[Clinical use of piperacillin on obstetrical and gynecological infections (Second report)].

We administered piperacillin (PIPC) to patients with obstetrical and gynecological infectious diseases and obtained the following results. 1. Sixteen patients were administered PIPC at a dose of 4 or 9 g by dripping infusion 2 or 3 times a day for a period of 4 approximately 14 days. Eight cases had cancer as their underlying disease (including cervical cancer etc.). The clinical effect was good in 3 of these 8 patients with cancer so that the efficacy rate was 37.5%. But the other 8 cases without cancer all responded effectively, so their efficacy rate was 100%. The efficacy was good in 11 out of 16 cases so the overall efficacy rate was 68.8%. 2. No side effect was observed. 3. No adverse reaction in laboratory findings (including hematological findings, hepatic function tests, renal function tests) was observed.

[Ataxia in rolling mouse Nagoya and the norepinephrine system--a psychopharmacological study].

Rolling mouse Nagoya (RMN), an ataxia mutant mouse, is adequate to checking up the efficacy of drugs on the ataxic movement disorders. The effects of some drugs on the ataxia of RMN were investigated by open field study. The drugs administered to RMN were as follows: TRH, FLA-63, DL-threo-DOPS, phenoxybenzamine and propranolol. Ataxic gait of RMN were remarkably improved by TRH, but remained unchanged by other drugs. The pretreatments with FLA-63, DOPS and phenoxybenzamine were not influenced to the antiataxic effects of TRH, while the pretreatment with propranolol increased the antiataxic effect of TRH in RMN. These results suggest that mechanism of the improvement of ataxic gait by TRH may not be sufficiently explained through only changing norepinephrine metabolism.

Regulation by human chorionic gonadotropin of protein kinases in rabbit endometrium.

The effect of human chorionic gonadotropin (CG) on protein kinase levels has been studied in rabbit endometrium. A good correlation was observed between alterations of total protein kinase activity and DNA content in the tissue. The level of type I cyclic AMP-dependent protein kinase had slightly increased by 6 h after human CG treatment, and then decreased later. In contrast, the type II enzyme gradually increased after 3 days. The increase of the activity at 7 days was mainly due to that of type II enzyme, and the ratio of type II to type I enzyme increased. The activity pattern at the later stage closely resembled that of treatment with progesterone.

Ventricular septal rupture secondary to myocardial infarction. Clinical approach and surgical results.

Twenty-four patients underwent operation for ventricular septal rupture secondary to acute myocardial infarction. There were 14 hospital survivors (58%) and two late deaths (8%). There were eight hospital deaths (62%) of 13 patients referred in cardiogenic shock, but only two deaths (18%) of 11 patients not in shock at time of referral. All 12 current survivors showed clinical improvement, and 11 of them are in New York Heart Association functional class I or II. Eleven patients had bedside catheterization with a balloon catheter and were operated on immediately thereafter, and eight survived (73%) with no late deaths at five years. With formal heart catheterization followed by operation, there were only six survivors of 13 operated on (46%).

Aortic valve replacement in patients with poor ventricular function-early and late results with long-term follow-up.

A series of 62 consecutive patients with an ejection fraction of 0.4 or less (mean 0.28 with a range from 0.10 to 0.40; 22 between 0.10 and 0.20, 18 between 0.21 and 0.30, and 22 between 0.31 and 0.40) who underwent aortic valve replacement from January 18, 1972 to December 20, 1976 was reviewed. Preoperatively two patients were in Class II, 35 in Class III and 25 in Class IV of the New York Heart Association functional classification (N.Y.H.A.). Thirty-nine patients (Group 1) underwent isolated aortic valve replacement and 23 patients (Group 2) underwent aortic valve replacement with associated procedures including aortocoronary bypass in 15. The operative mortality was 8 percent in Group 1, 17 percent in Group 2, and 11 percent overall. In the group of 15 patients with coronary artery disease, the operative mortality of aortic valve replacement and aorto-coronary bypass was 27 percent. Since January 1974, isolated aortic valve replacement was performed with no operative deaths in 25 consecutive patients in Group 1 including 10 patients with an ejection fraction of 0.2 or less. Five-year survival rates were 70 percent in Group 1, 64 percent in Group 2 and 68 percent overall. In the 38 currently living patients, 32 showed clinical improvement and 27 are in Class I or II of N.Y.H.A. In conclusion, isolated aortic valve replacement can be performed with a low mortality and a high survival rate in patients with impaired left ventricular function.

Occurrence of two types of Ca2+-dependent protein kinases in the cytosol fraction of the brain.

Two types of Ca2+-dependent protein kinases were demonstrated and partially purified from the cytosol fraction of rat brain by DEAE-cellulose, Sephadex G-200, and calmodulin-affinity column chromatography, using endogenous proteins and chicken gizzard myosin light chains as substrates. The molecular weights of the enzymes were 88,000 (peak I) and 120,000 (peak II) on gel filtration. Peak I had no affinity for calmodulin, whereas peak II had a high affinity for it, with a Ka value of 16.7 nM. The Ka values of peaks I and II for Ca2+ were 2.4 and 1.6 microM, respectively.
