[Granuloma formation in mice by liposomes of new glycolipid containing mycolic acids, "trehalose 2,3,6'-trimycolate"].

After intravenous administration of liposomes containing a novel glycolipid, trehalose 2,3,6'-trimycolate, isolated from Rhodococcus aurantiacus, the granuloma formation was observed distinctively in the lungs, spleen and liver of ICR mice. The concentration of the glycolipid in liposomes influenced profoundly on the inducibility for granulomas and the liposomes containing 10 mol% of trehalose 2,3,6'-trimycolate showed the highest activity for the granuloma formation in mice without a significant loss of body weight of mice with a less toxicity. EggPC liposomes are of more highly inducible for the granuloma formation in mice than eggPC:PS (7:3, mol ratio) liposomes or eggPC:Chol (9:1, mol ratio) liposomes, suggesting that cholesterol in liposomes was not necessarily essential for inducing granulomas in mice. Among the various PC liposomes, synthetic dimyristoyl-phosphatidylcholine (DMPC) or dipalmitoylphosphatidylcholine (DPPC) liposomes were of highly inducible, although a high responsiveness was also observed with natural lecithines such as egg or soy PC. However, distearoylphosphatidylcholine (DSPC) liposomes did not show any activity for the granuloma formation in any organs of mice. Organ responsiveness differed significantly in the micelle forms injected. Glycolipid entrapped with eggPC-Chol liposomes showed a lower lung index below 1.0, in contrast to w/o/w micelles containing Freunds' incomplete adjuvant, which showed a higher granuloma formation in the lungs. It was also noted that the toxicity of glycolipid containing liposomes was comparatively lower than the toxicity of glycolipid containing w/o/w micelles, indicating that the liposomes appeared to be more suitable for the induction of the immunomodifying activities of mycolic acid-containing glycolipids than w/o/w micelles.

Liver-associated large granular lymphocytes: morphological and functional aspects.

Large granular lymphocytes (LGLs) differ from other lymphocytes in their recirculation pattern and are distributed preferentially in nonlymphoid organs such as the liver and lung. The liver-associated LGLs adhere strongly to the sinusoidal endothelium and show a natural killer (NK) cytotoxicity against incoming metastatic tumor cells; this reaction occurs very rapidly because, in contrast to the immune response, it does not require complex processes in the lymphoid tissue. They have been extensively studied morphologically in terms of pit cells. LGLs have two characteristic cell organelles which participate in the NK cytolysis, i. e., dense granules and rod-cored vesicles. The former are lysosomes derived from multivesicular bodies and contain pore-forming proteins. The latter are the secretory vesicles exclusively seen in LGLs and are markedly increased in number when the NK function is augmented by biological response modifiers. These two structures are believed to be exocytosed in the space between LGL and the conjugated tumor cell. The microenvironment of the liver sinusoids, which includes Kupffer cells, endothelial cells and other lymphocytes, is considered to regulate the function of the liver-associated LGLs. Liver-associated LGLs, as well as Kupffer cells, are intrinsically involved in the defense system of the liver under various physiological and pathological conditions.

Rapid and precise diagnosis of atypical mycobacterial infection by chemotaxonomical and immunological methods.

The species of 205 strains of acid fast bacteria isolated from swine and human mycobacteriosis were identified chemotaxonomically and numericaltaxonomically. The species of the isolates which were identified numericaltaxonomically as Mycobacterium avium intracellulare (MAI) complex were further classified by using both thin-layer chromatography of the antigenic glycopeptidolipids (GPL) from the bacteria and seroagglutination test devised by Schaefer. These MAI complex from swine fell into serotype 8 (45 strains), serotype 4 (32 strains), serotype 9 (9 strains) and untypable (9 strains), respectively. In contrast to swine, human isolates covered more wide ranges of serotypes such as serovar 7, 12, 16 besides serovar 4, 8 and 9. Furthermore, enzyme-linked immunosorbent assay (ELISA) which is based on the type specific glycolipid antigen and infected swine/human sera was applied to distinguish serological variants of the MAI complex. Of the fourteen cases in swine and five in human that had been typed by both the seroagglutination reaction and the thin layer chromatography (TLC) the thirteen in swine and two in human cases showed clear coincidence with the results of ELISA. The results demonstrated that enzyme-linked immunosorbent assay using infected sera was especially useful, and it was recommended from the sensitivity and rapidity as an adjunct to seroagglutination test and thin layer chromatography for the identification of serotypes of MAI complex.

[Measurement of changes in distance between the femoral and the tibial drill holes for the anterior cruciate ligament reconstruction].

