Administration of truncated secretory leukoprotease inhibitor ameliorates bleomycin-induced pulmonary fibrosis in hamsters.

The purposes of this study were to investigate the effect of our recently developed truncated secretory leukoprotease inhibitor (SLPI) on bleomycin (BLM)-induced lung injury and fibrosis in hamsters, which is widely used as a model of interstitial pulmonary fibrosis, and to assess the possible involvement of neutrophil elastase in the pathogenesis of pulmonary fibrosis. The fibrosis model was prepared by administration of BLM (10 mg/kg) intratracheally to hamsters. The truncated SLPI was administered at a dose 5 or 15 mg/kg intraperitoneally twice a day only for 10 d starting from the time of BLM administration. BLM administration resulted in decreases in body weight and survival rate. Elastase activity in the supernatant of bronchoalveolar lavage (BAL) fluids was increased at 3 and 7 d after BLM administration in parallel with the increase in the number of neutrophils in the fluids. Histopathologically, at 14 and 28 d after BLM administration, diffuse thickening and fibrosis of alveolar walls with marked cell infiltration and severe distortion of alveolar structure, including honeycomb lung, were observed to have occurred. In this model, the decreases in body weight and survival rate and the increase in elastase activity were inhibited by the truncated SLPI dose-dependently, and pulmonary fibrosis was significantly ameliorated by the administration of this shortened form of SLPI. These results suggest that neutrophil elastase may be implicated in the pathogenesis of BLM-induced pulmonary fibrosis and that the truncated SLPI might be a promising therapeutic in the treatment of interstitial pulmonary fibrosis.

Type D dural carotid-cavernous fistula. Results of combined treatment with irradiation and particulate embolization.

PURPOSE: To assess the efficacy of irradiation combined with particulate embolization for treating type D dural carotid-cavernous fistulas, supplied by the meningeal branches of both external (ECA) and internal carotid arteries. MATERIAL AND METHODS: Nine patients were treated by particulate embolization via the ECA branches, followed by radiation therapy with doses of 30 Gy. One of these patients with recurrent symptoms received an additional 30 Gy one year after initial treatment, making a total dose of 60 Gy. RESULTS: In all cases, symptoms were improved immediately after the particulate embolizations. In 8 cases, complete resolution of symptoms were obtained in 4-19 months and an average of 8 months after treatment. In one case, chemosis was markedly improved and persisted minimally 41 months later. CONCLUSION: Particulate embolization is effective for immediate symptomatic improvement, and complete resolution of symptoms can usually be achieved by subsequent radiation therapy.

Absence of lysosomal cleavage in the cytotoxicity mechanism of an immunoconjugate composed of anti-alpha-fetoprotein monoclonal antibody and vindesine analog.

The effect of lysosomal enzyme inhibition on the cytotoxic activity of an immunoconjugate composed of anti-alpha-fetoprotein monoclonal antibody and vindesine analog (VDS) was studied in vitro using human tumor clonogenic assay (HTCA). Addition of the lysosome enzyme inhibitors, leupeptin and ammonium chloride, to the HTCA system had little influence on the cytotoxicity of this immunoconjugate. In separate experiments, no released VDS was detected by HPLC after incubation with the supernatant of rat liver homogenate without inhibitor. These results show that the immunoconjugate may bypass the lysosomal process and exert its activity as an intact or similar form.

Anti-tumor-promoting activities of triterpenoids from ferns. I.

To search for possible anti-tumor-promoters (cancer chemopreventive agents), we carried out primary screening of 23 triterpenoid hydrocarbons (1-23) isolated from ferns using an in vitro synergistic assay system. Of these triterpenoids, hop-17(21)-ene (2), neohop-13(18)-ene (3), neohop-12-ene (4), taraxerane (17), multiflor-9(11)-ene (18), multiflor-8-ene (19), glutin-5(10)-ene (21) and taraxastane (23) exhibited remarkable inhibitory effects on Epstein-Barr virus (EBV) activation induced by the tumor promoter, 12-O-tetradecanoylphorbol-13-acetate (TPA). Further, compounds 2 and 3 exhibited remarkable anti-tumorpromoting effects on mouse skin tumor promotion in an in vivo two-stage carcinogenesis test using 7,12-dimethybenz[a]anthracene (DMBA) as an initiator and TPA as a promoter.

