[Correlation between fetal abdominal longitudinal and lateral diameters and the baby's weight on discharge (author's transl)].

By ultrasonic diagnostics, fetal abdominal longitudinal and lateral diameters were measured in 200 pregnant women from 37th to 41st weeks of pregnancy before the start of the birth for the study of the correlation coefficients between the diameters and the baby's weight on discharge. The following results were obtained. 1) Before the determination of the above coefficients, the great lateral diameter, the girths of the head, chest and abdomen and sitting height were measured in newborn infants in from six to 12 hours after birth. The correlation coefficient was 0.66 for the great lateral diameter, 0.71 for the head girth, 0.85 for the chest girth, 0.81 for the abdominal girth and 0.66 for the sitting height. The chest and abdominal coefficients were found to be low. 2) By ultrasonic diagnostics, measurements were taken of the fetal heads, chest, abdomen and head length. In consideration of dimensions and distinctness of ultrasonically delineated images, the chest and head length seemed unsuitable for measurement. On the contrary, the head lateral diameter and abdominal longitudinal and lateral diameters can be measured with relative ease. 3) The fetal head lateral diameter can be measured with ease and high accuracy, but its correlation coefficient becomes 0.62 which is low. Due to soft tissues, difficulty is encountered in measuring the fetal abdominal longitudinal and lateral diameters. However, if check points are properly set, they can be measured with relative ease. Their correlation coefficient to the weight on discharge is very high at 0.87. Clinically for the estimation of the weight on discharge, the method of measuring the fetal longitudinal and lateral diameters is presumably the better method at present, based on the above results.

[Changes in the criteria for planned delivery in the National Defense Medical College Hospital with statistical analysis].

Planned deliveries have been attempted with the administration of prostaglandins E2 and F2 alpha since 1978. Eight hundred and eighteen prostaglandins-induced and 729 uninduced deliveries from January, 1978 to July, 1980 were statistically analyzed. The uninduced deliveries included 465 spontaneous and 264 stimulated labors. The cervical ripening was considered the basic requirement stimulated labors. The cervical ripening was considered the basic requirement for labor induction for the first year of study. However, spontaneous labor onset or the membranes rupture occurred before the induction in 52.4% of cases, where babies were large enough (3,052.6 +/- 33.4g, mean +/- SE, n = 154). Therefore, the cervical factor was excluded from the essential requirement for induction. Since the beginning of 1979, labor induction has been performed based on gestational age (38 weeks or more) and uterine fundal length (32cm or more). Laminaria tents were inserted into the unfavorable cervix the night before induction. Delivery time in the day, duration of labor, and birth weight of babies in the induced group were 2.04 +/- 0.18 p.m., 8.0 +/- 0.3 hours and 3,138.8 +/- 12.3g, respectively. Apgar scores and the amount of postpartum hemorrhage were not significantly different from those in the uninduced group. Similar results were obtained between the laminaria subgroup with unfavorable cervix and the non-laminaria subgroup with ripe cervix in the induced primigravidas.

Virus-induced fusion of human erythrocyte ghosts. I. Effects of macromolecules on the final stages of the fusion reaction.

Human erythrocyte ghosts prepared by hypotonic hemolysis can be fused by Sendai virus, provided that certain macromolecules (bovine serum albumin, dextran and others) are sequestered in the ghosts. Since fusion of the ghosts is dependent on intactness of the F(fusion)-glycoprotein of the virion, and since the other requirements for this reaction are also similar to those for the Sendai virus-induced fusion of intact erythrocytes, this system can be used as a model for the Sendai virus-induced cell fusion reaction. Sequestered macromolecules seem to be required for rounding of locally fused ghosts. Under low osmotic swelling conditions, such as use of ghosts sealed without macromolecules or using bovine serum albumin-loaded ghosts sealed in the presence of external macromolecules, no apparently complete cell fusion (large spherical polyghost formation) could be observed. Even under these conditions, however, occurrence of local cell fusion could be demonstrated either by transfer of fluorescent-labeled albumin from one ghost to an other, or by observation of polyghost formation after osmotic swelling in the cold. Thus, final stages of the fusion reaction can be divided into local cell-cell fusion which could not be observed by phase-contrast microscopy, and rounding (i.e. formation of spherical polyghosts). For the observation of fusion of ghosts, the last step seems to be important.

Anticancer drug sensitivity in vitro in the bladder cancer cell line, KK-47 and prophylactic use of carbazilquinone and urokinase in bladder cancer.