We have developed an experimental system in which a new Gallium-Indium containing transducer can continuously measure the changes of separation distances between the femoral and tibial points. The measurements provides information for the attachment location in the anterior cruciate ligament (ACL) reconstruction and used for various combinations of extra-articular and intra-articular methods. At the first experiment, the distance between each pair of points at the level of the capsule for fifteen combinations during simple flexion-extension knee motion were measured on six cadaveric knees. At the next experiment, in an ACL-deficient knee the distances of ten combinations in the intra-articular method were measured. These results indicated that for an isometric placement the combination of the center of tibial insertion and the postero-proximal of the femoral origin of the ACL appeared to furnish a better location for intraarticular reconstruction. No combination was recommended for extraarticular reconstruction.

[A taxonomic study on a mycobacterium which caused a skin ulcer in a Japanese girl and resembled Mycobacterium ulcerans].

A taxonomic study has been made on a mycobacterial strain isolated from skin ulcer lesion of a 19 year-old Japanese girl who has lived only in Japan. By numerical classification, in which 108 characters were used, 6 isolates of this strain formed one cluster together with Mycobacterium ulcerans strains. However, the isolates were distinguished from the M. ulcerans strains, forming a subcluster. The isolates were differentiated by the characters shown in Table 2 from authentic M. ulcerans strains. The mycolic acids of the strain ATCC 33788 isolated from Japanese girl were distinguished from those of the type strain ATCC 19423 of M. ulcerans. The number of carbon atoms of alpha-mycolate of the former centered at 80, whereas those of the strain ATCC 19423 at 76. The number of carbon atoms of the alpha-unit was 24 in both strains and the number of double bonds was 2 in both strains. The strain ATCC 33788 showed a 100% similarity with the strain ATCC 19423 in DNA hybridization. In view of the above results, we have concluded that the strain belongs to a new subspecies of M. ulcerans (M. ulcerans subsp. shinshuense). Until now, M. ulcerans infection has occurred only in three area of the world, Africa, Oceania, and South and Middle America. Our case is the first case of M. ulcerans infection discovered in an area other than the above. However, the strain was not the same as the M. ulcerans described previously but differed from it in mycolic acids and some biological and biochemical characteristics.

Maturation of autogenous tendon grafts in anterior cruciate ligament reconstruction of the human knee--an arthroscopic study.

Fifty-six arthroscopic observations on 53 knees at various intervals after anterior cruciate ligament (ACL) reconstruction using the autogenous quadriceps and patellar tendon were analyzed to clarify the morphological maturation process of the grafts in human patients. Arthroscopic observations of the grafts were divided into four types, except for one rerupture case. In type I, thick synovial tissue with abundant capillary blood vessels filled the intercondylar fossa. In type II, synovial tissue mass was decreased and ligamentous tissue showed through the thin synovial tissue. In type III, relatively thick synovial tissue with many folds and abundant capillary blood vessels enveloped thick ligamentous tissue. In type IV, thin synovial tissue with relatively few blood vessels enveloped thick ligamentous tissue. Correlation between these graded types and intervals after reconstruction was significant (chi-square test, p less than 0.001). These four types of arthroscopic findings are thought to represent four phases of the maturation process of autogenous grafts after ACL reconstruction in human patients.

Arthroscopic observations of autogeneic quadriceps and patellar tendon grafts after anterior cruciate ligament reconstruction of the knee.

Fifty-six arthroscopic observations on 53 knees at various periods after anterior cruciate ligament (ACL) reconstruction using the autogeneic quadriceps and patellar tendon were analyzed to clarify the morphologic maturation process of such grafts in human patients. Arthroscopic observations of the grafts were divided into four types, excluding one rerupture case. Type I was observed in three knees less than six months after ACL reconstruction and in one knee six to 12 months after; Type II, in seven knees six to 12 months after; Type III, in six knees six to 12 months after, in 13 knees 12-18 months after, and in five knees more than 18 months after; and Type IV, in eight knees 12-18 months after and in 12 knees more than 18 months after. The types and the intervals after reconstruction were correlated with four phases of healing, including the maturation process of the implanted autogeneic grafts: (1) synovial envelopment, (2) fibrous tissue ingrowth, (3) transformation into ligamentlike tissue, and (4) hypertrophy or maturation of striated ligamentlike structure.

[A clinical evaluation of cefpodoxime proxetil in pediatrics].

