Biomechanical analysis for osteoarthritis of the ankle.

The study of three-dimensional model of the human arthritic ankle joint by the stress freezing photoelastic technique showed regional increases of stress distribution in contact area of the medial side of the talocrural joint and medial shift of the stress distribution center in the subtalar joint. From the study of the ankle joint in a contact stress analysis under a variety of conditions, the possible cause of primary or secondary osteoarthritis is discussed, and the joint instability resulting from lateral collateral ligament injuries might be one of the major cause of the disease. For the treatment of the disease, low tibial osteotomy with ligament plasty is considered to be effective.

Comparison of the effects of nifedipine on ureter and coronary artery isolated from the dog.

The effect of nifedipine (Adalat) on isolated dog ureter was compared with that on isolated coronary artery. Nifedipine at a concentration of 10(-8) mol/l significantly decreased the frequency of ureteral rhythmic contractions evoked by potassium, and suppressed the force of these contractions at a concentration of 3 X 10(-8) mol/l. In potassium-contracted coronary artery strips, nifedipine at concentrations more than 3 X 10(-9) mol/l produced significant relaxations in a concentration-dependent manner. The results indicate that nifedipine is able to act inhibitorily on ureteral contractions, and inhibition of pace making activities in ureteral smooth muscle was suggested as a mode of action of nifedipine.

[The concomitant effects of SM-4300 with some antibiotics on severe bacterial and/or fungal infections in surgery].

SM-4300, a newly developed human immunoglobulin preparation, has been studied for the safety and effectiveness/efficacy in the 16 patients with severe bacterial and/or fungal infections which are resistant to antibiotic therapy. Clinical effect of 16 cases were excellent 1 case, good 4, fair 4, poor 4 and unknown 3. The efficacy rate was 38.5% and the efficacy rate including fair was 69.2%. No remarkable adverse reaction due to administration of SM-4300 were observed.

[Two cases of papillary tumor of the pancreas].

Two patients with papillary tumors of the pancreas are reported. In both cases, the tumors occurred in women (62 and 60 years of age) who presented with upper abdominal mass and abdominal fullness. The resected tumors were 14 X 16 cm and 13 X 10.5 X 8.3 cm, respectively. Their histology resembled that of the papillary tumor of the pancreas reported by Frantz (1959). The ultrastructural details, including numerous mitochondria, basement membrane and no evidence of secretory or zymogen granules, suggest pancreatic duct cell origin. The first patient died of colon cancer six years and 10 months postoperatively. The second patient is without signs of recurrence five months after resection.

Resolution of human mercapt- and nonmercaptalbumin by high-performance liquid chromatography.

Gel-exclusion high-performance liquid chromatographic (HPLC) analysis of human serum albumin (HSA) on PGP 2000 column (0.10 M sodium phosphate buffer, 0.30 M NaCl, pH 6.86) showed at least two peaks, the principal component corresponding to human mercaptalbumin (HMA) and the second one to human nonmercaptalbumin (HNA). Mechanism for the separation of HMA and HNA might be due to weak resin-HSA interaction. HPLC analysis of bovine plasma albumin (BPA) showed a single peak on PGP 2000 column. The elution volume of HSA was larger than that of BPA, resulting in a clear resolution of HSA and BPA.

Possible alpha-adrenoceptor blocking activity of arotinolol (S-596), a new beta-adrenoceptor blocking agent in isolated dog coronary artery.

Effects of arotinolol on dog coronary arteries were investigated in vitro. In distal portions of left circumflex coronary arteries contracted with 3 X 10(-2)M KCl, norephinephrine relaxed the strips in a concentration-dependent fashion. Propranolol (10(-6)M) converted the norepinephrine-induced relaxations to contractions, and arotinolol (10(-6)-10(-5)M) inhibited the relaxations induced by norepinephrine in a concentration-dependent manner. In proximal portions of the strips after potassium-contracture, norephinephrine produced concentration-dependent contractions which were augmented by propranolol (10(-6)M) and inhibited by arotinolol (10(-6)-10(-5)M). These results suggest that arotinolol has an alpha-adrenoceptor blocking activity in addition to a beta-adrenoceptor blocking action in dog coronary arteries.

Effects of an antihypertensive agent, nipradilol, on isolated coronary artery of the dog.

Effects of nipradilol on proximal, middle and distal portions of left coronary arteries of the dog were investigated in vitro. Nipradilol (10(-7)-10(-3) M) produced a concentration-dependent relaxation in helical strips of isolated coronary artery under potassium contracture. The relaxation in the middle portions was most pronounced, it was 10% of the equimolar concentrations of nitroglycerin and significantly greater than that of propranolol. In the proximal portions of the coronary arteries, nipradilol (3 X 10(-8)-3 X 10(-7) M) showed a tendency to augment the contractile responses to norepinephrine; in the middle portions it reversed the relaxant responses to norepinephrine into contractile effects in a concentration-dependent manner. In distal portions, nipradilol significantly decreased the relaxant responses of the strips to norepinephrine. The findings demonstrate direct vasodilating and beta-adrenoceptor blocking actions of nipradilol in isolated coronary arteries of the dog.

Alpha adrenergic receptor activity of epicardial coronary artery in the anesthetized dog.

The epicardial coronary artery plays an important role not only as a conduit artery but also as a locus of coronary spasm. Accordingly, diameter change of the large epicardial coronary artery of the in situ canine heart was continuously measured by ultrasonic dimension gauge technique and the effects of alpha adrenergic stimulation on the caliber were studied. After the intravenous administration of phenylephrine the aortic pressure increased by 28 mmHg, however, the left circumflex coronary artery diameter decreased by 3.4% (p less than 0.01). Norepinephrine and sciatic nerve stimulation after propranolol (1 mg/kg) induced coronary vasoconstriction by 1.3 (p less than 0.025) and 2.5% (p less than 0.05) against the obvious increases of aortic pressure by 16 and 19 mmHg, respectively. Thus alpha adrenergic stimulation, in situ in dogs, was shown to directly constrict the large epicardial coronary artery.

