Merkel cell carcinoma: a challenge.

A 70-year-old man presented with an enlarging, recurrent tumor of the left buttock which, on initial biopsies, was interpreted as a histiocytic malignant lymphoma. Further biopsies showed numerous monomorphic, hyperchromatic, noncohesive cells, arranged in compact sheets, throughout the dermis, the fat, and the underlying muscle. Neuron specific enolase stain for neuroendocrine granules was positive. Electron microscopy showed cytoplasmic perinuclear microfilaments and round membrane-bound dense core granules. The histologic, enzymatic, and ultrastructural findings confirmed the diagnosis of Merkel cell carcinoma. Early diagnosis and treatment are imperative for a favorable outcome.

Newly developed circulating blood volume-monitoring system and its clinical application for measuring changes in blood volume during hemofiltration.

Blood and ultrafiltrate electrical resistivities were continuously monitored during hemofiltration. By substituting these values into a previously developed equation that was modified by the authors, the hematocrit value was determined; and blood volume change was obtained from the change in this value. The following facts were discovered as the result of monitoring the blood volume change during treatment: When the body position was changed from the reclining to the sitting position, the blood volume decreased by 4.2 +/- 0.3%. With the body water removal rate kept constant, the body position recumbent, and the subject fasted, the blood volume gradually decreased. However, when the position was changed from reclining to sitting for food intake and once more returned to the supine position after the meal, the blood volume was greatly affected by the change in position and the food intake.

[Behavioral pharmacological properties of nicergoline. Effects on gross-behavior in rats and monkeys and on DRL response, CER, and CAR in rats].

Whether nicergoline has psychotropic-like pharmacological properties was examined through the gross-behavioral and operant behavioral observations in rats and monkeys. In gross-behavioral observations, slight decrement of spontaneous motor activity, lying on the abdomen and relaxation of abdominal tone were observed in rats when nicergoline was administered intravenously (1 mg/kg or more) and intraperitoneally (4 mg/kg or more). However, when it was administered orally, slight decrement of spontaneous motor activity was observed only at large doses of 32 and 128 mg/kg. In monkeys, nicergoline produced decrement of spontaneous motor activity, palpebral ptosis, and lacrimation when administered intraperitoneally at doses of 1 mg/kg or more. Under a differential reinforcement of low rate (DRL) schedule for food reinforcement in rats, nicergoline depressed the response at 4 mg/kg, i.p., or 128 mg/kg, orally. In conditioned emotional response (CER), nicergoline had no effect on the responses during both the alarm and safe periods at doses of 0.25, 1, and 4 mg/kg, i.p., or 8, 32, and 128 mg/kg, p.o. In Sidman continuous avoidance response (CAR), nicergoline (0.25, 1, and 4 mg/kg, i.p., or 8, 32, and 128 mg/kg, p.o.) slightly depressed the response and increased the total shock. These results were compared with those of chlorpromazine, chlordiazepoxide, pentobarbital, and methamphetamine and the following conclusion was drawn: Inhibitory effects of nicergoline on gross and operant behaviors seem to be non-specific, and its behavioral pharmacological properties are qualitatively different from those of anti-psychotics, anti-anxietics, hypnotics, and stimulants.

[Health problems among workers engaged in the education of handicapped children. Status of daily care and trends in physical complaints].

Workers engaged in compulsory education and training of severely handicapped children have a high prevalence of neck pain, shoulder pain, low back pain and general fatigue. These complaints are mainly due to heavy physical burden placed on these workers in caring for a large number of handicapped children who are unable to satisfy by themselves daily needs such as excretion, putting on and off their clothes, and eating. To prevent these physical disturbances, it was found effective to reduce the number of assigned handicapped children, to introduce physical exercise especially designed for low back pain prevention, and to provide individual guidance to workers through health examination. Though these measures are expensive, they are considered necessary in preventing disturbances and in executing the government's program of providing compulsory education and training to all handicapped children in the country.

