[A examination of conversation process in social context].

The purpose was to reveal a set of conversation rule and to examine the effect of social context on the importance of the rule. We collected 51 items concerning conversation rule from 29 male and female undergraduates. Another 128 male and female undergraduates were asked to rate the importance of each item in given social contexts. We manipulated three factors of (a) goal orientation in conversation, (b) intimacy of a dyadic relationship, and (c) combination of sex, in the descriptions of the social contexts. Factor analysis of the rating scores led to identification of six factors of conversation rule: 1) acceptance rule, 2) rule for partner, 3) rule for accomplishment of speech, 4) rule for intention of speech, 5) emotion-directed expression rule, 6) comprehension-directed expression rule. The results of ANOVA indicated that the importance of each factor varies with social context.

Specific induction of 68 KD protein synthesis in rabbit smooth muscle cells by growth stimuli in platelets: comparison of intimal and medial SMC.

We examined the influence of growth stimuli in heat-treated platelet extract on specific protein synthesis in the early phase of the cell cycle prior to the initiation of DNA synthesis in cultured rabbit aortic smooth muscle cells (SMC). The extract preferentially stimulated the synthesis of a cytoplasmic protein with a molecular weight of 68000 (p68) and an isoelectric point of around 6.3. Stimulation of p68 synthesis occurred within 1 h after the addition of heat-treated platelet extract to growth-arrested and quiescent SMC, continued until 4 h after stimulation, and then returned to the baseline level. Actinomycin D preferentially inhibited p68 synthesis. In SMC prepared from atheromatous plaques from the aorta of hyperlipidemic rabbits (I-SMC), the amount of DNA synthesis and of p68 synthesis by heat-treated platelet extract were less than those of SMC prepared from normal media (M-SMC), suggesting that the decreased capacity for cell proliferation of I-SMC in response to heat-treated platelet extract was due to the down-regulation of signal transduction in the early G0/G1 phase of the cell cycle.

[Measurement of blood glucose variations due to surgical stress].

It has been confirmed that the oral surgical invasion not only arouses emotions, such as anxiety and fear, but causes physiological changes at the same time through 2 different pathways, the autonomic nervous system and endocrine system. The change in blood glucose concentration is known as one of representative vital reactions associated with these 2 functional systems. Although secure measurement manipulation producing errors as small as possible is essential to the comparison of blood glucose concentrations obtained from a same individual at different times, various errors are often made during specimen preparation and measurement. Resulting values of glucose concentration are known to be especially influenced by specimen pretreatment, kinds of specimens, meals, menstruation and anesthetics among factors affecting the measurement. Thus we determined these factors in order to establish a measurement procedure to produce the least errors. As a result, it was thought to be desirable that the measurement is performed using the following procedure: 1) blood is sampled more than 90 minutes after meal; 2) whole blood collected from the median basilic vein is used as a specimen; 3) measurement is performed as soon as possible, within 60 minutes after sampling; 4) quantitative values of blood glucose concentration are obtained using dextrosticks and a dextrometer; 5) every specimen is measured 5 time to obtain a mean value used as blood glucose concentration in that specimen through averaging 3 measurement values, omitting the highest and lowest values, and 6) as for female patients, measurement (as well as operation) is performed on a day 2 weeks after the beginning of menstruation. It was concluded that blood glucose concentration can be used as a highly reliable parameter for the observation of vital reactions to external stimulation and for the determination of internal environment at a certain point of time if the measurement is performed with careful consideration of the above-mentioned factors.

Clinical and cytogenetic features in six patients with chronic myelogenous leukemia and a complex Philadelphia translocation.

Six patients with chronic myelogenous leukemia and a complex Philadelphia (Ph1) translocation are described. The complex Ph1 translocations were a three-way translocation in five patients and a five-way translocation in one. Additional chromosomal aberrations were detected in four of five patients when the blastic crisis supervened. The median survival time was 42 months. The remaining patient died of acute myocardial infarction 23.5 months after the diagnosis of CML was made. There seems no difference between these six patients and those with the standard Ph1 with respect to clinical, hematologic and cytogenetic findings.

[Early operation for postinfarction ventricular septal perforation; report of three cases].

Three patients underwent surgery for postmyocardial infarction ventricular septal perforation (VSP) within 3 to 21 days after onset of infarction. The hemodynamic stabilization was not obtained despite aggressive medical treatment including Intra-aortic Balloon Pumping (IABP) in one patient. The others had sudden hemodynamic deterioration during IABP support. In two of the three cases, the VSP were closed via transinfarct ventriculotomy with double Dacron patch, and ventricular wall reconstruction was performed to sandwich the double septal patch between ventricular free walls with Dacron felt strips. Two of the three patients survived. Our experience suggests that early surgical intervention is essential unless medical therapy results in clinical improvement and the double patch method may provide a successful operative repair and comeout.

