[A double blind clinical trial of the alpha-adrenergic blocker, Ea-0643 tablet, in the treatment of neurogenic bladder].

The clinical effectiveness, safety and usefullness of the alpha-adrenergic blocker, Ea-0643 tablet, for the treatment of the neurogenic bladder were evaluated by the double blind test method. A total of 61 patients was treated. Five of them were excluded or dropped out. Eighteen patients (group-A) received a 1.0 mg tablet of Ea-0643, 20 patients (group-B) received a 0.5 mg tablet of Ea-0643 and 17 patients (group-C) received a placebo tablet. Each tablet was administered orally three times a day for two weeks. There was no significant difference in the background factors between the three groups. The rate of effectiveness for subjective symptoms was 22.2% in group-A, 30.0% in group-B and 35.3% in group-C. There was no significant difference between the three groups. The rate of effectiveness for residual urine, judged by decrease in value of residual urine rate after treatment of less than 25%, was 55.6% in group-A, 35.0% in group-B and 17.6% in group-C. The rate of effectiveness in group-A was significantly higher than that in group-C. The mean value of net decreased residual urine rate after treatment in group-A was significantly higher than that in group-C. Decrease of maximal urethral pressure by more than 5 cm H2O was seen in 44.4% of group-A, 35.0% of group-B and 41.2% of group-C. There was no significant difference between the three groups. The rate of improvement for flow rate was 66.7% in group-A, 46.2% in group-B and 36.4% in group-C.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical application of thyrotropin-releasing hormone to neurogenic bladder caused by olivo-ponto-cerebellar atrophy].

The effect of thyrotropin-releasing hormone (TRH) on vesicourethral dysfunction in two patients with olivo-ponto-cerebellar atrophy was studied by urodynamic examination. The two patients had difficulty of urination with 200 ml of residual urine. Cystometrogram and electromyogram showed autonomous contraction without detrusor-sphincter dyssynergia. After administration of TRH, the symptom and volume of residual urine was not improved, autonomous contraction was not changed, while prostatic urethral pressure in urethral pressure profile was significantly increased in both patients.

[A case of fibromuscular dysplasia with middle cerebral aneurysm].

A case of cervico-arterial fibromuscular dysplasia (FMD) associated with an aneurysm of middle cerebral artery was reported in a women aged 54. On December 12, 1983, she was admitted to our hospital, who had been diagnosed as head injury (cerebral concussion) and a cerebral aneurysm. On admission, her general condition was favorable, her conscious was clear and neurological examination was normal. The four vessels cerebral angiograms showed right M1M2 aneurysm and FMD which was characterized as typical changes of form "string of beads" in the extracranial internal carotid and vertebral arteries of both sides. Clipping of aneurysm was performed and favorable post-surgical progress was observed. FMD of the cervico-cephalic arteries has been seen rarely in Japanese, but the detection frequency of FMD from cerebral angiography is 0.3-0.9% and the frequency of complication with aneurysm is high at 13-51% on literature. The histologic similarities between intracranial aneurysms and the aneurysmal dilatations of FMD are the absence of the muscular media and a defective interval elastic lamella. This paper suggested strong relationship between FMD and cerebral aneurysm.

[Traumatic mitral insufficiency: a case report].

A 25-year-old man was admitted to Juntendo University Hospital with chief complaints of nocturnal dyspnea and shortness of breath on Sept. 22, 1983. He had no history of rheumatic fever or bacterial endocarditis. He was violently kicked in the chest while practicing Shorinji-Kempo (Karate) in July 1977. His heart murmur was first noticed in April 1978, but he was asymptomatic for six years after the accident until transient nocturnal dyspnea developed January 1983. Physical examination on admission revealed a grade 4/6 apical holosystolic murmur, a markedly accentuated third sound, and hepatomegaly of two finger breadth in the right midclavicular line. An electrocardiogram revealed sinus tachycardia (100/min), left atrial overload and left ventricular hypertrophy. Chest radiography showed slight cardiac enlargement with a cardiothoracic ratio of 55%, and slightly increased pulmonary vascular markings. Two-dimensional echocardiography showed a markedly prolapsed posterior mitral leaflet and fluttering in diastole. Cardiac catheterization showed elevated pressure of pulmonary capillaries (a: 16, v: 30, mean: 19 mmHg), the pulmonary artery and the right ventricle. Left ventriculography revealed grade four (Sellers) mitral regurgitation. Mitral valve replacement was performed on October 13, 1983. A chorda tendinae supporting the posterior leaflet of the mitral valve was found to be ruptured just above its origin from the posterior papillary muscle. Histological examination of the resected valve showed increased spongiosa tissue which mimicked so-called myxomatous changes, but it seemed preferable to interpret this as a "secondary change due to increased hemodynamic stress" rather than the "primary change".

