[A report of a resected case of malignant lymphoma combined with follicular carcinoma of the thyroid].

A 66-year-old female administered with complaining of cervical mass. All of the thyroid function tested were within normal limits. A diagnosis of Malignant lymphoma was done by fine aspiration cytology. Total thyroidectomy and dissection of bilateral regional lymph nodes were performed. And additional chemotherapy is continued. The histological findings revealed diffuse lymphoma, mixed type (LSG), but there is no invasion of the lymphoma cell to the lymph node. There are also two follicular carcinomas with metastasis to the dissected regional lymph nodes. In the residual thyroid tissue, the findings of chronic thyroiditis was shown histopathologically.

Glomerular lesions associated with liver cirrhosis: an immunohistochemical and clinicopathologic analysis.

Clinicopathologic and immunohistochemical analyses of the kidneys in 30 autopsy cases of liver cirrhosis of different types revealed glomerular lesions in 27 cases. Clinical nephritis was present in three cases. Characteristic diffuse mesangial proliferation with focal mesangial interposition and/or subendothelial deposits were present. Glomerular immunoglobulin deposition was observed in several cases, IgM in 23 cases, IgG in five, and IgA in 17; the latter were predominantly IgA1. Secretory component binding was found in 11 of the 17 cases with IgA deposits (65 per cent). Liver weight was related to IgA deposition (P approximately 0.05), and the volume of ascitic fluid was significantly correlated (P less than 0.05).

Neuron-specific enolase and Leu-7 immunoreactive small round-cell neoplasm. The relationship to Ewing's sarcoma in bone and soft tissue.

The authors present an immunohistochemical analysis of tissue from five cases of morphologically distinctive Ewing's sarcoma in bone and soft tissue. The mean age of the five patients was 16.6 years, with a range of 6-28 years. The tumors existed in chest wall, pelvis, and lower extremities. Two siblings with tumor in bone were clinically diagnosed as having Ewing's sarcoma. All cases had immunoreactivity for neuron-specific enolase (NSE), and four cases revealed positive staining for Leu-7. Neuron-specific enolase is highly specific for neurons and neuroendocrine cells. In addition, immunoreactivity for Leu-7, expressing a natural killer activity, has been demonstrated in peripheral nerve fibers and neuroendocrine cells. The authors suggest that NSE and Leu-7 immunoreactive small round-cell neoplasm is probably a primitive neuroectodermal tumor and should be categorized as Ewing's sarcoma in bone and soft tissue.

Conformations of fibroblast and E. coli-derived recombinant human interferon-beta s as studied by nuclear magnetic resonance and circular dichroism.

The conformations of fibroblast and E. coli-derived recombinant human interferon-beta s were studied by circular dichroism and nuclear magnetic resonance spectroscopy in the acidic pH region of 4.6 to 1.6. Both interferons have very similar conformations with high alpha-helix contents (approximately 70%). These results suggest that glycosylation does not appreciably change the conformation of human interferon-beta. Moreover, a slow conformational change is observed below pH 2.0, which induces the disruption of beta-sheets.

[Effect of isepamicin (HAPA-B) on reproduction. I. Fertility study in rats (intramuscular administration)].

Effect of isepamicin (HAPA-B), a new aminoglycoside antibiotic, on fertility and reproductive performance in Wistar rats was studied. The drug was injected intramuscularly to rats at a daily dose of 25, 100 or 200 mg/kg. Male rats were treated for 63 days before mating and during the mating period, and female rats were treated for 2 weeks and through the mating period to the 7th day after mating. In male rats, an inhibition of body weight gain and a development of a pale color and hypertrophy of the kidney were observed in 100 and 200 mg/kg dose-groups. In female rats, inhibition of body weight gain was observed in 100 and 200 mg/kg dose-groups and a pale color of the kidney was seen in the 200 mg/kg group. No significant differences were observed in mating and fertility performance of male and female rats between the control and treated groups. Numbers of corpora lutea, implantation sites and living fetuses in the 200 mg/kg group decreased significantly. No external, visceral and skeletal anomalies due to the treatment of HAPA-B were observed in the live fetuses. No effect dose levels of HAPA-B found in this study were 25 mg/kg on parent rats and 100 mg/kg on reproduction ability and their fetuses.

[Effect of isepamicin (HAPA-B) on reproduction. II. Teratological study in rats (intramuscular administration)].

Teratological study of isepamicin (HAPA-B), a new aminoglycoside antibiotic, was performed in Jcl: Wistar rat. The drug was given intramuscularly to dams at doses of 25, 100 and 200 mg/kg from day 7 to 17 after mating. The influence on dams, their fetuses and offspring was studied. A decrease in food intake and a development of pale color of the kidney of dams at a dose level of 200 mg/kg were observed in the prenatal study. The HAPA-B did not show any influence on prenatal development of fetuses. In the postnatal study, no effect of HAPA-B on parturition and lactation of dams was seen except that a discoloration of the kidney was observed at autopsy. However, some differences were observed between the control and treatment groups in postnatal development, behavior and reproduction performance of offspring. Dose-response effects of HAPA-B on offspring were not observed in the postnatal study. No effect dose levels of HAPA-B established in this study on rat fetuses and offspring were 200 mg/kg and 100 mg/kg on pregnant rat, respectively.

