Influence of autonomic nervous activity on variations in the R-R intervals in adult goats.

We used a heart rate memory apparatus to measure 2047 successive R-R intervals with an accuracy of 10 msec in adult goats. For 100 successive stationary R-R intervals out of the 2047 intervals, the R-R intervals sequence was recorded graphically and the means and standard deviations (SD) were calculated. The magnitude of the variation in the R-R intervals was represented by the SD. The usefulness of the analysis of R-R intervals as an indicator of autonomic nervous activity was determined. The diagram of 100 successive R-R intervals at rest showed a conspicuous periodic fluctuation. After the administration of atropine, atropine plus propranolol or after feeding, the R-R intervals were markedly shortened and the variations in the R-R intervals were abolished. After the administration of propranolol only, the R-R intervals were prolonged and the amplitude of the fluctuations in the R-R intervals was conspicuously increased. The respiration curve and the variation in the R-R intervals at rest fluctuated in periodic synchrony. A positive correlation between the mean and the SD of the R-R intervals was highly significant. This indicates that the variations in the R-R intervals in the adult goats are closely related to autonomic nervous activity and that the analysis of the R-R intervals is suitable for the noninvasive assessment of autonomic nervous activity.

Atrial natriuretic polypeptide (ANP) as protective agent of renal ischemia.

The effect of atrial natriuretic polypeptide (ANP) on hemodynamics and renal function was evaluated after the reconstructive surgery of the left renal artery in a patient with renovascular hypertension secondary to Takayasu's arteritis. The reconstructive surgery was done using the femoral artery, since we were unable to obtain adequate vein segments to fit the renal artery. The femoral artery was reconstructed by her saphenous vein segments. After 30 min of the aortorenal bypass operation, alpha-human ANP (alpha-hANP) was infused intravenously for 10 min at a rate of 0.1 microgram/kg/min. Although total peripheral resistance was decreased by alpha-hANP infusion, blood pressure was not changed because of the increased cardiac output. Glomerular filtration rate was increased markedly with concomitant increase in urine volume and urinary excretions of sodium, potassium and phosphate. Fractional excretions of water and sodium were not changed, but fractional excretion of phosphate and potassium clearance were increased. Thus, the infusion of alpha-hANP markedly improved the renal function of the ischemic kidney by the reconstructive surgery of the renal artery, suggesting that alpha-hANP seems clinically applicable as a protective agent in renal ischemia at renovascular surgery as well as the renal transplantation.

[Evaluation of a radioimmunoassay of alpha 1-microglobulin (alpha 1-m) with simplified procedures].

A newly established double antibody radioimmunoassay (RIA) was fundamentally and clinically evaluated. Original procedures were partially modified as follows: Sample volume for serum and urine was changed to 25 microliters, and thus 200 mg/l of alpha 1-m standard was prepared using 50 microliters of original standard solution (100 mg/l). The results were satisfactory in sensitivity (0.3 mg/l obtained from -2SD method), intra-assay precision with its coefficient variation (CV) ranging from 3.0 to 7.4%, interassay precision with its CV ranging from 3.0 to 10.7%, and recovery with the mean value of 102.4% in serum and 108.2% in urine respectively. There were no changes about alpha 1-m value between diluted (2 times) and undiluted with high concentration samples. Normal levels of alpha 1-m were less than 25 mg/l in serum and less than 10 mg/l in urine. The present results indicate that the determination of alpha 1-m could be very simple and useful for the most sensitive screening test for the evaluation of renal function.

Preparation of 3-amino-3-deoxy derivatives of trehalose and sucrose and their activities.

