur more strongly in the veins than the arteries.

Effect of terfenadine and KW-4679, a novel antiallergic compound, on action potential of guinea pig ventricular myocytes.

It has been reported that terfenadine caused torsade de pointes ventricular arrhythmias. The prolongation of action potential duration (APD) in ventricles is considered to be one of the mechanisms of this adverse effect. We examined the effect of antiallergic drugs, terfenadine and KW-4679 ((Z)-11-[3-(dimethylamino)propylidene]-6,11-dihydrodibenz[b, e]oxepin- 2-acetic acid hydrochloride), on action potentials in isolated guinea pig ventricular myocytes. Terfenadine (30 nM-1 microM) increased APD in a concentration-dependent manner. On the other hand, KW-4679 (0.1 microM-100 microM) exerted no significant effects on action potential parameters. These results present no evidence that KW-4679 has the possibility to cause ventricular arrhythmias.

Effect of KW-4679, an orally active anti-allergic drug, on antigen-induced airway hyperresponsiveness in actively sensitized guinea pigs.

We investigated the effect of KW-4679 ((Z)-11-[(3-dimethylamino)propylidene]-6,11-dihydrodibenz[b, e]oxepin-2- acetic acid monohydrochloride), an orally active anti-allergic drug, on antigen-induced airway hyperresponsiveness using two different indicators, pulmonary resistance (RL) and dynamic lung compliance (Cdyn), in actively sensitized guinea pigs. Oral administration of KW-4679 (0.1 and 1 mg/kg) 1 hr before aerosolized antigen exposure significantly inhibited the development of airway hyperresponsiveness to inhaled acetylcholine in RL and Cdyn in a dose-dependent manner. Terfenadine (10 mg/kg) also inhibited the development of airway hyperresponsiveness. These results indicate that KW-4679 could be useful in the treatment of bronchial asthma.

Inhibitory effect of KW-4679, an anti-allergic drug, on tachykinin-mediated airway response induced by electrical vagal stimulation in guinea pigs.

We examined the effect of KW-4679 ((Z)-11-[(3-dimethylamino) propylidene]-6,11-dihydro-dibenz[b,e]oxepin-2-acetic acid monohydrochloride), an anti-allergic drug, on the tachykinin-mediated airway response in guinea pigs. Electrical stimulation of the vagus nerve in atropine-treated and propranolol-treated guinea pigs caused a 38.1% decrease in dynamic compliance (Cdyn), which was suppressed by the combination of the tachykinin NK1-receptor antagonist (+/-)-CP-96345 and NK2-receptor antagonist SR 48968. KW-4679 at a dose of 3 mg/kg significantly reduced the decrease in Cdyn (P < 0.05). On the other hand, KW-4679 did not inhibit substance P or neurokinin A-induced decrease in Cdyn. These results suggest that KW-4679 may inhibit the tachykininergic airway response by prejunctional inhibition of peripheral sensory nerves.

Long-term follow-up review of suspension laminotomy for cervical compression myelopathy.

This study compared the long-term outcome of cervical spondylotic myelopathy (CSM) with that of the ossification of the posterior longitudinal ligament of the cervical spine (OPLL) after suspension laminotomy, which was developed in the authors' clinic. Seventy-six patients who received follow-up care for more than 5 years were available for analysis. The duration of the follow-up period averaged 97.8 months (range 61-160 months). Radiological and neurological analyses were performed in these 76 patients (50 with CSM and 26 with OPLL). There were no differences in sex, age, follow-up period, and preoperative neurological status between the two groups. In the quantitative study of the dural configuration, 43 patients (86%) with CSM and 17 patients (65.4%) with OPLL attained complete decompression 1 month after surgery. At long-term follow-up review, complete decompression was maintained in 42 patients (84%) with CSM but in only seven patients (26.9%) with OPLL. The neurological evaluation improved markedly at early follow up in both groups but declined insignificantly at the last follow-up review, particularly in the OPLL group. Of 12 patients (24%) with CSM and 10 patients (38.5%) with OPLL whose neurological recovery grades later deteriorated, four (8%) with CSM and nine (34.6%) with OPLL demonstrated reconstriction causing spinal cord compression at long-term follow-up review. For the remaining eight patients (16%) with CSM, who were older than 70 years on average at last follow-up review, no radiological explanation was found. These long-term results indicate that OPLL does not resolve as well as CSM after suspension laminotomy; they both may have late deterioration due to reconstriction that occurs occasionally in CSM and frequently in OPLL.

