[Cooperative research of UFT E phase II study. Cooperative Study Group of UFT E in Tohoku Area].

Phase II clinical research of UFT E granules (enteric coated) was carried out in 18 institutes (21 clinical dept.) by Study Group of UFT E in Tohoku Area, to investigate its effect and safety on cancer of the digestive organs. Of all the registered 26 cases, 21 cases were available for evaluation (perfect cases: 17 and imperfect cases: 4). Patients were administered UFT E 600 mg/body/day in principle. The response rate of the overall 26 cases was 14.3% and that of perfect 17 cases was 17.6%. PR was seen in 2 cases with far advanced gastric cancer and in 1 case with sigmoid colon cancer metastasized to lung, NC in 8 cases and PD in 6 cases. Side effects more than Grade II were seen in 4 cases (19%), of which 1 case caused diarrhea with leucopenia, 1 case caused diarrhea with anorexia, fever, pigmentation, stomatitis and general tiredness, 1 case caused anorexia and the other 1 case caused paresthesia on both legs with diarrhea and anorexia. Side effects in upper digestive tract were slight, making it possible to continue administration. But 1 case, which caused simultaneously Grade II anorexia, Grade II diarrhea and Grade III paresthesia on both legs, refused administration and dropped out. He recovered from those symptoms 5 days after discontinuance of administration. UFT E is able to administer for a long term because of its slight side effects on the upper digestive tract.

[Brachial plexus injury and fracture of the first rib as complications of median sternotomy].

In 192 patients following median sternotomy during from 1978 to 1988, we studied the incidence, cause, prognosis and correlation of brachial plexus injury and fracture of the first rib as complications of median sternotomy. Four brachial plexus injuries were identified, an incidence of 2.1%, seven first-rib fractures were identified, an incidence of 3.6% and one patient had both complications in this series. Symptoms of this patient were more severe and difficult to improve than those of patients without first-rib fracture. Stretching of the nerve by the displaced first rib, ischemia of the nerve by pressure on it, and direct injury by the fractured rib end are causes of the brachial plexus injury following median sternotomy. It is due to the difference of position of the fracture, the difference of direction and degree of the protrusion of the fractured rib end, whether presence or absence of the brachial plexus injury in patients with first-rib fracture.

Studies on reflux nephropathy--clinical investigation of renal lesions.

Renal biopsies were performed on 25 patients with reflux nephropathy to clarify the relationship between the pathological findings and clinical parameters. Biopsy specimens were obtained at anti-reflux operation by open renal biopsy. Glomerular lesions were classified as focal, diffuse, segmental and global sclerosis. Tubulo-interstitial changes were defined according to the principle advocated by Cotran and expressed as the percentage of the lesions. The clinical parameters consisted of the grades of VUR, the grades of renal scarring, renal function and the daily urinary protein excretion. A significant relationship was noted between the grades of VUR and those of renal scarring. The percentage of global sclerosis in the kidney revealed a close relationship with the grades of VUR and the percentage of tubulo-interstitial changes but not with the renal function or the daily urinary protein excretion. Focal segmental hyalinosis and/or sclerosis (FSHS) lesions were presented in 3 of 25 cases (12%). The pathological findings of the FSHS lesions had a close relationship with the renal function and the daily urinary protein excretion and the prognosis of the kidney. The other patients without FSHS lesions displayed an unchanged renal function. Based on these observations, it is clear that the prognosis of the patients with vesico-ureteral reflux could be predicted from the histopathological findings; the presence of FSHS lesions suggests a poor prognosis for the kidney. The degree of daily proteinuria represents a useful parameter for evaluating the progression of the reflux nephropathy.

[Ventriculoperitoneal shunts with the use of pressure-adjustable valve in the management of hydrocephalus].