[An autopsy case of primary cutaneous plasmacytoma].

An autopsy case of primary cutaneous plasmacytoma with very unusual extensive skin involvement resulting in death 9 months later, was reported. A 75-year-old female was admitted to our hospital in November, 1985 because of an enlarging skin nodule on the right neck of 5 month's duration. The nodule was a 5 x 7 x 4-cm, firm and mobile mass. Light- and electron-microscopic studies of its biopsy specimen revealed a cutaneous plasmacytoma which was composed of dense aggregates of plasma cells. Cytogenic study on the biopsy specimen revealed hypotetraploid and structural abnormalities such as 7q+, 11q+ and 20q+. After radiotherapy, the right neck nodule became smaller, but subcutaneous indurations with erosions extended to the surrounding skin. The biopsy specimen of these skin lesions microscopically revealed massive infiltrations of plasma cells in the dermis. The PAP method revealed a definite evidence of monoclonal kappa light chain production by these cells. 3H-thymidine were incorporated in 5.6% of the plasma cells. The skin lesions were refractory to chemotherapy and gradually extended. The clinical course showed a progressive one leading to persistent deterioration and she died in August, 1986. Repeated examinations including immunoelectrophoresis of serum and concentrated urine, bone marrow aspirations and skeletal x-ray films, excluded the diagnosis of myeloma. At autopsy, massive infiltrations of plasma cells in the skin of chest wall and neck, small metastatic tumors in the liver and bilateral axillary lymph nodes were found, but there was no evidence of bone marrow involvement.

Studies on intimal smooth muscle cells in rabbits: decreased growth response to the tumor promoter.

The growth behavior of intimal smooth muscle cells (SMC) prepared from atheromatous plaques of the thoracic aorta in hyperlipidemic rabbits was studied in a culture system. Specimens of intimal and normal medial SMC were examined in terms of their proliferative response to various growth factors, polypeptide hormones or 12-O-teradecanoylphorbol-13-acetate (TPA). Intimal SMC showed lower rates of growth and DNA synthesis when the cells were exposed to TPA, but there was no difference in growth response between intimal and medial SMC to the other growth-promoting stimuli such as fibroblast growth factor (FGF), epidermal growth factor (EGF), insulin or serotonin. 3H-phorbol-12,13-dibutyrate (3H-PDBu) binding assays showed the number of binding sites to phorbol esters in intimal SMC to be decreased by 65% as compared with that in medial SMC. These results suggested that intimal SMC have different growing characteristics, which seemed to be acquired during the process of intimal thickening.

[Two cases of spontaneous pneumomediastinum in healthy young men].

We report two cases of spontaneous pneumomediastinum. Both were young males, and had no causal diseases or apparent precipitating episodes. Both recovered rapidly with bed rest only. Symptoms, signs, radiological diagnosis, and pathogenesis of pneumomediastinum are discussed.

Two cases of pancreatic cystadenocarcinoma with elevated CA 19-9 levels in the cystic fluid in comparison with two cases of pancreatic cystadenoma.

A 39-year-old woman was hospitalized with left hypochondralgia. Ultrasonography and abdominal computed tomography showed a cystic mass in the tail of the pancreas. Echo-guided aspiration of the cyst was performed, and a markedly elevated CA 19-9 level in the cystic fluid was found. A surgical operation was performed, and the tumor was radically resected. The pathologic diagnosis was papillary adenocarcinoma of the pancreas. Another case of pancreatic cystadenocarcinoma also had an extremely high cystic CA 19-9 level, whereas two cases with benign pancreatic cysts had very low cystic CA 19-9 levels. Moreover, positive localization of CA 19-9 in the cancerous tissue was clearly demonstrated by an immunohistochemical technique, indicating active secretion of CA 19-9 into the cyst, while CA 19-9 in the tissue of pancreatic cystadenoma was almost non-existent. Measurement of the CA 19-9 level in the cystic fluid might therefore be a valuable additional aid in the diagnosis of cystadenocarcinoma of the pancreas.

Immunohistological analysis of thymic tumors with PE-35 monoclonal antibody reactive with medullary thymic epithelium.