Children with acute infections were treated with cefpodoxime proxetil (CPDX-PR, CS-807), a new oral cephalosporin. 1. A girl of 4 years old, weighing 17 kg, and another girl of 12 years old, weighing 33 kg, were administered orally each 3 mg/kg of CPDX-PR. Blood levels of CPDX reached peaks of 1.39 and 2.26 micrograms/ml at 4 hours-post-dose, and T1/2's were 2.09 and 2.63 hours, respectively. Cumulative urinary recovery rates for 8 hours were 57.3 and 80.9%, respectively. 2. A total of 30 patients was treated with CPDX-PR. These patients included 10 with acute tonsillitis, 6 with acute bronchitis, 5 with bronchopneumonia, 2 with scarlet fever and 2 with urinary tract infections, and one each with acute pneumonia, acute otitis media, acute otitis media plus sweat gland abscess, staphylococcal scalded skin syndrome and acute lymphadenitis. The treatment was effective in 27 cases out of 29 (except one with an unknown response) with a clinical efficacy rate of 93.1%. 3. Bacteriological responses to CPDX-PR were as follows; eradication of pathogen in 7, and unknown in 2 out of 9 cases from whom pathogens had been isolated prior to the treatment. 4. As a side effect, diarrhea was observed in 1 patient, but it was possible to continue the treatment. With regard to laboratory tests, a slight elevation of GOT and slight elevations of GOT and GPT were found in 1 case each.

[A clinical study on cefteram pivoxil in the field of pediatrics].

Cefteram pivoxil (CFTM-PI, T-2588), a new oral cephem antibiotic of ester type, was evaluated for its safety, efficacy and pharmacokinetics. 1. One child, 4 years of age (18 kg body weight), was administered orally 3 mg/kg after meal. The peak serum level of CFTM was 0.78 microgram/ml after 2 hours, and cumulative urinary excretion rate during the first 6 hours was 15.0%. 2. Clinical studies on CFTM-PI were carried out in 17 pediatric patients; 1 with acute pharyngitis, 2 with acute tonsillitis, 1 each with pertussis, acute bronchitis, 2 with broncho-pneumonia, 4 with scarlatina, 3 with acute otitis media, and 1 each with lymphadenitis, acrobystitis and urinary tract infection. Clinical responses were excellent in 9, good in 6, fair in 1, poor in 1, and the overall clinical efficacy rate was 88.2%. 3. Bacteriological efficacy was investigated with 10 strains of 5 species (Streptococcus pyogenes 4, Streptococcus pneumoniae 2, Haemophilus influenzae 2, Enterococcus and Bacteroides 1) isolated from 9 patients. All strains were eradicated. 4. As to adverse reactions, mild diarrhea was observed in 1 patient. But therapy had to be continued without procedure and the diarrhea disappeared after 6 days. No adverse hematological, renal or hepatic effects were noted.

Ornithine-containing lipids of some Pseudomonas species.

Ornithine-containing lipids purified by thin-layer chromatography were found to represent 2-15% of the total extractable cellular lipids in two or three strains each of four Pseudomonas species: P. aeruginosa, P. fluorescens, P. stutzeri and P. cepacia. The structures of the ornithine-containing lipids were elucidated by chemical analysis, thin-layer chromatography, gas-liquid chromatography, gas-liquid chromatography/mass spectrometry (electron impact or secondary ion) and infrared absorption spectroscopy. At least six molecular species of ornithine-containing lipids were present in common in all of the preparations of the four Pseudomonas species. The structure which was the most abundantly in P. fluorescens (about 60% of the total amount of the ornithine-containing lipid) was 3-hydroxyhexadecanoic acid amide-linked to ornithine and esterified to hexadecanoic acid. In addition to this structure, 3-hydroxyoctadecenoic acid amide-linked to ornithine and esterified to hexadecanoic acid was a dominant structure in the ornithine-containing lipids of P. aeruginosa, P. stutzeri or P. cepacia. In P. cepacia, another ornithine-containing lipids with a terminal polar fatty acid, 3-hydroxyhexadecanoic acid amide-linked to ornithine and esterified to 2-hydroxynonadecacyclopropanoic acid or 2-hydroxyoctadecenoic acid, was found; its content, which represented 8-11% of the total extractable cellular lipids, was higher than that of the ornithine-containing lipids with a terminal nonpolar fatty acid. These ornithine-containing lipids exhibited hemagglutinating activity. Additionally, it was very interesting that hydroxy fatty acids included in the ornithine-containing lipids were not found in the phospholipids which represented more than 80% of the total extractable cellular lipids.

Various kinds of lipoamino acids including a novel serine-containing lipid in an opportunistic pathogen Flavobacterium. Their structures and biological activities on erythrocytes.

