[Clinical experience with ceftizoxime suppositories in bacterial infections in children].

A clinical trial of ceftizoxime suppositories (CZX-S) was performed to evaluate the therapeutic effectiveness in children with bacterial infection. The subjects were 10 children comprising 4 with pneumonia, 3 with lacunar tonsillitis, 2 with pharyngitis, and 1 with UTI. They were given 1 suppository containing either 125 mg or 250 mg of CZX 2 to 4 times a day. The daily per kg body weight dose ranged from 17.1 to 60.0 mg. The result was "markedly effective" in 3, "effective" in 6, and "failure" was recorded in 1. Bacteriologically, successful eradication of causative organisms was confirmed in all the 4 children who underwent the test. No clinical side effects were observed. The only laboratory test abnormality recorded in a single patient was eosinophilia, which was not definitely ascribable to CZX-S. In conclusion, CZX-S have proved to be a clinically safe and effective antibiotic preparation in infantile infection, even in children whose treatment with conventional antibiotics is associated with difficulties.

[Long-term follow-up of surgical patients with ischemic heart disease].

Long-term follow-up of a consecutive 333 patients who underwent coronary artery operations from 1974 through December of 1982 is reported. The surgical procedures in this series consisted of aortocoronary bypass grafting for angina pectoris in 269 patients (Group I), various operations for myocardial infarction and its mechanical complications in 44 patients (Group II), and combined operative procedures in 20 patients (Group III). The 5 year survival rate of Group I, Group II and Group III were 92.9% 72.7% and 80% respectively. Nine out of 14 late deaths in the whole group were not related cardiac deaths. Sixty eight patients with aorto coronary bypass grafting were asymptomatic and 27% were less symptomatic in the long term periods. The postoperative study indicates that aorto coronary bypass grafting increased coronary blood flow estimated by regional coronary venous flow associated with improvement of left ventricular function and myocardial metabolism resulting in so called "better quality of life". The successful aorto-coronary bypass grafting prevented the incidence of myocardial infarction at the time of subsequent combined vascular or malignant operation in the same individuals. Although the early death was somewhat high in the patients with acute myocardial infarction, the patients who survived the operation had the satisfactory long term results.

[The association of cerebral aneurysm with malignant brain tumor--report of a case (author's transl)].

A rare case was reported in which a brain tumor was associated with a cerebral aneurysm. A 59-year-old woman was admitted with right spastic hemiparesis and motor aphasia. She was in somnolence and complained of headache. She had two attacks of loss of consciousness about 12 years previously. Since then she had right spastic hemiparesis and motor aphasia as sequelae. Preoperative CT scan and carotid angiograms showed a malignant brain tumor in the left temporal region and an anterior communicating artery aneurysm. Operation was performed in one stage. At first the neck of the aneurysm was clipped by right sphenoidal approach and the tumor was partialy removed by left temporal approach. The histological diagnosis was glioblastoma multiforme. The cerebral aneurysm associated with brain tumor should be treated as soon as possible. The operative result in our case was good.
