Reliability of scintigraphic diagnosis of liver metastases.

Three experienced specialists in nuclear medicine independently read 52 liver scans for the presence or absence of metastases. The accuracy of the assessments was, for each of the observers and for the observers' agreed-upon assessments, determined against findings at laparotomy or autopsy. Overall accuracy, predictive values of a positive and negative test result, sensitivity, and specificity were 0.71-0.79, 0.80-0.90, 0.63-0.72, 0.63-0.77, and 0.77-0.91. The accuracy was not higher for the agreed-upon assessment than for each of the observers' assessments. The interobserver variation was calculated as overall agreement between pairs of observers and ranged from 0.85 to 0.94. After adjustment of overall agreement for chance agreement, kappa values from 0.70 to 0.88 were obtained. In the determination of intraobserver variation overall agreement rates and kappa values ranged from 0.90 to 0.94 and 0.84 to 0.88. The scintigraphic diagnosis of hepatic metastases is reliable but, to increase the reliability further, it should be examined whether observer error reflects error in the primary detection of findings in the scans or error in the overall interpretation.

Food-stimulated serum calcitonin concentrations in patients with ulcer negative dyspepsia, duodenal ulcer and in controls.

In 12 normals serum calcitonin (CT) concentration decreased significantly when eating wtih a return to basal value and a significant postprandial fall. No difference was observed in the food-stimulated CT secretion between duodenal ulcer patients and normals. In 25 patients with ulcer negative dyspepsia a significant increase in serum CT concentration occurred during food intake followed by a return to basal level and a secondary significant rise. The pattern in patients with ulcer negative dyspepsia differed significantly from patients with duodenal ulcer and normals.

The role of serum gastrin in the secretion of calcitonin: studies in patients with pernicious anaemia and in healthy subjects.

Basal concentrations of serum gastrin and serum calcitonin were measured in 38 patients with pernicious anaemia and in 32 healthy control subjects. The fasting level of serum gastrin was greatly elevated in 32 patients and was within normal range in 6 patients, the difference between pernicious anaemia patients and controls being highly significant (P less than 0.001). No significant difference between fasting serum calcitonin concentrations in patients and healthy subjects was found. Furthermore, these hormones were studied before and after a beef meal in eight of the patients with hypergastrinaemia and in six healthy control subjects. After the meal serum gastrin concentrations increased significantly from basal values in both patients and controls, the peak values being reached 60 and 30 min after the meal, respectively. No significant changes in serum calcitonin concentrations were observed after the meal in either group. In five patients a transient rise in serum calcitonin occurred 5-10 min after ingestion, beginning, however, before any rise in serum gastrin was observed. Our results indicate that serum gastrin is without influence on calcitonin release, either in hypergastrinaemic patients with pernicious anaemia or in healthy subjects.

Acquired hydrocephalus. IV. Determination of the absorption rate of albumin from the cerebrospinal fluid.

By quantitative isotope ventriculography (QIV), the absorption rate (lambda 2) of albumin in the cerebrospinal fluid (CSF) was determined. Regard was paid to the rate constants for the exchange of the albumin molecule between the serum and extravascular phase. The rate constant lambda 2 and mean biological transit time Tmb-2 were determined in 22 patients suspected of acquired hydrocephalus. QIV revealed that 8 patients were hydrocephalic, while 13 were non-hydrocephalic, and the findings were uncertain in one. In the patients with acquired hydrocephalus, it was demonstrated that lambda 2 and Tmb-2 were significantly reduced as compared with the non-hydrocephalic patients. By means of QIV, a curve was calculated and plotted for the biological decay from the head--the retention curve. From the initial slope of this curve (0--24 hours), the rate constant lambda 2 was calculated. A comparison of lambda 4 and lambd 2 revealed a highly significant correlation, which means that the retention curve gives an acceptable measure of the absorption rate of CSF-albumin.

Determination of free thyroid hormone fractions using a simple T3-uptake test.

Results from a simple, fast and inexpensive Sephadex T3-uptake test (T3U) have been compared to free fractions of serum thyroxine (T4) and triiodothyronine (T3) determined by a dialysis procedure in forty-six sera from hypothyroids, 154 sera from euthyroids and ninety-one sera from hyperthyroids. Sera with high TBG levels were included in these groups. A second order correlation (r = 0.90) was found between T3U and free T4 and T3 fractions. The T3U had a low diagnostic value for all three groups. Free hormone indices calculated as the product between the T4 or T3 and T3U were linearly correlated to total free T4 (r = 0.94) and total free T3 (r = 0.96) if these are less than 2.5 times the upper nu described.

Serum calcitonin in patients with chronic renal disease.

