Fetal weight estimation was tried in each fetus in 16 cases of twins and 2 cases of triplets using the formula derived from the regression analysis of fetal biometric data and actual neonatal weights in 26 cases of singleton pregnancies. The fetal biometric data used were biparietal diameter (BPD), abdominal antero-posterior diameter (APD), abdominal transverse diameter (TD), abdominal circumference (AC) and fetal femur length (FFL). The correlation between the estimated and actual weight was significant (r = 0.926) and the estimation error in neonates smaller than 2,500 g was less than 10% of the birth weight. Estimated weight differences in 16 pairs of twin pregnancies correlated well with the actual weight differences (r = 0.678). A sensitivity of 70% and specificity of 100% were revealed in the diagnosis of IUGR using the estimation formula and the intrauterine fetal growth curve. These results showed how the estimation formula derived from ultrasonically measured fetal biometric parameters, can be applied clinically but two kinds of formula with or without BPD are necessary for fetal weight estimation in multiple pregnancies as only 53% of BPD were available for fetal weight estimation.

A middle-aged man with chronic renal failure received a renal transplant from his healthy daughter. Two years later, malignant lymphoma developed in the transplanted renal pelvis. The lymphoma was found to be B-cell immunoblastic lymphoma bearing monoclonal IgG and kappa immunoglobulin, and it was of recipient origin.

Two hundred twenty-nine patients with primary breast cancer seen from 1970 through 1983 were investigated in terms of the survival rate and factors relevant to the prognosis. The operative methods were as follows: so-called radical in 191, modified radical in 24, super radical in eight, and other limited operations in six. Fifty-four patients had Stage I, 128 had Stage II, 45 had Stage III, and four had Stage IV diseases. The curabilities of these operations were absolute curative in 190, relative curative in 26, and non-curative in 13 cases. The overall cumulative survival rate was 80.9% and 78.2% at five and 10 years, respectively.
The changes in 5 parameters in 1080 5 minute periods obtained by automated FHR analysis were evaluated in 140 normal and premature labor patients at 26 to 41 weeks of gestation. The FHR-baseline level was between 140.29 +/- 5.04 and 146.90 +/- 6.94 bpm and showed no significant change between 29 and 31 weeks, but then gradually declined to 134.55 +/- 15.74 bpm by the 41st week. FHR-baseline variability (LTV) showed no significant change in the period between 26 and 32 weeks, and increased to 12.82 +/- 3.16 bpm by the 41st week from 9.96 +/- 3.78 bpm in the 33rd week. Variability in a minute was 4.09 +/- 1.03 at 28 weeks, then increased to 5.63 +/- 0.81 by the 41st week. Variability of which the amplitude was 5 bpm or more in 5 minutes showed no change from the 26th to the 41st week. FHR acceleration in 20 minutes was 1.714 +/- 1.204 at 26 weeks, then increased to 4.357 +/- 2.805 by the 40th week, and the mean values were always more than 2 after 30 weeks.
Basic research has revealed that hydrophobic residues increased on the molecular surface of the denatured IgG and that the denatured proteins in the plasma can be removed by several sorbents of different amino acids bonded from polyvinylalcohol hydroxide residue with a covalent bond. Thus, an artificial reticulo-endothelial system was developed to supplement the human reticulo-endothelial system (RES) by removing denatured proteins from the patient's plasma. Clinically, angiitis and Raynaud's phenomenon associated with collagen disease such as systemic lupus erythematosus were remarkably reduced following a series of ARES treatments. The proteins adsorbed on the ARES were analyzed by two-dimensional electrophoresis. Virtually no albumin was detected, and IgG (constant isoelectric point), IgM, IgA, IgA dimer, C3, C4, fibrinogen and other unidentified spots were found in the elute from the ARES column.

For rapid identification of Escherichia coli, changes of urinary metabolites incubated with E. coli were investigated by gas chromatography--mass spectrometry. Hydroquinone and uracil were detected and the normal urinary constituent 4-deoxythreonic acid was found to diminish in urine incubated with E. coli. Hydroquinone could not be detected in urine incubated with Klebsiella pneumoniae, Serratia marcescens or Pseudomonas aeruginosa. Although uracil was detected in normal urine, urine incubated with E. coli showed an increased uracil level. Urine incubated with K. pneumoniae, S. marcescens or P. aeruginosa evidenced no such change. A decrease of 4-deoxythreonic acid was noted in urine incubated with S. marcescens or P. aeruginosa. In 7.0 X 10(7) cells of E. coli, 0.33-2.36 micrograms of hydroquinone and 13.4-42.0 micrograms of uracil were detected after 3 h of incubation at 38 degrees C, and production was not changed after 4, 5 or 8 h of incubation. These results suggest that the detection of hydroquinone and uracil in urine is useful for rapid identification of E. coli.
This study characterized, by means of immunocytochemistry and electron microscopy, four cases of "unusual" cutaneous T-cell lymphoma (CTCL) other than classical mycosis fungoids and Sézary syndrome. Cases 1, 2, and 4 were diffuse lymphoma of a pleomorphic type, and Case 3 was of a mixed type. Case 4 shared a feature common to pagetoid reticulosis. A fairly large number of inflammatory cells were seen in Cases 1, 3, and 4. Functionally, the neoplastic cells of Cases 1, 3, and 4 were of a helper/inducer T-cell subset, whereas those of Case 2 were of a suppressor/cytotoxic T-cell type. Epidermotropic cells with pagetoid growth in Case 4 failed to show these specific surface phenotypes, although they still retained pan T-cell markers. Neoplastic large or intermediate-sized cells revealed a marked difference in the development of cytoplasmic organelles and their nuclear profiles, ranging from a few simple indentations (Cases 2 and 3) to forms with many deep indentations (Case 1) and highly cleaved shapes (Case 4). All of these cells, however, possessed dense-cored granules located in a portion of the cytoplasm. This study indicated the clinicopathologic and immunologic heterogeneity of CTCL, which may be classified, according to the reactivity with monoclonal antibodies and the fine structural features, into subtypes that correspond to functionally distinct subsets of T-cells and their stages or types of differentiation.
Effects of hypophysectomy and treatment with testosterone or estradiol on the sex-specific forms of cytochrome P-450, P-450-male and P-450-female, were examined. The amounts of P-450-male as well as drug oxidation activities were decreased by hypophysectomy of male rats. In female rats, drug oxidation activities were increased by hypophysectomy, which was associated with the disappearance of P-450-female and the appearance of P-450-male. Treatment of hypophysectomized female rats with testosterone or estrodiol effected minor changes in the amounts of P-450-male.