The 3-keto derivatives of trehalose and sucrose respectively were prepared by a one-step enzymatic route using D-glucoside 3-dehydrogenase from Flavobacterium saccharophilum. Reductive amination of 3-ketotrehalose with ammonium acetate and sodium cyanoborohydride gave three compounds, 3-amino-3-deoxy-alpha-D-allopyranosyl alpha-D-glucopyranoside, 3-amino-3-deoxy-alpha-D-glucopyranosyl alpha-D-glucopyranoside (3-trehalosamine) and 3-amino-3-deoxy-alpha-D-mannopyranosyl alpha-D-glucopyranoside. From the reductive amination of 3-ketosucrose, 3-amino-3-deoxy-alpha-D-allopyranosyl beta-D-fructofuranoside and 3-amino-3-deoxy-alpha-D-glucopyranosyl beta-D-fructofuranoside were obtained. The antibiotic and glycohydrolase inhibitory activities of these 3-amino-3-deoxy derivatives were determined.

[Application of neuronal cell culture in human degenerative brain disorders].

Neuronal cell culture system has been used for the study of pathochemical evaluations in human degenerative brain disorders, particularly for Krabbe's disease and neuronal ceroid lipofuscinosis. To understand the pathochemistry of Krabbe's disease, we added psychosine into neuronal cell cultures and psychosine treated cells showed the destruction of cytoskeleton and pathy intracellular changes. Electron microscopic finding showed the swelling of the mitochondria. Oligodendrocytes and Schwann cells were isolated from the brains and sciatic nerve of twitcher mouse as an authentic murine model of globoid cell leukodystrophy. Oligodendroglial cells cultured for 22 days were stained by anti-galactocerebroside antibodies. In twitcher oligodendrocyte processes were wirelike and progressively degenerated and there were few membranous expansion. Schwann cells from twitcher could not elongated their processes. These data suggest that psychosine might be important factor to result in these pathological conditions. Furthermore, we studied the effect of protease inhibitors, E-64 on dissociated primary cultures from fetal rat brain. After treated with E-64 in a concentration from 0.1-50 micrograms/ml, numerous cytoplasmic accumulations appeared in neuronal cells. These morphological pictures resemble with those of neuronal ceroid lipofuscinosis, Batten disease. We will discuss the relationship between the deficiency of catepsin H in Batten disease and inclusion bodies found in E-64.

[A case of tardive dystonia].

Tardive dystonia in a chronic dystonia caused by neuroleptics. A 53-year-old man suffering from a neuroleptic induced dyskinesia began to show an abnormal posture. His abnormal posture was caused by changes of muscle tonus and thought to be a dystonic posture. He had no family history of dystonia. This posture was similar to that of idiopathic dystonia in that the muscle tonus was hypertonic in sitting but was hypotonic in lying, and in that the activity of daily living was not disturbed in spite of hypertonia. But he also showed lingual dyskinesia and hyperreflexia and the electromyographic analysis disclosed the fact that his dystonia was similar to that of secondary dystonia. Brain CT showed the atrophy of the head of caudate nucleus but superconducting MRI disclosed no abnormality in basal ganglia. No effective therapeutics was as yet found in tardive dystonia. So it was proposed that the tardive dystonia was due to irreversible functional damage to the basal ganglia.

[The kinesiological, chemical and pathological analysis in pulsed magnetic stimulation to the brain].

Pulsed magnetic stimulation of the human brain and spinal region has been reported recently. Unlike electrical stimulation, magnetic stimulation excites the motor cortex without discomfort to the subject. This method will be used as a new clinical test to study the central motor pathway. Although no deleterious effects have been observed thus far, the safety of this technique is regarded as unproven. We have investigated kinesiological, neurochemical and pathological analysis. Our pulsed magnetic discharge system consists of a high voltage capacitor bank and flat circular coil of insulated copper wire. The high voltage capacitor bank has a maximum voltage of 900 V, a maximum current flow of 8,000 amp and 1,637 uF in condenser capacitance. Sixty four normal wistar rats each weighing 200 g were used in this study. The rats were separated into two groups. Rats in one group received pulsed magnetic stimulation 50 times in 0.5 Hz by a flat circular coil which surrounded the head of rat at 1 cm in front of the interauricular line. The rats were housed in a long circular chamber. Rats in the other group did not receive the pulsed magnetic stimulation in the long circular chamber. The details of kinesiological analysis by Animex II measurement were described in an other paper (Act Neurologica Scandinavica 73; 352-358, 1986). The measurement of monoamines, dopamine (DA), homovalinic acid (HVA), noradrenaline (NA), and 5-hydroxytryptamine (5-HT), were made according to the Mefford's method 1 hour and 4 days after the magnetic stimulation. The analysis of the pathological state was also studied 1 hour and 4 days after the magnetic stimulation.(ABSTRACT TRUNCATED AT 250 WORDS)

[Coronary artery bypass grafting in a patient with Sjögren's syndrome and other autoimmune diseases].