Sixteen patients with hydrocephalus of varied etiology who underwent ventriculo-peritoneal shunts utilizing pressure-adjustable valves were reviewed, and the usefulness of the pressure-adjustable valve in the management of hydrocephalus was evaluated. Before shunting all patients had CT evidence of hydrocephalus with Evans' Index greater than 30%. High-, medium- and low-pressure settings were chosen in 3, 11 and 2 patients respectively, according to the CSF pressure as measured during lumber puncture or ventricular drainage. Following the shunting procedures with programmable pressure valves, hydrocephalus was alleviated with Evans' Index being 27% on the average, as evaluated by CT scans. Such clinical symptoms as disturbed consciousness, dementia and increased intracranial pressure all subsided. There were no mortality, infection or revision during the follow-up period of 5 to 16 months. Of particular note was that, among these 16 cases, there were three illustrative cases in which programmable pressure valves were found useful in coping with newly developed subdural effusion and low-intracranial-pressure syndrome, by manually adjusting the valves to higher pressure settings.

[Automated image processing by PACS as a simple tool for designing craniotomy; synthesis of sagittal reconstructed CT images and cerebral angiograms].

Software has been developed to produce sagittal images of the brain recontracted from axial CT slices and automatically superimpose them on the lateral views of cerebral angiograms. A commercially available system for digitization, processing, display and computer storage of film radiographs (EFPACS-500) was used in the present studies. Axial CT scans are obtained in either 3, 5 or 10mm thicknesses parallel to the base line at the 10 degree angle to the OM line, and processed into the digital storage. Plain skull films and subtracted angiograms, on which two reference points on the same base line for superimposition were marked prior to processing, are also digitized and stored in the optical disk. On each CT slice, the ratio is calculated of a lesion's longitudinal length to the midsagittal distance of the head. By simply adjusting a linear cursor, which is parallel to the base line, to the highest and the lowest levels of the lesion, the sagittal image with the lesion clearly depicted is automatically reconstructed. And on this image an angiogram can be superimposed automatically simply by indicating two reference points on the base line on both images. The resulting images have markedly facilitated simple designing of craniotomy for intracranial lesions by graphically indicating their locations along with cerebral vessels in a lateral view.

[Intrapleural administration of CDDP against malignant pleural effusions in breast cancer].

Malignant pleural effusion in the patients with breast cancer commonly occurs, and is a life-threatening factor. The present paper shows the usefulness of intrapleural administration of CDDP in six cases. A decrease of pleural effusions was observed in all cases. Treatment was effective in two cases of CR and four cases of PR. A median survival from initiation of intrapleural therapy is 17 months (range 2-47 months). This procedure produced distinctly fewer side effects than intravenous administration. The results of this trials suggest that CDDP should be considered as an active agent in the treatment of malignant pleural effusion in the patients with breast cancer.

Neurite outgrowth from mouse neuroblastoma and cerebellar cells induced by the protein kinase inhibitor H-7.

The protein kinase inhibitor H-7 (1-(5-isoquinolinesulfonyl)-2-methylpiperazine dihydrochloride) or staurosporine was found to induce neurite outgrowth from mouse neuroblastoma N18TG2 cells and cultured cerebellar cells by light microscopy. Examination by electron microscopy revealed some morphological differences between the neurites of N18TG2 cells induced by H-7 and dB-cAMP, in which H-7 formed longer and thinner neurites and more abundant localized varicose structures along neurites than dB-cAMP. These results indicate that protein kinase inhibitor itself can act as a potent stimulus for inducing neurite outgrowth in a different manner from dB-cAMP.

Identification by monoclonal antibody of the tumor antigen of a human autologous breast cancer cell that is involved in cytotoxicity by a cytotoxic T-cell clone.