Immunohistological study with peroxidase-antiperoxidase staining in a case of generalized prepubertal periodontitis.

A case of prepubertal periodontitis was observed and examined immunohistologically with peroxidase-antiperoxidase staining. The patient was an 11-year and 7-month-old Japanese girl, well-developed and well-nourished. Her parents were first cousins. Her chief complaint was the loosening and loss of the permanent teeth. There was a similar history of primary dentition. Her remaining permanent teeth were loosened with severe alveolar bone loss, but calculus deposit was minimal. Significantly, there was no palmar-plantar hyperkeratosis. General examination showed normal data except for the increase of the immunoglobulin concentrations. In neutrophil function tests chemotaxis was depressed, although phagocytosis, random migration and superoxide production were within normal limits. Histologically, neutrophils were seen in the gingival tissue and other findings were also similar to those of adult periodontitis. In immunohistological examination, IgG-bearing cells which mostly consisted of plasma cells predominated in the lesion. Considering the past history, the immunodeficiency and the absence of palmar-plantar hyperkeratosis, the case was diagnosed as prepubertal periodontitis.

Acid ATPase from chicken liver lysosomes. III. A metal ion-activated ATPase combines with membranous phosphatidylinositol.

A metal ion-activated acid ATPase was present in chicken liver lysosomes. We used Zn2+ as an activator. Lysosomal extract containing octylglucoside from chicken liver was centrifuged at 100,000 xg for 60 min. The supernatant was analyzed by gel filtration on a Sepharose 6B column. Two peaks of metal ion-activated acid ATPase activities were obtained according to the distribution patterns. Each of the two active fractions was incubated with phosphatidylinositol-specific phospholipase C at 37 degrees C for 60 min. The resulting solution was analyzed by gel filtration on a smaller size column of Sepharose 6B again. Molecular weight of the major peak was altered from approx. 1,600,000 to 130,000, whereas that of the minor one, 700,000, remained unchanged.

[Study on utero-placental circulation by pulsed-Doppler technique--blood flowmetric study in placental pores].

The flow velocity waveform (FVW) was obtained by means of the pulsed-Doppler technique from the placental pores visualized as anechoic areas on the ultrasonic tomogram in 36 cases of singletons and 7 cases of multiple pregnancies. Utilised parameters for analysis of the sonogram were resistance index and systolic to diastolic ratio of flow velocity (S/D ratio) which were independent of the angle between the ultrasonic beam and the direction of blood flow. Diagnostic apparatus were composed of real-time sector scanner and Doppler unit with FFT analyser by which sampling points were selectable within 15 cm of depth. The FVW from the placental pores was recorded as the toward flow in the case of posteriorly attached placentas and was similar to the one from the arcuate arteries which has poor pulsatility and high diastolic flow in the normal pregnancies. There was a tendency for the increase in the gestosis index (GI) to be related to that of RI. Multiple pregnancies revealed lower RI than singletons in the same GI group. There was reverse correlation between mean birth weight and mean RI in the cases who received pulsed-Doppler examination within 2 weeks prior to delivery. The RI and S/D ratio showed a high correlation in the each cases (r = 0.99).

[Cefotiam concentration in exudate from suction drain of patients with breast cancer following intravenous administration].

Cefotiam (CTM) in a dose of 2 g was given by intravenous bolus injection to 16 patients with breast cancer operated upon radical mastectomy with suction drainage. The materials of exudate from drain were taken at intervals by sterilized paper disc and determined by the paper disc method with Proteus mirabilis ATCC 21100 as the test organism to CTM concentrations. CTM concentrations in exudate from suction drain of patients increased quickly after intravenous injection, and reached to peak early time on 2 to 3 days after operation. While on 6 days after operation, the CTM concentrations were comparatively lower and reached to peak at 2 approximately 3 hours after injection. The CTM concentrations in exudate exceed the antibacterial levels of CTM and are thought to be sufficient to produce prophylactic effects in the operation.
