An extremely rare case of isolated histioproliferative lesions arising from the subarachnoid space of the left occipital convexity and the orbit is presented. The presence of histiocytes showing lymphophagocytosis and positivity for S-100 protein staining confirmed that the lesions represented extranodal forms of sinus histiocytosis with massive lymphadenopathy.
The effect of a single day treatment with 600 mg norfloxacin 600 mg ofloxacin or 1,920 mg trimethoprim-sulfamethoxazol was determined on 114 patients with acute cystitis. The overall clinical efficacy was excellent in 101 patients (89%), moderate in 9 patients (8%) and poor in 4 patients (3%). Recurrence was observed in 8 cases (8%) within 6 weeks after the treatment. The effectiveness rate and the recurrence rate were inferior in those caused by S. epidermidis compared with those caused by E. coli.
The changes in serum CA125, CA19-9, CEA, Ferritin, TPA, IAP and LDH concentrations were measured in 22 primary cases and 7 recurrent cases of adenocarcinoma of the uterine cervix and endometrial carcinoma in order to examine the clinical usefullness of CA125 and CA19-9 as a tumor marker. Localization of CA125 and CA19-9 was also examined in adenocarcinoma, normal endocervix and endometrium immunohistochemically. 1. Twenty-seven percent of primary cases had increased serum CA125 and CA19-9 which decreased rapidly after operation or chemotherapy, reflecting reduction of the tumor mass. 2. In these cases, CA125 or CA19-9 was localized in carcinoma tissues immunohistochemically. On the other hand, in normal endocervical and endometrial glands, CA125 was localized, but CA19-9 was not. 3. In most recurrent cases, serum CA125 and CA19-9 increased early and markedly, compared with the other markers. 4. The change in serum Ferritin, CEA, TPA, IAP and LDH didn't reflect clinical courses such as operation and chemotherapy. In recurrent cases they increased even more slowly and slightly than CA125 or CA19-9. These results show that CA125 and CA19-9 are useful tumor markers in the management of adenocarcinoma of the uterine cervix and endometrial carcinoma, especially in advanced or recurrent cases.
The therapeutic effects, safety and side-effects of clocapramine and sulpiride were evaluated in 52 hospitalized chronic schizophrenic patients using a single-blind method during an 8-week trial period. While the final global improvement rating showed clocapramine to be superior to sulpiride, the differences were not statistically significant. The time course of the total psychiatric rating scales (PRS) showed a progressive decline during treatment for both drugs, and at the end of treatment clocapramine proved significantly lower in total PRS than did sulpiride. In the improvement of psychotic symptoms, clocapramine seemed to be superior to sulpiride with respect to motor retardation, delusion, hallucination or disturbance of self-consciousness, social isolation or withdrawal, and recreation or work. Side-effects appeared more frequently with clocapramine than with sulpiride, but abnormal laboratory-test results appeared less in clocapramine-treated patients than in sulpiride-treated ones. Neither side-effects nor abnormal laboratory-test results induced by the two drugs were severe enough to terminate administration. Clocapramine is concluded to have a more favourable effect on negative symptoms, as well as on some positive symptoms of chronic schizophrenia, than sulpiride.

A case of a non-jaundiced ampullary carcinoma with a unique tumor spread is reported. A 3.2 X 1.0 cm-sized tumor at the ampulla of Vater was resected in a 64-year-old female. The primary focus of the well differentiated adenocarcinoma was confined to the intraampullary common channel, but the most of the tumor consisted of lymphatic permeation. A wide spread lymphogenous metastases had been noticed at operation, and the patient died nine months thereafter from metastases of the brain. The mechanism which prevented jaundice from developing in this case might be due to the unique way in which this tumor spread, without obstruction via the lymphatic space and only stenosing the bile duct.
A study was undertaken to evaluate the efficacy of the Cytobrush compared with the cotton-tipped applicator for obtaining cells from the uterine cervix. Four consecutive Papanicolaou smears were obtained in 1,000 patients during a single clinic visit. In every patient, a total of four smears on two slides were submitted: one slide contained endocervical material obtained with a cotton-tipped applicator and with the Cytobrush and the other slide contained ectocervical material obtained with both a cotton-tipped applicator and the Cytobrush. In endocervical smears, the Cytobrush technique produced numerous groups and clusters of cervical columnar cells in contrast to the use of the cotton swab. This was particularly true in older patients and pregnant women. In ectocervical smears, the detection of the squamous component was also higher in the Cytobrush smear than in cotton swab smears. However, the cell yield was not influenced by the cell sampling sequence in either endocervical or ectocervical specimens. It is the conclusion of this study that the utility of the Cytobrush technique for collecting specimens for endocervical and ectocervical smears is greater and provides a higher yield of cells than the cotton swab technique. Wider use of this simple procedure is encouraged.
