Blastic transformation of essential thrombocythemia. A case report.

A case of blastic transformation of essential thrombocythemia (ET) is reported. A 69-year-old male was first admitted to hospital because of fever in February, 1982. He was diagnosed as having ET because of marked thrombocytosis (205.5 x 10(10)/1), absence of erythrocytosis, absence of splenomegaly, normal karyotype and no increment of blasts in the bone marrow, and normal levels of neutrophil alkaline phosphatase, vitamin B12 and folate. He was treated with busulfan, and subsequently his platelet count was well controlled for about five years. At the second admission, blasts were present in the peripheral blood, and later accounted for 49% of the total leukocyte count. Histological examination of a bone biopsy specimen showed homogeneous proliferation of blastic cells and slight reticulin fibrosis. At autopsy, the degree of bone marrow fibrosis had increased. This was considered to be a very rare case of ET with blastic transformation in the terminal phase.

Recombinant interleukin-1 triggers the increase of circulating fibronectin level in rats.

Interleukin-1 (IL-1) is one of the mediators responsible for the acute phase protein response during inflammation. We evaluated the effect of recombinant IL-1 alpha given intra-abdominally on rat circulating fibronectin (Fn) levels. Circulating Fn level, sampled between 3 h and 7 days after injection, was maximum at 24-48 h in Wistar rats treated with IL-1 at doses of 10 U and 100 U/100 g body weight (BW), respectively. Circulating Fn level in rats injected with 100 U/100 g BW IL-1 was higher than that of rats given 10 U/100 g BW IL-1. Circulating Fn level peaked at 24 h, whereas albumin levels decreased by 10% at this time. When frozen tissue sections of rat liver were examined using anti-Fn antibodies, the immuno stainings were denser in IL-1-treated animals than in controls. These findings suggest that IL-1 triggers plasma Fn production by the liver.

[Case of the maxillofacial defect patient. Prosthetic treatment after reconstruction of the palate and face using deltopectral flap].

The patient, a 72-year-old edentulous man, had an extensive maxillofacial defect caused by treatment of the maxillary cartinoma. As his left maxillary and zygomatic bone, a portion of the soft palate, orbital cavity contents and soft tissues of the cheek had been excised, his face was ugly, and his speech and eating were extremely poor. For this patient, first of all, plastic surgeons reconstructed the cheek and palate using deltopectral flap. Secondly, dentists prepared a facial prosthesis and a set of complete dentures. Four months after prosthetic treatment, plastic surgeons corrected the left corner of the mouth using temporal muscle flap and fascia lata, and strengthened infraorbial region of deltopectral flap by grafting the left 5th rib. Lastly, dentists remade the facial prosthesis. As a result, the patient had great improvement in his face, speech and eating. Periodic examination of the surgical site can be practiced through the remaining facial defect. The advantages of the surgical reconstruction in this case were as follows. (1) As reconstruction of the cheek narrowed a facial defect, it was easy to improve patient's appearance by a small facial prosthesis. (2) The closure of the palate was effective to improve hypernasal speech, to prevent fluid leakage into the nasal cavity and to improve the retention and stability of the upper complete denture.

Effects of deoxyspergualin on hematopoiesis: studies of murine hematopoietic progenitor cell and peripheral blood cell levels.

The effect of a novel immunosuppressive agent, deoxyspergualin (DSG), on hematopoiesis in mice was studied with measurements of peripheral blood counts and assays of granulocyte-monocyte colony-forming cells (cfu-C) and spleen colony-forming cells (cfu-S) in bone marrow, during and after successive intraperitoneal administration of DSG. When DSG was administered at a strong immunosuppressive dose of 6.25 mg/kg daily for 15 days, mice developed significantly decreased peripheral blood counts and decreased bone marrow cells (BMC) during administration. After the completion of DSG administration a marked rebound in leukocytosis was observed and BMC returned to normal. In contrast, total cfu-C in the femur significantly increased during the DSG administration, and subsequently returned to normal. Moreover, total cfu-S in the femur were normally sustained in contrast with a decrease of BMC during the DSG administration. These findings suggest that DSG does not show generalized overt cytotoxicity against hematopoietic stem cells and freezes the ability of the stem cell in the proliferation or the differentiation.

In vitro immunosuppressive properties of spergualins to murine T cell response.

Deoxyspergualin has strong immunosuppressive activity in animals. However, it shows less in vitro immunosuppressive activity at the therapeutic concentration used for in vivo administration. Recently, we reported that there are some technical problems with in vitro experiments. In this report, the effects of spergualins were examined under in vitro conditions which excluded these problems, and compared with cyclosporine A (CYA). Spergualins have suppressive effects on mixed lymphocyte response (MLR) and cytotoxic T lymphocyte induction. Furthermore, interleukin-2 (IL-2) induced proliferation of concanavalin A blasts and CTLL-2 were inhibited at low concentration. However, spergualins have little effect in the early stage of MLR or the mitogen response. These results suggest that spergualins act on the proliferation and differentiation of T cells which respond to growth factors, such as IL-2.

