[Fundamental and clinical studies of ceftizoxime in pediatric infections (author's transl)].

Experimental and clinical trials were carried out with ceftizoxime in pediatric infections. Results were as follows. 1. The antibacterial activity of ceftizoxime against clinically isolated organisms was determined. Ceftizoxime was more active than cefazolin, cefotiam and cefmetazole against K. pneumoniae, E. coli and H. influenzae. 2. The serum concentrations of ceftizoxime following intravenous injection of 10 mg/kg were 16.2, 10.1, 6.2, 1.8 micrograms/ml at 30, 60, 120, 240 minutes after injection, respectively. Ceftizoxime was excreted with the rate of 97.2% in the 6-hour urine after injection. 3. Twenty-one patients comprising 5 with urinary tract infection, 16 with respiratory tract infection were received ceftizoxime at the doses ranging from 32 to 109 mg/kg divided 3 or 4 times a day. The results were excellent in 8 and good in 13 patients. The rate of satisfactory clinical response was 100%. 4. Slight and transient elevation of GPT was observed in 1 patient. Other side effects were not observed in any of these 21 patients.

[Laboratory and clinical studies of cefmenoxime in pediatric infections].

The antimicrobial activity of cefmenoxime (CMX) against clinical isolated organisms was measured; CMX was more active than cefotiam and cefazolin against Escherichia coli and Haemophilus influenzae. The serum concentrations of CMX following intravenous injection of 20 mg/kg were 25.6, 10.3, 3.0 micrograms/ml at 30, 60, 120 minutes after injection, respectively. CMX was excreted 60.9% in urine within 6 hours after injection. CMX was administered clinically to 22 pediatric patients with various infections (respiratory tract infection 16 including 1 pyothorax, urinary tract infection 4, tonsillitis with sinusitis 2) at the dose of 39 approximately 96 mg/kg/day for 4 approximately 9 days, and the following satisfactory results were obtained; excellent in 11, good in 9, and poor in 2. The rate of satisfactory clinical response was 90.9%. Eosinophilia 2 cases, slight elevation of transaminase 3 cases, slight elevation of BUN 1 case and transient diarrhea 1 case were observed. But no other serious side effects were observed.

Epidemiology of familial predisposition for breast cancer in Japan.

The physical and reproductive traits known to be risk factors for breast cancer were correlated with familial predisposition for this disease by the use of hospital records of 3,867 patients with breast cancer, of whom 186 had one or more relatives with breast cancer and were called "familial patients." Various characteristics in the total series of patients were compared with familial patients matched for year of birth. The mean age at menarche was significantly less in familial compared with the total series of patients, especially for premenopausal patients; however, no difference was observed in the mean age at marriage between the two groups. Familial patients were slightly taller as compared to the total series of patients, but no difference was detected in body weight between the two groups. Because age at menarche and the stature are highly correlated, they may be influenced by an underlying common hormone effect that may influence the familial nature of breast cancer.

[Changing patterns in surgery of breast cancer].

The ideal surgical procedure for the treatment of breast cancer has been a matter of controversy. Halsted radical mastectomy has been the treatment of choice, at the cancer institute hospital, in the management of breast cancer. Over the past five years, there has been a definite trend toward modified radical mastectomy preserving the pectoral muscles in the treatment of patients with early breast cancer. From 1975 to 1890 we treated 755 breasts with radical mastectomy, 723 with Patey's operation, 373 with Auchincloss operation and 58 with extended mastectomy. Extended mastectomy has been gradually abandoned.

[Intramural injection of anticancer emulsion by gastroendoscopy].

Attempting to establish a topical chemotherapy for stomach cancer, injectable 5-FU emulsion (O/W type) has been developed. The distribution of 5-FU was analyzed by the procedures of quantification and X-rays. The combination of lipiodol having a contrast ability in the oil phase was studied. During the endoscopic examination before operation, 1.0 ml of 5-FU was injected at a site, a total of 50 ml at 2 to 4 sites in the submucosal tissue of the lesion. From the result of examination of 5-FU distribution in the extracted stomach and lymph node, it was confirmed that the drug was retained at the site of injection. In addition, 0.017-0.056 microgram/g on the average of 5-FU concentration was transferred to the secondary lymph node. At the same time, its distribution was monitored by X-rays. In those cases which received intermittent administration for twice or more before operation, the 5-FU concentration was detected at a higher rate than in cases receiving a single dose. This suggests that the therapy is markedly effective for stomach cancer and its metastasis to the lymph node as topical therapy.

Hormone receptors and obesity in Japanese women with breast cancer.