[A retrospective evaluation of suprapubic prostatectomy].

Suprapublic prostatectomies performed on 89 patients from January 1977 to December 1981 were retrospectively evaluated as regard to operative risk, preoperative and postoperative complications. The patients were from 56 to 87 years old with a peak distribution in the seventies. Common preoperative complications were urinary tract infection, coronary sclerosis followed by hypertension. The operation time for resection and weight of resected adenoma averaged 46.5 minutes and 35.1 g, respectively. Average blood loss during operation was 179 ml. The most frequent postoperative complication was acute epididymitis. There were no operative deaths. Prostatic incidental carcinoma was found in 3 cases of resected adenomas by histological examination. There were statistically significant correlations between adenoma weight and duration of postoperative urinary tract infection (r = 0.38, p less than 0.01) and between operation time and blood loss (r = 0.42, p less than 0.01). In contrast, preoperative urinary tract infection had no influenced on duration of postoperative urinary tract infection or postoperative gross hematuria.

HPLC analysis of E. coli rRNA on ASAHIPAK GS-520 and nucleobases, nucleosides and nucleotides on ASAHIPAK GS-320.

High-performance gel filtration chromatography using packed column Asahipak GS-520 was employed to separate E. coli 23S, 16S and 5S rRNAs. Low-molecular-weight components of nucleic acid were separated with Asahipak GS-320. Concurrent, rapid analysis of nucleobases, nucleosides and nucleotides was obtained isocratically. The elution of these substances is also described.

Effects of trapidil and nitroglycerin on coronary circulation in conscious dogs.

Effects of trapidil and nitroglycerin (glyceryl trinitrate) on coronary blood flow or epicardial coronary diameter were studied in conscious dogs, instrumented with a Doppler flow probe or a pair of ultrasonic dimension crystals on the left circumflex coronary artery. Bolus intravenous injections of trapidil (0.5, 1.0 and 2.0 mg/kg i.v.) increased coronary blood flow, dose-dependently, such being comparable at the peak value seen with nitroglycerin (5, 10 and 20 micrograms/kg i.v.). Coronary blood flow following the intravenous administration of trapidil or nitroglycerin increased biphasically and returned to pre-drug levels in 1.4 +/- 0.2 min (trapidil 1 mg/kg i.v.) or 1.0 +/- 0.1 min (nitroglycerin 10 micrograms/kg i.v.), while the mean coronary diameter increased monophasically and approached the control level 5 min after drug administration. The first peak was observed before the maximal decrease in aortic pressure and the second peak was associated with concomitant increases in heart rate and myocardial contractility induced by a sudden hypotension. Propranolol, a beta-blocker, did not modify the initial peak but attenuated markedly the second peak (P less than 0.05) in case of trapidil (1.0 mg/kg i.v.) or nitroglycerin (10 micrograms/kg i.v.), which corresponded with reduced changes in reflex tachycardia and positive inotropism. Therefore, the direct effects of trapidil and nitroglycerin on coronary circulation of conscious dogs are an initial transient dilatation of the resistance vessels followed by a continuation of the dilatation of the conductance coronary vessels.

A study of correlation between histological, staging classification and prognosis of malignant lymphoma.

In order to study the relationship between histological staging classification and prognosis, 62 cases of malignant lymphoma were analyzed retrospectively according to each histological type and staging classification during the last eight years (1976-1983). The findings obtained from this study were as follows: The incidence of malignant lymphoma in this department (Fourth Department of Internal Medicine, Tokai University) almost coincided with that reported before. Patients with Hodgkin's disease had almost the same results as that reported before even in the advanced stage. The prognosis of patients with non-Hodgkin's lymphoma was almost the same as that previously reported.

In vivo stimulation of murine granulopoiesis by human urinary extract from patients with aplastic anemia.

Significant in vivo stimulation of granulopoiesis was induced in mice by the administration of an extract from the urine of patients with aplastic anemia (AA). Sialic acid has been identified as an important molecular component for the in vivo biological activity of this granulopoietic factor, "granulopoietin," which is distinct and different from endotoxin. Urine from patients with AA was successively fractionated by Sephadex G-50 and DEAE-cellulose chromatography. The resultant extract, which we refer to as AA urinary extract, contained approximately equal to 44 international units of erythropoietin per A unit of protein and induced 15,000 colonies of granulocyte/macrophage precursor cells (granulocyte/macrophage colony-forming units, CFU-gm) per A unit of protein with mouse bone marrow. Eight daily intraperitoneal injections of this extract in mice induced a 6.2-fold increase in peripheral blood granulocytes and a 14.6-fold increase of splenic CFU-gm, with concomitant increases in the proliferation rates of CFU-gm in both bone marrow and spleen. Pretreatment of the AA urinary extract with sialidase significantly diminished these granulopoietic effects in vivo (P less than 0.001). In contrast, both extracts (i.e., native AA and sialidase-treated AA urinary extracts) revealed high granulocyte/macrophage colony-stimulating factor activity in vitro when clonal assays were performed with mouse bone marrow. Increased in vivo and in vitro granulopoietic activities were found in the concanavalin A "break-through" fraction, indicating that these activities were due to protein(s) that did not bind to the lectin. These results reveal that this urinary extract from patients with AA is capable of inducing significant granulopoiesis in mice and that sialic acid is an important component in the maintenance of this granulopoietic effect in vivo but not in vitro.
