Full-thickness reconstruction of cheek defect involving oral commissure with forearm tendinocutaneous flap.

An 82-year-old man underwent full-thickness reconstruction of the cheek for a defect of the oral commissure resulting from a T3N1M0 squamous cell carcinoma. He had previously had both radiotherapy and chemotherapy. A tendinocutaneous flap from the forearm incorporating palmaris longus was used for static reconstruction. The vermilion was then reconstructed with rotation flaps from the contralateral vermilion. Oral function was restored, and he had no problems with drooling or speech.

Morphology of the mandibular fossa and the articular eminence in temporomandibular joints with anterior disk displacement.

The depth, length, and depth:length ratio of the mandibular fossa, and the angles of the posterior slope of the articular eminence were measured for 91 temporomandibular joints (TMJ) with anterior disk displacement (ADD) in 79 female patients with TMJ dysfunction. As a control, 48 joints in 24 females without TMJ dysfunction were similarly measured. One of the angeles of the posterior slope of the articular eminence was larger for joints with ADD than for the controls (P < 0.01, Student's t-test). There were no other statistically significant differences between the two groups. Among the 91 joints with ADD, there was no statistically significant difference in any variable between joints with ADD with reduction (n = 46) and those with ADD without reduction (n = 45); likewise, there was no association between the magnitude of angle of the posterior slope of the articular eminence and overjet, overbite, canine relationship, or molar relationship.

Immuno-inflammatory responses in the tissue adjacent to titanium miniplates used in the treatment of mandibular fractures.

The immuno-inflammatory responses to titanium miniplates used in the treatment of mandibular fractures were studied immunohistochemically at light and electron microscope levels. Titanium miniplates were stably situated on the cortical bone surface. In the soft tissue adjacent to the surface of titanium miniplates, double layered connective tissue was observed, which consisted of dense fibrous connective tissue, and relatively loos connective tissue contained proliferated blood vessels with hypertrophied endothelial cells. These vascular endothelial cells expressed HLA-DR, CD54 and CD62P antigens. In some cases they were CD62Epositive. CD68+ and CD11c+ round or spindle-shaped macrophages had infiltrated around the small vessels. Fine titanium particles were observed in the cytoplasm of these macrophages. Both CD4+ and CD8+ T lymphocytes had also infiltrated around venules in some cases. They were CD4+ T lymphocyte-dominant. Immunoelectron microscopically, CD68+ and CD11c+ macrophages contained titanium particles in the lysosomes. Most of the macrophages showed varying degrees of degenerative change. The presence of titanium was confirmed by energy-dispersive X-ray analysis.

Electromyographic and kinesiographic study in patients with nonreducing disk displacement of the temporomandibular joint.

OBJECTIVE: The purpose of this study was to clarify electromyographic and mandibular kinesiographic properties of the chewing movements in patients with unilaterally painful nonreducing disk displacement of the temporomandibular joint. STUDY DESIGN: Chewing movement in 50 female patients was evaluated by electromyograph and mandibular kinesiograph, and the results were compared with those in 31 normal controls. RESULTS: In the analysis by electromyograph, some differences between patients and controls were found. In the analysis by mandibular kinesiograph, chewing movement showed deviation to the chewing side in the TMJ-affected-side chewing but did not show deviation in the TMJ-unaffected-side chewing in the horizontal plane. The maximal anteroposterior width between opening and closing paths in the sagittal plane was smaller in the experimental subjects. CONCLUSION: These differences between patients and controls may be helpful to diagnosis for painful nonreducing disk displacement of the temporomandibular joint.

Effects of acyclovir and vidarabin 5'-monophosphate on anti-duck hepatitis B virus in an in vitro culture system.

Various anti-viral agents, e.g., interferon, have recently been used for the treatment of viral hepatitis. In the present study, duck hepatitis B virus (DHBV) was cultured in vitro and the anti-DHBV effects of acyclovir (ACV) and vidarabin 5'-monophosphate (VMP) were studied. The portal perfusion method was applied to the livers of 7-day-old white ducks weighing 100 g, bred in Japan, and hepatocytes were infected with DHBV in vitro. Duck hepatocytes infected with DHBV were cultured in medium containing ACV or VMP, and the anti-DHBV effects of these drugs were assessed by determining DHBV-DNA and duck hepatitis B surface antigen in the medium. Both ACV and VMP had anti-DHBV effects when used immediately after infection; however, both drugs were ineffective in hepatocytes obtained from a DHBV carrier duck. In conclusion, the anti-DHBV effects of these drugs were very limited. However, this culture system appears to be useful for studies of hepatitis virus and anti-viral drugs.

Intraoral reconstruction with innervated forearm flap: a comparison of sensibility and reinnervation in innervated versus noninnervated forearm flap.

