Successful treatment of oligospermic and azoospermic men with alpha 1-blocker and beta-stimulator: new treatment for idiopathic male infertility.

We studied the effect of oral administration of alpha 1-blocker and beta-stimulator on 20 idiopathically infertile men. Bunazosin (alpha 1-blocker, 2 mg/day) and procaterol (beta-stimulator, 100 micrograms/day) were given orally twice daily for 5 months. The administration of alpha 1-blocker and beta-stimulator elicited an increase in sperm output and seminal volume in 16 patients (80%). The increase in sperm output seems to be associated with relaxations of myoid cells, which lead to dilatation of stenotic areas of seminiferous tubules, occurring discontinuously, and subsequent maintenance of good tubular fluid flow. No adverse effects were observed in this series.

Gastrointestinal absorption of sulfaguanidine in neonatal and adult rats.

The pharmacokinetics of sulfaguanidine in rats was studied after its intravenous or oral administration. In adult rats, its elimination from plasma, after intravenous administration of 2.5 or 25 mg/kg doses, could be described by a two-compartment open model, while its plasma concentration after oral administration of 25 mg/kg doses, agreed with the one- or two-compartment open model. Neonatal rats displayed a lower elimination of sulfaguanidine than adult rats. Comparison of the drug's gastrointestinal absorption showed that the maximum plasma concentration after oral administration was significantly higher for neonatal than for adult rats. However, there was no significant difference between the times required to reach maximum plasma concentrations. The area under the plasma concentration-time curve at 0-infinity h was significantly higher for neonatal than for adult rats. The absolute bioavailability (57.86%) in neonatal rats was approximately five times that (12.76%) in adult rats. Thus, sulfaguanidine was poorly absorbed by adult rats, but was efficiently absorbed by immature gastrointestines of neonatal rats.

Pharmacological and histological evidence for adrenergic innervation of the myoid cells in the rat seminiferous tubule.

Although the existence of seminiferous tubule contractions attributed to peritubular myoid cells is established, the control of the contractions is poorly understood. In this communication, it is shown that the rat seminiferous tubule responds to autonomic drugs and to stimulation of the perivascular nerve running along the spermatic vessels, by means of recording the intratubular pressure with a servo-null micropressure measuring device. Furthermore, the presence of nerve fibers close to the myoid cells is shown using a silver impregnation technique. Furthermore, the nature of the neurotransmitter contained in synaptic vesicles using 5-hydroxydopamine and L-DOPA as markers of adrenergic nerve elements with electron microscopy is shown in this report. It is concluded that there are adrenergic alpha- and beta-receptors and muscarinic receptors in myoid cells of the rat seminiferous tubule and that the contractions of seminiferous tubules are regulated by adrenergic nerve fibers.

Combined prostatectomy and retropubic prostatectomy on patients eighty years old or older with benign prostatic hyperplasia.

Combined prostatectomy and retropubic prostatectomy were performed on 20 patients eighty years old or older with benign prostatic hyperplasia. A comparative assessment of two surgical methods regading to operative blood loss, operating time and postoperative complications was presented. The procedure of combined prostatectomy revealed a smaller amount of operative blood loss, less operating time and lower incidence of complications when compared to retropublic prostatectomy. This modified method of suprapubic-retropubic prostatectomy appears to offer several advantages over other open methods of prostatectomy.

[A case in which alpha-interferon showed remarkable effect against the brain, bone, and lung metastases arising from the renal cell carcinoma].

A 37-year-old man was sent to our department from the Radiology Department on July 19, 1984 with the diagnosis of right renal cell carcinoma with metastases to the right humerus and bilateral lungs. He was nephrectomized immediately and administered 10 X 10(6) units of alpha-interferon i. m. every other day for 3 months during the admission and 30 X 10(6) units once a week for more than 3 months after discharge. Although in addition to humeral and lung metastases, brain metastasis was found a week after the operation, interferon showed a remarkable effect and produced get a complete response in the humeral and brain metastases, and partial response in the lung metastasis. We think this is, perhaps, the first case of CR in brain metastasis so far. We conclude that interferon can be the first choice drug as an adjuvant chemotherapy against renal cell carcinoma.