We have already established a pair of human autologous clones, tumor-specific cytotoxic T-lymphocyte clone TcHMC-1 and tumor target clone HMC-1-8, that were derived from the metastatic pleural effusion of a patient with mammary carcinoma. In this paper, we describe the target antigen that was defined by monoclonal antibody 3A2. This monoclonal antibody selectively inhibited the cytotoxic action of TcHMC-1 against HMC1-8 autologous tumor target cells, but not the cytotoxicity of lymphokine-activated killer and possibly natural killer cells against HMC-1-8 cells. Western blot analysis using the 3A2 monoclonal antibody identified a molecule with an approximate molecular weight of 92,000. This antigen was highly expressed on autologous primary cancer cells of breast carcinoma tissue, but not on the normal mammary gland in the same patient. Moreover, this antigen can be detected on approximately 50% of human allogeneic breast carcinomas, but not on other neoplastic tissues such as gastric and colonic carcinomas except for one out of 10 prostatic carcinomas. Nonneoplastic normal cells did not express this antigen. It was also suggested that the antigen is not murine mammary tumor virus-related products. These data suggest that 3A2-defined antigen could participate in the cytotoxicity by human autologous cytotoxic T-lymphocytes as the target molecule expressed on tumor cells.

Functional analysis of mononuclear cells infiltrating into tumors. V. A. soluble factor involved in the regulation of cytotoxic/suppressor T cell infiltration into tumors.

We have analyzed the mechanisms controlling the accumulation of cytotoxic/suppressor T lymphocytes in tumor tissues. We found that tumor-infiltrating helper/inducer T cells isolated from T-9 gliosarcoma-sensitized rats between 4 and 6 days after T-9 gliosarcoma inoculation produced a lymphocyte migration factor (LMF) during in vitro culture. Four peaks of LMF activity (A through D) were detected upon fractionation of LMF by using a Mono Q anion exchange column chromatography. Peak C exhibited the strongest activity among the four peaks of LMF. The action of peak C was chemotactic, but not chemokinetic. Peak C had an isoelectric point of 8.0 and a Mr of 26,000 Da. Only cytotoxic/suppressor T cells were found to be sensitive to peak C in vitro as well as in vivo. It is thus likely that peak C is responsible for the infiltration of cytotoxic/suppressor T cells into tumor tissues. The infiltration of lymphocytes into tumor tissues might also be regulated by the expression of lymphocyte sensitivity for LMF. The target molecule for LMF at 4 days may involve an asparagine-linked oligosaccharide.

Diagnosis of gastric cancers with fluorescein-labeled monoclonal antibodies to carcinoembryonic antigen.

Forty gastric adenocarcinomas in 30 resected stomachs were examined by an endoscopic immunofluorescent technique using fluorescein isothiocyanate (FITC)-labeled antibodies to carcinoembryonic antigen (CEA). Fluorescence images of the tumors were obtained with a newly developed endoscopic television system for detecting faint fluorescence. Computer-assisted processing was used to enhance the detection and localization of the tumors visualized by the immunofluorescent technique. Twenty seven (90%) of 30 tumors showed positive fluorescence after treatment with FITC-labeled antibodies for 60 minutes, whereas only two (40%) of five cancers could be detected by the immunofluorescent technique after treatment with antibodies for less than 60 minutes. There was no significant relationship between positive fluorescence and the gross and histological types or the stages of the tumors. In contrast, no positive fluorescence could be demonstrated in cases of benign gastric lesions or after pretreatment with FITC-labeled alpha-fetoprotein. This study therefore suggests that the immunofluorescent technique using FITC-labeled anti-CEA antibodies can be useful in detecting the early stages of gastric cancer.

Mitogenic activities for skin fibroblasts in the sera from untreated scleroderma patients at the early stage.

Mitogenic activities for adult skin fibroblasts in 17 sera from untreated patients with scleroderma at the early stage were studied. Mitogenic activities in 10% test sera were measured by the [3H]-thymidine incorporation assay using confluent fibroblasts; there was no significant difference between scleroderma-affected and normal controls. Growth-stimulatory activities for subconfluent fibroblasts in 10% test sera were measured by increased cell numbers; again, there was no significant difference between scleroderma-affected and normal controls. There have been contradictory reports on growth-stimulatory activities in scleroderma sera; our data do not suggest the presence of a specific mitogen in scleroderma sera.

