Two cases of Hashimoto's thyroiditis with pupillary disturbances.

Two cases of Hashimoto's thyroiditis with pupillary disturbances are reported. Case 1 was a 58-year-old housewife, displaying pandysautonomia. Case 2 was a 65-year-old housewife, suffering from chronic polyneuropathy and polymyopathy. Both patients showed similar pupillary disturbances, suggesting both sympathetic and parasympathetic postganglionic denervation of the pupil. Hashimoto's thyroiditis may have caused the pupillary disturbances in association with generalized polyneuropathy or pandysautonomia.

Studies on antitumor agents. IX. Synthesis of 3'-O-benzyl-2'-deoxy-5-trifluoromethyluridine.

A practical synthesis of 3'-O-benzyl-2'-deoxy-5-trifluoromethyluridine (1), a candidate antitumor agent for clinical testing, was developed from 2'-deoxy-5-iodouridine (3). Benzylation of 2'-deoxy-5-iodo-5'-O-trityluridine (14) with benzyl bromide and sodium hydride in tetrahydrofuran gave the 3'-O-derivative (16). Benzoylation of 16 afforded the N3-benzoyl derivative (17). Coupling of 17 with trifluoromethylcopper, prepared from bromotrifluoromethane and copper powder in the presence of 4-dimethylaminopyridine, gave the 5-trifluoromethyl derivative (19) minimally contaminated with the 5-pentafluoroethyl compound. Deprotection of 19 furnished 1.

Operations for forearm deformity caused by multiple osteochondromas.

We reviewed 36 cases of forearm deformity caused by multiple osteochondromas in 30 patients and classified them into three types: Type I showed a combination of ulnar shortening and bowing of the radius secondary to osteochondromas of the distal ulna (22 forearms). Type II showed dislocation of the radial head, either with osteochondromas of the proximal radius (Type IIa, two forearms) or secondary to more distal involvement (Type IIb, five forearms). Type III had relative radial shortening due to osteochrondromas at the distal radius (seven forearms). Operations were performed on 16 forearms in 13 patients, with 92% of satisfactory results. For Type I deformity, excision of osteochondromas, immediate ulnar lengthening and corrective osteotomy of the radius are recommended. For Type IIa, excision of the radial head is necessary, and for Type IIb, we advise gradual lengthening of the ulna using an external fixator. Excision of osteochondromas alone gave good results in Type III deformity. Our classification gives a reliable indication of the prognosis and is a guide to the choice of surgical treatment.

Detection of nuclear protein antigens to antinuclear antibodies in serum of NOD mouse.

Nuclear protein antigens to the antinuclear antibodies in serum of non-obese diabetic (NOD) mice were investigated. In the serum of diabetic NOD female mice (20 weeks old), the antinuclear antibodies were detected by indirect immunofluorescence assay using frozen sections of liver of C 57 BL/6 J or NOD mice as antigen. Nuclei were separated from the liver of C 57 BL/6 J mice and solubilized. Solubilized nuclear antigens were analyzed by SDS PAGE-Western immunoblotting techniques. Nuclear protein antigens with molecular weights of 26,000, 32,000 and 65,000 showed strongly positive reactions with the antinuclear antibodies in the serum of the NOD mouse.

[Change of incidence of antinuclear antibodies in serum of NOD mouse with aging].

Extractable nuclear antigens (ENA) were prepared from liver of C57BL/6J mouse and analyzed by SDS PAGE Western-immunoblotting techniques. Some protein components of the ENA, with molecular weights of 94 K, 65 K, 32 K, and 26 K, reacted with antinuclear antibodies in the sera of NOD mice. Incidence of antinuclear antibodies in the sera of NOD mice with aging were measured by ELISA method using the ENA as antigen. The antinuclear antibodies were not detected in young NOD mice (10 weeks old). However, the incidence increased with aging and reached 100% in the female NOD mice of 40 weeks. In the male NOD mice, the incidence of antinuclear antibodies was delayed and low in comparison with that in female.

