[Experimental and clinical studies of sulbactam/cefoperazone in pediatric field].

Clinical trials were carried out with sulbactam/cefoperazone (SBT/CPZ) (combination ratio of 1:1) in pediatric infections. Results were as follows. The mean half-lives of SBT and CPZ in the serum following intravenous injection of SBT/CPZ were about 0.7 and 1.2 hours, respectively. Urinary excretions of SBT and CPZ within 6 hours after intravenous injection of SBT/CPZ were 81.9% and 28.1%, respectively. SBT/CPZ was administered to 33 pediatric patients with various infection; 18 respiratory tract infections, 12 urinary tract infections and 3 Salmonella enterocolitis. The overall efficacy rate was 87.9%. In particular, 7 of 8 urinary tract infections caused by beta-lactamase producing organisms were improved after administration of SBT/CPZ. Diarrhea in 8 and soft stool in 3 of 33 patients occurred, and slight elevation of GOT/GPT was observed in 2 patients.

[Laboratory and clinical studies on ceftazidime in the field of pediatrics].

Laboratory and clinical studies on ceftazidime ( CAZ ), a new cephem antibiotic, were carried out in the field of pediatrics. The results were as follows: Antibacterial activities of CAZ against clinically isolated strains of S. pneumoniae, H. influenzae, E. coli and P. aeruginosa were compared with those of cefotaxime (CTX), ceftizoxime (CZX), latamoxef ( LMOX ), cefoperazone (CPZ) and cefmetazole (CMZ), and also with cefsulodin (CFS) and gentamicin (GM) against P. aeruginosa. Against S. pneumoniae and H. influenzae, CAZ was almost as active as CTX, CZX and CPZ. Against E. coli, it was almost as active as CTX, CZX and LMOX . Against P. aeruginosa, it was almost as active as CFS and GM. Serum concentrations and urinary excretion rates after intravenous bolus injection of CAZ at doses of 20 mg/kg and 10 mg/kg for 5 minutes in each 2 cases (4 cases in total) were determined. The mean serum concentrations of CAZ were 78.9 and 52.0 micrograms/ml at 15 minutes, 38.5 and 27.4 micrograms/ml at 1 hour, and 6.5 and 4.8 micrograms/ml at 4 hours, with serum half-lives (T 1/2) of 1.39 and 1.80 hours respectively. Mean cumulative urinary excretion rate within 6 hours after administration was 84.6%. In a patient with chronic renal failure, serum half-life was 3.22 hours and urinary excretion rate within 6 hours was 22.8% (after intravenous bolus injection of CAZ at a dose of 10 mg/kg). CAZ was administered at a dose of 55.5 mg/kg by intravenous bolus injection to a child with purulent meningitis. The levels of CAZ in the cerebrospinal fluid (CSF) at 1 hour after administration were 2.7-38.9 micrograms/ml with CSF/Serum ratios of 3.2-28.8%. Forty-two pediatric patients with various bacterial infections (pyelonephritis 14, tonsillitis 1, bronchopneumonia 3, pneumonia 17, purulent meningitis 1, bacteremia 2, SSSS 1, enterocolitis 3) were treated with CAZ at a daily dose of 49-222 mg/kg t.i.d. or q.i.d. (as a rule 60 mg/kg t.i.d.). The efficacy rate was 97.6% clinically and 97.8% bacteriologically. No adverse reactions were observed except 1 case with mild diarrhea. Abnormal laboratory findings were also only mild; eosinophilia in 1, slight elevation of GOT in 5 and that of GOT & GPT in 3 cases. These results indicate the usefulness of CAZ in the treatment of bacterial infections in children.

[Influence of family history, irradiation and anti-cancer drug (mitomycin C) on the occurrence of multiple primary neoplasms in breast carcinoma patients].

The influence of family history, irradiation and anti-cancer drug (Mitomycin C) on the occurrence of multiple primary neoplasms was analysed using the person-year method in 1359 Japanese breast carcinoma patients. There were 111 multiple primary neoplasms, including bilateral breast cancer, in 109 patients; the incidence rate was 0.0072 per person-year. The incidence rate in patients with a family history of cancer was 1.29 times higher than in those without. In the bilateral breast cancer group there was about a 3 times higher frequency of family history of breast cancer. Irradiation therapy raised the occurrence of multiple primary neoplasms 1.28 fold, and Mitomycin C (40 mg) had no effect on the occurrence of neoplasms during a 10-year observation period.

[So-called carcinosarcoma of the breast--report of 4 cases].

Four cases of so-called carcinosarcoma of the breast were studied histopathologically. The patients were females aged 44, 37, 44 and 77 who were admitted to the Cancer Institute Hospital for breast tumors. Histologically, the tumors showed proliferation of sarcomatous cells with relatively small amounts of papillotubular carcinoma or scirrhous carcinoma. There were transitions between the sarcomatous portion and carcinoma. Two cases showed squamous metaplasia merging into the sarcomatous component, whereas the other 2 showed transition from adenocarcinoma to sarcomatous element. Lymph nodal metastases in 2 cases were papillotubular carcinoma. One patient died of lung and brain metastases, and autopsy revealed only sarcomatous element.

