[Fracture of the coracoid process: its classification and pathomechanism].

Twenty-eight cases of fractures of the coracoid process were assessed regarding general characteristics, causes of injury, regions of contused wound, fracture types and complications. From the results of the assessment, the fractures were roughly divided into five types: type I; apical, type II; middle, type III; basal, type IV; intraarticular (including upper 1/3-1/4 of the glenoid cavity), and type V; others. Subtypes A and B were present in type III and IV. From the specifics of the contusions and complicated osseous-articular injuries around the shoulder, the mechanisms of injury in each type may be suggests as follows: type I and II; tractive force of attached muscular tendons, type III and IV; shearing force acting between the scapula shifting inward due to blow from the lateral aspect of the shoulder and the clavicle countering this movement, and type V; unspecified. The main cause of fractures of the superior scapular margin was considered to be the tractive force of the superior transverse scapular ligament, because almost all cases of subtype A were complicated with them. For the benefit of the clinical use, they should be classified into two groups: group A; without loss of connection between scapula and clavicle (mainly types I and II), and group B; with loss of the same (majority of types III and IV).

[Coronary artery disease and the results of coronary bypass surgery in diabetics].

To evaluate the influence of diabetes mellitus on coronary artery disease and the results of coronary bypass surgery, a review was made of 63 consecutive patients undergoing isolated saphenous vein aortocoronary bypass, of whom 38 patients (G-1) were nondiabetic, 9 patients (G-2) had impaired glucose tolerance, and 16 patients (G-3) were diabetic. The severity of coronary artery disease was assessed using an angiographic grading system. Among three patient groups, there was no difference in total coronary score per patient reflecting total extent of disease (G-1; 15.7 +/- 5.5, G-2; 14.1 +/- 5.9, G-3; 17.1 +/- 6.3, mean +/- S.D.) and the incidence of diffusely diseased vessels. The mean number of diseased vessels (greater than 50% stenosis) per patient was 2.5 +/- 0.6 in G-1, 2.6 +/- 0.7 in G-2, and 2.5 +/- 0.7 in G-3, and the mean number of bypass grafts per patient was 2.3 +/- 0.9, 2.2 +/- 0.9, and 2.1 +/- 0.7, respectively. Coronary luminal diameters and the severity of atherosclerotic changes of coronary arteries at the site of graft anastomosis, studied intraoperatively, were similar in all groups. Vein graft blood flows and early graft patency in diabetics (112 +/- 55 ml/min, 96.9%, respectively) were also similarly good as those in the other groups (98 +/- 49 ml/min, 91.4% in G-1, and 92 +/- 39 ml/min, 94.1% in G-2). Overall hospital mortality was 1.6% (one of 63). The only death from meningoencephalitis occurred in the diabetic group.(ABSTRACT TRUNCATED AT 250 WORDS)

[CT evaluation of blow-out fractures].

Fifty patients with blow-out fracture were analysed in detail by using CT. Among these cases, 38 and 12 cases were classified as pure type and impure type fractures, respectively. The lesion of pure type noted in five positions of the orbit; floor, roof, medial wall, corner, and floor with medial. The shape of fracture fragments were observed as trap door, punched out, broad and linear in floor type fracture. Herniated ophthalmic muscles were detected in approximately half of the pure type fractures. CT scan plays an important role in the decision to operate on blow-out fracture.

[Renal accumulation of technetium-99m-labeled bone imaging agents in patients treated with cisplatin].

