Effects of dynamic leg exercise on subcutaneous blood flow rate in the lower limb of man.

Subcutaneous blood flow (SBF) was studied simultaneously in the upper arm at heart level and in the lower limb during positional changes and during leg exercise in seven healthy males. SBF was estimated by local clearance of 133Xenon registered by portable cadmium telluride detectors. Venous pressure was recorded directly on dorsum on the foot. Changing the position from supine to head-up tilt, SBF decreased by 43% (P less than 0.01) at the arm level, 40% at the thigh (P less than 0.01), 47% at the calf (P less than 0.01) and decreased by 51% at the ankle level (P less than 0.01). Performing 20 heel-raisings per min in nearly erect posture, SBF increased by 96% at the thigh (P less than 0.01), 25% at the calf (P greater than 0.1) and increased by 18% at the ankle level (P greater than 0.01). At 40 heel-raisings per min SBF increased by 99% at the thigh (P less than 0.01), 121% at the calf (P less than 0.01), but only 44% at the ankle level (P greater than 0.1). During leg exercise subcutaneous vascular resistance was significantly increased at arm and ankle levels. In contrast, a vasodilatory response was noticed at the thigh and calf levels and seemed associated with a decrease in local venous pressure to below the trigger level of the sympathetic veno-arteriolar reflex mechanism. In conclusion, SBF in the lower limb of man was increased during exercise. The increase in SBF could only partly be ascribed to the concomitant increase in perfusion pressure. The local blood flow response seemed modified by changes in sympathetic nervous activity and metabolic rate.

Treatment of cryptorchidism with human chorionic gonadotropin or gonadotropin releasing hormone. A double-blind controlled study of 243 boys.

We have conducted a modified double-blind study on the effect of human chorionic gonadotropin (hCG), gonadotropin releasing hormone (GnRH) and placebo on bilateral and unilateral maldescended testes. One hundred and fifty-five boys with bilateral and 88 boys with unilateral cryptorchidism fulfilled the inclusion criteria and completed the treatment protocol. The boys were between 1 and 13 years of age. hCG was administered as intramuscular injections twice weekly for 3 weeks. GnRH and placebo were given intranasally. hCG was superior to GnRH and placebo in the treatment of bilateral maldescended testes (p = 0.0009). Both testes descended in 25% of the boys following treatment with hCG, and improvement in the position of the testes was obtained in a further 25% of the cases. hCG administration resulted in complete testicular descent in 14% of boys with unilateral cryptorchidism compared with 3 and 0% after placebo and GnRH, respectively (p = 0.07). The testis had moved to a more distal position in 46% of the boys treated with hCG. There was no significant difference between the treatment groups with regard to age or initial position of the testes. We conclude that a success rate of 25% justifies the use of hCG in the treatment of maldescended testes, whereas the study did not support a general use of GnRH administered intranasally.

Stereological estimates of nuclear volume correlated with clinical stage and progression of prostatic cancer: clinical aspects.

Fifty-one transurethral prostatectomies were reviewed regarding the possible prognostic value of stereological estimation of cell nuclear volume in prostatic cancer. Patients with manifest cancer had a larger nuclear volume than those with incidental cancer, even though the two groups were matched concerning age and histological grade of the cancer. The nuclear volume was larger in patients with incidental cancer than in those with prostatic hyperplasia. Stereological estimation of nuclear volume may be a useful objective supplement to other prognostic parameters in prostatic cancer.

Stereological estimates of nuclear volume in the course of primary tumors of the renal pelvis.

The aim of this retrospective study was to provide quantitative data which make grading of primary tumors of the renal pelvis objective and reproducible by stereological estimation of nuclear volume using the principle of estimation of the mean volume of particles of arbitrary shape. The study includes 35 kidney specimens with primary transitional cell tumor of the renal pelvis. After standard fixation, embedding, sectioning and hematoxylin-eosin staining, an unbiased estimate of the mean volume of nuclei sampled with a change proportional to volume: (formula; see text) was calculated. Here 1(0) is the length of the intercept through a test point hitting a nucleus measured in a random direction. The mean nuclear volumes of the tumors are spread over a wide range, from 159 microns 3 to 1.555 microns 3. Ten of 11 patients with nuclear volume above 800 microns 3 died of the disease while only 8 of 24 patients with a mean nuclear volume below 800 microns 3 died of the disease. This simple and fast estimate of mean nuclear volume seems to provide objective data with a high prognostic value.

