[Lipiodol transcatheter arterial chemoembolization for non-resectable hepatocellular carcinoma--multicenter cooperative study].

A cooperative group study was carried out on the effect of Lipiodol transcatheter arterial chemo-embolization (L-TACE) for non-resectable hepatocellular carcinoma (HCC). Thirty-seven hospitals in Japan participated in this study and a total of 157 eligible patients included 138 males and 19 females with an average age of 60.3 y.o. In the chemo-embolization, Lipiodol mixed with 20-50 mg/m2 of doxorubicin (adriamycin) was given through a catheter, and this was followed by embolization with gelatin sponge. Effect of additional oral 5-FU (150-200 mg/day) was also studied as an open trial. Levels of serum alpha-feto protein decreased at 10 days after L-TACE, and this decrease lasted for 5 weeks. CR was observed in one patient, PR in 33, MR in 24, NC in 66 and PD in 16. The response rate was 24.3%. Cumulative one-year, two-year and three-year survival rates were 56.0%, 30.8% and 26.4%, respectively. It was concluded that higher survival rates after L-TACE were observed when (1) patients had better functional reserves of the liver, (2) HCC was in the less advanced stage and (3) L-TACE was carried out more than twice. A reduction of the tumor size after L-TACE did not necessarily mean a good prognosis for the patients. There was no significant difference in the survival rate between the patients taking or not taking 5-FU.

[A new method of preparation of doxorubicin-in-oil suspension using ultrasonificator attached with Cuphorn].

The property of selective deposition of oily contrast medium, Lipiodol (LPD), in tumor tissue was utilized for targetting intraarterial infusion chemotherapy for hepatic cancers. For this therapy anti-cancer agents need to be suspended in LPD. In this report the new suspension device using ultrasonificator attached with Cuphorn was studied. Doxorubicin (Dx) was stable to ultrasonification for 1 hour. Ten mg/ml of Dx was mixed with LPD and this mixture was treated 2 times for 5 minutes with the ultrasonification method. This procedure was simple and sterile, as the commercially used Dx vials into which LPD was injected were set in the Cuphorn and ultrasonificated just as sealed. Microscopic examination of the suspension showed uniform dispersion of Dx particles without formation of aggregates. Dx particles were finely and regularly fragmented. In vitro the suspensions showed a gradual release of Dx from LPD to water phase. In one case with hepatocellular carcinoma received intraarterial infusion of this suspension, the size of the tumor and serum level of alpha-fetoprotein was prominently decreased. This ultrasonification method was simple and convenient to prepare Dx-in-oil suspension.

[Changes in the immune response in postoperative patients while running the ventricular assist device (VAD)].

We examined the immunological parameters in two patients who were assisted with LVAD in postoperative LOS. Both patients demonstrated transient consumption of complements immediately after running the LVAD. The serum concentrations of immunoglobulins (IgG, IgM, IgA) showed no significant decrease. Both patients had significant postoperative lymphopenia, but recovered in a week. A decrease in the number of T cells was observed in both patients. The OKT4/OKT8 ratio initially increased in one patient, but gradually declined postoperatively. In the other patient, the OKT4/OKT8 ratio decreased. The lymphocyte response to mitogen (PHA) remained significantly depressed when the number of lymphocytes returned to the normal level, suggesting that the lymphocytes were functionally impaired. Such findings are generally observed after cardiac surgery. Thus, LVAD does not affect the immune response.

Blood group antigens in the intrahepatic biliary tree. II. Type 1 chain N-acetyllactosamine-related carbohydrate antigens in the proliferated bile ductules.