[HLA class II alleles and leprosy (Hansen's disease) classified by WHO-MDT criteria].

Human leukocyte antigens (HLA) class II alleles were analyzed in Japanese leprosy patients to ascertain whether immunogenetic differences exist among the forms of leprosy in classification of World Health Organization-recommended multidrug therapy (WHO-MDT). The subjects were 86 unrelated Japanese leprosy patients, including 62 multibacillary leprosy (MBL), 24 paucibacillary leprosy (PBL). Controls were 114 unrelated healthy subjects. Genotyping of HLA class II alleles was performed by using the polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP) and PCR-restriction fragment length polymorphism (RFLP) methods. The frequencies of HLA-DRB1* 1501, * 1502 and DRB5* 0101,* 0102 and DQA1* 0102 and DQB1* 0602 were significantly increased in the whole patients (44.2%, 34.9%, 44.2%, 34.9%, 53.4% and 41.9%, respectively) as compared with the control subjects (14.0%, 21.1%, 14.0%, 21.1%, 27.2% and 13.2%, respectively). On the other hand, the frequencies of HLA-DRB1* 0405, * 0803, * 0901 and DQA1* 03 and DQB1* 0401 were significantly decreased in the whole patients (10.5%, 5.8%, 16.3%, 41.9% and 9.3%, respectively) as compared with the control subjects (29.8%, 17.5%, 30.7%, 78.1% and 29.8%, respectively). When MBL and PBL patients were compared, the frequencies of HLA-DRB1* 1501, DRB5* 0101 and DQB1* 0602 were significantly increased in the MBL patients (51.6%, 51.6% and 48.4%, respectively) as compared with the PBL patients (25.0%, respectively). Our results suggest that HLA-DRB1* 1501, DRB5* 0101 and DQB1* 0602 contribute to the susceptibility to the Japanese MBL.

[Endoscopic treatment of gastrointestinal cancers--indication and limitation].

Recently, endoscopic treatments have been applied for the curative treatment of early gastrointestinal cancers and for the palliative treatment of end-stage gastrointestinal cancers. As a curative treatment for early gastric cancers, the endoscopic Nd-YAG laser therapy was initially applied and a satisfactory results were obtained. However, the endoscopic mucosal resection (EMR) is evaluated as the reliable endoscopic treatment for early gastric cancers because curability can be histologically detected by resected specimen. EMR technique is also applied for early esophageal cancers and early flat or depressed colon cancers. EMR-L (EMR with the use of Ligating device) developed by us is mainly practiced in our department. By the use of EMR-L technique. Resectability has been improved compared with 2 channel EMR technique. As the palliation of end-stage gastrointestinal cancers endoscopy recanalization of malignant esophageal stenosis is very effective to improve the patient's QOL. The memorial metallic stent for esophageal stenosis is very useful to keep cavity for a long term after endoscopic recanalization. PEG (percutaneous endoscopic gastrostomy) is also very effective to reduce the symptoms of the patients with peritonitis carcinoma or ileus. In this paper, the present status of curative and palliative endoscopic treatment for gastrointestinal cancers was reported. In recent future, endoscopy will be more widely applied as a less invasive therapeutic procedure for gastrointestinal cancers.

[Evaluation of the usefulness of 111In-DTPA-IgG scintigraphy for the detection of focal inflammatory lesions].

We evaluated the usefulness of 111In-DTPA-IgG scintigraphy for the detection of focal inflammatory lesions. We examined 9 patients with focal inflammatory lesions that had been diagnosed based on the findings of various types of laboratory data and other imaging modalities. Scanning was performed at 6, 24, 36 and 48 hours after the administration of 80 MBq of 111In-DTPA-IgG. The sensitivity was 78%. A good diagnostic ability was observed by the scintigraphy findings at 24 hours. In conclusion, 111In-DTPA-IgG scintigraphy at 24 hours is thus considered to be useful for detecting focal inflammatory lesions.

Serum levels of hyaluronic acid indicate the severity of joint symptoms in patients with systemic and polyarticular juvenile rheumatoid arthritis.