[Rhabdomyosarcoma of the prostate].

We report a case of rhabdomyosarcoma of the prostate. The patient was a 56-year-old man who complained of anal pain and dysuria. Tumor of the prostate was suspected after rectal examination. Multiple metastatic lesions were found in the lungs and liver. A needle biopsy of the prostate revealed rhabdomyosarcoma. He received chemotherapy, using Etoposide and responded slightly. Subsequently VAC-therapy was also performed. Although the patient improved temporarily, he died 4 months after admission.

Outward transport of fluorescein from the vitreous in aphakic eyes.

By administering fluorescein intravenously to 95 patients we calculated the ratio of fluorescein concentration in the vitreous at the time of its peak level compared with the estimated unbound concentration of fluorescein in the plasma at the same time. We studied 12 normal phakic and 83 aphakic eye approximately two months, one year, and more than two years after cataract extraction. All the eyes had undergone intracapsular cataract extraction or extracapsular cataract extraction, with or without posterior capsulotomy, because of senile cataract. The calculated ratio in patients with intracapsular and extracapsular lens extraction was statistically significantly reduced at two months and one year after cataract extraction and was normalised at more than two years after the operation in comparison with normal subjects. The ratio was statistically extracapsular extraction at two months and one year after surgery. Posterior capsulotomy had no effect on the ratio. The ratio, we considered, at least partially reflects the outward transport of fluorescein from the vitreous cavity. Although the findings reflect subclinical phenomena, they are of importance when considering postoperative sequelae. The posterior lens capsule, zonule, and intact anterior vitreous face may be essential for the anterior uvea to function in the outward transport of fluorescein from the vitreous cavity.

[Clinical application of a new fibrin adhesive (Tisseel) in urologic surgery].

Our experience with the use of a new biological tissue adhesive (Tisseel) consisting of highly concentrated human fibrinogen, thrombin and factor XIII in urologic surgery is reported. In 21 operations (18 patients), Tisseel was used for the purpose of tissue adhesion and reduction of sutures of the renal parenchymal wound in nephrolithotomy, water tight sealing in suture part of expanded pyelolithtomy and vasovasostomy, and local hemostasis of oozing in prostatectomized fossa. Tisseel was not responsible for the two unsuccessful trials; and, undesirable complications due to this adhesive were few. Our results revealed that this new fibrin adhesive will be useful for urologic surgery.

[A case of primary adenocarcinoma of the urinary bladder occurring in a patient with type 1 multiple endocrine adenomatosis (MEA)].

A case of primary adenocarcinoma of the urinary bladder occurring in a patient with type 1 multiple endocrine adenomatosis (MEA) is presented. The patient was a 36-year-old female who had a past history of type 1 multiple endocrine adenomatosis, namely, adenomatosis of the parathyroid gland, insulin and gastrin-producing adenomatosis of the pancreas, and prolactin-producing pituitary adenoma. She was admitted in January 1981 with the complaints of gross hematuria, pollakisuria and micturition pain lasting for about one year and a half. Cystoscopic examination revealed four solid tumors in the posterior and left lateral walls of the bladder with diffuse mucosal hyperemia. Biopsy of the tumors disclosed that they were adenocarcinoma. Clinical examinations revealed that there was no extravesical primary malignant neoplasm in this case. Radical cystectomy with urinary diversion by ileal conduit was performed on January 22, 1981. Histological examination revealed that the tumor was adenocarcinoma originating from the vesical mucosa. Follow-up for over three years since the time of surgery has not shown any sign of tumor recurrence or occurrence of extravesical malignant neoplasm. In addition, 28 cases of primary adenocarcinoma of urinary bladder in Japan reported during the last 25 years are reviewed and analyzed.