[Bilateral ureteral obstruction by periarterial fibrotic reactions in aortoiliac arteriosclerosis: report of a case].

A 60-year-old man visited our hospital with the chief complaint of right flank pain. A right ureteral stone was suspected by the findings of KUB and DIP, and conservative treatment was carried out. Repeat KUB and DIP a month after treatment showed that the stone shadow was not visible and right hydronephrosis had increased. Closer examinations by RP and CT scan revealed right ureteral obstruction caused by periarterial fibrosis accompanied with arteriosclerotic changes of the aortoiliac region. The arteriosclerotic changes were confirmed by pelvic arteriography, and then right ureterolysis and lateral displacement were performed. DIP four months after the operation showed improvement of right hydronephrosis, but left hydronephrosis was observed. As the same cause as the right side was suspected, left ureterolysis and intraperitoneal transposition were also performed. Histological findings of each periureteral tissue resembled idiopathic retroperitoneal fibrosis. DIP three months after the second operation showed no hydronephrosis. Case reports of ureteral obstruction due to perianeurysmal fibrosis have not been infrequent recently, but our case showed severe arteriosclerotic changes instead of conspicuous aneurysm. The possibility of occurrence of periarterial fibrosis caused by severe arteriosclerotic changes is discussed.

[Percutaneous nephrostomy for unusual complications occurring in patients with ureteral stones: a report of two cases].

The first case is a 59-year-old man who had left flank pain and nausea. KUB, excretory urograms and CT scan showed a left ureteral stone at the ureterovesical junction associated with spontaneous rupture of the left renal pelvis. Percutaneous nephrostomy was performed. The ureteral stone was spontaneously discharged on the 4th postoperative day and extravasation of contrast medium from the left renal pelvis disappeared. The second case is a 42-year-old man who was admitted with bilateral flank pain, nausea and vomiting. KUB and excretory urograms showed bilateral hydronephrosis due to small bilateral ureteral stones. Serum BUN and creatinine had risen to 41 and 5.1 mg/dl, respectively, on the day after admission. Percutaneous nephrostomy to the left kidney was performed. BUN and creatinine were normalized immediately and the bilateral ureteral stones were spontaneously discharged by the 9th postoperative day. After the nephrostomy catheters were removed, no complications occurred in either case and KUB and excretory urograms showed normal findings.

Preparation and enzymatic degradation of monosulfogangliotriaosylceramide.

Gangliotriaosylceramide 3'-sulfate (GgOse3Cer-II3-sulfate) contains the sugar sequence similar to that of GM2 ganglioside except that the NeuAc in GM2 is replaced by a sulfate group. Due to this structural similarity, we have studied the in vitro synthesis of GgOse3Cer-II3-sulfate using the system for GM2. Our results showed that GgOse3Cer-II3-sulfate could be synthesized from lactosylceramide 3'-sulfate and UDP-GalNAc catalyzed by N-acetylgalactosaminyltransferase prepared from rat brain (Dicesare, J. L., and Dain, J. A. (1971) Biochim. Biophys. Acta 231, 385-393). As in the case of GM2, the GgOse3Cer-II3-sulfate biosynthesized in vitro or isolated from rat kidney could also be cleaved by human beta-hexosaminidase A in the presence of GM2-activator (Li, S.-C., Hirabayashi, Y., and Li, Y.-T. (1981) J. Biol. Chem. 256, 6234-6240). The fact that the GM2-activator could stimulate beta-hexosaminidase A to hydrolyze both GM2 and Gg-Ose3Cer-II3-sulfate indicates that these two glycolipids may be catabolyzed by the same mechanism.

Multiple paraganglioneuromas.

We report multiple paraganglioneuromas which occurred in a 40-year-old-man. Thirty-two tumours with similar histological appearance have been reported previously and most of them showed a striking predilection to occur in the second portion of the duodenum. In this case, three masses were detected; one was located in the periampullary region of duodenum with a polypoid appearance, the others were well defined masses in peri-pancreatic tissue adjacent to large vessels. Histology revealed two cellular components, epithelioid cells with NSE immunoreactivity and S-100 protein containing spindle-shaped cells. Moreover, on electron microscopical examination, three different epithelioid cell types were seen. Type I was a figure differentiating to ganglion cells, type II to paraganglion cells, type III was a hybrid form of ganglion and paraganglion cells. Paraganglioneuroma revealed the histopathology of ganglioneuroma, paraganglioma and also a mixed appearance. In this respect the tumour may be considered to originate in undifferentiated neural crest cells and develop organoid differentiation.