[Influence of a repetitive commanded acoustic signal with constant frequency on jaw tapping movement].

Influence of a repetitive commanded acoustic signal with constant frequency of 1.3 Hz on jaw tapping movement was studied. 10 healthy subjects were made to perform anticipated jaw tapping movement corresponding to the signal. Jaw tapping movement with and without signal were analyzed for their opening distance and a ratio of duration of opening phase I (depressing phase), opening phase II (plateau phase), closing phase and tooth contact phase to the cycle time. 1. Opening distance in loaded tapping was shorter than that in the tapping ad lib. (P not equal to 0.01), and independently of commanded signal it became to be longer with increasement of trial numbers (P not equal to 0.01). 2. The ratio of duration of tooth contact phase in loaded tapping became smaller with increasement of the trials, although that in the tapping ad lib. became greater (P less than 0.001). 3. The ratio of duration of opening phase I in loaded tapping became greater with increasement of the trials, while that in the tapping ad lib. became smaller (P not equal to 0.01). 4. It could be interpreted that in the loaded tapping "an instant performance control" acted from the beginning of the trials and the signal might make it facile or easy to detect and confirm errors in movement.

Investigation on the effect of dipyridamole and papaverine on regional blood flow and cardiac hemodynamics in anesthetized dogs.

Vasodilating effects of dipyridamole were compared with those of papaverine in anesthetized open-chest dogs. Dipyridamole i.v. produced a significant fall in aortic blood pressure and significant increases in aortic and coronary blood flows. Vertebral blood flow did not alter, but renal blood flow was transiently decreased. Left ventricular dP/dt, heart rate and left ventricular enddiastolic pressure were hardly affected by dipyridamole. Papaverine i.v. significantly increased aortic, vertebral and coronary blood flows, left ventricular dP/dt and heart rate and significantly decreased aortic blood pressure and left ventricular enddiastolic pressure. Renal blood flow was instantly decreased by papaverine. Percent change in each vascular resistance showed decreases in total peripheral, vertebral and coronary vascular resistances in response to both drugs, and the decrease in coronary vascular resistance was the most remarkable. Renal vascular resistance was increased by dipyridamole, while it was decreased by papaverine. Percent change in double product (heart rate x systolic arterial blood pressure) showed a monophasic decrease pattern with dipyridamole and a biphasic pattern with an initial, transient decrease followed by an increase with papaverine. The present results indicate that dipyridamole and papaverine produce marked coronary vasodilatation, and that papaverine is different from dipyridamole in cardiotonic properties.

A clinical study of patients with idiopathic thrombocytopenic purpura.

Fifty patients with chronic idiopathic thrombocytopenic purpura (ITP) were initially treated with corticosteroids. Seven patients had an excellent response (normal platelet count after therapy), 17, 18, and 8 had a good response (normal platelet count during therapy), fair response (improved platelet count during therapy) and poor response (no improvement), respectively. Nine patients, who had a fair or poor response to the corticosteroids, were splenectomized. Five of the 9 had an excellent response to splenectomy. The other four patients were then treated with azathioprine. Three of the four responded well to the immunosuppressive therapy. The effectiveness of three main therapies for chronic ITP was confirmed in the present study. Especially, it indicated that splenectomy can be considered for relatively young patients who show a fair to good response to corticosteroids. Of the 6 patients with acute ITP, 5 had an excellent response to corticosteroids, and 1 recovered without therapy.

Production in Escherichia coli of human thymosin beta 4 as chimeric protein with human tumor necrosis factor.

