[The study of washing operative wound with amikacin sulfate solution in the orthopedic surgery].

For the purpose of preventing postoperative orthopedic infections, investigation was carried out on effectiveness and safety of amikacin (AMK) after washing wound surface with its solution. 500 ml saline solution containing 400 mg AMK was used for washing. As the result, no postoperative infections were observed in all the 19 patients operated in our department. Neither abnormal findings to be regarded as abnormalities in blood test nor side-effects caused by washing were also observed. The blood samples obtained after washing did not show measurable AMK in all cases. It is said that use of antibiotics with 1) a wide-range of spectrum, 2) a bactericidal action and 3) low cross resistance against other antibiotics is preferable in washing wound surface in order to prevent postoperative infections. Under these circumstances, administration of aminoglycoside antibiotics is used frequently. Concerning the use of aminoglycoside antibiotics, however, it is necessary to pay attention to auditory and renal impairment. The previous study has revealed that AMK is lower in auditory and renal toxicity than other aminoglycoside antibiotics. AMK is thought, therefore, to be free from fear of auditory and renal impairment after washing and also to be one of effective antibiotics.

The role of metal and bone cement fixation in the management of malignant lesions of the vertebral column.

Fifteen patients with malignant spinal metastases underwent fixation of the spine using metallic implants and bone cement. The operation provided considerable relief of pain in all 15 cases and a significant improvement in mobility in 12 cases. These results compared favourably with 77 patients with fatal metastatic cancer of the spine who were treated without spinal surgery. The technique seems to be best indicated for use in patients with breast and thyroid cancer with a relatively prolonged terminal course.

Congenital non-progressive myopathy, associated with scoliosis--clinical, histological, histochemical and electron microscopic studies of seven cases.

Five cases of central core disease, one case of multicore disease and one case of non-specific myopathy, all of which were associated with scoliosis, were studied in regard to clinical features, histology, histochemistry and electron microscopy. The characteristic clinical features were non-progression, delayed motor development and proximal muscle weakness. Muscle biopsy was the most important diagnostic procedure. Routine histology might not reveal core regions where oxydative enzymatic activity was decreased or was absent in the muscle fibers, but histochemistry disclosed clearly single or multiple core regions. Electron microscopy showed a structured or unstructured core in central core disease. Scoliosis was commonly associated and was very similar to idiopathic scoliosis. Some curves did not progress while others progressed and did not cease progression with maturation. Surgical correction and stabilization was carried out on two cases without complication. The importance of recognition of these myopathies was emphasized.

Adenylate cyclase activity in rat myocardium following chronic infusions of angiotensin II and epinephrine.

Adrenergic receptor response coupling pathways have been shown to differ in hypertrophied hearts in different models of hypertension. To mimic this, chronic subcutaneous infusions of epinephrine (80 nmol/h for up to 13 days) and angiotensin II (AII) (4.3 nmol/h for up to 4 weeks) were given. Myocardial-, basal-, and isoproterenol-, glucagon-, forskolin-, and Gpp(NH)p-stimulated adenylate cyclase were measured. No changes in enzyme activity were seen following AII infusion, even though myocardial hypertrophy was significant. After epinephrine infusion for 6 days, there was a decrease in isoproterenol stimulated enzyme. After 13 days of infusion, cyclase activity, both basal and stimulated, was reduced. We conclude that in hearts from different models of experimental hypertension associated with cardiac hypertrophy, there are different biochemical alterations in the beta-adrenergic receptor response coupling mechanism.

Beta adrenergic receptor response coupling in hypertrophied hearts.

Reports in the literature have indicated that inotropic responsiveness to isoproterenol (ISO) of hypertrophied rat myocardium is decreased. We have studied possible biochemical mechanisms to explain this. Adenylate cyclase activity in myocardial membranes from 13-week-old spontaneously hypertensive rats (SHR) was unchanged compared to enzyme activity in Wystar-Kyoto (WKY) when measured under basal conditions or following NaF, a guanosine triphosphate analog, MnCl2, or forskolin stimulation. However, ISO-stimulated cyclase activity was decreased as were beta-adrenergic receptor density with no change in receptor affinity. On the other hand, hearts from two-kidney, one clip renal hypertensive rats 6 weeks after initiation of hypertension showed decreased basal, ISO, NaF, and GTP analog-stimulated cyclase activity with no change in Mn++ or forskolin-stimulated activity. In this model, receptor density increased. When the clipped kidney was removed, these changes returned toward normal as did the blood pressure and heart weight. We interpret these data to indicate that in SHR the biochemical defect leading to decreased inotropic responsiveness of hypertrophied hearts is due to decreased beta-adrenergic receptor density, leading to decreased ISO-stimulated cyclase activity. The nucleotide regulatory protein component (N) and the catalytic subunit (C) are not changed. In the renal hypertensive rat, on the other hand, although the C is unchanged, there is a decrease in the activity of N, and this is probably the primary reason why inotropic responsiveness to ISO is decreased. Increases in receptor density seen in this model may possibly be compensatory.

Clinical correlates of prolonged pain in Japanese patients with acute herpes zoster.

To determine which risk factors are relevant to the occurrence of post-herpetic neuralgia in Japanese patients with acute herpes zoster, correlations between the prolongation of pain and various disease factors were examined in 263 adult patients presenting within 10 days of the onset of rash at 17 institutions in the Hyogo region of Japan. All patients in whom pain persisted for more than 3 months were over 60 years of age. The pain also tended to be more prolonged in those with clustered vesicles, disturbed sleep and hypanaesthesia. Other factors such as underlying disease states, critically involved regions, scar tissue, generalized rash and allodynia were not relevant to the duration of pain. Although decreased pain persistence was observed in patients in whom acyclovir therapy was initiated within 72 h of the onset of symptoms in comparison with those in whom it was initiated after this time, the difference between the two groups of patients was not statistically significant. Our results suggest that advanced age, the presence of clustered vesicles, and disturbed sleep and hypanaesthesia influence the prolongation of herpes zoster pain.

Type 2 segmental manifestation of disseminated superficial porokeratosis showing a systematized pattern of involvement and pronounced cancer proneness.

Nine tumors of squamous cell carcinoma developed in a 61-year-old Japanese woman with linear porokeratosis. She had disseminated superficial porokeratosis, but a linear arrangement of pronounced lesions was found only on the left side of the body, and all of the tumors arose on the linear lesions on the left side of the body. Some of her family members had disseminated superficial porokeratosis. This case may represent a type 2 segmental manifestation of disseminated superficial porokeratosis showing a systematized pattern of involvement and pronounced cancer proneness.

Extramammary Paget's disease of the genitalia with clinically clear margins can be adequately resected with 1 cm margin.

For the treatment of extramammary Paget's disease (EMPD), wide excision has been recommended because of unpredictable spread of tumor cells. EMPD lesions are often well circumscribed. Should all the lesions of EMPD be resected with a 3 cm margin? Forty-six patients with EMPD were surgically treated with a 1 cm margin. Width of tumor cell free area from the last lesional cells at the borders to the resected edge was measured with micro-oculometer. The microscopic gap between the histopathological tumor border and the clinical border scored by scalpel tract was also measured. The tumor cell free area measured 10.2 +/- 2.48 mm. The microscopic gap between the histopathological and clinical borders measured 0.334 +/- 1.183 mm. Thus, the clinically determined border of well-defined lesions of EMPD corresponded well to the histopathologic border. No local recurrence was observed in 24 to 115 months of follow-up. Well-demarcated lesions of EMPD can be adequately managed with 1 cm margin resection.