[Chest wall reconstruction after the resection of primary chest wall tumors].

Between April 1983 and June 1994, we performed chest wall reconstructions on 9 patients (7 males and 2 females) out of 27 all of whom underwent the radical resection for the primary chest wall tumors. Histological diagnosis of tumors included 6 chondrosarcomas, 1 Ewing's sarcoma, 1 plasmacytoma and 1 giant cell tumor. Six lesions were located on the rib and 3 on the sternum. For the materials for reconstruction of the chest wall use a polyethylene plate in 2 patients, a Marlex mesh in 5 patients and a Marlex mesh with a methyl methacrylate utilizing the sandwich method in 2 patients. The polyethylene plate proved to be satisfactory for maintaining stability of the chest wall. However, our experience using this plate showed intensive foreign body reactions, and discovering these we have not use the polyethylene plate. Marlex mesh had an affinity with human tissue and was very easy to use and produced no complications. We use Marlex mesh for flat surfaces on the anterior or posterior chest wall. Marlex mesh and methyl methacrylate utilizing the sandwich method, combined the solidity and shaping of methyl methacrylate with the benefit such as easy fixation and excellent incorporation of the mesh.

[Effect of KW-4679, an antiallergic drug, on antigen-induced airway microvascular leakage in rats passively sensitized with mouse monoclonal IgE antibody].

We studied the effect of KW-4679, an antiallergic drug, on inhaled antigen-induced airway microvascular leakage in rats passively sensitized with mouse monoclonal IgE antibody. Inhaled antigen increased extravasation of Evans blue dye content in tracheal and main bronchus tissues. KW-4679 (0.3-10 mg/kg, p.o.) reduced antigen-induced microvascular leakage in trachea with an ED40 value of 4.9 mg/kg. KW-4679 also reduced antigen-induced microvascular leakage in main bronchus with an ED40 value of 0.47 mg/kg. The inhibitory effect of KW-4679 was more potent than those of ketotifen and terfenadine. These results suggest that KW-4679 clearly reduces airway microvascular leakage mediated by antigen-antibody reaction.

[Diagnostic magnetic resonance imaging (MRI) in fracture of the penis: a case report].

A 23-year-old man was admitted to our hospital complaining of swelling and pain in the penis. Examination revealed fracture of the penis. The tear in the tunica albuginea was not palpable on physical examination of the penis, but magnetic resonance imaging (MRI) revealed the exact site of the tear in the tunica albuginea. MRI was very useful in localizing the site of fracture.

[A case of labial adhesions with urinary retention].

A 69-year-old woman was admitted to our hospital complaining of urinary retention. On examination, the labia was found to be fused extensively, with a pinhole opening at the midline. The adhesions were separated under spinal anesthesia. One year after operation, there remains no recurrence. Etiological factor in labial adhesions are thought to include a combination of local inflammation and estrogen deficiency. In our case, the patient had hip joint disease. Hip joint disease is shown to influence the formation of adhesions by interfering with perineal hygiene and decreasing sexual activity.

[Primary pulmonary cryptococcosis treated by thoracoscopic resection].

A 37-year-old man was admitted to the hospital because of an abnormal shadow in the right lower field on a chest roentgenogram. The lesion was calcified and was therefore suspected to be benign. Thoracoscopic resention was performed. The pathological diagnosis was cryptococcosis of the lung. The lesion was completely resected, so no antifungal drug was given. The patient was doing well as of 12 months after the operation.

[A case of giant cell tumor originating in the rib].