As the routine follow-up studies in patients with malignant diseases, bone scintigraphy is often performed to detect bone metastasis, before, during and/or after the treatments. Among these patients who were treated, especially, with CDDP, we often encountered the markedly increased renal radioactivity during the follow-up period by bone scintigraphy. In this study, 68 patients with 76 bone scintigraphy performed within 30 days after the administration of CDDP were evaluated. Forty-five of the 76 bone scintigraphy (60.5%) in the 68 patients exhibited more prominent accumulation of the radionuclide in the kidneys than that normally seen. Among them, twenty-four cases (31.6%) showed markedly increased accumulation of the radionuclide in the kidneys so called the "hot kidneys", when the bone scintigraphy was performed later than 3 weeks after the treatment with CDDP. Moreover, the frequency of the "hot kidneys" were well correlated with the degree of renal damage in terms of serum creatinine levels. These findings suggest that the "hot kidneys" might represent the presence of transient renal damage caused by the administration of CDDP. As the causes of this finding called as the "hot kidneys", various factors and/or many causes are reported recently, however, the highest incidence of this phenomenon is thought to be occurred by CDDP as the factor of drug incidence.

[Clinical application of FCR (Fuji Computed Radiography) for portable chest radiographs in the ICU].

The diagnostic value of the portable chest radiography obtained with Fuji computed radiography (FCR) was evaluated in comparison with conventional images in 23 patients who were admitted to ICU in Yokohama City University Hospital. On the whole, FCR produced more excellent images than conventional screen-film radiographs. Especially the evaluation for the demonstration of the catheters, the endotracheal tubes and the tracheal shadows obtained high score. We discuss the clinically utility of FCR for the portable chest radiographs in ICU.

B cell response pathways regulated by IL-5 and IL-2. Secretory microH chain-mRNA and J chain mRNA expression are separately controlled events.

We have examined the effect of IL-5 and/or IL-2 on the expression of the secretory form of microH chain (microsecond) and J chain mRNA in a homogeneous neoplastic B cell clone, in which proliferation, IL-2R up-regulation and entry into the IgM secretory state are separately controlled events. The IL-5 signal triggers a partial induction of CL-3 cells into the IgM secretory state, characterized by a striking increase of microsecond mRNA expression and an increase in the ratio of the secretory to membrane forms of microH chain mRNA, with a modest increase of J chain mRNA. In contrast, amplification of J chain mRNA is accomplished by the late-acting B cell differentiation stimulus, IL-2, acting on IL-5-pretreated CL-3 cells or acting simultaneously with IL-5 on CL-3 cells. Such dually stimulated cells now are fully induced into IgM secreting cells. These results define the relative roles of IL-5 and IL-2 in B cell differentiation by showing important regulatory effects at the mRNA level. In addition, these results substantiate that appearance of mRNA for J chain, a molecule key to the formation of pentameric IgM, is a limiting factor for high level IgM secretion. The separate control of microsecond and J chain mRNA found in CL-3 cells stimulated with IL-5 and IL-2 elucidates a molecular mechanism by which these two lymphokines synergize in the development of CL-3 cells into IgM secreting cells.

[High-resolution ultrasound of the thyroid in patients with Graves' disease].

The thyroid glands in 53 patients with Graves' disease were scanned with 10-MHz high-resolution ultrasound. Homogeneous echo pattern was noted in 21 patients (39.6%), and heterogeneous echo pattern was noted in 31 patients (60.4%). Each mean value of the serum triiodothyronine (T3), thyroxine (T4), and free thyroxine (Free T4) in patients with heterogeneous-echo thyroid was noted higher than that of the group with homogeneous-echo thyroid. (p less than 0.05) However, between the two groups, there is no significant difference in mean age and mean duration of treatment with antithyroid drugs. The heterogeneous echo pattern is strongly suggested to be correlated with the severity of hyperthyroidism.

Establishment of tumor cell lines from a patient with head and neck cancer and their different sensitivities to anti-cancer agents.

