[Coronary flow reserve index corrected by exercise load using Tl-201 SPECT: evaluation by PTCA].

For simple, non-invasive evaluations of coronary flow reserve during exercise, we investigated the coronary flow reserve index using the ratio of change of fractional distribution in the regional myocardium to oxygen demand. The subjects included 24 patients with effort angina pectoris and 11 normal volunteers. PTCA was performed for 11 patients. Using the biological properties of Tl-201, single photon emission computed tomography (SPECT) was performed at submaximal exercise and at rest by two successive injections of Tl-201 within a short time interval (double dose method). The correlation between the fractional distribution in the myocardium (delta-Fract) and the oxygen demand during exercise and at rest were evaluated. delta-Fract correlated well with the rate of change of the pressure rate product (delta-PRP), and delta-Fract/delta-PRP (CRI) was a good indicator of the coronary flow reserve index. CRI in the ischemic region correlated well with the percent area stenosis of the supplying coronary artery and improved with resolution of percent area stenosis of the target coronary artery after PTCA, but in some cases they were not above the normal range. CRI as obtained by exercise SPECT using the double dose method is useful for non-invasively estimating the coronary flow reserve during exercise, and it can be used for evaluating the effect before and after PTCA.

[Regional myocardial blood flow measurements in resting and dipyridamole induced vasodilative state by Xenon-133 clearance method].

Myocardial imaging with Xenon-133 and a gamma camera was employed to evaluate the relationship between angiographically demonstrated coronary artery stenosis and regional myocardial blood flow (r-MBF) in 22 patients with ischemic heart disease and 9 control subjects. After a left anterior oblique cineangiogram was obtained, the cinecamera was replaced by the mobile gamma camera in the same plane and location over the pericardium of patients. Then, 370 MBq (10 mCi) of 133Xe was injected into the left coronary artery as a bolus. After the first measurement of r-MBF in resting state, a second injection was made 3 minutes after intravenous injection of 0.56 mg/kg of dipyridamole for 4 minutes. The r-MBF in different regions of the heart was calculated by Kety formula. Coronary flow reserve (CFR) was defined the rMBF ratio between resting and dipyridamole induced vasodilative state. In the resting state, good correlation (r = 0.711) was observed between pressure rate product and the r-MBF at the areas perfused by non stenotic vessels. The r-MBF of the areas perfused by severe stenotic vessels (greater than 90%) was smaller than that of the areas perfused by non stenotic vessels, however there was overlapping of flow. In the dipyridamole induced vasodilative state, the r-MBF of the areas perfused by intact vessels increased approximately 2.4 times of the resting state, and showed good correlation with pressure rate product. In contrast, the r-MBF of the areas perfused by stenotic vessels did not correlate with pressure rate product, but depended on the degrees of coronary stenosis (50-75% stenosis; 1.9 times, greater than 90% stenosis; 1.3 times, respectively).(ABSTRACT TRUNCATED AT 250 WORDS)

[Regional myocardial coronary blood flow reserve in hypertrophic cardiomyopathy assessed by digital subtraction coronary angiography].

Previous observations suggest the presence of ischemia in the disproportionately thickened interventricular septum (IVS) of patients with hypertrophic cardiomyopathy (HCM), although the details remain obscure. Utilizing digital subtraction coronary angiography (DSA) with LAO projection before and after intracoronary papaverine (P) injection, we evaluated regional myocardial coronary blood flow reserve (rMFR) consecutively 18 patients with HCM, and compared it with that of 8 patients without apparent cardiac abnormality (C). Time-density curves were obtained from digital angiograms of the myocardial region of interest. We measured peak contrast density (Cm) and time to peak contrast (Tm). An index of rMFR was calculated as the quotient of Cm/Tm before and after P. In HCM, rMFR in IVS and apex was significantly lower than that of C (Mid-IVS: 1.9 +/- 0.5 vs 3.9 +/- 0.5, p less than 0.001; Low-IVS: 2.0 +/- 0.5 vs 4.4 +/- 0.9, p less than 0.001; Apex: 2.0 +/- 0.7 vs 4.5 +/- 1.6, p less than 0.01). There was correlation between the impairment of rMFR and the extent of hypertrophy in HCM. In conclusion, we could state that, in HCM, the region of impaired myocardial coronary blood flow reserve is localized. In HCM, DSA is useful in evaluating myocardial coronary blood flow reserve.

[Endoscopic treatment of early gastric cancer in patients undergoing chronic hemodialysis].

Endoscopic treatment of early gastric cancer (type Ila in all patients) was performed in four patients undergoing chronic hemodialysis. Surgical treatment was contraindicated because of various risk factors in these patients. Endoscopic treatment was performed by concomitant use of YAG laser irradiation, local ethanol injection and mucosal resection. After endoscopic treatment, stomach biopsy revealed no cancer in four patients. In one patient who is alive now, no sign of cancer recurrence has been noted for about two years after endoscopic treatment. This patient has undergone hemodialysis on outpatient department. The remaining three patients died from diseases other than gastric cancer. When one of them was autopsied, no gastric cancer demonstrated. Taking the prognosis related to life in to consideration, endoscopic treatment should be used positively when surgical treatment of early gastric cancer cannot be performed in patients undergoing chronic hemodialysis.