We have produced thymosin beta 4 protein in Escherichia coli as a chimeric protein with tumor necrosis factor (TNF). The protein was abundantly expressed, was immunoreactive against both anti-thymosin beta 4 and anti-TNF antibodies, and retained cytotoxicity in a TNF assay using mouse L929 fibroblasts. This latter characteristic enabled the easy and simple purification of thymosin beta 4 merely by following the TNF activity. The chimeric protein was designed to have an Asp-Pro bridge between thymosin beta 4 and TNF which could be specifically cleaved under suitable acidic conditions to release the thymosin beta 4 from the chimeric protein. These results indicate that the expression system in E.coli of a chimeric protein composed of thymosin beta 4 and TNF is appropriate for obtaining an abundant amount of the beta 4 peptide, especially since its purification from tissues is usually difficult because of limited yield and obscurity of its biological activity.

[A review of colorectal cancer in atomic bomb survivors--treated cases at Nagasaki Genbaku Hospital from 1971-1984].

Clinical and pathological characteristics of patients with colorectal cancer amongst atomic bomb survivors, who had undergone operations from 1971-1984, have been reviewed and compared with that of a control group. The survival rate of the atomic bomb survivors over the age of 60 years was statistically better than that of the same age group in the controls. In this age bracket, the control group were in a more advanced stage of the disease than were the survivors, this accounting for the reason why the survivors had a better prognosis. Further, the fact that the survivors continually have received more medical attention than have the aged in the control group affects this statistic.

[Usefulness of fine needle aspiration cytology in the diagnosis of prostatic carcinoma].

Eighty-six cases of cytological diagnosis of the prostate by fine needle aspiration performed between January, 1986 and December, 1987 were reviewed. Twenty patients were diagnosed as positive cytology in prostate (malignant: class V, suspicious: class IV or atypical: class III) and were admitted and further evaluated with conventional needle biopsy followed by histological diagnosis. Then, 18 of them were diagnosed with prostatic carcinoma. False positive rate and false negative rate of this aspiration cytology were 10% and 3.0%, respectively. There was only one minor complication (fever onset) (1.2%) following this procedure. These results indicate that fine needle aspiration cytology of the prostate gland is an easily performed, diagnostically reliable outpatient procedure with minimal complications.

Antitumor effect of systemic administration of novel recombinant tumor necrosis factor (rTNF-S) with less toxicity than conventional rTNF-alpha in vivo.

This study reported that acute toxicity (expressed as the LD50 value) of rTNF-SAM1 and rTNF-SAM2 which we constructed was considerably lower than that of rTNF-alpha (1). Following this finding, the antitumor effects of systemic administration of these novel recombinant tumor necrosis factors (TNF-S) (named rTNF-SAM1 and rTNF-SAM2) were examined in vivo on murine tumors (Meth A fibrosarcoma, MH134 hepatoma, and B16 melanoma). Both rTNF-SAM1 and rTNF-SAM2 could be administered systemically to tumor-bearing mice at doses up to 3 X 10(4) to 10(5) U. Growth of all tumors was significantly inhibited by systemic administration of rTNF-SAM1 or rTNF-SAM2 at a dose of greater than 3 X 10(4) U; at this dosage conventional rTNF-alpha could not be administered systemically to any of the mice strains due to its toxicity. These results confirm our previous finding that the rTNF-SAM group can be administered with safety at higher doses than rTNF-alpha, and revealed that these novel rTNF-S could be more promising antitumor drugs than rTNF-alpha in view of their lower toxicity compared with antitumor effect.

[A case report of pulmonary arterio-venous fistula complicated with hereditary hemorrhagic telangiectasis].

A 5-year-old girl with multiple pulmonary arterio-venous fistulae was reported. Both her mother and grandmother have been diagnosed as the same disease previously. Her grandmother was treated with resection of the right basal segment for arterio-venous fistulae. Her mother is well with fistulae for more than 20 years. The patient was operated upon to enucleate two larger fistulae in segments 3 and 6 of right lung. Her postoperative course was uneventful. She was discharged from the hospital on the 22nd postoperative day. Pulmonary arteriovenous fistulae are thought to be a part of the hereditary hemorrhagic telangiectasis. Enucleation of the larger fistulae of the lung are preferable even for multiple lesions.