OBJECTIVE: To evaluate the cutaneous sensibility and sensory reinnervation in patients who underwent intraoral reconstruction with an innervated or noninnervated forearm flap. STUDY DESIGN: Results of the use of innervated forearm flaps in oral reconstruction was compared with the use of noninnervated flaps. The evaluation of sensibility and reinnervation comprised clinical sensibility tests and immunohistochemical investigation of postoperative biopsy specimens against S-100 and neurofilament. RESULTS: The innervated flaps (4 patients) provided earlier and qualitatively better recovery of sensation than the noninnervated flaps (9 patients). Immunohistochemical investigation revealed the existence of a larger number of regularly arranged sensory nerve fibers in the cutaneous tissue of the innervated flaps than in the noninnervated flaps. Examination with an electron microscope found the structure of these nerve fibers to be well preserved in the innervated flaps, whereas nerve fibers in the noninnervated flaps were degenerative. CONCLUSION: These findings suggest (1) that the innervated flaps are superior to the noninnervated flaps not only for the repair of defects but also for the restoration of function and (2) that the innervated flaps contribute to the improvement of the quality of life for patients.

Management of nonreducing temporomandibular joint disk displacement. Evaluation of three treatments.

OBJECTIVE: The purpose of this study is to evaluate the responses of patients with anterior disk displacement without reduction to natural course, stabilization splint, and surgery. STUDY DESIGN: Thirty-one patients refused any treatment (natural course group), 20 patients were treated with a stabilization splint (stabilization splint group), and 24 patients who had not responded to nonsurgical treatment for a mean period of 19.0 months underwent surgery (surgical group). The success rate was evaluated in each of the three groups. RESULTS: The success rate was 41.9% in the natural course group, 55.0% in the stabilization splint group, and 76.9% in the surgical group. CONCLUSIONS: No statistically significant benefit from treatment with a stabilization splint over no treatment was identified although both groups of patients experienced alleviation of discomfort. The patients who had not responded to nonsurgical treatment for a mean period of .19 months benefited from surgery.

Systemic lupus erythematosus: a case-control epidemiologic study in Japan.

BACKGROUND: Systemic lupus erythematosus (SLE) is designated by the Japanese government as one of the intractable diseases and all patients, who suffer from these diseases, are registered to get financial aid for treatment. Using newly registered SLE patients, a case-control study was conducted to evaluate potential risk factors. METHODS: Two-hundred and eighty-two women SLE patients, newly registered to receive financial aid for treatment, and 292 randomly selected health examination participants at public health centers (controls) were surveyed from April 1988 through March 1990. By means of a self-administered questionnaire, data concerning demographic variables, smoking and drinking habits, past medical and reproductive history, and family history were collected. RESULTS: Based on unconditional logistic regression analysis, the risk of SLE was significantly increased for current smokers (age-adjusted odds ratio (OR) = 2.31, 95% confidence interval (CI) 1.34-3.97). Alcohol and milk intake were inversely associated with risk. Family histories of asthma and collagen diseases, including SLE, were associated with significantly elevated risk of SLE (OR = 2.07, 95% CI 1.14-3.77; OR = 5.20, 95% CI 1.08-24.95, respectively). Regarding reproductive function, women with menarche at age 15 or later had significantly higher risk than those, who started menstruating before age 12 (OR = 3.82, 95% CI 1.66-8.81 for menarche at > 15 years and OR = 2.90, 95% CI 1.14-7.39 for menarche at > or = 16y). CONCLUSIONS: Our study suggests several risk factors, including smoking, family history, and reproductive history that may increase the risk of SLE.

Fixation of mandibular body fracture with combination of one-piece cast splint and miniplate.

A method for fixation of mandibular body fracture within the dental arch using a combination of a rigid one-piece cast splint and a pliable miniplate screwed at the inferior border of the mandible was developed with the highest priority placed on restoration of occlusion. This paper presents the results obtained in 36 patients treated by this method. The postoperative occlusion was analyzed using the T-scan system in 20 fully dentate patients without major caries. The data obtained in the patients were well comparable to those in normal subjects.

Experimental methods for immunization and challenge in antigenicity studies in guinea pigs.