We have experienced a rare case of giant cell tumor originating in the rib. A 25-year-old male was admitted to our hospital with the abnormal shadow in chest X-ray film. Chest CT, MRI, Bone scintigram showed tumor originating from the right 4th rib. Needle biopsy was performed. As the result, this tumor was suspected giant cell tumor of bone. The tumor was completely resected, and the chest wall defect was covered with the marlex mesh. The pathological diagnosis was a giant cell tumor of bone. The patient is now doing well without recurrence for 14 months after the operation.

[Cyclic alteration in atheroma plaque morphology observed by intracoronary ultrasound: initiation mechanism of acute myocardial infarction].

Rupture of plaque is an important factor in the initiation of acute myocardial infarction. Plaque rupture is related to stress distribution in atheroma, but morphological alteration in coronary atheroma plaque in vivo is little understood. Atheroma plaque was observed during the cardiac cycle using 3.5 or 2.8 F, 30 MHz intracoronary ultrasound (ICUS) in seven patients with coronary artery disease. Coronary ostial pressure (P) measurements were simultaneously performed. Seven of the eccentric stenotic sites were analyzed. Intima-medial thickness (Th) and regional radius of curvature (R) at the middle and lateral portions of the atheroma were measured. Delta Th and % delta Th were calculated from change in thickness from end-diastole to peak systole. These parameters of the middle portion were compared with those of the lateral portion. The lateral % delta Th was 20.1 +/- 13.6% (delta Th 0.13 +/- 0.08 mm; 0.69 to 0.56 mm), and was significantly greater than the middle % delta Th [5.8 +/- 6.6% (delta Th 0.06 +/- 0.08 mm; 1.24 to 1.18 mm), p < 0.05]. Regional radius of curvature did not significantly differ. Wall stress calculated as P x R/Th was greater at the lateral portion (peak systole 3.7 x 10(5) dynes/cm2, end-diastole 1.9 x 10(5) dynes/cm2) than at the middle portion (peak systole 1.4 x 10(5) dynes/cm2, end-diastole 0.9 x 10(5) dynes/cm2). The change in stress from end-diastole to peak systole at the lateral portion was significantly greater (1.8 x 10(5) dynes/cm2) than that at the middle portion (0.5 x 10(5) dynes/cm2). The change in stress during the cardiac cycle, caused by the increase in intracoronary pressure, was greater at the lateral than at the middle of the plaque. This imbalanced distribution of stress may result in greater thinning of plaque at the lateral portion. Therefore, the distortion of plaque may be a mechanism for atheroma rupture at the lateral portion.

Effect of beraprost sodium, a stable prostaglandin I2 analogue, on platelet aggregation in diabetes mellitus.

Test results concerning platelet behavior in vitro, particularly aggregation, are frequently abnormal in diabetic patients. The concept has therefore arisen that platelet hyper-reactivity is one factor contributing to diabetic microangiopathy. We report here the antiplatelet effect of beraprost sodium, a chemically stable prostaglandin I2 analogue made in Japan, in 6 diabetic patients. Platelet aggregation induced by adenosine 5'-diphosphate (10 microM) after administration of beraprost sodium (40 micrograms every 8 h for 14 days) was significantly decreased as compared with levels before beraprost sodium administration. These results indicate the possibility that the occurrence of vascular complications in diabetes mellitus can be suppressed by long-term administration of beraprost sodium.

Hemifacial hyperplasia with meningeal involvement: a variant of proteus syndrome?

We report on a sporadic case of hemifacial hamartomatous hyperplasia. The patient is male, and has sebaceous nevus-like skin change, subcutaneous lipomatous mass, cranial bone hyperplasia, and bony change of meninges. His lesion involves the anterior half of the face and cranial base, and, medially, is delimited by the midline. No case has been found identical to this in the literature, particularly with respect to the unusual meningeal lesion. However, without this meningeal lesion the rest of the manifestations are not uncommon in Proteus syndrome. Also, this case may be an indication of somatic mosaicism, lethal in its nonmosaic state. Thus, this case may be thought of as a variant of Proteus syndrome.