[Nuclear cardiological examination for evaluation of hypertrophic cardiomyopathy with midventricular obstruction and apical aneurysm: a case report].

Midventricular hypertrophy (MVO) is a rare and complicated myocardial disease. Its pathophysiology and prognosis remain unknown, and few nuclear cardiological findings for MVO were reported. On a 44-year old man with MVO, thallium-201 (Tl) myocardial scintigraphy and gated blood pool scintigraphy (GPS) were performed to evaluate their usefulness. GPS revealed a characteristic hour-glass deformity of the left ventricular cavity, apical aneurysm and asynchrony due to obliteration of the midventricle. Anteroseptal hypertrophy in midventricle and myocardial damage in apico-inferior region were detected by Tl. Thus, nuclear cardiological studies are proved to be useful and essential in not only diagnosis but also evaluating pathophysiology and observing natural history noninvasively.

Effect of postpartum intrauterine treatment with 2% polyvinyl-pyrrolidone-iodine solution on reproductive efficiency in cows.

The effect of a routine postpartum intrauterine infusion of 50 to 100 ml 2 % polyvinylpyrrolidone (PVP) -iodine solution on the reproductive performance of dairy cows was investigated in field trials. At a regular herd visit, alternate cows (n = 531) at around one month after calving (maximum 50 and minimum 20) were treated in utero with PVP-iodine. Additional cows (n = 474) were left untreated or were infused in utero with physiological saline and served as controls. The first service conception rate, overall conception rate within 180 d after calving, and average interval in days from parturition to conception were 48.5 and 74.5 %, 98 +/- 36 d in the treated group and 54.9 and 75.9 %, 90 +/- 34 d in the controls. Thus, the routine postpartum treatment with 2 % PVP-iodine solution was not effective in improving the reproductive efficiency of cows. The results were nearly the same in all groups, across herd, year of trial, parity, interval from parturition to treatment and dose of PVP-iodine solution. Besides, for cows with endometritis, an intrauterine infusion of PVP-iodine solution was not only ineffective but it was detrimental to fertility.

[Controlled study of MQF-OK therapy with FT and with UFT on various advanced gastrointestinal cancers. Hirosaki Cooperative Study Group of Cancer Chemotherapy].

Our cooperative study group carried out a controlled study of MQF-OK therapy with tegafur (FT) (group A) and with UFT (group B), a compound of FT and uracil in the molar ratio of 1:4, on various advanced gastrointestinal cancers. From January 1985 to May 1987, 91 patients were entered into this study, and 11 cases were ineligible for the protocol (11%). Fifty of 80 cases had advanced gastric cancer, 30 had pancreatic cancer or other cancers, such as colon cancer, and biliary tract cancer. They were divided into group A or B at random. There was no difference in the patient characteristics between group A and B. In gastric cancer, 23 of 50 cases were randomized into group A and 27 cases into group B. Two cases in group A (8.7%) and 7 cases in group B (25.9%) showed PR. The response rate of group B was better than that of group A, but the value of P by Kruskal-Wallis test was 0.27. Thirteen of the 30 other cancers were randomized into group A and 17 cases into group B. Three cases in group A and B, respectively, showed PR. There was no significant difference between the two groups in terms of antitumor effect, prolongation of life or side effects. In conclusion, it was suggested that the MQF-OK therapy with UFT was beneficial for the Remission Induction Therapy in advanced gastric cancer.

Effects of pertussis toxin on the alpha 2-adrenoceptor-inhibitory GTP-binding protein-adenylate cyclase system in rat brain: pharmacological and neurochemical studies.

Behavioral excitement and the increase in locomotion were observed in male adult rats four days after an intraventricular injection of 5 micrograms pertussis toxin (IAP). Clonidine (100 micrograms/kg s.c.)-induced locomotor hypoactivity was not observed in animals pretreated with 1 and 5 micrograms IAP. IAP caused a significant (P less than 0.05) decrease in the KD value of [3H]clonidine binding and enhanced GTP (1 microM)-induced decrease in the binding to cortical membranes from rat brain. In addition, the inhibition of adenylate cyclase induced by alpha 2-receptor stimulation (100 microM adrenaline plus 100 microM propranolol) was completely suppressed in the cerebral cortical membranes by IAP pretreatment. It is suggested that the system consisting of alpha 2-receptor, the inhibitory GTP-binding protein (Ni) and adenylate cyclase inhibits some animal behaviors and cyclic AMP formation. Moreover, IAP seems to inactivate Ni, subsequently producing behavioral excitement and it inhibits clonidine-induced sedation.

Suppressing effect of pertussis toxin on clonidine-induced inhibition of noradrenaline release from cerebral cortical slices of rats.

Clonidine at 1.0 microM significantly decreased 20 mM K+-evoked release of L-[3H]noradrenaline (NA) from rat cerebral cortical slices preloaded with L-[3H]NA. Inhibitory effects of clonidine, however, were not observed in slices pretreated with 20 micrograms/ml pertussis toxin, an islet-activating protein, together with NAD and adenosine triphosphate. It is suggested that the inhibitory guanine nucleotide-binding protein (Ni) could be involved in alpha 2-adrenoceptor-mediated inhibition of NA release from nerve terminals in the central nervous system.

