Ultrastructure of hypertrophic hemangioma.

A hypertrophic hemangioma of a 6-month-old girl was studied electron-microscopically. Hypertrophic hemangioma is characterized by few patent vessels, high cellularity, local aggressiveness, occasional mitotic figures and frequent recurrence after operation. The ultrastructural examination showed the tumor to be composed of three types of cells: endothelial cells, pericytes and phagocytic fibroblasts.

[In vivo effects of cefoperazone in local infection].

The in vivo antibacterial activity and the penetration into inflammatory tissues of cefoperazone (CPZ) were compared with those of cefotiam (CTM) in local infection systems using mouse subcutaneous abscess and rat granuloma pouch. The serum levels of CPZ in subcutaneous abscess in mice caused by Staphylococcus aureus F-230 (penicillinase producing strain) were lower than that of CTM, but there was no significance between the therapeutic effects of both drugs. The same results were obtained using Staphylococcus aureus F-196 (penicillinase non-producing strain). When infected with Enterobacter cloacae H-27 (cephalosporinase producing strain) in rat granuloma pouch, the exudate levels of CTM were lower than those of CPZ. Judging from these results, it was suggested that CPZ was a useful antibiotic for the treatment of bacterial-inflammatory tissues.

Localized chronic pemphigoid of Brunsting-Perry. Ultrastructural localization of IgG and complement components.

Immune electron microscopic data are presented of a typical case of localized chronic pemphigoid (LCP) of Brunsting-Perry. The peroxidase-antiperoxidase multistep method for the electron microscopic localization of immunoglobulins and complement components showed that IgG, C3, and C4 were distributed in the lamina lucida and on the undersurface of the basal keratinocytes. These findings support the concept that LCP, bullous pemphigoid, and benign mucosal pemphigoid are closely related diseases. Unlike in bullous pemphigoid, however, complement was occasionally found in this case of LCP beyond the basal lamina in the uppermost portion of the dermis. This finding may explain the occurrence of scar formation in LCP.

In vitro and in vivo antibacterial activity of T-1982, a new semisynthetic cephamycin antibiotic.

The activities of T-1982 (sodium 7 beta-[(2R, 3S)-2-(4-ethyl-2, 3-dioxo-1-piperazine-carboxamido)-3-hydroxybutanamido]-7 alpha-methoxy-3-[(1-methyl-1H-tetrazol-5-yl)thiomethyl]-3-cephem-4-carboxylate) against various gram-positive and gram-negative bacteria were compare with those of cefmetazole, cefoxitin, cefazolin, and cefoperazone. T-1982 was active against both gram-positive and gram-negative bacteria, including genera resistant to the other cephalosporins. T-1982 exhibited greater activity than did cefmetazole, cefoxitin, cefazolin, or cefoperazone against Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, and Serratia marcescens and was also highly active against Bacteroides fragilis. T-1982 was as stable to various beta-lactamases as were cefmetazole and cefoxitin. The therapeutic activities of T-1982 in mice experimentally infected with various gram-negative bacteria were superior to those of cefmetazole, cefoxitin, cefazolin, and cefoperazone.

Pulmonary function before and after surgical correction of scoliosis.

We compared the preoperative and postoperative pulmonary function of thirty-one scoliotic patients ranging in age from nine to twenty-five years. The mean postoperative follow-up period was three years and eight months. Twenty patients were treated by a posterior procedure with Harrington instrumentation. At more than two years postoperatively a significant improvement in the pulmonary function was noted, particularly in patients with a preoperative curve of less than 90 degrees (Cobb angle) and in those in whom the correction was greater than 30 per cent. The remaining eleven patients were treated by an anterior procedure, primarily a Dwyer operation, with or without posterior Harrington instrumentation. These patients manifested no remarkable improvement in pulmonary function more than two years after surgery; three patients showed deterioration. The results of tests performed less than two years postoperatively showed no improvement in pulmonary function, irrespective of the types of assessments used. We attribute our long-term improvements to a shortening of the postoperative period of plaster-cast immobilization and to the use of a plastic corset which allowed relatively free chest motion. We suggest that the Dwyer operation should be restricted to patients with a severe spinal deformity.

Detection of hepatitis B surface antigen subtype adr in an epidemic of papular acrodermatitis of childhood (Gianotti's disease).

Papular acrodermatitis of childhood (PAC) has recently been reported to be associated with hepatitis B surface antigen (HBsAg) subtype ayw. Between September, 1978, and June, 1979, we saw 14 patients with PAC in a small epidemic occurring in Iwakuni City, Japan. HBsAg was detected in sera from all patients. Subtyping of HBsAg in 11 patients showed that 8 had a determinant adr and 3 had no detectable determinant because of low antigen titers. The result suggests that factors other than the specific HBsAg subtype contribute to the development of PAC.