OBJECTIVE: Elevated serum levels of hyaluronic acid (HA) correlate with joint inflammation in adult rheumatoid arthritis (RA). There are no laboratory indices for specifically assessing joint inflammation. Therefore, serial measurements of HA were assessed as a possible tool for measuring the severity of arthritic symptoms in children with juvenile rheumatoid arthritis (JRA). METHODS: Serum levels of HA, measured by a sandwich assay method using HA binding protein, were correlated with the severity of joint symptoms and with laboratory test values in 71 patients with JRA, 30 children with other rheumatic diseases, and 138 children without rheumatic disease. RESULTS: Serum levels of HA showed significant correlation with the severity of joint symptoms, but not with systemic symptoms, in patients with systemic and polyarticular JRA. No other laboratory tests, including C-reactive protein and erythrocyte sedimentation rate, reflected the severity of joint symptoms. This correlation of serum levels of HA with joint symptoms was observed in patients with systemic and polyarticular JRA, but not in pauciarticular JRA, other rheumatic diseases, or nonrheumatic diseases, even when signs of arthritis were present in the latter 3 groups. CONCLUSION: Serum levels of HA are useful in objectively evaluating arthritic symptoms in patients with systemic and polyarticular JRA, and may have diagnostic value in this disease.

Age-related change of technetium-99m-HMDP distribution in the skeleton.

UNLABELLED: To understand age-related changes of whole-body and regional skeletal metabolism, it is important to investigate the mechanisms of age-related bone loss and to develop suitable treatments for it. Bone biopsies show metabolism of the particular site examined while biochemical markers for bone metabolism reflect total skeletal metabolism. Bone scintigraphy is a convenient and simple way to analyze whole-body and regional skeletal metabolism. We attempted to study and understand age-related changes in bone metabolism by quantifying the bone scan and correlating it with biochemical bone metabolic markers. METHODS: The whole-body skeletal uptake (WBSU) and tracer distribution pattern were studied in men and women by bone scintigraphy using 99mTc-hydroxy-methane-diphosphonate (HMDP). Bone scans were performed using a standard protocol and quantified by setting regions of interest (ROIs) on selected regions. WBSU and the skeletal distribution pattern were compared with simultaneously obtained serum biochemical markers. RESULTS: WBSU showed an increase with age in both sexes, but in women, uptake in the head and legs increased more relatively than in the thoracic region, while in men no such tendency was observed. Increase of WBSU and relative increase of uptakes in the head demonstrated a weak correlation with the serum levels of alkaline phosphatase and type 1 collagen metabolites. CONCLUSION: These results show an age-related increase of skeletal turnover and sex-dependent regional skeletal metabolism. The age-related changes seen in bone scintigrams might be a sign of progressive bone loss, reflecting changes in local bone metabolism.

Hyperperfusion and hypermetabolism in brain radiation necrosis with epileptic activity.

We report a case of high uptake of 11C-methionine (MET), 18F-FDG (FDG) and 201Tl-Cl (Tl) in brain radiation necrosis. Twenty-one years previously, the patient had undergone surgery and radiation therapy consisting of 60-Gy for ependymoma in the anterior horn of the right lateral ventricle. The clinical features consisting of frequent seizures of the left face and arm suddenly appeared 2 wk before admission. MRI depicted a Tl and T2-prolonged lesion in the right frontal lobe. Abnormally high uptake in this area demonstrated by MET-PET, FDG-PET, Tl-SPECT or HMPAO-SPECT suggested the presence of a recurrent tumor. A craniotomy was then performed and an intraoperative electrocorticogram showed continuous epileptic spikes in the lesion. The epileptic foci were resected and the histological features of the lesion were consistent with radiation necrosis. After surgery, the seizures disappeared and the postoperative examinations with MET-PET, FDG-PET, Tl-SPECT and HM-PAO-SPECT no longer showed abnormally high uptake. Hypermetabolism and hyperperfusion related to epileptic fits are therefore thought to result in high uptake of MET, FDG and Tl in radiation necrosis.

Current trends in cataract and refractive surgery in Japan--1995 survey.

A fourth annual survey was carried out by mail in February 1996 to investigate the current trends in cataract and refractive surgery in Japan. A questionnaire was sent to 911 ophthalmologist members of the Japanese Society of Cataract and Refractive Surgery. Data received from 46.7% of the recipients were cross-analyzed and compared with those from the previous surveys.