[Effect of pre-operative biopsy for breast cancer on survival rate].

To study the effect of pre-operative biopsy for breast cancer on the 10-year-survival rate, 1123 cases with single cancer radically operated at the Cancer Institute Hospital between Jan. 1960 and Dec. 1969 were analyzed. The survival rate in 292 biopsied and non-biopsied patients with cancers less than 2 cm in diameter and no lymph node metastasis was not significantly different. There was no difference between incisional- and excisional-biopsied patients. However, the survival rates tended to decrease as the interval between biopsy and radical surgery increased. We conclude that pre-operative biopsy does not worsen the prognosis in patients with breast cancer. Early detection and treatment are of great importance.

[Lymphedema and function of the arm after mastectomy for breast cancer].

We made an investigation of lymphedema and function of the arm in 1,115 patients after mastectomy. Slight edema was observed in 26.7 per cent, moderate edema in 3.9 per cent and severe in 0.9 per cent. Lymphedema is dependent upon the extensiveness of surgical operation. The incidence of moderate and severe edema is 2.5 per cent for 637 patients with modified radical mastectomy, 7.7 per cent for 440 patients with standard radical mastectomy and 10.5 per cent for 38 patients with extended radical mastectomy. The development of edema is more frequent in irradiated patients than in those without irradiation. No difference of the incidence of arm movement limitation is observed between modified radical mastectomy and standard radical mastectomy. Modified radical mastectomy, especially Auchincloss operation, offers a much cosmetic appearance.

[Previous abortions exert favourable effects on the prognosis of breast cancer patients].

The effect of previous abortions, both induced and spontaneous, on the prognosis of breast cancer patients was analysed, applying the hazard regression model of 1190 radically mastectomized Japanese women. The 10-year disease-free survival rate of 800 patients without abortion, 312 with one or two abortions and 78 with 3 or more abortions was 64.8%, 69.8% and 77.3%, respectively. Previous abortions had a significantly favourable effect on the prognosis when the effects of 8 other co-variants were taken into account (p = 0.008). Histologic findings on resected materials showed that patients with previous abortions had a higher incidence of well-differentiated breast cancer (25%) than those without abortions (19%) (p = 0.026). The former group had significantly less metastasis to visceral organs (11.8%) than the latter (17.5%) (p = 0.011) when the first recurrence site was investigated.

The prognostic value of EEG in neonatal meningitis.

EEG recordings were made polygraphically in 29 newborn infants with meningitis. Visual and auditory evoked potentials, and CT scans were also obtained in some of them. The neonatal findings of these examinations were well correlated with the clinical outcome. Background EEG activity was a good prognostic tool but could not indicate the type of complications, although persistent abnormality correlated with severe brain damage or development of complications. The polygraphically recorded EEG was of great value to diagnose the apneic seizures often encountered in neonatal meningitis.

[Laboratory and clinical studies of T-1982 (cefbuperazone) in pediatric infectious diseases].

Laboratory and clinical studies were carried out with T-1982 (cefbuperazone) in pediatric infectious diseases. Results were as follows. 1. The average serum concentrations of T-1982 following intravenous injection of 10 mg/kg and 20 mg/kg were 35.3, 64.7 micrograms/ml at 30 minutes, 25.5, 41.5 micrograms/ml at 1 hour, 12.4, 21.8 micrograms/ml at 2 hours, respectively. Dose-response was observed. Urinary recovery rates of T-1982 during 6 hours after injection of 10 mg/kg and 20 mg/kg were 57.3% and 73.6%, respectively. 2. The antibacterial activity of T-1982 against clinically isolated organisms was determined. T-1982 was more active than cefazolin and cefmetazole against K. pneumoniae, H. influenzae and E. coli. It was also effective against ampicillin-resistant E. coli. 3. Thirty-seven patients received daily 30-69 mg/kg of T-1982 t.i.d. for 5-9 days. The rate of satisfactory clinical response was 91.9%. 4. Side effects were diarrhea in 4 cases, diarrhea and rash in 1 case and slight elevation of GOT and GPT in 1 case. But these were transient and mild.

[Conditions suitable for the adjuvant chemotherapy of gastric cancer].

From a retrospective analysis of 1,169 cases of gastric cancer treated with curative gastrectomy and randomized controlled adjuvant chemotherapy of six different protocols from 1961 to 1981, good candidates for adjuvant chemotherapy were found to be those who had lymphatic spread of n1 to n2 or serosal involvement of S1 to S2 or stage II or III diseases. Multidrug combination therapy produced better survival than single drug therapy. Long term oral maintenance therapy was not yet concluded to be essential for the adjuvant chemotherapy of gastric cancer. Patients with relatively late stage disease should be treated earlier than two weeks after surgery.

[Evaluation of cefotetan in pediatrics].