[Changes in bone mineral content and bone metabolism related to menopause].

Although the phenomenon of postmenopausal osteoporosis has attracted attention, there have been no parameter-matched studies of pre- and postmenopausal difference in metacarpal bone mineral status and bone metabolism. Therefore the authors compared metacarpal bone loss in a group of 41 postmenopausal volunteers (average postmenopausal duration: 39.3 months) and 28 age- and body size-matched controls (age 50-54), using the MD method. Perimenopausal changes in bone metabolism were also examined in these groups. The result revealed that the reduction in the metacarpal bone mineral status due to menopause occurs mainly in the bone mineral density such as GSmin and sigma GS/D. The reduction rate was the highest for the index of GSmin, corresponding to 5.1% per year. Serum levels of Alp and osteocalcin were significantly higher and the U-Ca/cr ratio tended to be higher in the postmenopausal subjects than in controls. The serum levels of calcium regulating hormones such as PTH and calcitonin in these two groups did not differ significantly. However the estrone and the estradiol levels, were significantly lower in postmenopausal subjects. Significant correlations were found between Alp and the postmenopausal period (r = 0.4684), and between the U-Hpr/cr ratio and postmenopausal period (r = -0.5332). It therefore appeared that estradiol is mainly related to postmenopausal bone metabolism causing insufficient suppression of bone resorption. However, calcium regulating hormones appeared to be not directly related to the postmenopausal bone loss mechanism. Bone turnover decreased with the postmenopausal period.

[Assessment of osteopenia following oophorectomy by quantitative computed tomography and its relation with Ca-regulating hormones].

In order to determine the pathogenesis of post-oophorectomy osteopenia, we investigated the bone mineral status and biochemical parameters, and identified high-turnover bone metabolism. The present study was undertaken to determine whether there is a decrease in bone mineral content of peripheral bone or axial bone and cortical bone or trabecular bone using the microdensitometry method and a modified quantitative computed tomography method in 40 [24 premenopausal females (controls) and 16 premenopausal females who underwent oophorectomy]. We also investigated what biochemical and endocrinological parameters are changed, for example, Ca-regulating hormones and sex steroids, etc. As a result, it found that the reduction in bone mineral content due to oophorectomy occurs mainly in the axial bone, especially in the trabecular bone. It was considered that oophorectomy causes high-turnover osteopenia, in which bone formation is promoted but is outweighed by bone resorption, resulting in an overall decrease in bone mineral content. It also seemed that E2 is significantly related to bone metabolism, causing insufficient suppression of bone resorption. However, Ca-regulating hormones appeared not to be directly related to bone metabolism during the period (average: 39.6 months) after oophorectomy.

[Effects of acute hypoxia on cardiovascular dynamics, myocardial metabolism and ECG in dogs].

In pentobarbital-pancuronium anesthetized open chest dogs, left ventricular pressure (LVP), first derivative of LVP (dp/dt) and left ventricular end-diastolic pressure (LVEDP) were measured by a micromanometer tipped catheter, stroke volume and cardiac output (CO) by an electromagnetic flow meter and coronary sinus blood flow (CSBF), by a thermal dilution catheter, respectively. Mean arterial pressure (mAP), mean pulmonary arterial pressure (mPAP), lead II of ECG and esophageal temperature were monitored. Blood gas, catecholamines and lactate of arterial blood and coronary sinus blood were measured and myocardial lactate extraction ratio (MCL) was calculated. As an index of contraction, maximum positive dp/dt (+dp/dt max), and as an index of relaxation, the time constant (T) of isovolumic left ventricular pressure fall were employed, respectively. Acute hypoxia was induced by the inhalation of a 5% O2-95% N2 gas mixture and arterial blood oxygen pressure was maintained between 15 and 20 mmHg. Along with the advancement of hypoxia, +dp/dt max, CO and mAP gradually increased and peaked in 5 minutes. Thereafter, these parameters began to decrease and CO fell to zero in 20 minutes. mPAP and HR slowly increased and peaked in 9 and in 12 minutes, respectively. T was progressively prolonged throughout the hypoxia. CSBF/CO ratio was increased and stayed at a high level even after CO began to decrease. MCL decreased and changed from positive values to negative ones in 5 minutes. Epinephrine and norephinephrine concentrations slowly increased and peaked in 16 minutes. A gradual decrease in the R wave, shortening of the R-R interval and lengthening of the QTc interval were observed in ECG. This study indicated that the left ventricular relaxing function was impaired earlier than contracting function by acute hypoxia and that conversion of the augmented left ventricular contractile state to the depressed one coincided with conversion of myocardial lactate extraction to its production.

[Effects of alfacalcidol (1 alpha-OH-D3) on post oophorectomy osteopenia as measured by microdensitometry].