[A study on transfer of ceftizoxime into prostatic and vesical tissues].

Prostatic and vesical tissue levels of Ceftizoxime (CZX) were determined in 21 patients with prostatic hyperplasia after the intravenous administration of 1 g of CZX. Serum concentration on prostatic surgery continued to be 1.3 fold higher than that of healthy adults. Prostatic tissue levels attained a peak of 40.7 +/- 1.6 micrograms/g (mean +/- SE) at 30 minutes after administration and vesical tissue levels attained a peak of 72.4 +/- 24.8 micrograms/g at 30 minutes. The mean value was 21.0 +/- 6.4 micrograms/g in prostatic tissue after administration for 120 minutes with the tissue/serum ratio of 0.91. Prostatic tissue level in glandular dominant were slightly higher than those of myoglandular hyperplasia (P less than 0.1). Judging from its favorable transfer into the prostatic tissue and MIC against clinically isolated E. coli, Proteus sp. and Klebsiella, CZX seemed to be clinically useful in the treatment of prostatic infection.

[Clinical application of eperisone hydrochloride to neurogenic bladder].

Eperisone hydrochloride, a striated muscle relaxant, was tried on 20 patients suffering from neurogenic bladder due to various reasons. Clinical effectiveness was evaluated by subjective symptoms, residual urine, cystometry and electromyography of the external urethral sphincter. Difficulty of urination was improved in 9 of the 20 patients. Decrease of residual urine by more than 40% was seen in 10% of the 20 patients. No significant changes were observed in the cystometrograms. On the electromyograph, external sphincter spasms were seen in 6 of the 8 patients, and partially improved in 2 patients. No marked side effects were observed.

Quantitation of the epsilon-(gamma-glutamyl)lysine cross-link using a high-speed amino acid analyzer without purification of the dipeptide. Application to enzymatic digested mixtures of keratin and the membranous fraction of human stratum corneum.

epsilon-(gamma-Glutamyl)lysine in an enzymically digested mixture of keratin and the membranous fraction of human stratum corneum was directly quantitated using a high-speed amino acid analyzer without purification of the dipeptide. The analytical conditions were improved so that epsilon-(gamma-glutamyl)lysine clearly separated from other amino acids and eluted directly after tyrosine. The enzymatically digested mixtures were filtered through an Ultra Free membrane and deammoniated before analysis. By our present method, keratin and the membranous fraction of human stratum corneum were analyzed and 5.8 and 43.5 nmol/mg of epsilon-(gamma-glutamyl)lysine, respectively, were detected.

Comparative study of prostaglandin I2 (PG I2) and thromboxane A2 (TXA2) synthetase inhibitor (OKY-1581) on hemodynamics in pregnant dogs.

Effects of intravenous injections of prostaglandin I2 (PG I2) and thromboxane A2 (TXA2) synthetase inhibitor (OKY-1581) on mean arterial pressure (MAP), cardiac output (CO), heart rate (HR), renal blood flow (RBF), and uterine blood flow (UBF) were investigated in the anesthetized pregnant dog. A dose-dependent fall in MAP was observed with these two agents. HR was almost unchanged by both PG I2 and TXA2 synthetase inhibitor. PG I2 and TXA2 synthetase inhibitor produced increases in CO and RBF in a dose-dependent manner. PG I2 caused a dose-dependent decrease in UBF, while TXA2 synthetase inhibitor was followed by an insignificant decrease in UBF. PG I2 and TXA2 synthetase inhibitor did not induce any increase in intrauterine pressure (IUP). The possibility of treatment of pregnancy-induced hypertension with PG I2 or TXA2 synthetase inhibitor is discussed.

[Echocardiography of ischemic heart disease simulating dilated cardiomyopathy, with special reference to abnormal wall movement on the short-axis].

