Blast crisis of chronic myelogenous leukemia with tumor formation characterized by T-cell features--a case report.

We report a case of chronic myelogenous leukemia (CML) associated with pronounced peripheral lymphadenopathy, with the cells having the philadelphia (Phl) chromosome and T-cell features. A 23-year-old man who was diagnosed as having CML and treated with busulfan was admitted to our hospital because of increasing hepatosplenomegaly and pronounced lymphadenopathy. An axillary lymph node biopsy disclosed that the malignant cells formed rosettes with neuraminidase-treated sheep red blood cells (En) (95.0%) and were positive for Leu 1 (91.8%). Of the cytochemical reactions, peroxidase was negative and periodic acid-Shiff, acid alpha-naphthyl acetate esterase and beta-glucuronidase were all positive. The karyotype of the bone marrow cells was 46 XY Phl positive (22q-), and that of the lymph node cells was 51 XY Phl positive +8, +9, +18, +19, +21, 22q-. He was treated with various anti-leukemic agents and irradiation. Despite such treatments, he died of pneumonia. This is a report of a CML patients with blast crisis and tumor formation characterized by T-cell features.

Effects of 2,4-dinitrophenol and other metabolic inhibitors on the thermograms of Ehrlich ascites carcinoma cells registered with a microcalorimeter.

The heat produced by Ehrlich ascites carcinoma cells was measured microcalorimetrically to investigate the characteristics of the energetics of these cells. 2,4-Dinitrophenol and other inhibitors were used to obtain the standard thermograms. The heat level of starved Ehrlich ascites carcinoma cells (endogenous) was almost constant and linear and depended on the cell number used in the experiments (about 1 cal/1 X 10(8) cells/hr). The endogenous heat generation was suppressed strongly by potassium cyanide but not by iodoacetic acid or 2,4-dinitrophenol. On the other hand, heat evolution was directly related to the amount of glucose added to the medium but not to the number of cells (19 cal/mmol of glucose). The addition of iodoacetic acid markedly suppressed heat generation, but potassium cyanide did not. In contrast, glucose with dinitrophenol increased heat production markedly to about 10% higher than that of the control, possibly due to the uncoupled energy from oxidative phosphorylation. Malonic acid (1 mM) suppressed slightly the endogenous heat but caused a 15% reduction in exogenous heat. The amount of heat produced by the addition of glucose (19 cal/mmol) was equivalent to 40% of the theoretical value of energy released through glycolysis (47 cal/mmol). Thus, the heat produced by exogenous glucose appears to be mainly dependent on glycolysis.

Purification and characterization of myosin light-chain kinase from porcine myometrium and its phosphorylation and modulation by cyclic AMP-dependent protein kinase.

Myosin light-chain kinase was purified from porcine myometrium to apparent homogeneity at about 262-fold with an Mr of 130 000 as determined by SDS-polyacrylamide gel electrophoresis and a sedimentation coefficient of 4.5 S. The approximate content of the soluble myosin light-chain kinase was estimated to be about 0.85 microM. The purified enzyme exhibited strict substrate specificity only for 20-kDa myosin light chain and Ka values of 0.6 nM and 0.3 microM for calmodulin and Ca2+, respectively. The enzyme was phosphorylated by the catalytic subunit of cyclic AMP-dependent protein kinase, which resulted in a decrease in the affinity for calmodulin of 4-7-fold without effect on the Vmax. The maximal amount of phosphate incorporated into the enzyme was 0.5-0.8 and 1.0-1.4 mol per mol of the enzyme in the presence and absence of Ca2+ and calmodulin, respectively. In the presence of a subsaturating concentration of calmodulin, the enzyme showed a lower sensitivity for Ca2+ by phosphorylation.

[Design of experiments and algebraic cycles].

It is well-known that balanced incomplete block designs are closely connected with finite geometry. A block design can be obtained by identifying rational points of an algebraic variety with treatments. The number of GF(qs)-rational points follows from the theory of étale cohomologies. It is, however, difficult to give an explicit formula for the number. We shall discuss the number of the GF(qs)-rational points of the variety of non-isotropic subspaces.

8-Fluoro-8-demethylriboflavin as a 19F-probe for flavin-protein interaction. A 19F NMR study with egg white riboflavin binding protein.

Binding of 8-fluoro-8-demethylriboflavin to riboflavin binding protein was studied 19F-NMR spectroscopically. The pH dependence of 19F chemical shift of this fluoro derivative both free and bound to the protein revealed a shift of pKa associated with N(3)-H to alkali when bound to the protein. 19F chemical shift of the fluoro derivative bound to the protein is in a higher magnetic field than that of the free derivative in the neutral to acidic pH region. These results are interpreted in terms of the environmental changes at the 8- and 3-positions of isoalloxazine, indicating the applicability of this derivative for probing flavin-protein interaction at these positions.