A simple method for the preparation of phosphorylcholine derivatives of bovine serum albumin (PC-BSA) by reductive alkylation of the amino groups of bovine serum albumin with choline phosphoryl glycoaldehyde is described. Choline phosphoryl glycoaldehyde was generated by periodate oxidation of glyceryl phosphorylcholine. PC-BSA was immobilized on SH-derivatized Toyopearl HW 65 by reacting the single SH group of PC-BSA with a bismaleimido reagent and then coupling maleimidated PC-BSA to the thiolated gel. The affinity purification of C-reactive protein (CRP), which is based on the Ca2+-dependent affinity of CRP for the phosphorylcholine residue of PC-BSA, was readily accomplished using the PC-BSA Toyopearl HW 65 column. The resulting CRP preparation from serum and pleural fluid was homogeneous as assessed by native polyacrylamide gel electrophoresis. PC-BSA derivatives were also shown to be reactive with Limulus polyphemus CRP.

An immunohistochemical and clinicopathologic analysis of glomerular alterations was carried out in 20 autopsy cases with obstructive jaundice. The 20 cases without clinical nephritis had primary carcinoma of various locations, including the stomach, rectum, pancreas, and biliary tract. Mesangial IgA deposition was present on immunofluorescence staining in four cases, IgG in two cases, IgM in five cases, and C3 in four cases. Glomerular polymeric IgA containing A1, A2, and J chain was considered to originate from the gastrointestinal tract. All immunofluorescence-positive cases except one had electron-dense deposits in mesangial regions. These findings suggest that glomerular IgA deposition develops in a handful of patients suffering from obstructive jaundice, presumably due to the passive trapping of circulating immune complexes. Furthermore, glomerular IgA deposition in patients with obstructive jaundice is not influenced by duration or intensity of jaundice, and it unlikely induces clinical nephritis.
Strains carrying only one species of pock-forming plasmid, designated as pSK3, were isolated from two different derivative strains of Streptomyces kasugaensis MB273 which contained three species of plasmids, pSK1, pSK2 and pSK3. Single and double digestion of pSK3 with seven restriction endonucleases yielded fragments identical with those of pSK3 and assignable to those obtained from pSK1 and pSK2. In particular, digestion with BglII alone or in combination with other restriction endonucleases afforded the same size fragments as those of pSK1 and pSK2. Strains containing pSK3 induced pocks on lawns of strains carrying pSK1 or pSK2 and resisted pock formation by the latter strains. Therefore, it was concluded that pSK3 was a pSK3 derivative with elevated pock-forming ability and represented a composite plasmid consisting of two elements, pSK1 and pSK2, without any loss of their plasmid functions. Deletion derivative plasmids constructed from the BglII fragments of pSK3 provided evidence supporting the above conclusion. Pock formation by a pSK3-containing strain against strains carrying pSK1, or pSK2 or no plasmid accompanied the transfer of pSK3 from the former to the latter. Segregation of pSK1 and pSK2 from pSK3 was observed in mycelium from pocks caused by pSK3-containing strains and on subculture of pSK3-containing strains.
Selective sodium loading attenuated the development of hypertension in the deoxycorticosterone acetate (DOCA) treated rat. The DOCA treated rat fed a diet equimolar in sodium to a 7% sodium chloride diet and in chloride to a standard diet, differed in various parameters from the DOCA treated rat fed a 7% sodium chloride diet: it had higher sodium concentration in both erythrocytes and muscles, a higher erythrocyte ouabain sensitive 22Na efflux rate constant (Kos), and a lower norepinephrine turnover rate in the heart and the spleen. These results suggest that the suppressed sympathetic nervous system activity and the activated cell membrane sodium pump contribute in part to the mechanism for the suppression of the development of hypertension in the DOCA-selective sodium loaded rat.
In order to reduce radiation dose to the hands of examiners who prepare and aspirate radiopharmaceuticals, we made a prototype of simplified manually-operated dispense system, which the syringe and the vial shield with lead were set in the small box made of lead and lead glass. The result showed that our dispense system allowed substantial reduction of radiation dose to the hands and rapid preparation of radiopharmaceuticals compared with the conventional lead shield syringe system, and allowed closer operation, smaller dead volume and lower cost compared with the conventional automatic system.
