Landmarks of the knee joint of the lateral radiograph during rotation.

On specimens of human knee joints a study was made of the contours on the lateral radiograph during rotation up to 45 degrees inwards and outwards. The most important characteristics of the femoral condyles are the situation of the limiting groove on the condylar joint surface. The best for identifying the tibial condyles is the lateral intercondylar tubercle which proceeds backwards in an even, convex arch continuing direct in the posterior contour of the lateral tibial condyle.

Occult traumatic dislocation of the patella.

In series of patients with clinically established traumatic patellar dislocation other authors have found surgical and radiological lesions pathognomonic for this condition. In a series of patients admitted with hemarthrosis, and with no knowledge of having had dislocation of their patella, we found the same lesions thus illustrating that a syndrome of occult traumatic patellar dislocation exists. The vertical location of the patients' patellae was found normal measured by the index of Insall and Salvati (9). The intercondylar groove measured by the method of Brattström (4) was found in almost all cases to be under the upper normal limit. Their knee caps were normal following the descriptions of Knutsson. None had valgus knees beyond normal upper limits and none had general laxity of joints, thus indicating that this syndrome may be found in normal knees.

Stress radiographical measurement of the anteroposterior, medial and lateral stability of the knee joint.

A hydraulically operated machine for applying well-defined forces to the knee joint is described. The measurement of anteroposterior as well as lateral and medial stability in the knee joint is based upon roentgenograms exposed while the forces are being exerted on the patient. The advantage of the radiological measuring method over methods using external measurements is the elimination of inaccuracies due to displacements of the soft tissues of the limb. The accuracy was evaluated by test/retest examination of 50 healthy subjects. The radiographs were measured at random after cessation of examinations in the gonylaxometer. The accuracy is +/- 1.2 nm for medial/lateral, +/- 2.4 nm for drawer sign. Diagnostically the difference between the two knees must exceed 2.0 nm (collateral instability) or 3.1 nm (drawer) to exceed standard values. The standard values for the different ligament laxities are given. The applied force to be used for evaluation of medial/lateral stability is recommended to be 9 kg, and for anteroposterior stability between 20 and 30 kg.
