The value of the prophylactic use of doxycycline and tinidazole in elective colorectal surgery.

The present study was undertaken in order to compare the effect of different antimicrobial regimens on the incidence of postoperative wound infections after elective colorectal surgery. The series consists of 84 patients, randomized in the following three groups: Group I was treated with bacitracin/neomycin tablets during the 3 days immediately prior to operation. Group II received doxycycline 200 mg orally 12-18 hours preoperatively and 200 mg intravenously during the subsequent 4 days. Group III is identical with group II, but in addition, 2 grams of tinidazole were given orally as a single dose 12-18 hours preoperatively. The ordinary preoperative treatment with low residue diet and bowel cleansing was the same for all groups. The wound infection rate was 37% (10 patients) in group I, 18.2% (6 patients) in group II, and 8.3% (2 patients) in group III. No serious side effects were observed in any of the groups.

A morphological study on the effect of cyproterone acetate on human prostatic carcinoma.

Sixteen patients with prostatic carcinoma were treated with 200 mg of Cyproterone acetate daily. No other kind of hormonal treatment was given. Transrectal biopsies of the prostate were taken before the treatment was started, and at regular intervals afterwards. The treatment period lasted from 3 to 16 months, with an average of 9 months. A thorough examination of multiple sections from all specimens revealed no convincing signs of cellular involution. The study has demonstrated no specific or significant atrophic changes following Cyproterone acetate therapy. Some possible explanations regarding the effect of Cyproterone acetate on human malignant prostatic tissue are discussed.

Carcinosarcoma of the urinary bladder in an adult male.

Carcinosarcoma of the urinary bladder in a 64-year-old man is described. Total cystectomy with bilateral ureteroileostomy was performed. The patient is in good health 12 months after the operation. Histologically the main bulk of the tumour tissue was rhabdomyosarcomatous, but in some areas atypical glands were intermingled with the rhabdomyoblasts. This histologic pattern has not previously been described. The histologic features, clinical manifestations and treatment of these rare bladder tumours are discussed in relation to the literature.

Treatment of prostatic carcinoma with cyproterone acetate.

Sixteen patients with prostatic carcinoma were treated with 200 mg of Cyproterone acetate daily. No other kind of hormonal treatment was administered. Increasing skeletal metastases were observed in 6 patients, whereas significant reduction of metastases took place in 2 patients. Objective relief of stranguria was observed only in 3 patients. The amount of residual urine increased in 3 patients and was reduced in 5. In about one third of the patients, the prostate gland became smaller and softer. The acidic phosphatases decreased from pathological to normal values in 7 patients. There were no observed hepatic, renal or haemotological side-effects. However, serious cardio-vascular complications occurred in 6 patients, while arterial hypertension developed in 4. It is suggested that Cyproterone acetate cannot be recommended as the only kind of hormonal treatment of prostatic cancer.

Metabolism of testosterone in the human prostate and seminal vesicles.

Tissue from the normal, hyperplastic and the cancerous human prostate as well as tissue from the human seminal vesicles are capable of metabolizing testosterone in vitro. By incubating minced tissue with 3H-testosterone for 2 hours at 37 degrees C the following radioactive metabolites were identified: testosterone (17 beta-hydroxyl-4-androsten-3-one), androstenedione (4-androstene-3,17-dione), androstanedione (5alpha-androstane-3,17-dione), 5alpha-dihydrostestosterone (17 beta-hydroxy-5alpha-androstane-3-one, DHT), 3alpha-androstanediol (5alpha-androstane-3alpha,17beta-diol), 3beta-androstanediol (5alpha-androstane-3beta-17beta-diol) and androsterone (3alpha-hydroxy-5alpha-androstane-17-one). When normal human prostatic tissue was incubated with 3H-testosterone approximately 40% of the hormone was metabolized and 30-35% of the metabolites were identified as DHT. There were apparently no differences in testosterone metabolism between the dorsal and lateral prostatic lobes. A much lower conversion of 3H-testosterone was observed in the seminal vesicles (24%). The same metabolites were formed by prostatic carcinoma tissue, although distinctive quantitative differences from the normal prostate were observed. Thus, only 23% of the testosterone was metabolized by cancerous tissue of which 15% was present as DHT. The formation of 17-keto metabolites and androstanediols in the prostatic carcinoma tissue was approximately the same as in the normal prostatic tissue. The most extensive metabolism of testosterone was found by incubation of tissue from benign nodular prostatic hyperplasia. About 65% of the testosterone was metabolized, and 40% of the metabolites were identified as DHT. Hyperplastic prostatic tissue also showed a significantly higher formation of 5alpha-androstanedoils and the other tissues examined. The high formation of DHT and 5alpha-androstanediols in benign nodular prostatic hyperplasia in comparison with normal and cancerous prostatic tissue and seminal vesicle tissue might indicate that these metabolites should be studied more closely as possible aetiological factors for prostatic hyperplasia. The very low metabolism of testosterone in prostatic carcinoma tissue should be examined further in relation to tumour differentiation and clinical effect of endocrine therapy.

Duplication of the male urethra treated by septotomy. A case report.

In a case of incomplete duplication of the male urethra the interurethral septum was divided by scissors. After this simple operative procedure, the patient was relieved of his obstructive symptoms. Postoperative urethrography revealed considerable improvement of the calibre of the urethral lumen, and uroflowmetry showed almost normalized flow values. This technique is recommended as treatment in similar cases.

Emergency resection of distal tracheal stenosis. A case report.

A case of tracheal stenosis secondary to tracheostomy and respirator treatment is presented. The stenosis caused acute respiratory failure, which made immediate surgical intervention mandatory. It is stressed that usually the tracheal lumen is severely reduced when the stenosis causes alarming symptoms. Rational treatment, which must be instituted immediately, consists of resection of the stenotic area and end-to-end anastomosis.

Uretero-vaginal fistula.

Six cases of uretero-vaginal fistula are presented. Five patients developed this complication after gynaecological surgery and one after Caesarian section. Constant urinary incontinence combined with normal voiding is typical of this lesion. Excretory urography revealed a stricture in the distal part of the ureter in 5 cases, while in one patient the urographic findings were normal. Accumulation of contrast medium in the vagina was seen on urography in 2 patients. Surgical correction is the treatment of choice, and all patients were successfully treated by resection of the distal part of the ureter with reimplantation of the ureter into the bladder.