The purpose of this study was to determine whether two-dimensional echocardiography (2DE) can differentiate ischemic myocardial disease (IMD) from dilated cardiomyopathy (DCM). The subjects consisted of six cases of IMD which showed left ventricular dilatation (LVDd greater than or equal to 60 mm) and diffuse abnormality of wall motion, but did not show obvious localized myocardial infarction or left ventricular aneurysm on 2DE, and 16 cases of DCM. Two cases of IMD had previous myocardial infarction, and five cases of DCM had cardiomegaly following myocarditis. A short-axis image of the left ventricle was recorded at the chordal and the papillary muscle levels. Each image was divided into 4 segments, which were comprised of the septum, anterior wall, lateral (posterolateral) wall, and posterior (posteromedial) wall. Regional wall motion abnormality with reference to systolic thickening was analyzed qualitatively in each segment. The results were as follows: In ECG findings in IMD group, only one case showed abnormal Q waves and five cases showed left ventricular hypertrophy (LVH) similar to intraventricular conduction defect. On the other hand, in DCM group seven cases showed abnormal Q waves and five cases showed LVH. Two cases of IMD had two-vessel disease and four three-vessel disease, respectively. Left ventricular ejection fraction by cine-angiography ranged from 0.10 to 0.39 (mean 0.24) in IMD group and from 0.22 to 0.42 (mean 0.36) in DCM group. Mean LVDd showed no significant difference between these two groups. Five cases of DCM showed marked left ventricular dilatation (LVDd greater than or equal to 75 mm), but there were no such cases in IMD group. B-B' step was recognized in only one case of IMD, though it was present in eight cases in DCM. In regional wall motion, incidence of asynergy such as akinesis or dyskinesis was higher in IMD group than in DCM group. Left ventricular asynergy was more serious in the posteromedial wall than the posterolateral wall at the same image in five cases of IMD. However, in 12 cases of DCM, the degree of asynergy was equal at the both walls. In conclusion, it is recommended to examine echocardiographically the extent of severe asynergy in the posteromedial and posterolateral walls in order to differentiate IMD from DCM.

[Traumatic cerebral aneurysm: 94 cases from the literature and 5 cases observed by the authors].

Traumatic intracranial aneurysm is rare lesion which is complicated severe head injury. In spite of this rarity, this aneurysm is well known by the fact of high probability of rupture, and high mortality rate. Ninety-four cases of traumatic aneurysms, including 5 cases of our own are discussed. In 94 cases, 83 cases (88.3%) are located at the middle cerebral arteries and/or the anterior cerebral arteries. There are only 2 cases (2.1%) located at the posterior fossa. All of these 94 aneurysms are situated at the peripheral arteries and/or cortical branches, in the distance of Willis' arterial ring. There are 8 cases which lead to be cured spontaneously without any surgical treatment. It was reported by some authors that the traumatic intracranial aneurysms expecting spontaneous healing had some characteristics by the cerebral angiography, such as irregular shapes, and unclear connections from parent arteries. However, these characteristics are sometimes common in the traumatic intracranial aneurysms. Therefore, it is actually difficult to differentiate spontaneous healing aneurysms from deteriorating ones. As for the prognosis of these 79 cases which are able to be examined, the mortality rate of non-surgical group is 4 times higher compared with that of the surgical group, that is the former 40%, and the latter 11%, respectively. According to these facts, authors concluded that the principle therapy of traumatic intracranial aneurysm is surgical treatment.

Isolation and characterization of horny cell membrane.

The cornified cell membrane and thickened marginal band were obtained from human stratum corneum by a new, faster procedure involving extraction, tripsinization, and density gradient centrifugation. Observations revealed a fine membranous structure with a thickened inner surface of approximately 150 A (marginal band). Each of the membranous samples contained a level of half-cystines markedly higher in amount than that of earlier findings by Matotsy and Matotsy [5]. The samples also contained high amounts of glutamic acid and lysine residues many of which constitute the epsilon-(gamma-glutamyl) lysine cross-linkages of the cell membrane. This study presents the simple isolation procedure and its results highlight the high content of half-cystines in the basic membrane fraction. In addition, the study reveals the importance of S-S cross-linkages during the formation of the cell membrane.
