C-Reactive protein in dilated cardiomyopathy.

The prognosis for patients with idiopathic dilated cardiomyopathy (DCM) is poor, although clinical features are variable. Prediction of outcome has been difficult in individual patients based on laboratory data. In some patients with DCM, myocardial damage secondary to viral or immune-mediated myocardial inflammation may persist. To objectively assess inflammation, we measured plasma concentrations of C-reactive protein (CRP) in 188 patients with idiopathic DCM over 5-8 years. All had dyspnea and fatigue at rest; all patients had a left ventricular ejection fraction less than 40% by echocardiography or by contrast or radionuclide ventriculography. We divided these patients into two groups: patients dying within 5 years following admission (n = 49) and the remainder surviving for at least 5 years (n = 139). CRP concentrations in the patients dying early were significantly higher than in the long-term survivors (1. 05 +/- 1.37 vs. 0.49 +/- 1.04 mg/dl, p < 0.05). Sixty-two percent of the patients with CRP>1.0 died within 5 years. In addition to other laboratory tests including electrocardiography and echocardiography, routine CRP measurements proved to be valuable for identifying high-risk patients who require special treatment strategies.

Tile automation: a model for an architecture of a living system.

To understand an architecture of a living system, "Tile Automaton" is introduced as an abstract model of chemical reaction of molecules scattered over a space. The model consists of tiles of various shapes that stand for molecules. The chemical reaction, induced by the collisions of tiles, is represented by the change of the tile shapes. The rules for reaction are deterministic, and the evolution of the system strongly depends on mutual spatial relationship among tiles. The evolution often leads to self-organization of a "factory," a set of tiles that produces tiles continuously and keeps its structure. Several interesting phenomena, such as a deformation or a division of a factory, are also observed. It is proposed that the formation of the factory is due to the interference between different aspects of tiles - the shape and the motion. The concept of "entanglement" is introduced as a mechanism of living systems.

Definitive chemoradiotherapy for T4 and/or M1 lymph node squamous cell carcinoma of the esophagus.

PURPOSE: To investigate the efficacy and feasibility of concurrent chemoradiotherapy for locally advanced carcinoma of the esophagus. PATIENTS AND METHODS: Fifty-four patients with clinically T4 and/or M1 lymph node (LYM) squamous cell carcinoma of the esophagus were enrolled. Patients received protracted infusion of fluorouracil 400 mg/m(2)/24 hours on days 1 to 5 and 8 to 12, 2-hour infusion of cisplatin 40 mg/m(2) on days 1 and 8, and concurrent radiation therapy at a dose of 30 Gy in 15 fractions over 3 weeks. Filgrastim was prophylactically administered to 35 patients. This schedule was repeated twice every 5 weeks, for a total radiation dose of 60 Gy, followed by two courses of fluorouracil (800 mg/m(2)/24 hours for 5 days) and cisplatin (80 mg/m(2) on day 1). RESULTS: There were 21 patients with T4M0 disease, one with T2M1 LYM, 17 with T3M1 LYM, and 15 withT4M1 LYM. Forty-nine patients (91%) completed at least the chemoradiotherapy segment. The 18 patients (33%) who achieved a complete response included nine (25%) of the 36 with T4 disease and nine (50%) of the 18 with non-T4 disease. Major toxicities were leukocytopenia and esophagitis; there were four (7%) treatment-related deaths. Prophylactic filgrastim reduced the incidence of grade 3 or worse leukopenia without improving dose-intensity or response. With a median follow-up duration of 43 months, median survival time was 9 months. The 3-year survival rate was 23%. CONCLUSION: Despite its significant toxicity, this combined modality seemed to have curative potential even in cases of locally advanced carcinoma of the esophagus.

Isolation of bovine serum albumin fragment P-9 and P-9-mediated fusion of small unilamellar vesicles.