The expression of the carbohydrate antigens related to type 1 chain N-acetyllactosamine (1NAcLc) in the proliferated bile ductules was immunohistochemically examined in liver tissues of 10 cases of chronic hepatitis (CH), 9 of liver cirrhosis (LC) and 8 of alcoholic hepatitis. The ductular expression of the examined blood group antigens was essentially the same among these pathological conditions. The backbone structure, i.e., 1NAcLc (Gal beta 1----3GlcNAc beta 1----3Gal beta 1----3R), was not detected in the proliferated ductules as in the normal bile ductules. Its fucosylated structures (Le(a), Le(b) and type 1 chain H) were more strongly expressed in the proliferated ductules than in the normal ductules. Sialyl-Le(a) which was not found in the normal ductules was detected weakly but definitely in the proliferated ductules. Besides proliferated ductules, single or small numbers of epithelial cells expressing the 1NAcLc-related antigens were found intralobularly. This suggests possible migration of biliary epithelial cells into the hepatic lobules. In conclusion, within the proliferated bile ductules (a) synthesis of the 1NAcLc-related antigens is increased compared to the normal ductules, (b) the backbone structure is completely sialylated or fucosylated as in the normal ductules and (c) alpha 1----4fucosylation of sialyl-1NAcLc, i.e., sialyl-Lea, formation occurs despite its absence in the normal ductules.

Microcomputer-assisted liquid chromatographic separation system: application to toxic compounds identification in poisoned human fluids.

The computer-assisted liquid chromatographic system (MCASYST) is developed for automated identification and analysis. The system has six main functions: retention prediction system, liquid chromatographic data base system, automated identification system, optimization of separation conditions, data loading program from UV multichannel detector, and UV spectral data base system. The performance and potential of this MCASYST system is evaluated for toxic compounds identification in poisoned human fluids.

[Aortic aneurysm associated with coarctation of the aorta].

An 8-year-old female of coarctation of the aorta associated with thoracic aortic aneurysm is reported. Under cardiopulmonary bypass, a 7 cm section of the aorta containing the aneurysm and coarctation was resected and replaced by a 16 mm woven Dacron tube. The postoperative recovery was complicated by chylothorax treated by thoracic aspiration. She is doing well 3 months after the operation. Histologic sections of the resected portions of the thoracic aorta revealed intimal hyperplasia, medial fibrosis and decrease of elastic fibers, especially in the aneurysm.

[Adenosquamous carcinoma and squamous cell carcinoma of the liver: a resected case and three autopsied cases].

A resected case and an autopsied case of an adenosquamous carcinoma and two autopsied cases of a squamous cell carcinoma of the liver are presented. With regard to the first case, a 58-year-old male underwent an operation, based on the diagnosis of a hypovascular hepatoma or a cholangioma. The resected material showed a nodular whitish gray tumor at the S4 region which histologically proved to be an adenosquamous carcinoma. In the two autopsied cases, found was a huge, grayish necrotic mass at the right lobe of the liver and a defect of gall-bladder. The tumor, in both cases, proved to be squamous cell carcinoma originating in the liver.

Effect of column temperature on the retention of peropyrene-type polycyclic aromatic hydrocarbons on various chemically bonded stationary phases in reversed-phase liquid chromatography.

The effect of column temperature on the reversed-phase retention of polycyclic aromatic hydrocarbons has been investigated using various chemically bonded phases. Four solutes, coronene, tetrabenzo[a,cd,j,lm]perylene, tetrabenzo[a,cd,f,lm]perylene and benzo[lm]phenanthro[4,5,6-abcd]perylene, were used as the test probes. The temperature dependences of the retention are almost linear (the logarithm of capacity factor is proportional to the reciprocal of the column absolute temperature) with monomeric C18, monomeric C18 with endcapping and diphenyl bonded phases, while non-linear behaviour was observed with polymeric C18 phases. These differences in behaviour of the stationary phases are interpreted in terms of their structural differences.

Blood group antigens in the intrahepatic biliary tree. I. Distribution in the normal liver.

The distribution of six blood group-related antigens, A, B, H, Lea, Leb and sialylated Lea antigens, in the intrahepatic biliary tree was studied. These carbohydrate antigens in fixed normal liver tissues were immunostained with the use of highly specific monoclonal antibodies in an avidin-biotin-peroxidase complex method. Bile ducts expressed both ABH and Lewis blood group antigens. However, only Lewis blood group antigens could be detected in the bile ductules. Canaliculo-ductular junctions could be clearly delineated with the anti-Lewis blood group antibodies, especially with anti-Lea antibody. Small Lewis antigen-positive cells were scattered intralobularly. They were adjacent to parenchymal liver cells, mainly in Rappaport zone 1, and apparently in continuity with the portal biliary tree. Sialylated Lea antigen was found in some septal bile ducts. No blood group antigen could be detected in the bile canaliculi. These results indicate that (1) biliary tract of a given size has its own pattern of blood group antigen expression, and (2) biliary epithelial cells are not identical with regard to the phenotypic expression of their structural carbohydrates. In future, it will be possible to classify biliary epithelial cells by their blood group antigen expression.