97Ru-transferrin uptake in tumor and abscess.

The uptake of 97Ru-transferrin (Ru-TF) in tumor and abscess bearing animals was compared with 67Ga-citrate (Ga), 123I-transferrin (I-TF), and several other plasma proteins. Maximal concentration in tumor of Ru-TF in mice was three times higher than 67Ga-citrate (16.80 +/- 4.20 vs 5.08 +/- 0.58% D/g) although it occurred later (24 h compared with 67Ga which reached its maximum 2 h after injection). Whole body autoradiography (WBARG) with 103Ru-transferrin (103Ru-TF) in tumor and abscess bearing rats demonstrated details of the distribution within these lesions. Turpentine-induced abscesses in the rabbits could be visualized with the gamma camera as early as 30 min post-injection of Ru-TF. It seems, therefore, that Ru-TF can be used for tumor and abscess localization. The results indicate that Ru-TF may have some advantages over 67Ga-citrate because of the higher concentration in the lesions. 123I-transferrin reached a concentration in tumor similar to 67Ga (6.89 +/- 1.67 vs 5.08 +/- 0.58% D/g) but had a very low tumor to blood ratio (0.64). The three compounds investigated (Ru-TF, I-TF and ionic Ga, which binds instantaneously to TF in vivo) have a common ligand, transferrin. It appears, therefore, that tumor affinity is a property of the radionuclide-ligand complex rather than of the radionuclide itself.

A successful repair of tricuspid atresia by modified Björk's procedure--a case report.

A ten-year-old Japanese boy with tricuspid atresia type IB was treated by direct anastomosis from the right atrium (RA) to the right ventricle (RV), entirely with autogenous material. An incision of the RA anterior wall was designed for a flap of RA appendage. A valveless conduit was created, with this flap and a large piece of pericardium. Atrial septal defect (ASD) and ventricular septal defect (VSD) were closed with a patch. Cardiac catheterization and angiocardiography two months postoperatively, showed good results of the surgery.

Kay's mitral valve repair for mitral regurgitation due to ruptured chordae tendineae--clinical and hemodynamic observations.

Kay's mitral valve repair was performed in six consecutive patients with symptomatic mitral regurgitation due to ruptured chordae tendineae of the posterior leaflet. All patients including one with mild residual murmur showed a marked decrease in the heart size and significant clinical improvement. Postoperative hemodynamic studies, performed in four patients, showed restoration of normal or near-normal dynamics. The medium-term follow-up, 15 to 30 months after the operation, revealed continuing asymptomatic states in all. The function of the repaired valves was significantly better than that of the Hancock mitral bioprostheses. The advantages of this procedure compared with mitral valve replacement for the same condition were stressed.

Alteration of glomerular anionic sites by the development of subepithelial deposits in experimental glomerulonephritis in the rat.

Using highly cationic polyethleneimine, alteration of glomerular anionic sites were evaluated ultrastructurally in two types of rat glomerulonephritis (GN); chronic serum sickness GN and heterologous (passive) or autologous (active) Heymann's GN. Daily i.v. injections of egg white lysozyme in physiologic saline into presensitized rats led to the formation of numerous mesangial and subepithelial deposits. In the non-proteinuric period in which immune deposits were localized predominantly in the mesangium, anionic sites of the laminae rarae and the epithelial cell coat were clearly observed. In the subsequent proteinuric period in which numerous subepithelial deposits were superimposed, a broad loss of anionic sites in the epithelial cell coat was seen. Splitting and focal loss of anionic sites on the lamina rara externa adjacent to the subepithelial deposits were commonly observed both in passive and active Heymann's GN and in lysozyme GN. These findings indicate that the subepithelial deposits are closely involved in the development of proteinuria by injuring the anionic sites, especially those on lamina rare externa of the glomerular basement membrane.

8-Fluoro-8-demethylriboflavin as a potential active-site-directed reagent for flavoprotein. Reaction with some amino acids.

8-Fluoro-8-demethylriboflavin was shown to be a potential active-site-directed reagent for flavoproteins. It reacted with the nucleophiles of N-acetylcysteine (-SH), N-acetyltyrosine (-OH), alpha-N-acetyllysine, and glycine (epsilon- and alpha-NH2, respectively) under fairly mild conditions, and the reaction products were identified. The reactivity of the fluoroflavin was higher than that of 8-chloro-8-demethylriboflavin, which reacted only with the cysteine derivative among the amino acid derivatives used, and whose pseudo first order rate constant at 23 degrees C was 1/23 of that of the fluoroflavin. The reactivity of the fluoroflavin was also estimated by 13C and 19F NMR spectroscopy. The results showed that this compound is more reactive than the chloroflavin.