Coronary arterial involvement in collagen disease is a very rare complication. Particularly in Sjögren's syndrome is there no report of coronary artery bypass grafting (CABG). We report a 68-year-old woman with various autoimmune diseases who underwent successful CABG for unstable angina pectoris. She had a history of Sjögren's syndrome, hypothyroidism, thrombocytopenia and lupoid hepatitis, and had been on the regimens of prednisolone and levothyroxine sodium during the last 4 years. Two months prior to admission when the doses of these drugs were increased, she began to complain of angina on mild exertion and occasionally even at rest. Selective coronary angiograms revealed 75% stenosis at Seg. 2 and 99% stenosis at Seg. 6. Based on these findings and severity of the angina, saphenous vein CABG to the posterior descending and the left anterior descending branches was performed. Despite the associated autoimmune diseases, the patient's postoperative course was uneventful and she is now doing well with no attack of angina, 12 months following the operation.

[PTCA and coronary artery bypass operation prior to subsequent surgery of gastric cancer in patients with coronary artery disease].

Two patients with coronary artery disease required operation for gastric cancer. In one patient with single vessel disease, 37 days later after successful PTCA, gastric resection for early gastric cancer was performed without any cardiac complication. The other patient with severe three vessel disease initially underwent coronary artery bypass operation, and 13 days later gastric resection for advanced cancer was performed. This patient developed supraventricular arrhythmia as a sole complication which was easily controlled by medical treatment. In both cases, fentanyl was used as an anesthetic agent and nitroglycerin was continuously administered intravenously during the perioperative period. ECG, central venous pressure, and direct arterial pressure were also carefully monitored. We feel that noncardiac operation such as gastric resection can be performed without major cardiac complications even shortly after coronary artery bypass operation.

[Comparison of effects on carbohydrate metabolism of hypotension induced by sodium nitroprusside and a nitroprusside-trimetaphan mixture in rabbits].

Effects on carbohydrate metabolism of hypotension induced by sodium nitroprusside and a nitroprusside-trimetaphan mixture were studied in 29 male rabbits under halothane anesthesia. They were randomly divided into three groups; Nitroprusside (Group N; n = 10), A Nitroprusside-Trimetaphan Mixture (Group M; n = 10), Control (Group C; n = 9). No changes were noted in blood glucose, blood lactate, blood pyruvate and lactate/pyruvate ratio in Group C throughout the experiment. During induced hypotension, blood glucose, blood lactate, blood pyruvate and L/P ratio of group N were significantly higher than the control value. On the other hand, in group M, blood glucose and blood pyruvate tended to increase slightly but not significantly. In contrast, in group M, L/P ratio remained unchanged. The results suggest that sodium nitroprusside tended to cause tissue hypoxia more markedly than a nitroprusside-trimetaphan mixture. The mechanism may be that a mixture of nitroprusside and trimetaphan maintained the carbohydrate metabolism (including gluconeogenesis, glucogenolysis) during induced hypotension. In conclusion, our results suggest that a nitroprusside-trimetaphan mixture is a safe method for hypotensive anesthesia.

[Multiple cerebral venous angiomas associated with mixed pial and dural arteriovenous malformation].

Venous angioma is a relatively rare entity of vascular malformations of the brain and usually found as a solitary lesion. Cases of multiple lesions and/or coupled with other vascular malformations are extremely rare. We present the first case of multiple supratentorial venous angiomas associated with mixed pial and dural arteriovenous malformation (AVM) fed by the posterior cerebral artery, the occipital artery and the posterior meningeal artery.