The relationship between serum calcitonin, calcium-phosphorus metabolism and renal function was studied in 66 patients with chronic renal disease. Serum calcitonin was significantly elevated both in non-dialysed patients and in patients on chronic hemodialysis. In the non-dialysed patient group a highly significant inverse correlation was found between serum calcitonin and creatinine clearance. Between serum concentrations of calcitonin and phosphorus a significant positive correlation was found. In the dialysed patients a significant inverse correlation was found between serum calcium and serum calcitonin. It is concluded that the elevated serum calcitonin in patients with chronic renal disease might be explained by a reduced renal degradation of calcitonin and/or an increased production due to stimulation by serum phosphorus.

Serum calcitonin after renal transplantation.

A prospective study of serum calcitonin was made in 9 patients with chronic renal failure. The patients received a well-functioning renal transplant. At the time of transplantation serum calcitonin was significantly elevated as compared with the serum concentration in normal controls. During the first 3--5 weeks after renal transplantation serum calcitonin decreased. Afterwards it increased to a level of the same magnitude as the initial value. A significant relationship was found between serum calcitonin and serum phosphorus, whereas no significant correlation was present between serum calcitonin and serum creatinine or between serum calcitonin and serum calcium.

Studies on the handling of retinotoxic doses of iodate in rabbits.

Accumulation of iodate in eye tissues and fluids as a possible explanation of the retinotoxic effect of iodate has been studied by intravenous injection of NaIO3(30 mg/kg), 125IO-3 and 131I- in rabbits. 125IO-3 was determined in fluids and tissue extracts by precipitation with BaCl2 after addition of KIO3. 125IO-3 was rapidly broken down in blood (T 1/2 = 14 min.). 125IO-3 was not present in aqueous humour, vitreous or extracts from retina, choroid + pigmentary epithelium or liver. Concentrations of 125I were comparable in blood, choroid + pigmentary epithelium and liver tissue while in vitreous and aqueous humour low concentrations of 125I were found which, however, increased gradually during 5 hrs after injection to reach levels comparable with blood levels of 125I. Retina had a low concentration of 125I. The ratio 125I/131I (R) in blood decreases during the first 60 min. after injection followed by a slow rise. R in retina, choroid + pigmentary epithelium and liver was the same as in blood at the same time after injection. During the first 80 min. after injection R was higher in vitreous than in blood while it was lower in aqueous humour than in blood. At longer times after injection R was identical in the three fluids. The investigation has been supplemented with whole body scintigraphy of rabbits injected with NaIO3(30 mg/kg) and 131IO-3 or 131I-. The reduction kinetics of IO-3 to I- by some body fluids, tissues, cystein and glutathione was also studied. It is concluded that the retinotoxic effect of iodate is not due to accumulation of IO-3 in eye tissues, but more likely to damage to biochemical mechanisms involved in the reduction of IO-3 to I-.

Serum gastrin and serum calcitonin in patients with chronic renal failure.

The interrelationship between serum gastrin and serum calcitonin concentrations was studied in 73 patients with chronic renal failure. In both haemodialyzed and non-dialyzed patients increased serum concentrations of these hormones were found compared with normal controls. In non-dialyzed patients with creatinine clearance above 10 ml/min a highly significant correlation between serum gastrin and creatinine clearance was found, whereas no correlation was found in patients with creatinine clearance below 10 ml/min. Between serum gastrin and serum calcitonin, a significant positive correlation was found in non-dialyzed patients. These findings may be explained by a relationship between the two hormones or be secondary to a decreased elimination due to the reduced renal function.

Scintigraphic estimation of thyroid volume and dose distribution at treatment with 131I.

A scintigraphic method for estimation of thyroid volume and dose distribution for 131I in the thyroid gland is presented using a gamma camera with pinhole collimator connected to a minicomputer and digital system. The three dimensions of each lobe are measured and the volume calculated using the ellipsoid model. Problems regarding changes in magnification and sensitivity with object to collimator distance have been investigated. The iodine distribution in the thyroid gland has been followed for 72 hours after oral administration of 131I in 25 cases. No significant change in distribution was observed in any of the cases.

Increased serum calcitonin in patients with mammary carcinoma.

Serum calcitonin war within normal range in 11 patients with benign breast tumour. In cases with local malignant breast tumour increased calcitonin was found in 25 per cent (4/16), in cases with regional metastases in 40 per cent (2/5), and in cases with osteolytic bone metastases in 77 per cent (10/13). Two patients with osteosclerotic bone metastases had low calcitonin concentration. It is suggested that increased calcitonin in mammary carcinoma is a physiologic response to osteolytic bone metastases.