Detrimental effects of methyl hydroperoxy-epoxy-octadecenoate on mitochondrial respiration: detoxication by rat liver mitochondria.

Methyl hydroperoxy-epoxy-octadecenoate (ML-X) inhibited state 3 respiration of rat heart and liver mitochondria with glutamate and malate as substrates. It also inhibited the NADH oxidase and NADH-ubiquinone-1 reductase activities of rat heart and liver submitochondrial particles (SMP). In liver mitochondria and SMP, these inhibitory effects of ML-X were transient, whereas in heart mitochondria and SMP, recovery of the respiratory activities did not occur. Results from high pressure liquid chromatography and 13C-NMR studies indicated that ML-X was converted to hydroperoxy-epoxy-octadecenoic acid (L-X) by incubation with liver mitochondria or SMP but not with heart mitochondria. Purified L-X apparently inhibited state 3 respiration of heart and liver mitochondria with glutamate and malate as substrates, but the amount required for 50% inhibition was 2-3 times larger than that of ML-X. Heart mitochondria adsorbed 36% of the added ML-X, while only 10% of the added L-X was adsorbed. These findings suggest that the recovery of the ML-X-inhibited respiratory activities of liver mitochondria and SMP occurs by conversion of ML-X to L-X, which is only weakly adsorbable on the mitochondrial membrane.

Splanchnic clearance of plasma vasopressin in the dog: evidence for prehepatic extraction.

It is generally considered that the liver is primarily responsible for the extraction of vasopressin from the circulating blood by the splanchnic viscera. To investigate this matter further, measurements were made in the anesthetized dog of the concentrations of vasopressin in arterial, portal venous, and hepatic venous plasma, and of total splanchnic plasma flow and hepatic arterial plasma flow. The total splanchnic vasopressin extraction ratio was 12.9 +/- 1.0%. However, the concentration of vasopressin in portal venous plasma was consistently lower than in arterial plasma, and there was a substantial prehepatic extraction of vasopressin, averaging 10.5 +/- 0.8%. A quantitative evaluation of the contribution of the "prehepatic" viscera, i.e., viscera with venous drainage into the portal vein, is provided by the relevant clearances of vasopressin. The prehepatic and total splanchnic vasopressin clearances were 1.58 +/- 0.20 and 3.04 +/- 0.31 ml X min-1 X kg-1, respectively. Thus, the splanchnic viscera other than the liver were responsible for approximately half of the splanchnic clearance of vasopressin; the remainder could be attributed to the liver. Immunoreactive vasopressin was not found in the bile. In splenectomized dogs, in which venous blood was collected from the superior mesenteric vein, the vasopressin extraction ratio was 14.6 +/- 2.3%, suggesting that the prehepatic clearance of vasopressin occurs largely in the mesenteric bed. A more specific localization of the prehepatic clearance sites has not as yet been made.

Effects of changes in steady state plasma vasopressin levels on renal and urinary vasopressin clearances in the dog.

A study was made of the effects of changes in the plasma vasopressin concentration on the extraction ratio and the renal organ and urinary clearances of vasopressin. Plasma vasopressin levels were increased in a stepwise fashion in anesthetized dogs by the iv infusion of vasopressin at rates of 100, 400, and 800 microU/min . kg. A steady state was achieved by infusing vasopressin for 60 min at each dose. Before the infusion of vasopressin, the extraction ratio and the renal and urinary clearances of vasopressin (one kidney) were 0.30 +/- 0.04, 1.8 +/- 0.2, and 1.4 +/- 0.1 ml/min . kg, respectively. The urinary clearance of vasopressin did not differ significantly from the inulin clearance (1.5 +/- 0.1 ml/min . kg). The infusion of vasopressin, which increased the plasma vasopressin concentration from an initial value of 4.3 +/- 1.3 to 54.6 +/- 2.4 microU/ml at the highest rate of infusion, was without effect on the vasopressin extraction ratio and the renal and urinary clearances of vasopressin. The MCR of vasopressin was estimated to be approximately 16 ml/min . kg. The renal clearance of vasopressin, calculated for both kidneys in all periods of all experiments, accounted for approximately 27% of the total clearance of vasopressin from the plasma. Thus, over a broad range of plasma vasopressin concentrations, a constant fraction of the vasopressin delivered to the kidney was removed from the blood perfusing the kidney, and the mechanisms for the renal extraction of vasopressin were not saturated.
