Human focal nodular hyperplasia-like lesion in the liver of a cat.

A young cat clinically displaying marked ascites had multiple nodule hyperplasia in the liver. One of the nodules sampled at exploratory laparotomy was studied histopathologically. The nodule consisted mainly of hyperplastic hepatocytes with a central stellate scarlike connective tissue. The connective tissue had thick-walled blood vessels and radiating bands of fibrous connective tissue extending peripherally, dividing the nodule into pseudolobules. The lesions resembled focal nodular hyperplasia (FNH) in man. Serial section revealed a single large artery with the splitting of the internal elastic lamina running into the central connective tissue and branching to connect with the main vessels. The arterial branchings formed a spider-like structure. From these findings, it was suggested that the nodule had developed from a hepatic response to pre-existing vascular anomalies as had been reported in human FNH.

[Non-specific immunosuppressive effect induced by A. actinomycetemcomitans].

Previous studies have shown that immunosuppressive effects are related to the pathogenicity of periodontopathic bacteria and the development of periodontal disease. We reported soluble sonic extracts (SE) from A. actinomycetemcomitans and B. intermedius had a strong immunosuppressive effect on primary response of anti-SRBC plaque forming cells. SE from A. actinomycetemcomitans inhibited the IgM----IgG isotype switching. The purpose of this study was to investigate the effect of SE on secondary immunoresponse. As a control, mice (C3H/HeN) were immunized with intraperitoneal injection of SRBC and complete Freund's adjuvant, and additional SRBC after 30 days. In order to study the effect of SE from A. actinomycetemcomitans on secondary response, four groups were prepared in this study. One group of C3H/HeN mice were injected SE intravenously before the primary immunization of SRBC. The other three groups were given SE at different time on secondary injection of SRBC and designated the pre-, post-, or simultaneous injection, respectively. Hemolytic plaque assay was performed and estimated anti-SRBC plaque forming cells in each immunoglobulin class and IgG subclass, on the 5th day after the secondary injection. The immunosuppressive effect was found in all the groups which we examined. The present study strongly suggests that periodontopathic bacteria have strong adjuvant activity, and long-term Actinobacillus infection accompanied by periodontal fluctuation in bacterial flora may induce aberration in the immune system. The suppressed immune system explain the explosive growth of bacteria tested in our investigation, resulting in the mixed or opportunistic infection by bacteria harbored in the gingival crevice.

[Significance of second look surgery in the therapy of ovarian cancer].

Second look surgery (SLO) has two kinds of significance in therapy of ovarian cancer. One is a diagnostic value in persistence or not of ovarian cancer. The other is a second cytoreductive surgery. SLO as a diagnostic value is very much valuable to determine prognosis and therapies. On the other hand, SLO as a second cytoreductive surgery prolonged survival periods, however did not affect long term survival rate. Hence, their significance of SLO is not established to acquired good prognosis in long term.

[Treatment and overview for the improvement of prognosis in ovarian cancer].

The current treatment for ovarian cancer introduced the improvement of survival rate by the chemotherapy based on cisplatin and maximum debulking surgery. The prognosis of ovarian cancer is affected by various kinds of factor as age, ps, stage, histology, histological grading and the volume of residual tumor. By our analysis it was shown whether tumor diameter is less than 2 cm or not was critical point. On the other hand, dose intensity of cisplatin is also important. We could show the improvement of survival rate of advanced cases by the treatment of maximum debulking surgery and high intensity of cisplatin.

[A novel liquid-phase enzyme immunoassay AlaSTAT for IgE antibody determination].

To determine whether a novel liquid-phase enzyme immunoassay AlaSTAT for allergen-specific IgE antibody determination can be used to measure the absolute amounts of serum IgE antibodies, we examined the amounts of anti-house dust (HD) and anti-mite IgE antibodies in sera of 32 HD-sensitized allergic asthmatics and 14 normal subjects by this immunoassay. The amounts of anti-mite IgE antibody in serum measured by this immunoassay were found to be consistent with those assessed by the difference in serum total IgE proteins before and after the absorption with mite allergen (the value determined by this immunoassay was 113% of the latter one, n = 4). Neither nonspecific IgE protein (50 to 10000 IU/ml) nor the heat-inactivated blocking serum affected the measurement of allergen-specific IgE antibody by the liquid-phase immunoassay. There were strong correlations between the values of anti-HD and anti-mite IgE antibodies by RAST and those by AlaSTAT (r = 0.90, p less than 0.001 and r = 0.86, p less than 0.001, n = 46, respectively). Our results indicate that the commercially available liquid-phase allergen-specific IgE enzyme immunoassay can be used to measure the absolute amounts of serum IgE antibodies without interference of nonspecific IgE protein or IgG blocking antibody.

Simultaneous determination of disopyramide and mono-N-dealkyldisopyramide enantiomers in plasma and urine by use of a chiral cellulose-derivative column.

