[Effects of ceftizoxime suppository rectal dosing on cecal microflora in mice].

The effect of ceftizoxime suppository (CZX-S), a new rectal preparation of ceftizoxime (CZX), against cecal microflora of mice following consecutive rectal administration was compared with that after subcutaneous administration of CZX. The numbers of aerobic and anaerobic bacteria except Enterobacteriaceae of cecal microflora were not markedly changed by the rectal administration of CZX-S at a dose of 25 mg/kg 3 times daily for 10 days. Although the number of Enterobacteriaceae was decreased, recovery was rapidly observed after completion of the administration period. In the case of subcutaneous administration, a decrease in Enterobacteriaceae was also observed, but this was no more remarkable than that occurring with rectal administration. Moreover, effects on the state of feces, i.e., diarrhea, were not observed in any of the mice. The peak level of CZX in the cecum contents when administered rectally at a dose of 25 mg/kg was 13.8 micrograms/g at 4 hours, while it was less than the determination limit (2.0 micrograms/g) when administered subcutaneously at the same dose.

Angiostrongylus cantonensis infection in Ampullarius canaliculatus (Lamarck) in Kyushu, Japan.

Ampullarius canaliculatus is a large fresh-water snail of which the original habitat is said to be Argentina. Recently, the snail which grew wild has propagated in various parts of Japan, and is causing damage to aquatic plants by feeding on them. The present study was performed to find out whether the snail can be a suitable intermediate host for Angiostrongylus cantonensis. Natural infection with A. cantonensis was confirmed in the snails collected from 5 different places in 4 hamlets in Okinawa and in those from Ishigaki Island. All snails experimentally infected with first-stage larvae of the parasite showed second-stage and third-stage larvae developing at the time of examination later on. Thus, A. canaliculatus is a suitable intermediate host for A. cantonensis, and, if eaten raw, it can be a potential source of human infection with eosinophilic meningoencephalitis in the endemic areas.

[The effects of medical treatment on the long-term prognosis of variant angina pectoris].

To investigate the changing aspect of the long-term prognosis of patients with medically-treated variant angina, we studied 253 consecutive patients treated from January 1963 to August 1984. The patients were categorized into two groups according to the year of first admission to our hospital; 88 patients hospitalized before January 1975 and mainly treated with nitrates (group I), and 165 patients admitted thereafter and treated with calcium antagonists (group II). The average follow-up period was 63.1 months (ranged from one to 136) in the group I, and 32.4 months (ranged from one to 116) in the group II. Coronary arteriography was performed in 146 patients of the group II. In 82 patients (56%), no fixed coronary artery stenosis of greater than or equal to 75% of the luminal diameter was present; 47 patients (32%) had one-vessel disease, and 17 (12%) had multi-vessel involvement. Seventy-eight patients having both rest and effort angina showed a higher prevalence of fixed coronary artery stenoses (50 of 78 patients) compared with the patients without effort angina (14 of 68 patients) (p less than 0.001). The group II showed a significantly good long-term efficacy of medical treatment for anginal attacks (124 of 165 patients compared with 54 of 88 group I patients: p less than 0.05). Complete remission over three months or more occurred in 56% of the group II compared with 45% of the group I. Fourteen patients (16%) of the group I suffered from myocardial infarction (10 were within six months of onset of angina); whereas, only five (3%) of the group II developed myocardial infarction (two were within six months of initial anginal attack) (p less than 0.001). Cardiac death occurred in five patients (6%) of the group I in contrast to four patients (2%) of the group II. The patients having significantly effective medical treatment for anginal attack were 80%, 62% and 64% in those without significant stenosis, with one-vessel disease, and with multi-vessel disease, respectively. Myocardial infarction occurred in two of 17 patients with multi-vessel disease. Spontaneous remission of angina over at least three years without medical treatment occurred in seven of 32 patients who were followed for more than 10 years. It was concluded that the long-term prognosis of patients with variant angina who received calcium antagonists was reasonably improved compared with patients treated with nitrates.(ABSTRACT TRUNCATED AT 400 WORDS)

[General pharmacology of (2"R)-4'-O-tetrahydropyranyladriamycin, a new antitumor antibiotic].