[Mutagenicity of steviol: an analytical approach using the Southern blotting system].

Steviol is the aglycone of stevioside, which is a non-caloric sugar substitute commonly used in Japan. Our previous studies and Pezzuto et al. have demonstrated that steviol is mutagenic after metabolic activation in the forward mutation assay using Salmonella typhimurium TM677 (TM677), whereas it is non-mutagenic in the reverse mutation assay (Ames test) using S. typhimurium TA 100, TA98, TA102 and TA97. There is the possibility, therefore, that activated steviol selectively induces a deletion or insertion of more than one base pair which cannot be detected by strains commonly used in the Ames test. In this study, we confirmed first that the 8-azaguanine (8-AG) resistance of the TM677 mutants appears to reside in the chromosomal gpt gene, since it can be complemented by the gpt gene of E. coli (Ecogpt). The chromosomal DNA of TM677 and TM677 mutants were digested by several restriction enzymes: BamHI, Sau3AI, AluI, TaqI, HaeIII, HpaII and RsaI, and analyzed by the Southern blot hybridization technique with a probe to the gpt gene DNA of E. coli. No significant differences in DNA fragment length, however, were formed between the wild type and spontaneous or steviol-induced mutants.

Restorative effect of muroctasin on leukopenia caused by anticancer chemotherapy in lung cancer. Comparative study by envelope method.

N2-[(N-Acetylmuramoyl)-L-alanyl-D-isoglutaminyl]-N6-stearoyl-L-lysine (MDP-Lys(L18), muroctasin), a derivative of muramyl dipeptide, is known to have the activity to augment the number of white blood cells (WBC) via colony-stimulating factor. Muroctasin has been expected to be applied to leukopenia caused by anticancer chemotherapy. When WBC decreased to less than or equal to 3,000/mm3 after the 1st course of chemotherapy, 131 patients with lung cancer, who were previously classified by the combination regimens of chemotherapy, were enrolled in the study and randomized into 3 groups, 200 micrograms (H), 100 micrograms (L) and untreated control (C) groups. The patients were then subcutaneously treated once daily for 6 consecutive days. WBC and its differential count were measured on Days 4, 7 and 15 after commencement of the study. WBCs in H and L groups were recovered greater than in C group. In WBC differential count, the recovery of neutrophil was prominent in muroctasin treated groups. The portion of immature neutrophil in the bone marrow was also increased by muroctasin treatment. A restorative effect on WBC and neutrophil counts was also confirmed only in the second course of H group. On the other hand, fever and pain in the injected site as side effects were common in the H group and L group in both of courses. In this study, the usefulness of muroctasin in leukopenia was suggested when administered at dosages of 200 micrograms for 6 days.

[Comparative study of in vitro diagnosis by the skinned fiber test and clinical diagnosis of malignant hyperthermia].

Skeletal muscles from twenty patients suspected of malignant hyperthermia during general anesthesia were examined with the skinned fiber test for abnormality in the Ca2+ release function of their sarcoplasmic reticulum, and the result was correlated with the classifications of the patients in accordance with the clinical criteria now used in Japan. Among them, fourteen patients were diagnosed as clinical malignant hyperthermia syndrome (fulminant syndrome: 4, abortive syndrome: 10). Three out of four fulminant cases showed potentiated Ca-induced Ca release mechanism of the sarcoplasmic reticulum in their skeletal muscles. The others including six patients not diagnosed as malignant hyperthermia were normal in the sarcoplasmic reticulum function. This suggests that the skinned fiber test has comparatively high sensitivity to fulminant malignant hyperthermia syndrome. The present paper discusses, based on the present results, the problems resulting from the difference in diagnostic concept between in vitro criteria by the skinned fiber test to detect a specific pathogenetical factor in the skeletal muscle of a single abnormality and clinical criteria to pick up a series of relatively non-specific symptoms and signs of a clinical syndrome.