Lipoamino acids were found to represent 60% of the total extractable cellular lipids in Flavobacterium meningosepticum and F. indologenes which are known to be opportunistic pathogens. The structures of the lipoamino acids were resolved by chemical and physicochemical methods. Three kinds of lipoamino acids were included in the lipid of Flavobacterium: 3-hydroxyisoheptadecanoic acid amide-linked to serine and esterified to isopentadecanoic acid, and 3-hydroxyisoheptadecanoic acid amide-linked to ornithine and esterified to isopentadecanoic acid or 2-hydroxyisopentadecanoic acid. This type of serine-containing lipid is a novel, rare substance and exhibited high hemagglutinating activity (minimum hemagglutinating concentration was 0.25-0.5 micrograms/ml) with very good, stable dispersion of its liposomes. We called the serine-containing lipid 'flavolipin', based on the genus name of the bacteria. Of the two types of the ornithine-containing lipids, the one that had a nonpolar terminal fatty acid showed higher hemagglutinating activity than the one that had a polar terminal fatty acid. The reconstituted liposomes whose lipid composition was similar to that of the original bacterial cell membrane exhibited definite hemagglutinating activity. As soon as a terminal fatty acid of the lipoamino acids was lost, their biological activity on erythrocytes changed from hemagglutinating to hemolytic, being accompanied by the disappearance of animal species specificity. The mechanism of both the hemagglutination and the hemolysis was discussed.

Three-dimensional structure of endothelial cells in hepatic sinusoids of the rat as revealed by the Golgi method.

The three-dimensional structure of endothelial cells in the hepatic sinusoids of the rat was studied by application of light- and electron microscopy on Golgi-impregnated specimens. A number of endothelial cells could thus be individually delineated throughout the hepatic lobules. The cytoplasm, showing heavy silver deposits, consists of two distinct areas, a thick and thin portion. The thick portion, issuing from the region of the perikaryon, branches and tapers toward the cell periphery. The thin portion, occupying the remainder of the cytoplasm, consists largely of highly fenestrated sieve plates. Some intralobular variation can be noted; the thick portion of the endothelial cells is well developed in the periportal zone, while the cells in the centrilobular zone are relatively rich in thin portions. In addition, the area of distribution of an individual endothelial cell is larger in the centrilobular sinusoids than in the periportal zone. Some endothelial cells also possess unique cytoplasmic processes projecting into the intercellular space between hepatocytes and connecting the sinusoidal walls of neighboring sinusoids. These processes may anchor the endothelial cells to the hepatic plates.

Molecular species of mycolic acid subclasses in eight strains of Mycobacterium smegmatis.

Thin layer chromatographic and gas chromatographic separation and mass spectrometric identification of mycolic acid subclasses and molecular species from eight strains of Mycobacterium smegmatis were established. Two major adjacent spots and a lower minor one were detected on silica gel thin layer chromatograms of methyl esters. The most abundant subclass showing the highest Rf value on TLC was that of alpha-mycolic acids (M1), the second was that of alpha-mycolic acids (M1), a shorter homologue than alpha-mycolates, and the third was the hydroxy mycolic acids (M4) derived from epoxy mycolic acids. They were identified by gas chromatography-mass spectrometry as their trimethylsilylether derivatives. alpha'-Mycolic acids were monoenoic acids ranging from C60 to C66 and possessing an alpha-unit of C24:0. Such profiles of alpha'-mycolic acids were common in eight strains. alpha-Mycolates were dienoic acids ranging from C75 to C79 and possessing an alpha-unit of C24:0. In most strains, the major molecular species of alpha-mycolates were odd-carbon-numbered, centering at C77 and C79, possessing a methyl branch in the even-carbon-numbered straight chain. The average carbon number of alpha-mycolates, from seven strains examined, was about 78, but that of the Takeo strain was 76.3. The profiles of epoxy mycolic acid molecular species composition from eight strains ranging from C75 to C81 were very similar to their M1 subclass profiles.

Relationship between traumatic spinal canal stenosis and neurologic deficits in thoracolumbar burst fractures.

Using CT scans of 112 consecutive patients with thoracolumbar burst fractures, we investigated the relationship between traumatic spinal canal stenosis and neurologic deficits. We calculated the stenotic ratios of the area occupied by the retropulsed bony fragments to the estimated area of the original spinal canal. We also examined the shape of the narrowed canal and the disruption of spinal elements. Burst fractures having the following ratios are at significant risk of neurologic involvement: at T11 to T12 with 35% more, at L1 with 45% or more, and at L2 and below with 55% or more.

Free vascularized fibular strut graft in the treatment of kyphosis.

Anterior free vascularized fibular strut grafts were performed in four patients with structural kyphosis, whose curves were: 40 degrees (C3-C7) in a case of Larsen syndrome; 51 degrees (C6-T3) in a postlaminectomy kyphosis; 48 degrees (T9-L2) in a neurofibromatosis; and 125 degrees (T11-L4) in an old tuberculous spondylitis. Bony union at both fibular ends occurred mostly at 12 weeks postoperative. Vascular patency was evaluated by scintigram using 99m TcMDP. All patients showed positive uptake in the shape of the grafted fibula at 1, 3, and 8 weeks postoperative. Without exception, serial roentgenograms demonstrated rapid incorporation of the graft into the vertebral bodies, without any phase of creeping substitution. This method provides stable and long-standing anterior struts, essential in the management of high grades of kyphosis.