An association between hormone receptors in primary breast cancer and obesity determined prior to mastectomy was investigated in 128 Japanese women. The following criteria for obesity were used: (1) weight greater than or equal to 60 kg (132 lbs), (2) weight kg/height cm-105 greater than or equal to 1.3, (3) weight lbs/height in greater than or equal to 2.30, (4) body surface area greater than or equal to 1.56 m2. In view of the 4 criteria, tumor estrogen receptor (ER) values greater than or equal to 4 fmol/mg were observed in obese patients more often than in nonobese patients, though the difference was not statistically significant. The same tendency was observed in the postmenopausal subgroup, 62 patients, especially in the 36 patients more than 5 years beyond menopause. However, there was still no statistical difference between obese and nonobese patients because the number of subjects was small. The same tendency was observed in the case of progesterone receptor (PgR) (greater than or equal to 6 fmol/mg) as observed in the case of ER.

[Laboratory and clinical studies of cefoxitin in pediatric infections (author's transl)].

The antimicrobial activity of cefoxitin against clinical isolated organisms was measured: Cefoxitin was more active than cefazolin and cephalothin against Escherichia coli. The serum concentrations of cefoxitin following intravenous injection of 25 mg/kg were 267.7, 38.8, 8.3 microgram/ml at 5, 30, 120 minutes after injection, respectively. Cefoxitin was excreted 90.5% in urine within 6 hours after injection. Cefoxitin was administered clinically to 22 pediatric patients with various infections (urinary tract infection 9, respiratory tract infection 10, S.S.S.S. 1, Salmonella enteritis 1, and cervical lymphadenitis 1) at the dose of 45-98 mg/kg/day for 4-10 days, and the following satisfactory results were obtained; excellent in 16, good in 5, and poor in 1. The rate of satisfactory clinical response was 95.5%. Slight elevation of transaminase and A1-P were observed in 4 patients, but no other serious side effects were observed.

[Laboratory and clinical studies on cefroxadine (author's transl)].

Cefroxadine (CXD) was applied to infectious diseases in children and the following results were obtained. 1. Serum concentration and urinary excretion: CXD was given orally in dose of 10 mg/kg in dry syrup from 30 minutes after meals. The peak serum concentration was at 1 hour after administration in 3 cases and at 2 hours in 1 case, and the average peak serum concentration was 16.41 microgram/ml in these 4 cases. The average urinary excretion rate of the antibiotic during 6 hours after administration was 93.9% in another 4 cases. 2. Antibacterial activity: The MIC of CXD against E. coli was slightly superior to that of CEX, but against S. aureus, S. epidermidis, Str. pyogenes, Str faecalis, Str. pneumoniae, Kleb. pneumoniae, Kleb. oxytoca, Pro. mirabilis, Pro, vulgaris, H. influenzae, H. parainfluenzae, H. parahaemolyticus the MICs of CXD were almost equal to those of CEX. 3. Clinical study: Thirty seven patients with bacterial infections (7 cases of urinary tract infection, 2 of cystitis, 10 of tonsillitis, 9 of scarlet fever, 7 of bronchitis and 2 of bronchopneumonia) were orally treated with CXD dry syrup, 15 approximately 48 mg/kg/day divided into 3 doses. The overall efficacy rate was 91.7%, and side effects inclusive of abnormal laboratory findings were not observed.

[Fundamental and clinical studies of cefoperazone in children (author's transl)].

As a result of conducting experimental and clinical tests with the newly developed cephalosporin, cefoperazone (CPZ), the following conclusions were obtained: (1) When tested against 10 strains of Staphylococcus aureus and 16 strains of Staphylococcus epidermidis, the antibacterial activity of CPZ was found to be weaker than that of CEZ. Against 5 strains of A-beta-Streptococcus and 4 strains of Streptococcus pneumoniae, both CPZ and CEZ exhibited similar excellent antibacterial activity. CPZ was effective against 18 strains of Escherichia coli though its activity was influenced by the amount of inoculated bacteria present. Against 15 strains of Haemophilus influenzae and 10 strains of Haemophilus parahaemolyticus, CPZ was found to be more effective than CEZ though several high-resistant strains were noted. CPZ also showed more excellent antibacterial activity than CEZ against 4 strains of Haemophilus parainfluenzae, 5 strains of Klebsiella pneumoniae, 8 strains of Salmonella sp., 4 strains of Pseudomonas aeruginosa and 4 strains of Proteus sp. (2) The mean half-life in the blood following intravenous injections of 25 mg/kg and 10 mg/kg of CPZ to three children was 70 minutes. (3) One hour after intravenous injection of 25 mg/kg of CPZ to 3 cases of aseptic meningitis, drug concentration in the cerebrospinal fluid (CSF) was 1.20 mcg/ml, less than 0.39 mcg/ml and 1.55 mcg/ml. In one case, the CSF/serum ratio was 2.7%. (4) The average recovery rate in the urine of children who had received intravenous administrations of 25 mg/kg (3 children) and 10 mg/kg (1 child) was 17.8% between 0 and 6 hours. (5) Eighteen pediatric patients received CPZ in doses ranging from 48 to 170 mg/kg divided three-four times a day. They were RTI in 7, URI in 5, UTI in 5, SSSS in 1 and enteritis in 1 children. The clinical effectiveness of CPZ was judged to be remarkedly effective in 11 children, effective in 5 children and ineffective in 3 children, with an overall effective rate of 84.2%. One patient of tonsillitis combined sinusitis was considered 2 cases. The three cases in which the drug was found to e ineffective were 2 cases of pyothorax and 1 case of sinusitis. (6) Side effects were 1 case of eosinophilia, 2 cases of elevation of GOT and GPT, and 1 case of mild elevation of GOT. All were considered to be minor.

