[Clinical experiences with a new membrane oxygenator with low priming volume (D702 MASTER FLO 51), studies during pulsatile and constant flow perfusion].

The newly developed oxygenator "D702" is a compact hollow fiber membrane oxygenator with a priming volume of 170 ml. The maximum flow allowance is 4 liters per/minute. We used this oxygenator in 16 patients (11 infants and children, and 5 adults) undergoing various open heart surgery, and function of this oxygenator was studied. Pulsatile cardiopulmonary bypass was performed in 8 patients and nonpulsatile constant flow perfusion was employed in the remaining 8 patients. Our clinical experience showed excellent maintenance of PaO2 and PaCO2 during both pulsatile and constant flow bypasses. A low pressure drop was encountered across the membrane oxygenator, and therefore, this oxygenator is applicable for pulsatile cardiopulmonary bypass. The D702 is a very useful and applicable for a wide range of patients from infants to adults with a small body structure.

[Successful recovery and long-term survival following postcardiotomy (mitral valve replacement) cardiogenic shock by means of a left ventricular assist device].

A 48-year-old woman who had had mitral regurgitation associated with hypertension and left ventricular hypertrophy underwent mitral valve replacement with a 29 mm xenograft. Following the uneventful procedure, she could not come off cardiopulmonary bypass for 6 times even with an aid of IABP and with re-replacement of the mitral valve using a 29 mm Duromedics prosthesis because of the fear of xenograft malfunction. A left ventricular assist device (LVAD) was then utilized with a resultant dramatic recovery from profound postcardiotomy cardiogenic shock, and cardiopulmonary bypass was successfully terminated. Postoperatively, her hemodynamics were maintained with a LVAD and IABP. Heparin was used to keep the prosthesis from blood clotting by monitoring the ACT. The heart recovered gradually from intraoperative myocardial damage and the LVAD was successfully removed at the 5th postoperative day. Postoperative cardiac catheterization studies showed decreases in left ventricular wall motion and ejection fraction, although improvements of pulmonary hypertension and left ventricular end-diastolic pressure at rest were observed. At present, 1 year and 9 months after the use of LVAD, she is leading nearly normal life of NYHA grade 1. Because of the excellent present clinical condition of this patient, the benefit of a LVAD in perioperative cardiogenic shock is stressed.

[Two cases of Hermansky-Pudlak syndrome with interstitial pneumonia].

Two cases of Hermansky-Pudlak syndrome with interstitial pneumonia were reported. Both patients had evidence of pulmonary involvement characterized by diffuse bilateral infiltrates. They had oculocutaneous albinism. Case 1 was a 55 year-old female who had a history of easy bruising. Her two sisters were albino and had died of pulmonary fibrosis. One of them was diagnosed as Hermansky-Pudlak syndrome on autopsy. Bone marrow aspirate disclosed typical macrophages with ceroid-like pigment. Transbronchial lung biopsy showed alveolar wall thickening. Lumi-aggregometer showed a decrease of platelet aggregation and an absence of ATP release. Case 2 was a 43 year-old female and had a bleeding tendency during a surgical procedure. Lumi-aggregometer showed normal platelet aggregation but an absence of ATP release. BALF analysis did not disclose macrophages with ceroid-like pigment.

[Pharmacological studies of eicosapentaenoic acid ethylester (EPA-E) on high cholesterol diet-fed rabbits].

The effects of eicosapentaenoic acid ethylester (EPA-E) at daily doses of 3 and 30 mg/kg (p.o.) on vascular lesions and changes in blood components were investigated in rabbits fed with a 1% cholesterol diet for 12 weeks and partly compared with those given 30 mg/kg/day (p.o.) of ticlopidine. EPA-E at 30 mg/kg significantly improved the reduction in distensibility of isolated thoracic aorta caused by cholesterol diet feeding. Ticlopidine at 30 mg/kg also showed significant but milder improvement. EPA-E at 30 mg/kg, unlike ticlopidine, tended to lower cholesterol content in the thoracic wall and elastin fraction. Although EPA-E did not act on plasma total cholesterol, HDL-cholesterol and triglyceride contents, it tended to mildly reduce platelet aggregability in response to sodium arachidonate and the increased production of TXA2-like activity by platelets. In addition, daily doses of 30 mg/kg EPA-E caused a significant increase in plasma and platelet EPA-E contents without any change in docosahexaenoic acid and arachidonic acid contents. From these results, daily administration of EPA-E to cholesterol diet-fed rabbits caused a remarkable improvement of the reduction in distensibility of isolated thoracic aorta. The mechanism of this improvement by EPA-E appears to depend on the protection of elastic fibers from lipid deposition on the vascular wall.

[The effects of eicosapentaenoic acid ethyl ester (EPA-E) on arterial thrombosis in rabbits and vascular lesions in rats].

The effects of highly purified ethyl ester of eicosapentaenoic acid (EPA-E) on ellagic acid-induced thrombus formation in the stenosed femoral artery in rabbits and on the progression of laurate-induced vascular lesions in rats were examined. (1) EPA-E in single administration significantly prevented the thrombus formation induced by ellagic acid in the stenosed femoral artery of rabbits. (2) EPA-E restored the hypercoagulation activity at 5 min and 3 hr after the ellagic acid injection. Platelet aggregability induced by arachidonic acid decreased at 5 min and 3 hr after the ellagic acid injection; EPA-E had little effect on this change. EPA-E showed almost no effect on fibrinolytic activity, blood viscosity and filterability of washed red blood cells after the ellagic acid injection. (3) Moreover, EPA-E in daily administrations showed a significant preventative effect against the laurate-induced vascular lesions in rats. The efficacy of EPA-E on arterial thrombosis and vascular lesions observed in this study suggests that EPA-E is a candidate for the treatment of peripheral circulatory disturbance.