[Pacemaker implantation in 7 days or less old of early neonates].

We have implanted cardiac pacemaker (PM) in 3 early neonates ranging from 5 hours old to 7 days old of age with congenital complete or incomplete AV block. They were in advanced stage of rapidly developed cardiac failure with resistance to drug therapy and with ventricular tachycardia or fibrillation. Cardiac failure was improved immediately in the 3 cases after the operation and the effectiveness by the pacing was recognized for a long term observation (4 months to 2 years). There appeared PM implantation in-14 neonates, including our cases, in Japanese literatures. In review of the 14 neonates, we discussed the autoimmune disease, indication of the pacing, implantation site of the PM generator and prognosis. An immediate PM implantation after birth in neonate with cardiac failure complicated by congenital AV block is treatment of choice. The generator as well as the electrode design for neonates are still yet to be developed.

Separation of peptides by multimode chromatography on a column packed with vinyl alcohol copolymer gel.

Several mechanisms of peptide separation in high-performance liquid chromatography were observed to occur on the Asahipak GS-320 packed with vinyl alcohol copolymer. Neutral rather than acidic mobile phases were employed as they were found to result in higher retention of many peptides on the GS-320. For neutral peptides, the retention volume corresponded to the Rekker's hydrophobic fragmental constant, with a correlation coefficient of 0.71. Peptides with acidic residues eluted early, as an effect of ionic exclusion; those with basic residues were retained longer, owing to an ion-exchange effect. The ionic interactions were shown to involve the carboxylic group present on the gel polymer. The net result was found to be excellent separation of hydrophilic as well as hydrophobic peptides, related to differences in their isoelectric points. The combination of these complex mechanisms, together with the size-exclusion effect of the GS-320 gel for separation of proteins and other large molecules and for analysis with a mobile phase high in acetonitrile content, makes possible high-resolution isocratic analysis of peptides, which cannot be achieved on octadecylsilica or ion-exchange columns.

Determination of salsolinol by ion-exchange chromatography with glycylglycine as the post-derivatizing agent.

The determination of salsolinol in human urine was carried out by ion-exchange chromatography on two coupled columns of a weakly acidic ion exchanger with a hydrophilic matrix (Asahipak ES-502C). Salsolinol was first isolated from urine by adsorption on Amberlite CG-50. It was eluted together with catecholamines by 2/3 M boric acid solution. The amines were then separated by isocratic elution from the first column of Asahipak with 0.05 M sodium succinate buffer (pH 5.5) containing 0.015 M borate and 0.5 mM ethylenediaminetetraacetate. Epinephrine, norepinephrine, dopamine and salsolinol were eluted in that order. The salsolinol-containing fraction was then transferred, by column switching, to a second Asahipak column and eluted with the same mobile phase. Salsolinol was determined fluorimetrically by reaction with glycylglycine in the presence of hexacyanoferrate(III) at pH 7.5-8 and 65 degrees C. Samples could be analysed every 47 min. The detection limit for salsolinol was 2 pmol/ml.

Inositol trisphosphate-linked calcium mobilization couples substance P receptors to conductance increase in a rat pancreatic acinar cell line.

The action of substance P (SP) on a rat pancreatic acinar cell line, AR4-2J, was examined using the whole-cell voltage-clamp technique. Pressure application of SP evoked inward currents accompanying increased membrane conductance. The SP-induced response was suppressed in Ca2+-free or low-Na+ solution. Treatment of cells with caffeine or A23187 produced a transient inward current and depressed the response to SP. Intracellular application of inositol 1,4,5-trisphosphate or guanosine 5'-O-(3-thiotriphosphate) elicited sustained inward currents and suppressed the SP-induced response. The results suggest that activation of SP receptors stimulates the formation of inositol phosphates via a guanine nucleotide-binding protein and leads to the rise in intracellular Ca2+, thereby activating a cation conductance in AR4-2J cells.