Jararhagin and jaracetin: novel snake venom inhibitors of the integrin collagen receptor, alpha 2 beta 1.

Two novel proteins, jararhagin and jaracetin, were purified from Bothrops jararaca viper venom. Jararhagin is a 55-kDa member of the metalloprotease-disintegrin family. Jaracetin is a 60-kDa dimer representing a differently processed form of jararhagin. Like botrocetin, a previously described viper venom protein, jararhagin and jaracetin modulated binding of von Willebrand Factor to the glycoprotein Ib-IX complex on platelets through a specific interaction with the von Willebrand Factor A1 domain. Both jararhagin and jaracetin, but not botrocetin, also blocked alpha 2 beta 1-dependent platelet adhesion to collagen, a receptor interaction mediated through a homologous A domain on the integrin alpha 2 subunit.

Chronic effect of oral mexiletine administration on left ventricular contractility in patients with congestive heart failure: a study based on mitral regurgitant flow velocity measured by continuous-wave Doppler echocardiography.

The long-term effect of mexiletine on left ventricular (LV) contractility in patients with congestive heart failure is not clear. The authors therefore measured LV contractility before and after continuous oral administration of mexiletine in patients with congestive heart failure accompanied by mitral regurgitation (MR) using Doppler echocardiography. The study population consisted of 8 patients with congestive heart failure (6 due to dilated cardiomyopathy and 2 due to old myocardial infarction) accompanied by significant functional MR who had more than 1000 ventricular premature contractions (VPCs) per day or Class IV Lown classification arrhythmias before mexiletine administration. The LV contractility was evaluated by calculating a Doppler-derived index, the rate of increase in LV pressure during the isovolumic contraction time (ICT delta P/delta t), which has been confirmed to be nearly equal to LV Max dP/dt. The increase in LV pressure (delta P) between 1 and 3 m/sec of the MR flow velocity as measured by continuous-wave Doppler echocardiography was calculated using the simplified Bernoulli's equation, and ICT delta P/delta t was derived by dividing delta P by the time required for this change. The left ventricular ejection fraction and the left ventricular and left atrial dimensions also were measured by echocardiography. These parameters were obtained before and after 2 to 4 weeks of daily oral administration of mexiletine 300 mg. Values of ICT delta P/delta t were 640 +/- 202 mm Hg/sec and 650 +/- 210 mm Hg/sec before and after mexiletine administration, respectively, showing no change.(ABSTRACT TRUNCATED AT 250 WORDS)

A newly established megakaryoblastic/erythroid cell line that differentiates to red cells in the presence of erythropoietin and produces platelet-like particles.

In August, 1992, we established a leukemic cell line (NS-Meg) from a patient in megakaryoblastic transformation of Philadelphia chromosome-positive chronic myeloid leukemia. The NS-Meg cells were positive for alpha-naphthyl acetate esterase and periodic acid-Schiff (PAS) staining and for surface CD4, CD7, CD13, CD34, CD41a, and glycophorin A antigens. Ultrastructurally, the cells had alpha-granules, demarcation membranes, and platelet peroxidase activity. The NS-Meg cells spontaneously produced platelet-like particles which contained alpha-granules, mitochondria and dense bodies, strongly suggesting platelet production. Erythropoietin (Epo), granulocyte/macrophage colony stimulating factor(GM-CSF), and interleukin 3 (IL-3) promoted the growth of NS-Meg cells. Phorbol-12-myristate-13-acetate increased the expression of both CD41a and CD61 antigens. Ten-day exposure to Epo induced mature erythroblasts and red cells. These benzidine-positive cells were positive for hemoglobin F staining. Untreated NS-Meg cells expressed mRNA for the Epo receptor (EpoR), for GATA-1, and for alpha 1, alpha 2 and gamma globin genes. These results indicate that NS-Meg cells undergo terminal differentiation of both megakaryocytic and erythroid lineages. This cell line should be a very useful tool for the investigation of both megakaryocytic and erythroid maturation.
