Observing posterior capsule lens haptics in living postoperative eyes.

The cycloscope, an aid for observing the ciliary body, was used to study lens haptic fixation in 100 living eyes (200 haptics). Although the whole aspect of the lens could be viewed in only about 20 haptics (10%), in 102 haptics (51%), judgment about whether fixation was in or out of the bag could be made based on the relationship of the visible part of the lens haptic and the surrounding tissues. Thus, the method was found to be useful in studying the fixation mechanism of posterior chamber lens haptics in living postoperative eyes. Two cases of J-loop posterior chamber lens implantation studied by cycloscopy are reported.

[Pelvic recurrence of bladder carcinoma following total cystectomy].

The patients who undergo total cystectomy for high stage bladder carcinoma have poor prognosis because of local extension or distant metastases. In the case of recurrent lesions after total cystectomy, radiotherapy and chemotherapy are generally selected. In some cases of recurrent tumors localized within the pelvic cavity and with no distant metastases, however, surgical resection of the tumor is effective in local disease control combined with radio- and chemotherapy. Surgical treatment is not only effective in reducing the cell number in a bulky tumor, but also improves clinical symptoms and conditions such as pain, bleeding, rectal irritability, intestinal obstruction and so on. We present here three patients who underwent extensive operation for pelvic recurrence of bladder carcinoma following total cystectomy. Two of these cases died of progressive disease, nine months and five months after the extensive operations. The third patient died of acute pericarditis thirteen months after the second operation. Autopsy revealed that the surgical treatment was effective in controlling the local recurrence in the third case. Furthermore, some problems regarding pelvic recurrence and surgical treatment are discussed.

[Angiomyolipoma of the kidney: report of three cases and a statistical study of 194 cases in Japan].

Three cases of renal angiomyolipoma are reported. The first case was a 53-year-old female with the complaint of left flank pain. In both cases selective angiography revealed a renal tumor, and computerized tomography disclosed a renal mass with low density corresponding to the fat tissue. From several examinations these two cases were preoperatively diagnosed as renal angiomyolipoma and nephrectomy was performed. Histopathological diagnosis was renal angiomyolipoma. The third case was a 64-year-old female with the complaints of left flank pain and macrohematuria. In this case computerized tomography revealed a renal mass which did not have a low density area. The possibility of renal cell carcinoma was considered and nephrectomy was performed. Histological diagnosis was renal angiomyolipoma which was primarily composed of smooth muscle cells. The first case had a calcified lesion in the brain. But there was no evidence of tuberous sclerosis in the following two cases. Some problems in diagnosis and treatment of renal angiomyolipoma are presented, and a statistical study is made on 194 reported cases of renal angiomyolipoma in Japan.

Incidence of cystoid macular edema after retinal detachment surgery and the use of topical indomethacin.

We conducted a prospective, double-masked, controlled study to determine the effects of indomethacin in preventing the development of cystoid macular edema after retinal detachment surgery. In addition to routine medication, topical indomethacin was applied to 63 eyes (52 with intact lenses and 11 aphakic) and a placebo was applied to 61 eyes (51 with intact lenses and ten aphakic) that underwent retinal detachment surgery. Indomethacin drops or placebo were instilled once an hour for five hours preoperatively and three times daily for four weeks postoperatively. Fluorescein angiography, done before surgery and four to six weeks after surgery to detect cystoid macular edema, showed that cystoid macular edema developed in 11 of 63 eyes (13%) in the indomethacin-treated group and in 20 of 61 eyes (33%) in the placebo group (P less than .01). Five of 28 eyes (18%) with cystoid macular edema in both groups and 38 of 96 eyes (40%) without cystoid macular edema in both groups had visual acuities of 20/40 or better (P less than .05).

Giant scrotal hemangioma with azoospermia.

We report a case of giant scrotal hemangioma with azoospermia, which was 37 cm in maximum circumference and 20 cm in length along the raphe. The biopsy specimens of the testes were examined histologically and cytologically. The testicular damage was believed to be due to the heat generated by hemangioma.

Cytoplasmic inclusion bodies in Leydig cells from the testes of postpubertal cryptorchid patients.

During the course of electron microscopic studies on testes from cryptorchid patients, we have observed an inclusion body in the cytoplasm of Leydig cells. Such inclusion bodies were detected in Leydig cells of post-pubertal patients (16 to 30 years), but were not evident in the cells of pubertal and pre-pubertal patients (3 to 12 years), or in cells from the contralateral descended scrotal testes of cryptorchid patients or in cells from normal controls. These inclusion bodies are therefore believed to result from specific cytological changes which occur in cryptorchid testes during puberty.

Combination chemotherapy for small cell carcinoma of the lung: evaluation of four-drug combination of cyclophosphamide, vincristine, methotrexate, and procarbazine.

