Subintimal injection of contrast medium as a complication of selective abdominal angiography.

The frequency, cause and sequelae of subintimal injection of contrast medium as a complication of selective abdominal angiography have been evaluated. The frequency increased considerably with the degree of superselectivity. The risk of total occlusion and clot formation in the renal artery after subintimal injection is high. Injury and total occlusion of the celiac and hepatic arteries have better chance for healing than of the renal arteries. A subintimal injection in the proximal part of the superior mesenteric artery may result in the development of an aneurysm.

Ether-dissolved ethiodol for lymphangiography: an experimental study.

In order to decrease administration time, the viscosity of Ethiodol was reduced by mixing it with ether; this mixture was evaluated in lymphangiography of dogs. The ether-Ethiodol solution was injected into a cannulated lymph vessel of one hind limb and compared to pure Ethiodol in the other. For the ether-Ethiodol solution, the time required for complete injection was 10-30 min in most cases compared to 60-105 min for Ethiodol alone. The radiographic appearance of the lymph vessels and nodes appeared similar on both sides, and follow-up lymphangiograms in two animals after 1 and 3 weeks showed no difference from the original study. Microscopic examination of the opacified lymph nodes on both sides revealed no histologic differences. The techniques has potential clinical application.

Roentgenologic aspects of haematuria.

In the roentgenologic investigation of haematuria, urography is the main diagnostic tool, other examinations being only of supplementary value. The main importance of other roentgen examinations is the possibility of excluding certain types of suspected disease as the cause of haematuria, and to demonstrate certain vascular malformations in the kidneys with renal angiography. Urography and cystoscopy should be performed in all patients with haematuria, and the possibility of pathologic conditions in both upper and lower urinary tract must be kept in mind. A large number of overlooked calculi and the finding of unsuspected malignant renal tumors in cases where the urography was performed on an emergency basis, underlines the important fact that "emergency" in these cases only indicates that the patient is an acute distress, not that the examination should be hasty and incomplete, or if so, should be supplemented by a repeat examination under normal conditions.

Repeat scintigraphy of the internal mammary lymph nodes in rabbits.

Rabbits were subjected to different combinations of scintigraphy of the internal mammary lymph nodes, fractionated irradiation, and surgical trauma to the chest wall. No influence could be demonstrated on the image at repeat scintigraphy of the internal mammary nodes. Microscopic changes in the lymph nodes and vessels were relatively slight and inconstant.

Growth rate of renal carcinoma as demonstrated by repeat angiography.

Thirteen patients with renal carcinoma, primarily not operated upon, were re-evaluated 1 to 71 months after the first nephroangiography. Tumour progression could be demonstrated in 7 cases with a maximum linear diametric growth rate of 0.5 cm/2.5 months. The reasons for absence of growth in the remainder are not apparent, but probably related to the state of immunologic defense mechanism of the host.

Popliteal pterygium syndrome in a 74-year-old woman.

The case of a 74-year-old woman with the rare popliteal pterygium syndrome is presented. This syndrome is inherited as an autosomal dominant trait with incomplete penetrance and varying expression and consists of cleft lip and palate, lip pits, genital anomalies, popliteal pterygium, and malformations of the extremities. The various treatments our patient underwent over the years are reported. Treatment of popliteal pterygium involves special problems whem removing the skin fold because the nerve and vascular cords lie immediately anterior to the posterior fibrous cord. In the present case there are widespread arthrotic changes, both in the extremity joints and in the spine. These patients are short in stature. This, together with the general arthropathy, suggests a hereditary metabolic disturbance in the cartilaginous tissue.

Angiography of the liver and spleen in Hodgkin's disease.

Angiography of the liver and spleen was performed in 18 patients with Hodgkin's disease. The results of the angiographies have been correlated with the findings at staging laparotomy, which included wedge biopsy of the liver and splenectomy. At angiography nonspecific filling defects and dislocation of intrahepatic arteries were found in 1 patient with lymphoma in the liver. Similar filling defects in the spleen are nonspecific and may occur in both involved and uninvolved cases. No specific angiographic pattern was seen in cases with epithelioid cell granulomas in the liver or spleen. Angiography is thus of limited value in the preoperative evaluation of the liver and spleen. The only reliable method of establishing spread of the lymphoma is laparotomy.