Optimal experimental methods for antigenicity studies in guinea pigs were investigated on: (1) the effects of different immunizing methods using complete or incomplete Freund's adjuvants (CFA or IFA), and various injection sites, the number of immunizations, the immunizing doses, and the immunizing periods, (2) the relationship between the severity of active systemic anaphylaxis (ASA) reactions and passive cutaneous anaphylaxis (PCA) titers, (3) positive control for oral administration, and (4) the effects of incubation mixture of drug and serum protein as the challenge for the ASA assay. The following results provided useful information for designing more appropriate methods for antigenicity studies: (1) The optimal immunization method for benzylpenicillin (PcG), cephaloridine, 2,4,6-trinitrobenzene sulfonic acid and adriamycin, which were selected as positive controls for low molecular medicines in this experiment, involved subcutaneous administration of 1 ml of a test substance in CFA (1st immunization) or IFA (2nd and 3rd immunizations) at two doses, 1 and 10 mg/animal, 3 times at 2-week intervals on the back of a guinea pig. Blood collection for PCA assay was needed 2 weeks after the last immunization, and ASA assay, 1 or 2 days after the blood collection. (2) The insensitivity of ASA reactions in bovine serum albumin-immunized animals with very high PCA titers was overcome by increasing the challenge antigen dose from 1 to 10 mg/animal. (3) Most animals administered lysozyme at 0.1, 1 or 10 mg/animal by gavage for 2 weeks or more showed ASA and PCA reactions. (4) Incubation of a mixture of 20 mg/ml of PcG and 2 mg/ml of guinea pig serum albumin for 4 hr was the most effective as challenge for the induction of ASA reaction in PcG-immunized guinea pigs.

A case-control study of ulcerative colitis in Japan.

To determine whether risk factors of ulcerative colitis among Japanese are the same as those observed in other countries, we conducted a case-control study in Japan in 1988-90. Patients selected for the study were receiving financial aid for treatment of the disease; an equal number of population-based controls matched by age and sex were selected. Smoking decreased the risk of the disease (odds ratio = 0.30; 95% confidence interval, 0.16-0.56), and stopping smoking increased the risk (odds ratio compared with those who had never smoked = 1.44; 95% confidence interval, 0.73-2.85). In those who drank alcoholic beverages > or = 5 days a week, disease development was less likely than in others (odds ratio = 0.49; 95% confidence interval, 0.30-0.81). In addition, difficulties in human relations at work, a family history of asthma, a medical history of surgical procedures, and personality traits such as nervousness, anxiety, and obsessive behavior were also considered risk factors. Although some findings are similar to those in former studies, a few findings are different: surgical procedures, occupational status, and a family history of the disease. To clarify whether the differences are consistent will require other case-control studies.

Symmetrical lipomatosis of the tongue presenting as macroglossia. Report of two cases.

Symmetrical lipomatosis in the oral cavity is extremely rare. Two cases of symmetrical lipomatosis presenting as macroglossia are presented. Glossectomy was performed in order to reduce the size of the tongue and for diagnosis. Because of their multiplicity, non-encapsulation and invasiveness, the lesions were diagnosed histopathologically as symmetrical lipomatosis.

The effect of a change in dietary habit upon maze learning ability in rats.

An experimental psychological study was undertaken to assess the effect of a change in dietary habit upon behavioural development, using as a laboratory animal male Wistar rats maintained on natural foods and a processed food. Forty male Wistar rats were weaned at 21 days of age and divided into two equal groups. Group 1 was fed natural food (pressed wheat, dried fish, peanuts, cabbage and soybeans) and group 2 was fed powdered food. At 68 days of age, the rats were tested in a T-maze. The results showed that Group 1 excelled with regard to T-maze learning.

[Two cases of solid and cystic tumor of the pancreas (including one case of MRI)].

Solid and cystic tumor of pancreas is an uncommon neoplasm which is histologically low grade malignant but amenable to cure by surgical excision. It tends to occur in young women. US and CT depict a well circumscribed mass that can be solid, mixed solid and cystic and largely cystic. Their ratio depends on the degree of hemorrhage and necrosis. Angiography shows a mildly to moderately vascular mass which corresponds to solid parts. MRI is useful to detect the hemorrhage which is common in this tumor.

Unilateral parotid swelling after general anaesthesia. A case report.

An extensive swelling in the left parotid region, extending to the buccal and cervical areas, developed in a 30-year-old man immediately after a partial nephrectomy which was performed under general anaesthesia. Radiological examination immediately after the onset revealed no abnormality, but a large swelling was detected by computed tomography in the left parotid region. Serum amylase was significantly elevated. The clinical signs had almost disappeared approximately 2 weeks after the onset, following intravenous infusion of antibiotics and transfusion. This parotid swelling was considered to be acute postoperative parotitis induced during induction of anaesthesia by luxation of the temporomandibular joint or by the positioning of the patient during operation.

Reduction and fixation of jaw fractures using acrylic splints.

This paper proposes the criteria for application of acrylic splints in the treatment of various types of maxillofacial fractures. These were based mainly on the number of remaining teeth that are important for the restoration of occlusion. The acrylic splints studied consisted of intermaxillary, lingual and labiolingual types that hold a dental arch and a cap type that covers a dental arch. It was found that the intermaxillary type was best indicated for the loss of multiple teeth, the lingual type for the predicted intraversion of bone fragments and the labiolingual as well as cap types for the deciduous or mixed dentition. Clinical application based on the above standard, which was attempted in our 60 patients, produced successful results. In addition, the usefulness of these acrylic splints as an intraoral apparatus was discussed.