Fusion peptide P-9 was isolated from bovine serum albumin by controlled pepsin degradation in the presence of caprylic acid, followed by Sephadex G-75 gel filtration and ion-exchange chromatography of CM-cellulose. By this procedure, P-9 could be strictly separated from peptic fragment P-Phe, which has a molecular weight close to that of P-9. P-Phe has no fusogenic activity. The addition of P-9 to phosphatidylcholine (PC) liposomes containing cholesterol (Chol) gave rise to an increase of absorption intensity at around pH 4.0. The increase of turbidity by P-9 addition did not decrease with increasing pH, indicating P-9-mediated fusion of PC liposomes. The extent of the fusion of PC liposomes was strongly dependent on the PC chain length and temperature. The membrane fluidity close to the polar head groups of the fatty acyl chains of PC affected markedly the extent of P-9-mediated liposome fusion. However, there was no correlation between membrane fluidity near the hydrophobic end of the fatty acyl chains and the extent of liposome fusion. The rate of liposome fusion was dependent on both lipid composition and PC chain length. These results suggest that a contact or an interaction of P-9 with liposomal membrane occurs in the rigid regions. The character of the membrane-water interface region in the liposome controls a triggering effect for P-9-mediated fusion.

Synthetic inhibitors of DNA topoisomerase I and II.

A new type of synthetic inhibitor of DNA topoisomerase I and II was examined and several of these derivatives exhibited strong dual activity against these enzymes. This series of compounds showed high cytotoxic activities against cancer cells, but only a limited number of compounds showed any noticeable activity in an in vivo test against murine P388. Non-specific toxicity was observed in the in vivo tests.

Characteristics of monochorionic-diamniotic growth-retarded twins during the third trimester.

The goal of this study was to assess the characteristics of monochorionic-diamniotic (MD) growth-retarded twin infants with twin-twin transfusion syndrome (TTTS) compared with those without TTTS during the third trimester. Retrospective analyses of the growth patterns and amniotic fluid volumes were performed on 5 MD twin pregnancies in which one or both twins showed growth retardation with TTTS, and the results weve compared with those without TTTS. Eighty-three percent of growth-retarded twin infants without TTTS in MD twin gestation showed an asymmetric growth pattern, while all TTTS cases showed a symmetric pattern (p < 0.05). Polyhydramnios of the co-twin was found in 80% of TTTS cases, while no polyhydramnios was found in patients without TTTS (p < 0.05). Assessment of growth patterns and amniotic fluid volume may be useful to exclude the possibility of TTTS in MD growth-retarded twin pregnancies during the third trimester.

Morphology of intestinal colonization of Yersinia enterocolitica serovar O3 in mice.

The present study was made to know the morphology of the initial invasion and lesions involved in the intestinal colonization of Yersinia enterocolitica serovar O3 in the epithelium of Peyer's patches of mice. Microfold (M) cells formed a specific structure like a pseudopodium and the bacteria were observed on the surface of the pseudopodium-like structure 4 hr after oral administration of serovar O3. The colonies of serovar O3 were observed in the epithelium and the lamina propria of the Peyer's patches dome region, and the bacteria grown in the Peyer's patches were in direct contact with the lumen without covered with the host tissue 24 hr after the administration.

Prevalence and persistence of Salmonella in broiler chicken flocks.

Cecal contents of 2,345 broiler chickens consisting of 28 flocks originated from 12 farms were examined for the prevalence of Salmonella to know the actual status of infection with Salmonella in the chicken flocks. Salmonella was isolated from 336 (14.3%) samples. From these isolates, eight serovars were identified. Of the 336 Salmonella isolates, 242 (72.0%) were serotyped as S. Blockley, 60 (17.9%) S. Hadar, 15 (4.5%) S. Bredeney, nine (2.7%) S. Schwarzengrund, four (1.2%) S. Anatum, three (0.9%) S. Enteritidis, two (0.6%) S. Ohio, and one (0.3%) S. Livingstone. The same serovars of Salmonella were repeatedly found in the chickens from the same farms. S. Typhimurium and S. Enteritidis were detected in pooled broken eggshell samples collected from the hatchery. Analysis of plasmid profiles revealed 11 patterns of S. Blockley and seven patterns of S. Hadar. Strains of the same plasmid profiles of S. Blockley were isolated repeatedly from the same farm over one year after the first isolation.

Possible involvement of atypical protein kinase C (PKC) in glucose-sensitive expression of the human insulin gene: DNA-binding activity and transcriptional activity of pancreatic and duodenal homeobox gene-1 (PDX-1) are enhanced via calphostin C-sensitive but phorbol 12-myristate 13-acetate (PMA) and Gö 6976-insensitive pathway.