General pharmacology of (2"R)-4'-O-tetrahydropyranyladriamycin hydrochloride (THP) was studied in experimental animals. Intravenous administration of THP showed no significant effect on the respiratory and cardiovascular systems, such as blood pressure, heart rate, ECG and respiration in anesthetized rabbits and dogs. But in rats and cats, THP produced a transient decrease in blood pressure resulted from vasodilation. The hypotension was not inhibited by antihistaminics. Contraction of isolated guinea-pig atria was stimulated by THP at high concentrations (10(-4) g/ml). THP inhibited the spontaneous movement of isolated rabbit ileum and rat uterus (virgin and pregnant) at high concentrations (10(-4) g/ml). In some isolated guinea-pig ileum preparations, THP partially (6 approximately 36%) antagonized the contraction inducing by acetylcholine, histamine, serotonin and barium chloride. Urine volume and urinary excretion of electrolytes were increased by intravenous injection of 5.0 mg/kg THP. Vascular permeability was progressed when administered intracutaneously. Hemolytic effect was shown at high concentrations (10(-4) g/ml) but no effect on the coagulation was found. No significant effect of THP was observed on the general behavior and central nervous system, autonomic nervous and peripheral nervous systems. Also, THP had no significant effect on gastrointestinal propulsion in mice, the mucous membranes of the stomach and duodenum of rats, or gastric acid and bile secretion in rats.

[Therapeutic experiences of cis-diamminedichloroplatinum, adriamycin and 5-fluorouracil combination chemotherapy in advanced urothelial cancer, with special reference to adjuvant chemotherapy in invasive urothelial cancer].

The combination chemotherapy including cis-diamminedichloroplatinum (CDDP), adriamycin (ADM) and 5-fluorouracil (5-FU) is reported as one of the most effective regimens of urothelial cancer. We experienced one patient who had multiple metastatic urothelial cancer in the lung, liver and brain and who showed complete response, even in short duration, by this regimen. Since then, we have been using this regimen as the adjuvant chemotherapy of the invasive urothelial cancer. This regimen consists of 15 mg/m2 CDDP on days 1 to 5, 30 mg/m2 ADM on day 1 and 300 mg/m2 5-FU on days 1 to 5, and is repeated 3 times for 3 to 4 weeks. If toxicity is intolerable, the dosage of CDDP and ADM was decreased in thirty percent. As the maintenance, tegaful was administrated oral or suppository for 1 to 2 years. Seven cases which were followed for at least 12 months were evaluated. In one patient, local recurrence appeared after 16 months, but the other patients were disease free. General malaise, nausea, vomiting and alopecia were recognized in all patients in various degrees, but severe myelosuppression did not appear. Because of the toxicity, three patients had to have reduced dosage. In the adjuvant chemotherapy the most effective regimen should be used in the first therapy to eradicate the micrometastasis. At present, the combination chemotherapy of CDDP and ADM is the most effective for urothelial cancer as reported by many authors. Although the number of patients and the duration of follow up are inadequate to evaluate this adjuvant chemotherapy, we believe that this regimen is effective since invasive urothelial cancer usually recurs within 2 years.

[Experience with microsurgical epididymovasostomy (specific tubule technic: end-to-end anastomosis by the Silber method)].

Here, we report 2 cases of epididymovasostomy operation in which good results were obtained. These operations were done for patients whose complaints were male sterility due to obstructive azospermia after bilateral epididymitis. The method of this operation was reported by Silber in 1978 as "specific tubule technique", and excellent results have been reported. Herein, we introduce this technique perhaps for the first time in Japan, and the characteristics of this method are considered. The important factors and characteristics of this method are that it is very reasonable with a high success rate, it is more difficult than microsurgical vasovasostomy, it is a time-consuming operation. Epididymovasography should not be done preoperatively but intraoperatively, if possible and chronic epididymitis should be ruled out by serological study and culture study.