The authors established five cell lines from a human head and neck tumor. The five cell lines (HC-2, HC-3, HC-4, HC-7, and HC-9) exhibited different sensitivities to Adriamycin, cisplatin, bleomycin, 5-fluorouracil, vincristine, and daunomycin. The D50 was 200 ng/ml Adriamycin (doxorubicin) for HC-7 and 45 ng/ml for HC-2. At the inception of long-term culture (11 months) in the absence of any drug, the sensitivity to Adriamycin of HC-7-5 (subcloned from HC-7) was 3.4 times greater than that of HC-2-6 (subcloned from HC-2); by 11 months, it decreased to 1.6 times that of HC-2-6. The cytocidal action of Adriamycin on HC-2-6 and HC-7-5 was potentiated when Adriamycin was combined with verapamil or cepharanthine. Cepharanthine also potentiated daunomycin and vincristine (VCR) against HC-2-6 and HC-7-5 cells, and it almost completely overcame drug-resistance to daunomycin and vincristine in HC-7-5/VCR, a multidrug-resistant variant isolated after long exposure to vincristine of HC-7-5 cells in culture. The cellular accumulation of [3H]-daunomycin by HC-7-5 cells was about 70% that of HC-2-6 cells. By Northern blot analysis, using a multidrug-resistance gene (mdr-1) probe, neither HC-2-6 nor HC-7-5 expressed the mdr-1 gene, but HC-7-5/VCR or other multidrug-resistant variants showed active expression of the mdr-1 gene. Differential sensitivities among the five cell lines to 5-fluorouracil, cisplatinum, and bleomycin appear to be mediated through other mechanism beside the mdr-1 gene.

Electron-microscopic study on the anterior pituitary gland of spontaneous dwarf rats.

The spontaneous dwarf rat is a novel experimental model animal on the study of pituitary dwarfism. The fine structure of the anterior pituitary cells was studied in the immature and mature dwarf rats. Pituitary glands were removed from 5-, 10-, 20-day-old immature dwarfs, adult (45 days-16 weeks) dwarfs and normal 3-month-old rats and processed for electron-microscopic observation. In the control animals, growth hormone cells were readily identified by their ultrastructural characteristics, such as the presence of numerous electron-dense secretory granules, 300-350 nm in diameter, well developed rough endoplasmic reticulum and a prominent Golgi complex. In contrast, growth hormone cells were not found in the anterior pituitary gland of the spontaneous dwarf rat at any age examined. Other pituitary cell types, i.e., luteinizing hormone/follicle stimulating hormone, thyroid stimulating hormone, adrenocorticotropic hormone and prolactin cells, appeared similar in their fine structure to those found in the control rats. In the pituitary gland of dwarf rats, a number of polygonal cells were observed either with no or relatively few secretory granules. The rough endoplasmic reticulum was arranged in parallel cisternae and the Golgi complex was generally prominent in these cells. In addition, many were found to have abundant lysosomes. A few minute secretory granules were occasionally observed; however, the immunogold technique failed to localize growth hormone or prolactin in the granules. Quadricuspid aortic valve--report of a case associated with severe aortic regurgitation and review of the literature.

A case of a 59-year-old woman who had a quadricuspid aortic valve associated with severe aortic regurgitation is reported herein. The anomaly was revealed by aortography, after which the incompetent valve was excised and replaced by a St. Jude Medical prosthesis. A quadricuspid aortic valve must be considered as a malformation which leads to severe valve failure in later life.

Three-year disease-free survivors of small cell lung cancer treated with combination chemotherapy with or without chest irradiation.

One hundred and seventy-four patients with small cell lung cancer (SCLC) treated with combination chemotherapy, with or without chest radiation, were analyzed. Fourteen patients (8%) survived for 3 years or more. Three-year disease-free survival continued for 12 of the 101 patients (12%) with limited disease, and one of 75 (1%) with extensive disease (P less than 0.05). Patients' sex and performance status were not important in achieving long-term survival. All disease-free survivors, except two who could not be evaluated, achieved a complete response. Although the treatment programs had some influence on the long-term survival rates (P less than 0.05), thoracic radiation did not have significant impact on long-term survival. Three of the 13 patients (23%) developed second malignancies and died, and one of these patients also suffered from a progressive neurologic deterioration with dementia. Two other patients died free of SCLC. Consequently, eight have remained alive and free of disease. The last relapse was observed at 1.5 years from beginning of treatment. The disease-free survival may offer the hope of cure of SCLC. However, the survivors are at an increased risk of developing late complications including second malignancies and neurologic abnormalities. Therefore, careful follow-up will be necessary.