The present study included 24 relatively young patients (48.4 years on average) who had decreased bone mineral levels after oophorectomy (severity scores of 4 or above as judged by the MD method), and the efficacy of active vitamin D3 (D3) was assessed according to the mean percentage of change in MD indices. The patients were randomly divided into two groups of 12 cases each, the D3-treated group and the non-treated group. The D3 treatment was given in the form of 1 microgram/day alfacalcidol for 9.67 months on average. Before and after treatment, the MD indices and serum Al-p, calcium and potassium were measured. The non-treated group was observed for 9.92 months on average, and the same parameters were measured before and after the observation period. There were no significant intergroup differences with regard to age, height, body weight, duration after oophorectomy, severity of decreased bone mineral levels and other background factors. In the treated group, MCI and d tended to increase and to decrease, respectively, GSmin and sigma GS/D also tended to increase and GSmax tended to increase (p less than 0.10). In the non-treated group, MCI decreased and d tended to increase, GSmin and GSmax tended to decrease, and sigma GS/D decreased (p less than 0.05). In the intergroup comparison, GSmax increased in the treated group (p less than 0.05) and sigma GS/D also tended to increase more in the treated group (p less than 0.10).(ABSTRACT TRUNCATED AT 250 WORDS)

[Radiotherapy of esophageal cancer: the local effect and prognosis].

The correlation between the local effect and the prognosis of 124 esophageal carcinomas treated curatively with irradiation has been examined. The local effect at 40 Gy was found to correlate well with the prognosis. A high local control rate can be expected in cases of a superficial type, a tumorous type and in some cases of an ulcerative type, even though no change was seen in Ba study at 40 Gy. In well advanced ulcerative cases, in infiltrative cases, and in cases that show no change at 40 Gy, the radiation method should be altered positively after 40 Gy to achieve better local control because metastases are relatively few.

Antigen-dependent and -independent proliferative T-cell populations in the peritoneal exudate cells of immunized mice.

In vitro proliferative responses of T lymphocytes in the peritoneal exudate cells of C3H/HeN(Iak) mice immunized with horse red blood cells (HRBC) were examined by determining the uptake of tritiated thymidine [( 3H]TdR) into the cells. Although the cells showed a basal proliferative response in the absence of antigen, addition of specific antigen increased the response sharply. Both the basal response and that stimulated by antigen disappeared if the cells had been previously treated with complement and anti-Iak antibody (AIak), anti-MAC-1 antibody (AMAC-1) or anti-Thy-1 antibody, but not anti-Ig antibody. Adding macrophages prepared from the peritoneal exudate cells of normal mice to AMAC-1-treated T-cell (i.e. Iak+ plus Iak- T-cell) cultures restored both of the responses, while adding them to AIak-treated T cells (i.e. Iak- T cells) only restored the antigen-specific response. These findings indicate that the basal proliferation is due to or dependent on the proliferation of Iak+ T cells, while the antigen-specific response is mainly due to Iak- T cells. Furthermore, interleukin (IL)-2 production was also examined. Immune T cells produced some IL-2 in the absence of antigen. The production by AMAC-1- or AIak-treated cells was impaired, but adding macrophages to the AMAC-1-treated cell cultures restored production. This result also suggests that the mode of IL-2 production by the Iak+ and Iak- cells is different. Proliferative responses of AMAC-1- or AIak-treated T cells to IL-2 were also examined. The AIak-treated cells dose-dependently responded to IL-2, while the response of Iak+ cells, which could be estimated by subtracting the response of AIak-treated cells from that of AMAC-1-treated cells, did not depend on the doses. These results indicate that in the immune peritoneal exudate the Iak+ T cells are functionally different from Iak- T cells.

[Mechanism of the decrease in metacarpal bone mineral content due to oophorectomy].

In order to determine the pathogenesis and pathophysiology of post-oophorectomized osteopenia, we have investigated the bone mineral status and the serum levels of calcium regulating hormones in 250 females. The bone mineral status was measured in 250 [173 pre-menopausal females (controls) and 77 who received oophorectomy] females by microdensitometry in the metacarpal bone. Among these subjects, in some cases who were selected randomly, the serum levels of Alp, Ca, Pi, M-PTH, calcitonin (CT), bone gla protein (BGP), estrone, estradiol and delta 4-androstenedione, and the urinary excretion of Ca and hydroxyproline (OH-pro) as a function of the creatinine ratio were measured. The mineralization indices of bone such as GSmax, GSmin, and sigma GS/D in oophorectomized patients were significantly less than those in controls at ages up to 45 years. On the other hand, the index of periosteal bone resorption (MCI) in the oophorectomized group was less than that in controls at ages up to 50 years. Serum levels of Alp, Ca, Pi and BGP were higher in oophorectomized subjects than in controls. Despite these facts, the serum levels of calcium regulating hormones such as M-PTH and CT in these two groups were significantly different. Urinary Ca/cr and OH-pro/cr ratios were significantly higher in oophorectomized subjects at ages 40-44 than those in age matched controls. These data clearly indicate that oophorectomy causes high turn-over osteopenia.