[Current topics in glaucoma].

Among the many factors involved in the development of glaucoma, the elevation of intraocular pressure (IOP) is the most important. The treatment of glaucoma aims to lower IOP in order to maintain visual function. New anti-glaucoma drugs, latanoprost and nipradilol, have been shown to effect a reduction in IOP equal to that achieved with the equivalent dosage of timolol, with no adverse systemic side effects. The mechanism of the reduction of IOP by these drugs mainly involves the increase of uveoscleral outflow. The success rate of filtering surgery for glaucoma has been increased by using antimetabolites such as mitomycin C (MMC) and 5-fluorouracil (5-FU). The use of MMC during surgery has resulted in a better outcome than with 5-FU. Normal tension glaucoma (NTG) cases are reported to constitute more than 60% of total glaucoma cases in Japan. NTG is different from primary open-angle glaucoma not only in IOP but also in the pattern of the visual field defect, cupping and peripapillary atrophy of the optic nerve head (ONH). The first choice of treatment for NTG is using drugs for reducing IOP and, if necessary, argon laser trabeculoplasty. In addition to these treatments a drug for increasing the blood circulation in the brain, brovinecamine fumarate, has shown beneficial effects in the treatment of NTG. NTG patients whose visual field can be shown by static perimetry to be deteriorating are indicated for filtering surgery. The results of filtering surgery for NTG have been confirmed that it is more effective than drugs for maintaining the visual field. We have developed an instrument using the laser speckle phenomenon for determining the microcirculation in the eye, as well as in the ONH, noninvasively, quantitatively, and repetitively. With the same instrument, the effects of anti-glaucomadrugs on ocular circulation, especially in the ONH, can also be determined. Timolol has no effect on the circulation in the ONH, but carteolol and betaxlol increase the circulation significantly. The Ca(+2)-blocker, nilvadipine, increases the circulation in the ONH. These findings indicate that the drugs increasing the ONH circulation many be beneficial for the control of glaucoma.

[Effectiveness of the radioactive strontium (89Sr) chloride agent, SMS.2P for pain palliation in patients with metastatic bone tumor in phase III multicenter clinical trial].

The phase III clinical trial of strontium-89 chloride agent (SMS.2P) was performed in 90 patients with painful bone metastases secondary to prostate (53), breast (18) and other types of cancer (19). Some patients experienced a transient increase in pain or nausea and vomiting. However both symptoms subsided and serious side effects were not observed in any of the patients. As reported, we confirmed some abnormal changes in peripheral blood picture. A decrease in the number of white blood cells and platelets was considered to be partly a result of bone marrow suppression due to 89Sr irradiation. Pain was substantially improved after 89Sr therapy in 58% of the patients and there was some alleviation in 12%. The release from pain was accompanied by an improved quality of life for these patients including sleep patterns and morbidity. Some patients were able to resume their former life styles. Most of the improved patients experienced pain relief from days to one week following 89Sr therapy and in half cases, this remained effective for 2 or 3 months. There were even cases in which the pain relief continued over an observation period of time of clinical study.

Source localization of middle latency auditory evoked magnetic fields.

The magnetic counterparts of middle latency auditory evoked responses (MLR) were measured for seven normal subjects with a 7-channel de superconducting quantum interference device (SQUID). The source of each component (Na, Pa, Nb and Pb) was estimated and plotted onto the individual magnetic resonance images (MRI). The source of Na, as well as those of Pa, Nb and Pb, was estimated to be in the supratemporal auditory cortex. The positions of Pa, Nb and Pb sources were compared with one another. No significant difference was observed between the positions of Pa and Nb sources. On the other hand, the source of Pb was found to be anterior to the sources of both Pa and Nb. It was suggested that there are more than two separate areas activated in the human auditory cortex during MLR.

Immunological characterization and localization of a Porphyromonas gingivalis BApNA-hydrolyzing protease possessing hemagglutinating activity.