[Clinical evaluation of cefamandole in infants and children (author's transl)].

Clinical trials were carried out with cafamandole (sodium salt) in pediatric infections. Results were as follows; 1. CMD was applied to 13 patients with pneumonia, 1 patient each with submandibular abscess, urinary tract infection and bacterial meningitis. 2. Results were excellent in 1 and good in 13 patients, being overall efficacy rate 93.3%. 3. Slight elevations of GOT and GPT were observed in 1 patient. No other serious side effects were observed or reported.

[Falx images on CT scan (author's transl)].

It has been reported that falx images usually are not visualized on CT scan except in the case with brain atrophy of falx calcification. But we have often experienced the falx images on CT scan in the case of normal child, and we have an idea that falx images do not always mean a calcification of the falx. Evaluation of falx images on CT scan was done in 964 normal or abnormal cases in relation to different CT slice level, age, sex, pineal body and habenula and choroid plexus. Results are as follows; 1) Slice of higher level revealed higher incidence of falx image. 2) Incidence of falx images was lowest in the second decade, and showed gradual increase in relation to aging process. 3) Incidence of falx images revealed no sex difference. 4) Incidence of falx images showed a following tendency: a) Under 10 years; falx greater than pineal body and habenula greater than choroid plexus. b) Between 11 years to 20: pineal body and habenula greater than choroid plexus greater than or equal to falx. c) Over 21 years: pineal body and habenula greater than choroid plesus greater than falx. 5) The case with brain edema showed a higher incidence of falx images than the normal case, but the case with brain atrophy did not show a higher incidence. 6) Difference of CT number of falx region between the case of positive falx image and negative could not be recognized.

Diet and endocrine-related cancer.

In women, dietary modification and life style affect the risk of breast cancer and may alter the hormonal status, while in experimental animals diet can alter the incidence of induced mammary tumors. In this study, a high fat diet increased the incidence of DMBA-induced tumors in rats while this increase in incidence was lowered and the effect of a high fat diet obliterated by an anti-prolactin during CB154. Premenopausal Japanese women had a higher estradiol level than their Caucasian counterpart. In Japanese but not Caucasian breast cancer patients, the estradiol decreased. When nurses were transferred from the Western to a vegetarian diet, the menstrual cycle was shortened, while their prolactin and testosterone decreased. Data indicate that dietary factors influence tumor incidence and hormone profile in rat mammary cancer and the hormonal status in women.

Electroclinical studies of seizures in the newborn.

Electroclinical features of neonatal seizures were studied in 215 neonates who developed seizures during the first three weeks of life. In 62 of them, clinical fits occurred during the actual polygraphic recording. The electro-clinical correlations were very variable, although there were some statistically significant associations between certain types of seizures and electrical discharges. The relation between seizures and sleep states was investigated in 40 infants who had more than three fits during the recording. The state in which more than 70% of seizures occurred was active sleep in 28% of the cases, quiet sleep in 13%, indeterminate sleep in 8%, and wakefulness in 8%. In 50%, seizures did not tend to occur in a given state. The authors thank Professor Suzuki for his continued interest and encouragement.

[Clinical and laboratory studies on S-6437 (a new longacting granule of cephalexin) in pediatric field (author's transl)].

Clinical and laboratory studies on S-6437 were made, and the following results were obtained. 1. Thirty pediatric patients with various types of infections such as tonsillitis, bronchitis, pneumonia, cystitis, pyelitis, lymphadenitis colli and pyodermia were treated with S-6437 at the daily dosage of 50 mg/kg orally, the clinical effectiveness was 89.3%. 2. The peak blood level of cephalexin after a single oral administration (25 mg/kg) was observed after 4 hours and the average peak blood level of 10 cases was 7.4 microgram/ml. 3. The average urinary excretion rate of 5 cases was 71.2%. 4. Mild side effects were noticed in a few cases.