Excellent patency and growth potential of internal mammary artery grafts in pediatric coronary artery bypass surgery. New evidence for a "live" conduit.

Coronary artery bypass grafting in pediatric patients provides a unique opportunity to evaluate the characteristics of graft material. Twelve pediatric patients with severe coronary artery involvement secondary to Kawasaki disease underwent coronary artery bypass grafting with internal mammary artery and autologous saphenous vein grafts. Eleven patients were boys and one was a girl; their ages ranged from 5 to 13 years (mean age +/- SD, 8.7 +/- 3.0 years). These 12 patients had 17 internal mammary artery and 11 autologous saphenous vein grafts with a mean value of 2.3 +/- 0.7 grafts per patient. Five patients underwent bilateral internal mammary artery grafting with or without concomitant autologous saphenous vein grafting. There was no mortality, and all patients are leading almost normal lives, although strenuous exercise is still prohibited for some. Biplanar cineangiography was performed before and 1 month after (early postoperative period) coronary artery bypass grafting in all patients and was repeated more than 1 year after (late postoperative period) bypass in eight patients. Changes in graft diameter and graft length were assessed by computer-assisted graphic analysis of 10 internal mammary artery grafts and of seven saphenous vein grafts between the early and late postoperative periods. Patency rates of saphenous vein grafts and of internal mammary artery grafts were 91% and 100%, respectively, at the early postoperative period (p = NS), and 50% and 100%, respectively, at the late postoperative period (p less than 0.05). In addition to the excellent patency of internal mammary artery grafts in the late postoperative period, an in situ internal mammary artery graft was found to have grown 125% in length with 112% body growth (112% body surface area) (r = 0.88, p less than 0.001) and to have enlarged 149% in diameter when the native coronary stenosis was tight. In contrast, the lengths of saphenous vein grafts either failed to change or were slightly decreased at the late postoperative period (p = NS). Growth potential of the internal mammary artery graft provides a new demonstration of the viability and adaptability of this graft. We conclude that the internal mammary artery graft is a "live" conduit with potential for growth and adaptation. This growth potential may be the most important reason for its excellent long-term patency, which suggests that in situ internal mammary artery grafts are the graft of choice for pediatric coronary artery bypass grafting.

[A case of primary extramedullary plasmacytoma in the posterior cervical portion].

This case involves a 16-year-old woman with a posterior cervical tumor. Histologically, it was found to be a extramedullary plasmacytoma (EMP). Immunohistochemically, IgG was present in the plasma cells that were shown to be atypia by the PAP method. Laboratory data revealed no other abnormalities. A surgical excision of the tumor was performed with a dissection of the relevant lymph nodes. Although irradiation and chemotherapy were administered postoperatively, multiple bone metastasis was found a month after the operation, and the patient died 8 months later.

Expanding intracerebellar hematoma: a possible clinicopathological entity.

An adult case of intracerebellar hematoma with a definite capsule is presented. The hematoma is thought to have been enlarged by repeated hemorrhages inside the capsule, with consequent progressive neurological symptoms. Intracerebellar hematoma does not always disappear by absorption. On the contrary, it may expand, and symptoms and signs similar to those of cerebellar tumor may subsequently develop. The authors propose that the expanding intracerebellar hematoma is a possible clinicopathological entity with a definite capsule comparable to that of chronic subdural hematoma.

[Pharmacological studies of MO-8282, a new antidepressant].

We investigated the pharmacological characteristics of MO-8282 as a novel antidepressant. MO-8282 inhibited the specific binding of 3H-clonidine to cerebro-cortical membrane fractions from rats about five times more potently than mianserin, and it competed with clonidine in the twitch response of the isolated guinea-pig ileum under field stimulation. The results indicated that MO-8282 possessed alpha 2-adrenergic receptor blocking activity. MO-8282 in a dose of 30 mg/kg (p.o.) showed no inhibition against the uptake of noradrenaline, dopamine and serotonin in the rat brain, whereas mianserin inhibited the uptake of serotonin specifically. MO-8282, similar to mianserin, had no effect on spontaneous release of 3H-noradrenaline and slightly stimulated the release of 3H-serotonin from the rat cerebrocortical synaptosome. The turnover rate of noradrenaline in rat brain was accelerated by administration of MO-8282 (30 mg/kg) for 15 days; however, that of dopamine and serotonin was not affected. The above findings indicate that MO-8282, unlike tricyclic antidepressants, mainly exerts alpha 2-adrenoceptor blocking action on the central noradrenergic system, similar to mianserin. In addition, the fact that MO-8282 unlike mianserin showed no inhibition against uptake of serotonin in brain suggests that the alpha 2-adrenoceptor blocking of MO-8282 is more specific and potent than that of mianserin.

[Arteritis and fatty degeneration of the spermatic cord resembling periarteritis nodosa: a case report].

A 23-year old male was admitted to our hospital because of swelling of the left scrotum for one month without any particular past history. Blood count, chemistry, urinalysis, chest X-ray and electrocardiogram revealed normal findings. Scrotal exploration was performed. A soft, dumbbell-like tumor enveloped in a thin membrane was found above the left testis. Pathology revealed fibrinous exudation and fibrinoid necrosis in the whole vessel wall indicating resemblance to the panarteritis in periarteritis nodosa. Some granulomatous lesions with many histiocytes were also noted around these arteries. The findings suggested that a inflammation like periarteritis nodosa had occurred at the spermatic cord and subsequently developed into fatty degeneration in the surroundings. Since local periarteritis nodosa-like lesion may progress to systematic disease, further observation is mandatory for this case.
