[Laboratory and clinical studies on cefmenoxime in respiratory tract infections].

Cefmenoxime (CMX, Bestcall) was administered by drip infusion to 65 patients with various respiratory infections and its effect was evaluated. The rate of effectiveness was 86.2% in all cases including 40 cases of acute pneumonia and 12 cases of chronic bronchitis. CMX was examined comparatively with other drugs for antibacterial activity on clinically isolated strains of 2 bacterial species, i.e. H. influenzae and B. catarrhalis, each of which has a high frequency of clinical isolation from infected respiratory organs. On H. influenzae CMX exerted the strongest antibacterial activity among test drugs (ampicillin, piperacillin cefoperazone, latamoxef) regardless of the production of beta-lactamase by the organism, while on B. catarrhalis it also exerted an antibacterial activity strong enough to control the proliferation of all strains at a dose level of 0.39 microgram/ml. Drip infusion of this drug (2 gram) brought about an average maximum blood concentration of 127.2 +/- 21.5 micrograms/ml and an average half-life in blood of 1.10 +/- 0.28 hours. However, different values were obtained for different individual cases because these subjects were patients of chronic respiratory infections each having some underlying disease or other. Side effects of the drug were observed as allergic symptoms such as pyrexia, eruption and the like, but only 5 cases without any serious cases. Increases in transaminase suggestive of abnormal clinical test results were also observed in 5 cases. However, numerical recoveries to the normal values were obtained in all of these 5 cases with the withdrawal of the drug.

Hypotensive and natriuretic effects of nifedipine in essential hypertension. Role of renal kallikrein-kinin-prostaglandin and renin-angiotensin-aldosterone systems.

To assess the role of renal kallikrein-kinin-prostaglandin and renin-angiotensin-aldosterone systems in the diuretic and natriuretic actions of nifedipine, a calcium-channel blocker, 20 mg of nifedipine was administered orally to 15 patients with essential hypertension. Nifedipine promptly induced a hypotensive effect and an increase in pulse rate. Urine volume, urinary sodium excretion, and creatinine clearance were significantly increased after the administration of nifedipine by 63.5%, 48.5% and 12.4%, respectively. Urinary excretion of kallikrein and prostaglandin E were also significantly increased after the administration of nifedipine by 29.4% and 50.0%, respectively. The change in urinary kallikrein excretion was significantly correlated with that in urine volume (r = 0.70, p less than 0.01) or that in urinary sodium excretion (r = 0.86, p less than 0.01). In addition, the change in urinary prostaglandin E excretion was also significantly correlated with that in urine volume (r = 0.72, p less than 0.01) or that in urinary sodium excretion (r = 0.53, p less than 0.05). Plasma aldosterone concentration did not change despite of the marked increase in plasma renin activity, and plasma aldosterone concentration/plasma renin activity ratio decreased after the administration of nifedipine. These results suggest that the augmented renal kallikrein-kinin-prostaglandin system and the suppressed secretion of aldosterone may be associated with the diuretic and natiuretic action of nifedipine and may contribute to the reduction in blood pressure that is caused mainly by its vasodilatory action.

Immediate repair of mandibular defects following surgery for carcinoma of the lower alveolus and gingiva using a pectoralis major osteomyocutaneous flap.

During the two years from 1981 to 1983, a pectoralis major myocutaneous island skin flap, with the 5th rib or sternum, has been used for immediate repair of mandibular defects following surgery in five cases of carcinoma of the lower alveolus and gingiva. In these 5 patients with segmental and hemimandibulectomy, immediate mandibular reconstruction was performed simultaneously using a flap with the 5th rib or the sternum. Thereafter, in 3 of them, the contour and function of the mandible were restored by prosthetic appliances. In the remaining 2 patients prostheses could not be used because of mandibular discontinuity due to removal of a portion of the graft because of postoperative infection and pseudo-arthrosis. Mandibular reconstruction using the osteomyocutaneous flap is advantageous in the restoration of the contour and function of the mandible. Some problems concerning the reconstruction will be reported.