A monoclonal antibody (mAb-PC) was produced against a BA pNA-hydrolyzing protease possessing hemagglutinating activity (Pase-C) from Porphyromonas gingivalis. Other P. gingivalis BA pNA-hydrolyzing enzymes (Pase-B and Pase-S) did not react with this antibody. By ELISA or SDS-PAGE and Western immunoblotting analysis, mAb-PC recognized all P. gingivalis and P. endodontalis strains tested but did not recognize other members of the Porphyromonas genus nor other putative periodontopathogenic organisms. Pase-C, extracellular vesicles (ECV) and human strains of P. gingivalis showed two major immunoreactive bands (44 kDa and 40 kDa), whereas a different pattern was obtained with animal strains of P. gingivalis. Biotinylarginyl chloromethane, an irreversible inhibitor of trypsin-like proteases, did not affect the reactivity of Pase-C with mAb-PC on immunoblot. By reversed-phase electronmicroscopy following immunogold labeling, the antibody was shown to bind to the cell surface of P. gingivalis. mAb-PC inhibited the hemagglutinating activity of both P. gingivalis cells and ECV whereas a monoclonal antibody against LPS of P. gingivalis did not. These results suggest that Pase-C is located on the cell surface of P. gingivalis and may participate in erythrocyte binding.

The structures of the human calcium channel alpha 1 subunit (CACNL1A2) and beta subunit (CACNLB3) genes.

Calcium influx in pancreatic beta-cells is regulated mainly by L-type voltage-dependent calcium channels (VDCCs) and triggers insulin secretion. The alpha 1 subunit (CACN4) and the beta subunit (beta 3) of VDCCs, both of which are expressed in pancreatic islets, are major components for the VDCC activity, and so they may play a critical role in the regulation of insulin secretion. We have determined the structures of the human CACN4 (CACNL1A2) and the human beta 3 (CACNLB3) genes. The CACNL1A2 gene spans more than 155 kb and has 49 exons. Most of the positions interrupted by introns are well conserved between the CACNL1A2 gene and the previously reported L-type VDCC alpha 1 subunit, CACNL1A1, gene. On the other hand, the CACNLB3 gene distributes in approximately 8 kb and comprises 13 exons, most of which are located together within approximately 5 kb. Comparisons of the genomic sequences of CACNL1A2 with the previously reported cDNA sequences indicate that there are a number of polymorphisms in the human CACNL1A2 gene. In addition, the PCR-SSCP procedure of exon 1 of CACNL1A2 revealed a change from 7 to 8 ATG trinucleotide repeats in a patient with non-insulin-dependent diabetes mellitus (NIDDM), resulting in an addition of methionine at the amino-terminus of CACN4. The determination of the structures of the human CACNL1A2 and CACNLB3 genes should facilitate study of the role of these genes in the development of NIDDM and also other genetic diseases such as long QT syndrome.

Origin of the nitrogen atom of pyridoxine in Saccharomyces cerevisiae.

The origin of the nitrogen atom of pyridoxine was studied in Saccharomyces cerevisiae. The 15N atom of 15NH4Cl added to the growth medium as the nitrogen source was incorporated efficiently into the nitrogen atom of pyridoxine. The competitive effects of 14N-amino acids on the incorporation of 15NH4Cl were examined. Incorporation of 15N into pyridoxine was inhibited by glutamine. The label of L-[amide-15N]glutamine was incorporated effectively into pyridoxine in S. cerevisiae. On the other hand, the label from L-[amide-15N]glutamine was not incorporated into pyridoxine in Escherichia coli. These findings suggest that the biosynthetic pathway of pyridoxine in S. cerevisiae differs from that in E. coli.

Proteolytic fragmentation with high specificity of mouse immunoglobulin G. Mapping of proteolytic cleavage sites in the hinge region.

We report the results of fragmentation of mouse IgG by clostripain, lysyl endopeptidase, metalloendopeptidase, and V8 protease that have a narrower substrate specificity than papain and pepsin. A panel of mouse monoclonal switch variant antibodies with IgG1, IgG2a, and IgG2b subclasses were examined. Cleavage sites by these proteases were mapped on the hinge region of each of the IgG subclasses. It was demonstrated that lysyl endopeptidase can cleave the core hinge portion of IgG2a and IgG2b without perturbing the inter-chain disulfide bridges. Digestion products were successfully isolated by a combined use of protein A affinity chromatography and HPLC techniques. This is a first successful attempt of obtaining the F(ab')2 fragment of IgG2b by proteolytic digestion.