Protective immunity induced by porin in experimental mouse salmonellosis.

The induction of protective immunity to mouse salmonellosis by porin from Salmonella typhimurium LT2 was studied. The immunization with porin induced a high level of protective immunity to salmonellosis in BALB/c mice. Mice immunized with porin exhibited significant levels of delayed-type hypersensitivity response and interleukin-2 production, indicating that porin was capable of inducing cell-mediated immunity (CMI). Furthermore, we found that both T cells and sera taken from the porin-immunized mice could transfer the protection against salmonellosis into nonimmunized mice. These observations suggested that a high level of the protection to salmonellosis obtained by the porin immunization resulted from the induction of CMI in addition to humoral immunity.

Specificity of Salmonella porin as an eliciting antigen for cell-mediated immunity (CMI) reaction in murine salmonellosis.

The specificities of Salmonella porin on elicitations of delayed-type hypersensitivity (DTH) reaction and interleukin-2 (IL-2) production in BALB/c mice immunized with Salmonella typhimurium were examined. Only porin from S. typhimurium was capable of eliciting significant levels of DTH and IL-2 production in S. typhimurium-immunized mice, whereas no significant DTH and IL-2 production were induced by porin from Salmonella enteritidis or Escherichia coli. Our observations suggested that Salmonella porin was a serovar-specific antigen for the elicitation of cell-mediated immunity (CMI) in salmonellosis.

A case of infantile digital fibromatosis showing spontaneous regression.

A case of infantile digital fibromatosis (IDF) in a 3-year-old girl is reported. Two tumours were present on the left fourth toe and one over the lateral aspect of the left heel. Histological examination revealed that the tumor cells contained intracytoplasmic inclusion bodies, characteristic for this condition. The tumours showed spontaneous regression without therapy.

Attenuation of the osmotic release of vasopressin by enkephalins in dogs.

To assess the possible role of circulating enkephalins in the osmotic release of vasopressin, Met5 (M-Enk)- or Leu5-enkephalin (L-Enk) dissolved in 0.9 or 10% NaCl was infused to either anesthetized or conscious dogs at a dose of 3.5 nM.kg-1.min-1 for 15 min. Intravenous infusion of M-Enk or L-Enk produced 150- to 200-fold increases in their plasma levels, and the elevated levels were maintained during the infusion. Although blood pressure (BP) in anesthetized dogs decreased significantly during L-Enk infusion, BP in conscious dogs was unaffected by both enkephalins. Neither M-Enk nor L-Enk infusion affected plasma vasopressin concentration (PAVP) in these anesthetized and conscious dogs that were not osmotically stimulated. PAVP in conscious time control dogs increased significantly after start of 10% NaCl infusion in spite of an increase in BP. M-Enk or L-Enk significantly blunted the increase in PAVP induced by 10% NaCl infusion. This attenuation was not accompanied by any significant changes in plasma sodium concentration and BP compared with those of time control dogs. Thus increase in plasma enkephalins attenuates the osmotic release of vasopressin.

Relation of intrinsic heart rate and autonomic nervous tone to resting heart rate in the young and the adult of various domestic animals.

Intrinsic heart rate (IHR) and autonomic nervous tone (ANT) were measured using the young and the adult of horses, cows, pigs, goats and chickens in order to elucidate species differences in a decrease of resting heart rate (RHR) with growth or age. The IHR and ANT were estimated from the changes in heart rate after the administration of atropine and/or propranolol. The IHR in all species decreased progressively with an increase in body weight from young to adult, and moreover the ANT altered toward the direction of parasympathetic predominance by a decrease in sympathetic tone and/or an increase in parasympathetic tone. The decrease of the RHR with growth resulted from a decrease in the IHR primarily and from a parasympathetic predominance in the ANT secondarily. A considerable species difference existed in the alteration of the ANT.
