[Evaluation of chemotherapy in hepatocellular carcinoma with liver cirrhosis].

The present paper discusses the efficacy of cancer chemotherapy in 46 patients with advanced hepatocellular carcinoma. In most of patients, Adriamycin (20-40 mg) or Mitomycin C (20 mg) was given by one shot injection via the hepatic artery and followed by serial administration of anticancer agents such as 5-fluorouracil (300-750 mg/day), Adriamycin and Toyomycin. Results were as follows: 1) The complete response was not seen. 2) The partial response (more than 50% decrease of the tumor size) was observed in 6 of 46 patients (13%) for 34 to 457 days. 3) Absence of tumor thrombi in the portal vein which was observed by hepatic angiography, CT and ultrasonography, was closely correlated in the partial response and the prolongation of survival time. 4) There were no significant differences with the therapeutic response or survival time among 3 groups (E2: tumor occupation rate (TOR) 20-40%, E3: TOR 40-60%, E4: TOR above 60%). 5) Obstinate abdominal pain and abnormal liver function remarkably were improved during the chemotherapy in 11 of 18 cases (61%), and 6 of 46 cases (13 %), respectively. 6) Major causes of death were hepatic failure (45.7%), gastrointestinal bleeding (30.4%) and intra-abdominal rupture of the tumor (14.7%). 7) As side effect, some extent of hematopoietic suppression was observed in 25% of the patients treated.

[Clinical evaluation of cefroxadine in the field of obstetrics and gynecology].

Cefroxadine (CXD), an oral cephalosporin antibiotic was studied in the field of obstetrics and gynecology and the following results were obtained. CXD was orally given to 22 cases at daily dose of 1,500 mg 3 times a day. CXD administration was given to 22 cases in all; 4 with cervicitis, 6 with endometritis, 2 with puerperal fever, 4 with bartholinitis, 5 with adnexitis and 1 with vulvitis, respectively. Overall efficacy rate was 77.3% (17/22) (excellent 4, good 13, fair 5). As for side effects, a slight diarrhea was observed. CXD was considered to be a useful antibiotic in the field of obstetrics and gynecology by above the results.

An unusual portal-systemic shunt, most likely through a patent ductus venosus. A case report.

A 46-yr-old man with cirrhosis presented with mental confusion, headache, and nausea. Colloid liver scintigraphy showed a defect in the hilar area, and portograms after superior mesenteric arteriography demonstrated varicose veins in the same area. Computed tomography revealed a sausage-shaped low-density channel connecting the hepatic hilum and the inferior vena cava, and bolus injection contrast enhancement demonstrated transit of contrast medium from the portal vein directly into the inferior vena cava through this channel. Real-time ultrasound demonstrated a large conduit contiguous from the portal vein to the inferior vena cava through and behind the liver. The catheter introduced into the inferior vena cava opacified a large opening of this shunt below the right and left hepatic vein openings. Measurements of blood pressure, ammonia, and PaO2 clearly indicated that portal vein blood was being shunted through this channel. These findings are highly suggestive of a portosystemic shunt through a patent ductus venosus.

Preoperative use of 5-fluorouracil suppository for carcinoma of the rectum.

A total of 50 patients with carcinoma of the rectum were treated with 5-fluorouracil suppository before operation. The suppository, which was made of Witepsol suppository base containing 5-fluorouracil, yielded high drug concentrations in carcinoma, draining blood and regional lymph nodes. As a clinical response to the suppository, a significant decrease in the size of tumor mass was noted in 8 of 50 carcinomas, but in other cases the gross change was unmeasurable. Thirty-three percent of the surgically resected carcinomas were histologically judged to have responded to the suppository. In such cases, histologic changes correlated well with the total dose of 5-fluorouracil. The adverse effects of the suppository were confined to anal pain, tenesmus and anal bleeding, probably due to the topical effect of 5-fluorouracil on the rectal mucosa.

[Migration of subduro-peritoneal shunt into the subdural space of an infant (author's transl)].

A ten-month-old baby was admitted to our clinic with the diagnosis of bilateral chronic subdural hematomas. He had complained of general convulsion during the past 3 months. Evacuation of hematomas was performed on both sides through burr holes in the parietal region. Several weeks following the evacuation of hematomas, subduroperitoneal (S-P) shunting operation was further performed bilaterally for the collection of fluid in the subdural spaces, using a "Pudenz" shunting catheter with a reservoir. The catheter was fixed at the connector of the reservoir and the peritoneal catheter by several silk sutures to the epicranium. Three weeks after S-P shunting operation the right shunting system was not felt on his head on palpation, and the abnormal high density spots were shown inside the right subdural space on CT. The right craniotomy revealed that the whole shunting system was located in the right subdural space. Technical problems of the shunting operation and the several factors of the patient which may contribute to such a rare complication of this surgical procedure are briefly discussed.