Forty-one patients with small cell carcinoma of the lung were treated with a four-drug combination of cyclophosphamide, vincristine, methotrexate, and procarbazine. The response rate was 68% (28 responded among 41 patients), with 10 complete responses (24%) and 18 partial responses (44%). The median survival time from the initiation of chemotherapy was 11 months for patients with limited disease and 8 months for those with extensive disease. Patients who achieved complete response survived significantly longer than those who did not; the median survival time for complete responders was 14.5 months, compared to 8.5 months for partial responders and 6 months for non-responders. Myelosuppressive toxicity remained within acceptable limits, with 5% incidence of leukocytopenia (less than 1,000/microliter) and 7% incidence of thrombocytopenia (less than 50,000/microliter) following the first course of the regimen.

[A case of malignant fibrous histiocytoma occurring in the retroperitoneum with giant pyonephrosis].

A case of malignant fibrous histiocytoma (MFH) occurring in th retroperitoneum with giant pyonephrosis is reported. The patient was a 45-year-old male and his chief complaint was an abdominal mass. The abdominal fullness progressed so rapidly that he was admitted to our hospital. After examination, this case was diagnosed as a malignant tumor with left hydronephrosis, and an operation was performed on August 5, 1982. At operation, the left kidney contained about 11,000 ml of a pus-like fluid and in the retroperitoneum was found a hen-egg-sized solid tumor which was invading into the left kidney and the feeding vessels of the descending colon. So the tumor, left kidney and a part of the descending colon were resected en bloc. Pathological diagnosis was malignant fibrous histiocytoma. Chemotherapy (PPM regimen) and immunotherapy (OK-432) were administered after the operation, but multiple metastases appeared in the liver and bilateral lungs within 3 months. Then, the CY-VA-DIC regimen was followed. But, local recurrence was found in about 5 months, and the patient died on the 174 th day after the operation. Local recurrence and metastases in the liver, bilateral lungs, pleura and bones were confirmed at autopsy. Besides our case, a review of case reports of retroperitoneal MFH in Japan and comments are presented.

[Treatment of urinary tract infections by cefaclor, with special reference to the antibody-coated bacteria (ACB) detected by the immunoenzyme technique].

Twenty out-patients suffering from urinary tract infections (UTI) were treated with cefaclor. Cefaclor was administrated orally in daily doses of 1,500 mg for 7 days against 15 cases of simple UTI and 5 cases of complicated UTI. Of 20 cases with acute simple cystitis, acute pyelonephritis and complicated UTI, clinical responses were excellent in 10 cases, good in 6 cases and poor in 4 cases. The efficacy rate was 80%. No side effect was observed. The site of infection determined by the ACB detection method was in good agreement with that determined by clinical findings. Cefaclor appeared to be equally effective in the treatment of both ACB-positive and ACB-negative infections. The presence of absence of antibody-coated bacteria did not correlate with clinical symptoms or response to therapy.

[Studies on the pathogenesis of idiopathic male infertility by testicular biopsy].

We have been studying the cause of idiopathic male infertility by means of testicular biopsy, and already have made several reports. This is a summary of these reports and a presentation of our concept for hypothetic pathogenesis of idiopathic infertile testes. Testicular biopsies were performed by Charny's accepted technique and tissues examined by light microscopic, histochemical, and transmission and scanning electron microscopic procedures. Also the circulatory sexual hormones; follicle stimulating hormone, luteinizing hormone, and testosterone of all patients were measured. The primary pathogenetic changes occur in the seminiferous tubular walls which are thickened by increase in immature collagen fibers and lamellation of seminiferous basement membrane, and they are followed by increase of follicle stimulating hormone. These seminiferous tubular changes occur discontinuously, so the idiopathic infertile seminiferous tubules resemble a rope strangulated in various places, or a tail-like rope gradually becoming thinner. Also, activated mast cells increase around the strangulated places. Thus, spermatogenesis in strangulated tubular parts decreases because of the nutritional disturbance caused by thickened tubular walls, and the strangulation of tubules interferes with the travel of sperms in the seminiferous tubules by interrupted seminiferous tubular peristalsis. Sloughing or disorganization in the infertile seminiferous tubules can be observed. In the infertile testes, Leydig cells generally increase with the elevation in the level of luteinizing hormone, but serum testosterone does not increase. And this normal value of serum testosterone is statistically found to be caused by the degradation in the quality of Leydig cells in idiopathic infertile testes. Also, galactose deficiency in glycoproteins of idiopathic infertile testicular tissue is reported.

Blood-retinal barrier in eyes with long-standing aphakia with apparently normal fundi.

Vitreous fluorophotometry was used to evaluate the integrity of the blood-retinal barrier in 130 eyes with long-standing aphakia and apparently normal fundi. All patients had undergone intracapsular lens extraction because of senile cataract and had been followed up for approximately one year (group 1) or three years (group 2). Thirty normal eyes served as controls. Thirty minutes or one hour after intravenous administration of fluorescein sodium, 50 of 130 aphakic eyes (32 of 70 group 1 eyes; 18 of 60 group 2 eyes) and three of 30 control eyes were judged abnormal in breakdown of the blood-retinal barrier. In aphakic eyes, the older the age at operation, the higher the incidence of barrier breakdown. These results indicate that the blood-retinal barrier remains disrupted for a longer postoperative period than was previously thought, and that the incidence of barrier breakdown relates to the age at operation. Although these findings reflect subclinical phenomena, they are biologically important.