Intracranial invasion of scalp carcinoma. Report of five cases.

Among 10 patients with squamous cell carcinoma admitted to our hospital, 5 showed intracranial invasion and 4 died with regrowth of intracranial tumour 9 to 24 months after incomplete extirpation of tumour. We reviewed our experience in the management of advanced scalp carcinoma, from neurosurgical and neuroradiological points of view. Dural invasion failed to be removed completely because of involvement of the middle part of the superior sagittal sinus. Therefore, total resection, facilitated by early detection prior to intracranial invasion, is mandatory for successful treatment of these scalp carcinomas.

Physiological changes during temporary occlusion of the superior vena cava in cynomolgus monkeys.

To evaluate the physiological changes that occur after temporary occlusion of the superior vena cava, we clamped the vena cava for one hour in 6 cynomolgus monkeys, monkeys with a venous system most resembling that of humans. The data from arterial blood gas analysis, ie, pH, arterial oxygen tension, arterial carbon dioxide tension, and HCO3-, were within normal limits during and after occlusion of the superior vena cava. Intracranial pressure was 8.6 +/- 0.8 mm Hg (mean +/- standard error) before occlusion and rose to 22.1 +/- 2.2 mm Hg during clamping. It decreased significantly to 17.7 +/- 1.9 mm Hg just before removal of the clamp and recovered to 8.6 +/- 0.9 mm Hg after the clamp was removed. Regional cerebral blood flow was 45 +/- 9 mL/min/100 g before clamping and decreased to 37 +/- 3 mL/min/100 g during clamping. It recovered to 47 +/- 5 mL/min/100 g after removal of the clamp. Cerebral perfusion pressure was within the margin of safety during clamping. Histological findings in the brain showed the effect of congestion in 1 monkey, but the change was slight. The electroencephalogram and electrocardiogram showed no abnormalities in this experiment. In conclusion, one-hour clamping of the superior vena cava with the azygos vein ligated was safe in 6 cynomolgus monkeys. We believe that in the clinical setting, one-hour occlusion of the superior vena cava would result in findings similar to those in this study, unless particular complications, such as arteriosclerosis or a cerebrovascular disorder, exist.

Longevity of expanded polytetrafluoroethylene grafts for superior vena cava.

We previously reported that the expanded polytetrafluoroethylene (ePTFE) graft for superior vena cava (SVC) substitution presents the problems of flexion and kinking when the graft is long. We therefore replaced the SVC of dogs with two types of prosthetic substitutes, ePTFE (Gore-Tex) and spiral-supported ePTFE (Im/praflex), and evaluated the long-term patency of the prosthetic substitutes. Total replacement of the SVC was performed in 9 adult mongrel dogs. The substitutes were ePTFE and spiral-supported ePTFE in 5 and 4 dogs, respectively. The animals were killed about 3 years after replacement of the SVC, and the harvested specimens were histologically examined by light microscopy and scanning electron microscopy. Evaluation of ePTFE revealed late occlusion in 1 of 5 dogs. The spiral-supported ePTFE showed patency in all dogs. In the group with ePTFE grafts, light microscopic examination revealed abnormalities of endothelial cells, granulation, and necrosis. There was no hyperplasia of the subendothelial connective tissue near the center. In the animals with spiral-supported ePTFE grafts, the subendothelial connective tissue showed favorable growth even in the center of the reconstructed site. There was no granulation in the spiral-supported ePTFE group. Scanning electron microscopic examination in the ePTFE group showed that endothelial cells were spindle-shaped and had an irregular surface. The spiral-supported ePTFE group showed an almost regular form of endothelial cells and no abnormalities except for the slightly spindled shape in the center. Therefore, we recommend that spiral-supported ePTFE should be used as an SVC substitute in clinical situations.