Myocardial positron tomography with N-13 ammonia in assessment of aortocoronary bypass surgery.

A total of 20 patients were examined at rest and during stress with N-13-ammonia myocardial positron emission tomography (PET) before and after aortocoronary bypass surgery in an attempt to evaluate the effect of surgery on myocardial perfusion and to predict the graft status. The PET images were divided into anterior, septal, apical, lateral and posteroinferior segments for analysis and were evaluated as "normal" (no perfusion defects during stress and at rest), "ischemia" (stress-induced defects) and "fibrosis" (persistent defects both at rest and during stress). Approximately 90% of the segments which were ischemic before surgery became normal after surgery. Thus, ischemic changes are highly reversible, and the vessels perfusing these ischemic areas are most suitable for bypass surgery. However, most of the persistent defects failed to respond to revascularization surgery and, hence, represented irreversibly damaged myocardium. In predicting graft status, graft patency could be demonstrated by normal perfusion in postoperative images (p less than 0.01) or by improved perfusion when pre- and postoperative images were compared (p less than 0.01). However, graft occlusion could not be predicted reliably. This study demonstrated that PET with N-13 ammonia was useful in the assessment of the effects of aortocoronary bypass surgery. However, this technique was not significantly superior to thallium-201 single-photon emission computed tomography for only qualitative analysis.

[Arterial infusion chemotherapy and embolization in unresectable hepatocellular carcinoma using an intraarterial catheter].

We performed arterial infusion chemotherapy and embolization on 22 patients with unresectable hepatocellular carcinoma via an intraarterial catheter, which was inserted in the hepatic artery under laparotomy. The administration frequency of intraarterial chemotherapy on a patient was 1 to 155 with a median of 17, while that of embolization was 1 to 6 with a median of 2.5. The maximal and average durations of a catheter in place were 15 and 4 months, respectively. The survival rates were 50% in one year, 18% in two years, and 12% in three years. Insertion technique and management of the catheter are relatively safe and easy, so catheterization of the hepatic artery would enable us to develop new treatment modalities.

Hepatocellular expression of a novel glycoprotein with sialylated difucosyl Lex activity in the active inflammatory lesions of chronic liver disease.

Hepatic expression of sialylated difucosyl Lex antigen (SDLex, NeuAc alpha 2-3Gal beta 1-4(Fuc alpha 1-3)GlcNAc beta 1-3Gal beta 1-4(Fuc alpha 1-3)GlcNAc beta 1-) was studied with monoclonal antibody FH6, which defines this structure. Hepatocytes in the severe form of chronic active hepatitis and liver cirrhosis strongly expressed SDLex. The antigen was only weakly and focally detected in chronic persistent hepatitis. The mild form of chronic active hepatitis showed intermediate expression. SDLex expressed along the liver cell membranes displayed a honeycomb pattern when extensively expressed in the severe form of chronic active hepatitis or in liver cirrhosis. Cytoplasmic expression was faint and focal. Preferential tissue distribution was at the periphery of the hepatic lobules where the distruction of the limiting plate was present. The antigen was also expressed in sinusoidal lining cells and polymorphonuclear cells but not in the biliary epithelia. Hepatocytes expressing SDLex did not express related carbohydrate antigens, ie, Type 2 chain N-acetyllactosamine, Lex, and sialylated Lea. On subcellular fractionation, the microsome fraction contained the majority of the antigen activity. SDS-PAGE and Western blot analysis revealed one major SDLex-active glycoprotein with an apparent molecular weight of 110 kilodaltons. This glycoprotein was different from SDLex-active glycoproteins found in the sera of cancer patients. No ganglioside showed FH6 reactivity. These results indicate that liver cells in active inflammatory lesion expressed a novel glycoprotein carrying SDLex antigen in honeycomblike membrane-associated pattern.