Study on abnormalities in the appearance of finger and palm prints in children with cleft lip, alveolus, and palate.

The aetiology of abnormalities in the appearance of finger and palm prints in children with cleft lip, alveolus, and palate was examined by comparing the frequencies of abnormalities in hereditary cases and sporadic cases, and those in hereditary cases with those in their parents. The following results were obtained: Comparison of hereditary cases and sporadic cases. Abnormalities in the appearance of finger and palm prints were greater in hereditary cases than in sporadic cases. Abnormalities in the appearance of finger and palm prints were greater in parents of hereditary cases than in parents of sporadic cases. The rate of agreement in the appearance of finger and palm prints between parents and children was greater in hereditary cases than in sporadic cases. Comparison within hereditary cases. Abnormalities in the appearance of finger and palm prints were greater in affected children when one of the parents was affected (gamma = 1/2) than when both parents were normal (gamma = less than 1/2). Abnormalities in the appearance of finger and palm prints were greater in parents, one of whom was affected, than in normal parents. The rate of agreement in the appearance of finger and palm prints of parents, one of whom was affected, and their gamma = 1/2 children cases was higher than of normal parents and their gamma = less than 1/2 children. It was concluded that the above results could be explained by multifactorial inheritance.

Stability and effectiveness of chlorine disinfectants in water distribution systems.

A test system for water distribution was used to evaluate the stability and effectiveness of three residual disinfectants--free chlorine, combined chlorine, and chlorine dioxide--when challenged with a sewage contaminant. The test distribution system consisted of the street main and internal plumbing for two barracks at Fort George G. Meade, MD. To the existing pipe network, 152 m (500 ft) of 13-mm (0.5 in.) copper pipe were added for sampling, and 60 m (200 ft) of 2.54-cm (1.0 in.) plastic pipe were added for circulation. The levels of residual disinfectants tested were 0.2 mg/L and 1.0 mg/L as available chlorine. In the absence of a disinfectant residual, microorganisms in the sewage contaminant were consistently recovered at high levels. The presence of any disinfectant residual reduced the microorganism level and frequency of occurrence at the consumer's tap. Free chlorine was the most effective residual disinfectant and may serve as a marker or flag in the distribution network. Free chlorine and chlorine dioxide were the least stable in the pipe network. The loss of disinfectant in the pipe network followed first-order kinetics. The half-life determined in static tests for free chlorine, chlorine dioxide, and combined chlorine was 140, 93, and 1680 min.

Effects of an analog of PGF2(alpha) (ONO-1052) on cows with luteinized ovarian cysts following treatment with GnRH analog (fertirelin acetate).

Effects of PGF2(alpha) analog (ONO-1052) on cows with luteinized cysts following treatment with GnRH analog (fertirelin) in inducing estrus and shortening an interval from treatment to conception were investigated in a field trial. Seventy-five cows with follicular cysts diagnosed by rectal palpation were treated intramuscularly (i.m.) with 400 mug fertirelin (an analog of GnRH). Luteinization of follicular cysts were tentatively judged by rectal palpation. Forty-one cows were considered to have luteinized cysts and were either treated i.m. with ONO-1052 (28 cows) or left untreated as controls (13 cows). Thirty-four other cows were considered to have not responded to fertirelin and retreated with 10,000 MU hCG (19 cows) or fertirelin (five cows). This tentative judgment of luteinization of the cysts by rectal palpation after treatment was later confirmed by determining serum progesterone concentration before and 10 to 14 days after treatment. Only in the cows with luteinized cysts which were confirmed by serum progesterone analysis; increased from a pretreatment level below 1.0 ng/ml to a value of 1.0 ng/ml or higher 10 to 14 days after treatment, effects of ONO-1052 combined with fertirelin were investigated. Of the 18 cows with luteinized cysts thus confirmed following fertirelin injection and treated with ONO-1052, 15 (83.3 %) cows came into estrus within 26 +/- 14 (mean +/- SD) days after the first treatment; eight cows within three to five days and four cows within 22 to 28 days, and 14 cows (77.8 %) conceived within 100 days (42 +/- 26 days). On the other hand, the five control cows with luteinized cysts that were not treated with ONO-1052 required significantly longer to exhibit normal estrus (54 +/- 13 days; P<0.05) and to conceive (54 +/- 13 days). These results indicate that ONO-1052 combined with fertirelin may be useful to shorten the interval from treatment to normal estrus and to conception in cows with follicular cysts.

Coagulant property of an indigenous seed: home water treatment in Tamil Nadu.

Use of some vegetable substances as coagulant has been recorded in historical literature. A seed from the tree known as tettan cottai is used today in South Indian villages to purify water. The scrapings of the seed were shown in laboratory experiments to have coagulant properties. It would be extremely valuable to have additional measurements made in South India, where the practice is employed in removal of turbidity, and also to have evidence of the extent of removal microorganisms.