Pancreatic and duodenal homeobox gene-1 (PDX-1) is a transcription factor which regulates the insulin gene expression. In this study, we tried to elucidate the role of PDX-1 in the glucose-induced transcriptional activation of the human insulin gene promoter in MIN6 cells. Electrophoretic mobility shift assay (EMSA) and chloramphenicol acetyltransferase (CAT) assay demonstrated that both DNA-binding activity and transcriptional activity of PDX-1 were increased with 20 mmol/l glucose more than with 2 mmol/l glucose. The DNA-binding activity of PDX-1 induced by high glucose was blocked by phosphatase treatment, suggesting the involvement of PDX-1 phosphorylation in this event. In an in vitro phosphorylation study, PDX-1 was phosphorylated by protein kinase C (PKC), but not by cAMP dependent protein kinase (PKA) or mitogen-activated protein kinase (MAPK). Furthermore, increased PDX-1 function induced by high glucose was blocked by calphostin C, an inhibitor of all PKC isoforms, but unaffected by phorbol 12-myristate 13-acetate (PMA), an activator of classical and novel PKC, or Gö 6976, an inhibitor of classical and novel PKC, which suggested that the PKC family which activated PDX-1 in MIN6 cells was atypical PKC. Western blot and immunocytochemical studies with anti-PKC zeta antibody confirmed the presence of PKC zeta, one of the isoforms of atypical PKC, in MIN6 cells. Furthermore, PKC zeta activity was significantly increased by glucose stimulation. These results suggest that high glucose increased DNA-binding activity of PDX-1 by activating atypical PKC including PKC zeta, resulting in transcriptional activation of the human insulin gene promoter.

Evaluation of three types of blinks with the use of electro-oculogram and electromyogram.

Three kinds of blinks, spontaneous, reflex, and voluntary, were measured for 11 men using electromyogram (EMG) at the orbicularis oculi muscle on the right side and electro-oculogram (EOG) in the vertical direction to the right eyelid. The amplitude and the duration time were defined here from the raw waves of the EMG and the EOG for the three kinds of blinks. The amplitude of the EMG indicated that the mean value for the spontaneous blinks was significantly smaller than that of the voluntary blinks and the mean duration of the EMG shows the value for the spontaneous blinks was smaller than that for the reflex and the voluntary blinks. The EMG for the spontaneous blinks had a smaller amplitude and a shorter duration time than the other blinks. The EOG amplitude indicated that the mean value for the spontaneous blinks was significantly smaller than that for the voluntary blinks, but there was no significant difference between the spontaneous blinks and the other blinks for the duration of the EOG. The amplitude and the duration for both the EMG and the EOG discriminated any two types of blinks from three blinks by statistical test. Those values for the EMG and the EOG are effective indices for the evaluation of blinks. Moreover, the coefficients of variation of the amplitude and durations of EMG and EOG for three kinds of blinks denote their characteristics. Multiple discriminant analysis distinguished the types of blinks simultaneously. The resultant maximum correct rate for the three blinks reaches to be 81.8%. So far, it has been difficult to discriminate the blinks quantitatively, but the procedures presented here are able to solve the difficulty.

Bacterial contamination in the environment of food factories processing ready-to-eat fresh vegetables.

A total of 196 samples were collected from equipment for trimming, washing, slicing, soaking, dehydrating, blending, and packaging and from the floor and air of operation rooms before and after operation in two food factories processing ready-to-eat fresh vegetables located in the suburbs of Tokyo. Heavy contamination determined by an aerobic plate count of >5.0 log CFU/cm2 or ml was observed after operation in most of the samples examined, as were samples taken before operation on the interior surfaces of equipment for washing, slicing, dehydrating, and blending, the surfaces of blades for slicing, and the floor surfaces of operation rooms. From these environmental samples, the coliform group was detected before operation. Although 67 strains of 70 coliforms isolated were nonfecal, three Escherichia coli strains were detected in the surface of the operation room floors and the gloves of employees. Bacillus cereus was isolated from 9 of 86 and 17 of 85 samples examined before and after operation with the number of 2.0 to 3.0 log CFU/cm2 or ml. Listeria spp. were not detected in the environment of the food factories.