This assay allows simultaneous determination of the enantiomers of both disopyramide and its active metabolite, mono-N-dealkyldisopyramide, in 1 mL of plasma or 0.1 mL of urine within approximately 35 min by HPLC with a chiral cellulose-derivative column and ultraviolet detection. Recoveries for the analytes and the internal standard (racemic verapamil) with an extraction from alkalinized plasma or urine into diethyl ether were greater than 90%. Intra- and interassay CVs for disopyramide enantiomers were less than 5.5% at 2.5 mg/L in plasma and less than 6.5% at 25 mg/L in urine; for mono-N-dealkyldisopyramide enantiomers they were less than 6.3% and less than 8.9%, respectively. Intra- and interassay relative errors for determining these analytes in plasma and urine at 2.5 and 25 mg/L, respectively, ranged from -5.9% to +2.5%. The calibration curves for the respective analytes were linear (r = 0.995 or greater, P less than 0.01) from 0.025 to 5.0 mg/L in plasma and from 0.5 to 10 mg/L in urine. The lower detection limits (signal-to-noise ratio of 3) for S(+)-disopyramide and the other analytes were 0.010 and 0.025 mg/L, respectively. We evaluated clinical applicability of this method by determining steady-state plasma concentrations and urinary excretions of the respective analytes in a pediatric patient being treated with racemic disopyramide.

[Neutral endopeptidase activity in human peripheral blood leukocytes].

Neutral endopeptidase (NEP; EC 3.4.24.11) efficiently hydrolyses many neuropeptides. To determine the distribution of NEP, a possible regulatory enzyme for the neuropeptide-induced leukocyte activation, among human leukocytes, we investigated the enzymatic activity of NEP in each cell type of human peripheral blood leukocytes. The activity of NEP assessed by an NEP inhibitor phosphoramidon-sensitive Met5-enkephalin degrading activity was present in neutrophils (59.0 +/- 9.1 pmol/min 10(6) cells); however, the NEP activity was virtually absent in mononuclear cells, eosinophils and basophils. Common acute lymphoblastic leukemia antigen (CALLA) detected immunocytochemically with three anti-CALLA antibodies, whose amino acid sequence has been shown to be identical with that of NEP, was also found only in neutrophils, but not in other blood leukocytes. It is suggested that NEP might regulate the neuropeptide-induced activation of human neutrophils.

[An anti-human eosinophil monoclonal antibody AE500 that reacts only with granulocytes from hypereosinophilic patients].

To investigate the heterogeneity of human eosinophils, we established a murine anti-human eosinophil monoclonal antibody AE500 (IgM, kappa) by immunizing blood eosinophils from patients with idiopathic eosinophilia. The reactivity of AE500 with human leukocytes was analyzed by a fluorescence-activated cell sorter. AE500 reacted with all the populations of blood eosinophils and neutrophils from patients with eosinophilia (idiopathic eosinophilia, n X 4 or asthma, n =), but not with those from asthmatic patients without eosinophilia (n = 8) or normal subjects (n = 6). AE500 did not react with granulocytes from other family members of AE500-positive individuals. These results suggest that the anti-human eosinophil antibody AE500 reacts with a cell surface antigen of blood granulocytes in a hypereosinophilic state. This anti-eosinophilic antibody would be useful in investigating the mechanism of eosinophilia in a variety of diseases.

[Prognostic study of ovarian clear cell carcinoma].

Prognostic studies were performed on 17 cases with clear cell carcinoma of the ovary. The results are follows: 1) The average age was 45.9 years and ranging from 35 to 57 years. 2) Regarding the FIGO clinical stage, all of 5 patients with stage I and 3 of 5 with stage II survived over 2 years, while all of patients with stage III and 2 with stage IV were dead within 2 years. 3) Of 9 patients who had complete resection for primary operation, 8 (89%) survived over 2 years. All of 8 patients who had incomplete resection were dead within 2 years. 4) All cases had been received various chemotherapy; CQ + 5-FU (n = 5), CPM + ADM + CDDP + 5-FU (n = 7), 5-FU + CPM + MMC (n = 2), CDDP (n = 1), CDDP + MMC + 5-FU (n = 1), MCNU + VCR + 5-FU (n = 1), but none of them improved the prognosis of this histological type. 5) With or without administration of CDDP, there was no significant difference in the prognosis. These results suggest that early diagnosis of disease, curative surgical resection and selection of effective chemotherapeutic agents are important things to improve prognosis of clear cell carcinoma of the ovary.

Immunoadjuvant effects of periodontitis-associated bacteria.