Atrial natriuretic factor inhibits the hypertension induced by chronic infusion of norepinephrine in conscious rats.

To assess the physiological role of atrial natriuretic factors in the regulation of blood pressure and sodium-water excretion, we studied the chronic effects of continuous infusion of a synthetic atrial natriuretic factor of 25 amino acids for up to 3 days on systolic blood pressure, urine volume, and urinary excretion of sodium, prostaglandin E2 and kallikrein in conscious rats, and also evaluated the antihypertensive effect of this substance in rats with hypertension caused by chronic infusion of norepinephrine. Continuous infusion of atrial natriuretic factor (150 micrograms/kg per day) into the jugular vein via osmotic minipumps did not induce any changes in systolic blood pressure, urine volume, and urinary excretion of sodium, prostaglandin E2, and kallikrein for up to 3 days, compared with those in vehicle-infused rats. When the same dose of atrial natriuretic factor was administered simultaneously with 1.8 mg/kg per day of norepinephrine infused intraperitoneally by osmotic minipumps, the systolic blood pressure of conscious rats rose on day 1 to only 127.3 +/- 6.3 mm Hg compared with the rise to 146.3 +/- 1.6 mm Hg when norepinephrine alone was infused (P less than 0.05). The antihypertensive effect of atrial natriuretic factor was sustained for 3 days in rats infused with norepinephrine. The administration of atrial natriuretic factor to rats made hypertensive by 3 days of infusion with norepinephrine alone returned the blood pressure to control levels, and the antihypertensive effect was sustained throughout the experimental period lasting for 3 days.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of pentalogy of Fallot with numerous collaterals evaluated by magnetic resonance imaging: a report of an adult case].

In this report, the diagnostic value of magnetic resonance imaging (MRI) was compared with that of two-dimensional echocardiography, computed tomography and cardiac catheterization in a patient with pentalogy of Fallot who survived to her fortieth year. The advantages and disadvantages of MRI in diagnosing the present case were as follows: The cardiovascular system, with the exception of atrial septal defect, was evaluated precisely. Collateral vessels were detected using MRI, but impossible with other non-invasive methods. MRI was particularly suitable for imaging the cardiovascular system because of the high contrast between the lower intensity signal of the blood and higher intensity signal of the myocardium and blood vessel walls. Using MRI, data acquisition time was 1.5 min per section. Gated MRI required more time for data acquisition. However, various oblique tomographic projections and very clear static images could be obtained using gated MRI. MRI should be one of the best diagnostic techniques for diagnosing congenital heart disease.

Treatment of NZB/W F1 mice with NZW splenic T cells or with serum of mice experiencing the graft-versus-host reaction: suppression of ongoing anti-double-stranded (ds) DNA antibody formation and improvement of renal function.

Seven-month-old NZB/W F1 mice were inoculated with 5 X 10(7) NZW spleen cells. The mice so treated showed a clear fall of anti-double-stranded (ds) DNA antibody titers accompanied by a decrease in blood urea nitrogen (BUN) concentration. Mice so treated showed markedly increased survival times (more than 17 months) compared with untreated controls (mean +/- SD 8.38 +/- 0.75 months). A cell fractionation study suggested that T cells among NZW spleen cells play a major role. Recipients of the unfractionated or the T-cell fraction of NZW spleen cells had histologically less glomerulonephritis than untreated control NZB/W F1 mice. NZW B-cell transfer to NZB/W F1 mice had no effect. Serum obtained from NZB/W F1 mice receiving NZW spleen cells 10 days previously (graft-vs-host (GVHR) serum) was injected into 8-month-old NZB/W F1 mice. A clear fall in anti-ds DNA antibody titers and inhibition of age-associated sharp increases in BUN were observed. The mean life span of these mice (10.50 +/- 0.58 months) was significantly longer than that of untreated controls (8.69 +/- 0.38 months). GVHR serum had the ability to suppress anti-ds DNA antibody formation by NZB/W F1 spleen cells in vitro. Allogeneic GVHR serum prepared in C57B1/6 X DBA/2 (BDF1) mice by transferring C57B1/6 spleen cells was also effective in suppressing anti-ds DNA antibody formation in vitro.