Clinical trials were carried out with cefotetan (CTT) in pediatric infections. Results were as follows; The mean serum concentrations of CTT following intravenous injection of 20 mg/kg were 204, 97, 56, 15, 10 micrograms/ml at 15, 60, 120, 360, 480 minutes after injection. The serum half-life was 2.18 hours. 68.3% was excreted in urine within 8 hours after injection. In vitro, the antimicrobial activity of CTT was more active than CEZ and CMZ against E. coli, H. influenzae and K. pneumoniae. CTT was administered clinically to 22 pediatric patients with various infections; 9 pneumonias, 4 bronchopneumonias, 1 acute tonsillitis and 8 urinary tract infections. Overall efficacy rate was 95%. The favorable clinical response could be gained by the doses of 30 mg/kg with being given every 12 hours. Slight elevation of S-GOT and S-GPT with mild diarrhea was observed in 2 patients and eosinophilia in 1 patient. No other serious side effect was observed.

[Clinical evaluation of an ampicillin suppository (KS-R1) in infections in children].

The following results were obtained in clinical trials of new ampicillin suppository (KS-R1) in the pediatric infection. Twenty-four patients comprising 15 with respiratory tract infection, 9 with urinary tract infection were received KS-R1 at the dose ranging from 31 mg/kg to 75 mg/kg divided 3 or 4 times a day. The results in the respiratory tract infection were "excellent" in 8 and "good" in 6, and the efficacy rate was 93.3%. The results in the urinary tract infection were "excellent" in 4 and "good" in 3, and the efficacy rate was 77.8%. Diarrhea were observed in 4 patients, and anal pain was observed in 1. But the symptoms were transient. No abnormal laboratory findings were observed.

[Evaluation of cefpiramide in pediatrics].

Fundamental and clinical trials were carried out with cefpiramide (CPM) in pediatric infections. Results were as follows. CPM has a broad spectrum of activity against both Gram-positive and -negative microorganisms, including Pseudomonas. Half-lives of CPM were more prolonged than any others that have ever been reported on cephalosporin derivatives. The mean half-lives in the blood after infection were 4.76 hours and 4.14 hours, when the doses were 10 mg/kg and 20 mg/kg, respectively. The average recovery rates in the urine between 0 and 8 hours were 17.1% and 24.7%, when the intravenous doses were 10 mg/kg and 20 mg/kg, respectively. Thirty-two pediatric patients received CPM in doses ranging from 31.9 to 88.2 mg/kg divided mainly 2 times a day. They were respiratory tract infection in 23, urinary tract infection in 8, and SSSS in 1. The rate of satisfactory clinical response was 90.6%. Clinical side effect observed were mild diarrhea in 7 cases. Slight elevation of GOT and GPT were observed in 3 cases. All were considered to be minor.

[Case of carcinoid tumor of the breast].

A 74-year-old female with a left breast mass was admitted to the Cancer Institute Hospital and underwent mastectomy. Upon study of a frozen section, a diagnosis of breast carcinoma was made. Histopathological examination revealed a non-invasive tumor whose slightly eosinophilic rich cytoplasm was positive for PAS and Alcian-green staining and argyrophilic staining. The tumor showed medullary arrangement, partly with glandular and rosette-like structures. Electron microscopically, many dense core granules (170 +/- 30 nm in diameter) were diffusely distributed in almost all the tumor cytoplasm, they were thought to be neurosecretory granules. A diagnosis of mucocarcinoid of the breast was made. The patient experienced no dyshormonal disorder during the course of the disease and is alive and well without recurrence.

[Factors affecting estrogen and progesterone receptors in primary breast cancer].

The contents of estrogen receptor (ER) and progesterone receptor (PgR) in 323 primary breast cancers were analysed with respects to the influence of a patients menopausal status, age, body weight, height, childbirth, abortion, size of tumor, node involvement and pathological diagnosis. Influence of menopausal status on the distribution of ER indicate that 68% of premenopausal women and 67% of postmenopausal women were ER positive, while only 44% paramenopausal women were ER positive. ER level was found to be higher in postmenopausal than in premenopausal women. In postmenopausal women, 87% of overweight (55kg and more over patients), 52% of non-overweight were ER positive. There was no significant correlation between ER or PgR rate and height, childbirth, abortion, size of tumor, axillary node involvement, pathological diagnosis.

Characteristic echographic features of circumscribed cancer.

Of 964 amputated breast cancers which had been examined preoperatively by ultrasound and mammography, 147 breast (17.0%) contained macroscopically circumscribed tumors and 596 cases (69.0%) showed infiltrative tumors on examination of the cut surface. It is well known that infiltrative types of cancer show ultrasonic images that usually possess clear cut signs of malignancy, such as jagged augmented boundary echoes and acoustic shadows. On the other hand, circumscribed types of cancer do not usually show such signs, but exhibit lobulated shapes and strong, coarse and non-uniform internal echoes. These latter features are intermediate between those of infiltrative cancers and those of solid benign tumors. As such circumscribed cancers are often difficult to diagnose correctly by ultrasonic techniques. The diagnostic accuracy rates by ultrasound were 88.0% in the infiltrative types and 69.4% in the circumscribed types. The ultrasonic pictures of the 147 cases of such circumscribed tumors are classified and discussed.