Bovine IgM: does it fix guinea pig complement in the absence of bovine complement components?

Purified bovine isotypes IgM, IgG1, IgG2, and IgA (secretory), affinity purified with Brucella abortus, were tested in a complement fixation test (CFT) for their ability to activate guinea pig complement directly or in the presence of 'normal' bovine serum. Only IgG1 fixed guinea pig complement in the direct test and approximately 250 ng of antibody was required to activate 50% of 3 CH50 units with a standard amount of antigen. Addition of 'normal' bovine serum as an additional source of complement resulted in activation of guinea pig complement by IgM, IgG2 and secretory IgA at levels of approximately 1200, 700 and 2250 ng, respectively for 50% of 3 CH50 units. Addition of 'normal' bovine serum did not enhance complement activation by bovine IgG1.

Peroxidase-labelled monoclonal antibodies for use in enzyme immunoassay.

Desirable characteristics of enzyme-antibody conjugates for use in enzyme immunoassay are labelling uniformity, permanent availability and stability. The use of monoclonal antibodies (McABS) for preparation of enzyme conjugates, in place of polyclonal antibodies, ensures labelling uniformity and permanent availability. The problem of stability still exists. Monoclonal antibody-horseradish peroxidase (McAB-HRPO) conjugates produced in our laboratory showed variable stability. After extensive testing of McAB-HRPO conjugates it became obvious that sodium borohydride, used as a reducing agent, did not result in the production of stable conjugates without enzyme pretreatment with fluorodinitrobenzene (FDNB). Ascorbic acid or ethanolamine used as the reducing agent, resulted in McAB-HRPO conjugates which were stable for periods of ten months or more when stored filter sterilized at 4 degrees C.

Dysplasia in colorectal adenomas related to the presence of O-acylated sialic mucin and to morphometric measurements.

In a study of 41 surgically removed colorectal adenomas of unselected size, the dysplasia was graded as mild in 9, as moderate in 22, and severe in 10 cases. Twenty-four of the lesions were classified as tubular adenomas and 17 as tubulo-villous. The presence of O-acylated sialic mucin was for each of the adenomas assessed as normal (19 cases), intermediate (13 cases) or absent (9 cases). The absence of O-acylated mucin was in the present study found to be highly predictive for severe dysplasia (89%). The morphometric measurements of epithelial structures showed that nuclear shape factor and nuclear stratification index were most significantly related to the grades of dysplasia. In a multivariate analysis, the grading of O-cylated sialic mucin most significantly contributed to the grade of dysplasia, and to a lesser degree, nuclear shape factor and nuclear stratification index. The presence of villous elements and the size of the adenomas had no independent influence on the grade of dysplasia in this statistical analysis.

Congenital heart malformations in Jutland, Denmark: a three year necropsy study in children aged 0-14 years. Epidemiology and classification according to sequential segmental analysis.

The use of sequential segmental analysis for describing congenital heart malformations was the method of assessment used in a prospective necropsy study covering Jutland, a well defined geographical and demographic area of Denmark. The study group was 1,154 children of whom 261 (22.6%) had a congenital heart malformation. The most common malformations were ductus arteriosus and ventricular septal defect and there were 77 cases in which connections between chambers or between chambers and great arteries were anomalous (68 liveborn; 37 male and 31 female: nine stillborn; two male and seven female). No difference in sex distribution or seasonal variation was found between those with congenital heart disease and those without. Extracardiac malformations and chromosomal abnormalities were more often seen in children with congenital heart malformation than those without (30.3% vs 16.6%). The sequential segmental analysis is a logical and precise way of describing congenital heart malformations and it should be routinely used to classify cases of congenital heart malformation.

The reliability of transabdominal ultrasound scanning in the determination of prostatic volume. An autopsy study.

The prostatic volume as determined by transabdominal ultrasound scanning using the ellipsoid method was compared to the prostatic volume determined by autopsy in 26 post-mortem men. The regression line was nearly optimal (Y = 1.06 chi + 0.57) although it seems that ultrasound underestimates prostatic volume slightly. The parametrical correlation coefficient was 0.86 and the non-parametrical Spearmans rho was 0.76. It is concluded that transabdominal ultrasound scanning has a definite value in the assessment of prostatic size.