Auranofin, an oral chrysotherapeutic agent, inhibits histamine release from human basophils.

The effect of auranofin, an oral chrysotherapeutic agent, on histamine release from human basophils was studied. Auranofin inhibited IgE-mediated, anti-IgE-induced histamine release in a dose-dependent fashion with a concentration of drug required to produce 50% inhibition of 1.3 micrograms/ml. This compound also inhibited calcium ionophore A23187 and 12-0-tetradecanoyl-phorbol-13-acetate-induced histamine release with a concentration of drug required to produce 50% inhibition of 1.7 micrograms/ml and 0.9 micrograms/ml, respectively. Auranofin was less active for formyl-L-methionyl-L-leucyl-L-phenylalanine-induced release of histamine with 32 +/- 10% (mean +/- SEM) inhibition at 2 micrograms/ml. Auranofin effects were reversible when leukocytes preincubated with auranofin was washed with buffer. In contrast, gold sodium thiomalate enhanced IgE-mediated histamine release, suggesting that these two compounds exert their actions at different levels. These results suggest that auranofin may be beneficial to patients with allergic diseases such as bronchial asthma.

Sex and age-related differences in the urinary excretion of TXB2 in normal human subjects: a possible pathophysiological role of TXA2 in the aged kidney.

Urinary excretion of thromboxane B2 (TXB2), a stable metabolite of TXA2, was measured by radioimmunoassay in 58 normal subjects (17 males and 41 females) aged from 19 to 98 years. The basal level of urinary TXB2 in normal subjects was 100 +/- 6.7 ng/day (mean +/- SEM). In young males, contamination of urine specimens with seminal fluid, which is known to contain large amounts of prostaglandin E, did not change the level of urinary TXB2. Urinary excretion of TXB2 was significantly higher in male subjects than in female (124 +/- 12.6 vs 90 +/- 7.4 ng/day, p less than 0.05). There was no significant age-related difference in 24-h urinary excretion of TXB2 in normal subjects. However, when urinary excretion of TXB2 was expressed as a function of urinary creatinine excretion, it was significantly higher in the elderly (60-93 years old) than in the younger subjects (19-59 years old)(141 +/- 15.7 vs 99 +/- 7.7 ng/g creatinine, p less than 0.02). Renal TXA2 may have a pathophysiological role in the functional impairment of the kidney in elderly people.

Renal prostaglandin E in the hypotensive mechanism of MK-421 in conscious rats.

To assess the role of renal prostaglandin E in the hypotensive mechanism of MK-421, we evaluated the effects of chronic infusion of MK-421 (6 mg/kg/day i.p.) on systolic blood pressure and urinary prostaglandin E excretion in conscious rats in states of sodium repletion or depletion and also during chronic infusion of norepinephrine (1.8 mg/kg/day i.p.) or vasopressin (7.2 U/kg/day i.p.). The hypotensive effect of MK-421 was greater in sodium depleted than in sodium repleted rats. The hypertensive effect of norepinephrine or vasopressin was inhibited by the simultaneous administration of MK-421. MK-421 induced an increase in the excretion of urinary prostaglandin E, in both sodium repleted and depleted rats. However, simultaneous administration of MK-421 had no influence on the increase in urinary prostaglandin E excretion induced by norepinephrine or vasopressin. In addition, the combined administration of MK-421 with indomethacin (10 mg/kg/day s.c.) still abolished the hypertensive effect of norepinephrine or vasopressin. The disparate effect of MK-421 on urinary prostaglandin E excretion suggests that the renal prostaglandin system is not essential for the mechanism of the hypotensive effect of MK-421.