Sonic extract (SE) from periodontopathic bacteria were studied for their effects on the immune response and adjuvanticity. The preparations of SE from A. actinomycetemcomitans, B. intermedius and B. gingivalis were found to have strong immunomodulating activity against the antigen itself and also sheep erythrocytes (SRBC) in in vivo experiments using C3H/HeN mice. All the SE enhanced the immune response to antigen and SRBC at low concentration. A strong immuno suppressive effect, however, was found at high concentrations of A. actinomycetemcomitans and B. intermedius but not of B. gingivalis. Moreover, these immunosuppressive effects were enhanced by the 3-day-prior injection to SRBC even at low concentrations, including that of B. gingivalis. The suppressive effect of SE from A. actinomycetemcomitans was dependent on the protein fraction in the SE tested, because suppressed anti-SRBC immune response was restored by pronase treatment or heating. Failure to recover immune response, found in a similar experiment using Bacteroides intermedius, indicated that its suppressive effect was due to a heat-stable and pronase-resistant substance. These findings, along with the data presented in this paper, support the hypothesis that patients who harbor these bacteria could suffer systemic immune suppression not only from the initial periodontopathic bacteria but also from secondary infecting bacteria. These suppressive effects may also enhance the pathogenicity of other opportunistic organisms in the gingival crevice.

[Prognostic factors of common epithelial ovarian cancer treated by surgery and cisplatin based combination chemotherapy].

Retrospective analysis of prognostic factors in 171 patients who had common epithelial ovarian cancer (WHO) and treated by surgeries and cisplatin based combination chemotherapies were performed by survival assay and multivariate analysis. In FIGO stage 3, the estimated parameter values were in following order: residual tumour age grade performance status histological type. On analysing Stage patients, histological grading and histological typing had an effect on prognosis. Patients with grade 2 or tumours had a worse prognosis than did those with grade 1 tumours, and patients with clear-cell carcinoma or undifferentiated adenocarcinoma showed a poor prognosis.

[Comparison of a new specific IgE assay Quidel allergy screen to skin prick test, intradermal test, and RAST].

Quidel Allergy Screen (QAS), an enzyme-linked immunosorbent assay, has been developed for measuring IgE antibodies against 9 allergens (HD 1, HD 2, Mite 1, Mite 2, Japanese cedar, ragweed, cat dander, sweet vernal grass, and egg white) at the same time. To determine whether this assay is useful in screening allergen-specific IgE antibody, we compared the titers of IgE antibodies against the 9 allergens measured by QAS to the obtained in skin prick tests, in intradermal tests and by RAST in 93 atopic asthmatics and 25 normal subjects. We found a good agreement between the reactivity of skin prick tests and the reactivity of QAS. There was a significant correlation between the threshold doses of intradermal tests and the titers of QAS. We also found a good agreement between the reactivity of RAST and the reactivity of QAS, and a strong correlation between the titers of RAST and the titers of QAS. Thus, it is concluded that QAS is useful in screening IgE antibodies against multiple allergens simultaneously.

[Reassessment of second-look operations (SLO) in the treatment of ovarian cancer].

To reassess the role off SLO in the ovarian cancer, 96 patients who underwent second laparotomy from 1968 to 1987 in our department were studied. 59 out of 96 (60.5%) were completed primary surgery while rest of them (39.5%) were incompletely resected. SLO was carried out in 34 patients of former group (57.6%) and in 9 patients in later group (24.3%). Recurrence was found in 3 patients of complete primary surgery + SLO (group A) whereas 5 patients of incomplete primary surgery + SLO (group C). Recurrence rate was significantly lower in group A. There was no relationship between the site of recurrence and survival time. Until we obtain a good tool to detect recurrence site, SLO has an important role for the management of patients. Combination of radical debulking surgery and CAP regimen with 70 mg/m2 of CDDP at least for 2 years with SLO is considered to be important to improve prognosis and quality of life of ovarian cancer patients.

[A rare case of metastasis of the spleen from recurrent cancer of the residual stomach].

In May, 1988, a 75-year-old housewife, who had undergone a subtotal gastrectomy for a carcinoma 12 years earlier, visited our hospital because of vomiting. Biopsy specimens of the residual gastric mucosa revealed malignant cells and an abdominal CT scanning examination showed a solitary nodule in the splenic parenchyma. A non-curative resection of the residual stomach, a splenectomy and a distal pancreatectomy were performed on June 3, 1988. Histological examination of the resected specimens demonstrated a moderately differentiated adenocarcinoma of the stomach, that had invaded the pancreas and retroperitoneal nodes. A nodule of the spleen that was inspected showed a round, greyish solid mass, 0.7 cm in diameter on the cut surface, and microscopically metastatic focus from a gastric cancer. Only a few operated cases manifesting a splenic metastasis of a gastric cancer have been reported and a definitive routing pattern of the metastasis is often difficult to distinguish. However, our case seems to suggest that a hemodynamic route had been taken, since there were no metastatic nodes around the splenic hilus and the metastatic site was only the parenchyma in the spleen.