PE-35 mouse monoclonal antibody (MoAb) (IgG1) detecting an epithelial antigen with a molecular weight of 35,000 was characterized serologically. Immunoperoxidase staining and double immunoenzymatic staining showed that PE-35 antigen is predominantly on nonlymphoid cells in the medulla of thymus. By immunoelectron microscopy, thymic epithelial cells in the medulla were positive with PE-35 MoAb, but macrophages, interdigitating reticulum cells, and thymocytes were negative with this MoAb, which demonstrated that PE-35 is a valuable marker for medullary epithelium. Using PE-35 and other MoAbs detecting thymic epithelial antigens (TE-3A, RFD-4, TE-4, and HLA-DR), 25 thymomas were studied, together with 6 other tumors of thymic origin. Among 25 thymomas, all 6 cases of epithelial type and 8 of 14 mixed lymphoepithelial type were positive with PE-35 MoAb, but only one of 5 lymphocytic type was positive. PE-35 antigen has a tendency to be expressed in the cases retaining medullary type thymocytes, with the phenotype of cluster of differentiation (CD) 1-/CD3+/CD6+, and also in the area of medullary differentiation. TE-3A, RFD-4, and TE-4 MoAbs reacted with most thymoma cases regardless of the types. HLA-DR was, however, expressed on a part of thymomas and the phenotype combined with that of PE-35 was as follows: PE-35+/HLA-DR+, 8 cases; PE-35+/HLA-DR-, 8 cases; PE-35-/HLA-DR+, 8 cases; PE-35-/HLA-DR-, one case. The results suggested that thymoma may originate from different subsets and/or different stages of thymic epithelium.

Plasma cell leukaemia of non-producer type with missing light chain gene rearrangement.

A case of plasma cell leukaemia of non-producer type is described. The patient presented with typical clinical features of plasma cell myeloma, including multiple osteolytic lesions, hypercalcaemia, renal failure and reduced polyclonal immunoglobulins, except that M-component was not detected in either the serum or urine. Morphological examinations showed a plasmacytoid appearance of the neoplastic cells, while immunological studies failed to detect cytoplasmic immunoglobulin or secretory capacity. The surface phenotype of CD38+, PCA-1+, DR-, CD20-, CD24-, CD9-, CD10- and surface immunoglobulin- was compatible with mature plasma cells. Chromosomal analysis showed the 14q+ marker due to translocation (6;14) and deletion of the short arm of chromosome 1. Analysis of immunoglobulin genes revealed the presence of heavy chain gene rearrangement, but the light chain genes, both kappa and lambda, remained in germline configuration. Such defective immunoglobulin gene rearrangement may be responsible for the failure of immunoglobulin biosynthesis and secretion by the neoplastic plasma cells. Furthermore, it is suggested that the morphological and phenotypic development of B cells may not necessarily depend on immunoglobulin light chain gene rearrangement, and that the oncogenic event in myeloma may occur at an earlier stage of B cell differentiation.

Assignment of a gene coding for a human T-cell antigen with a molecular weight of 40,000 daltons to chromosome 17.

Hybrid human-mouse T-cell clones reactive with Tp40 antibody, which detects cluster of differentiation (CD)7 antigen on human T lymphocytes, were established. Karyotype analysis showed that human chromosome 17 was essential for the expression of CD7 antigen. The presence of this chromosome was confirmed by enzyme analysis of galactokinase, which is coded by a gene on human chromosome 17.

Concentration and localization of carbohydrate antigen 19-9 in tissues of pancreatic cancer.

Carbohydrate antigen (CA) 19-9 concentration in the tissue-extracts of cancerous and noncancerous pancreatic tissues were determined by radioimmunoassay. Pancreatic cancer tissues revealed significantly elevated CA 19-9 concentrations, when compared with chronic pancreatitis, normal adult pancreas, or fetal pancreas tissues. Metastatic liver tumors from pancreatic cancer also showed extremely high CA 19-9 concentrations, and there was no significant correlation between tissue CA 19-9 concentration and serum CA 19-9 level in patients with pancreatic cancer. In addition, positive localization of CA 19-9 was clearly observed in cancer cells of pancreatic cancerous tissues by immunohistochemical study, confirming the remarkable increase of CA 19-9 concentration in tissues of pancreatic cancer, although CA 19-9 was also partially found in non-malignant pancreatic tissues. The results indicated that CA 19-9 would be produced in great quantities by cancer cells in tissues of pancreatic cancer, and thus could be a valuable tumor associated antigen suggesting its clinical use as a tumor marker for cancer of the pancreas.

Morphology and increased growth rate of atherosclerotic intimal smooth-muscle cells.

Atherosclerotic intimal smooth-muscle cells (SMCs) in vitro showed higher growth activity than did medial SMCs obtained from either atherosclerotic or normal aortas. Using an electron microscope, it was proved in primary cultures by an explant method that intimal SMCs had rich organelles and fewer filaments in their cytoplasms. They were regarded as synthetic phenotype. In contrast, most medial SMCs had rich filaments and fewer organelles. They were regarded as contractile phenotype. When atherosclerotic intimal and normal medial SMCs were plated on type I collagen gel, cytoplasmic cyclic adenosine monophosphate concentration increased and DNA synthesis was suppressed. Intimal SMCs cultured on the gel showed contractile phenotype. Dibutyryl cyclic adenosine monophosphate added to culture media decreased DNA synthesis and altered cellular phenotype to a contractile state. Intimal SMCs were more resistant to injury by hyperlipidemic low-density liproprotein and homocysteine. Lysosomal enzyme activity was enhanced in intimal SMCs.