Using colony formation technique and KK-47 cell line established from a human bladder transitional cell carcinoma, the effect of 6 anticancer drugs, thio-TEPA, Bleomycin, mitomycin C, carbazilquinone, Adriamycin and cis-Platinum, were compared. On the results of tests performed to establish the drug concentration required to achive a 50% inhibition of cell survival, carbazilquinone was chosen for the prevention of recurrences of bladder cancer. The two groups studied consisted of 56 patients (previously untreated groups) who were rendered free of tumours by surgical intervention and of 19 patients (thio-TEPA failures group) who had experienced a persistent recurrence of tumours after prophylactic thio-TEPA instillations and were presumed free of the recurrence of tumours after the next surgical intervention. The 2 groups were subjected to prophylactic combined intravesical instillation therapy with carbazilquinone and urokinase. In the previously untreated group, 6 of the 56 patients (10.7%) had a recurrence of tumours, and the recurrence rate after 21 months was 16.7%, using the actuarial method. In the thio-TEPA failures group, 12 of the 19 patients (63.2%) had a recurrence of tumours, a rate at 21 months of 76.1%. A considerable drop in the recurrence rate was obtained by the combined instillation therapy in the previously untreated group. The results in the thio-TEPA failures group suggested the presence of a cross-resistance between both alkylating agents, and of a persistent susceptibility to multifocal lesions. No bone marrow depression was observed but an episode of anaphylactic shock attributable to the use of carbazilquinone occurred in 1 out of a total 75 patients.

Comparative study of F-type pyocins of Pseudomonas aeruginosa.

Pseudomonas aeruginosa strain PAF41 was found to produce a new F-type pyocin, pyocin F3, the action spectrum of which was different from those of previously reported pyocins F1 and F2. These three F-type pyocins were compared with respect to their structure and biological properties. These pyocins were almost the same with regard to the structure and the dimensions, and have similar amino acid compositions and S values. The particle weights of these pyocins were also suggested to be similar. Analyses of subunit proteins by SDS-polyacrylamide slab gel electrophoresis showed that these pyocins were composed of 5 major (bands 1, 2, 3, 4, and 6) and 2 minor (bands 5 and 7) subunit proteins and that no difference in the mobilities of these subunit proteins could be detected among the pyocins except that of the second major subunit protein (band 4), which did differ. Pyocins F1, F2, and F3 were immunologically cross-reactive, and carried common antigens as well as specific ones. It was shown that band 6 was a common antigen among the three pyocins and that band 4 was antigenically different in pyocins F1 and F3 by immunological reaction after protein blotting. Electron microscopic observation of pyocin particles treated with anti-sera revealed that the common antigens were located on the rod part and the specific ones were on the fiber part. Pyocin F3 was neutralized by both anti-F3 and anti-F1 sera showing apparent first order rate kinetics, whereas the neutralization for pyocin F1 by these sera did not show such kinetics, but a considerable increment of pyocin F1 activity was observed when small amounts of the sera were added. The increment seemed to be due to the antibodies common to pyocins F1, F2, and F3. A phage, which had a flexuous rod-like tail, was found to be immunologically cross-reactive with the three pyocins and was named KF1.

Studies on racemization during couplings using a series of model tripeptides involving activated residues with unfunctionalized side chains.

Racemization studies have been carried out using as model tests couplings of N-benzyloxycarbonylglycyl-L-X with benzyl NE-benzyloxycarbonyl-L-lysinate followed by deprotection and analysis for the diastereomeric tripeptides with an amino acid analyzer, for X = alanine, leucine, phenylalanine, valine and isoleucine. The order of susceptibility to racemization of residues during these segment couplings depends on whether the solvent is polar or apolar, with isoleucyl and valyl, followed by phenylalanyl, being the most susceptible ones in polar solvents. The racemization depressing effect of some additives on carbodiimide-mediated couplings has been examined. Reconciliation of apparent discrepancies in data in the literature on the relative merits of l-hydroxy-benzotriazole and N-hydroxysuccinimide is made on the basis of the nature of the model tests used in acquiring the data.

Pharmacokinetics of cefroxadine in healthy volunteers and patients with impaired renal function.

The pharmacokinetics of cefroxadine, a new orally active broad-spectrum cephalosporin, was studied in healthy volunteers and patients with impaired renal function after a single oral dose of 500 mg. The pharmacokinetic parameters of cefroxadine were obtained by analysing the serum level data of the drug based on a one-compartment open model. The mean serum half-life of cefroxadine was 0.97 h in healthy subjects, and was prolonged to 41.7 h in patients with a creatinine clearance of less than 5 ml/min. There was a significant linear correlation (p less than 0.001) between the elimination rate constant of the drug and the creatinine clearance. In healthy subjects, 71% of the administered dose was excreted in the urine collected over the first 6 h.

Home blood pressure and circadian variation of blood pressure in the evaluation of hypertensive patients.

Clinical blood pressures measured at clinic by physician were higher than home blood pressures measured at home by patients in the majority of untreated patients with essential hypertension, but equivalent or lower in some patients. Clinic minus home blood pressure (delta P) were correlated with the levels of clinic blood pressure (r = 0.51, p less than 0.005 for systolic; r = 0.35, p less than 0.02 for diastolic blood pressure, respectively). The systolic delta P might be greater in the middle-aged women, especially in the fifties of females than the age-matched males (p less than 0.05). The delta P could not be altered by any antihypertensive drugs with the exception of systolic delta P with diuretic alone. The blood pressure tended to remain more stable throughout the 24-hour period in proportion as the severity of hypertension increased. The observation of circadian variation in blood pressure disclosed that the blood pressure was lower in the morning, but increased gradually, resulting in the relatively high blood pressure between the afternoon and evening in the low renin and volume expanded type of hypertension. On the contrary, the blood pressure was already high in the early morning in hypertensive patients characterized by the accelerated renin-angiotensin system and contracted volume factor.
