[Radioimmunoimaging of malignant melanoma with In-111-labeled monoclonal antibodies 96.5 and ZME 018].

This paper includes the results of imaging and kinetic studies on two kinds of In-111-labeled monoclonal antibodies (MoAbs), 96.5 and ZME 018, which are known to have capability of recognizing different surface antigens present in malignant melanoma cells. These MoAbs were supplied by Hybritech Inc. through Teijin Ltd. The former MoAb (96.5) was used on 11 cases of malignant melanoma and one case of basal cell carcinoma, and the latter MoAb (ZME 018) was used on 6 cases of malignant melanoma and one case of basal cell carcinoma. As for the malignant melanoma, the results obtained from both patients groups were compared to each other. Twenty-four out of 31 lesions in the former group and 9 (including 2 occult lesions) out of 10 lesions in the latter group were visualized. However, these positive ratios can not be compared to each other because of the very small number of the lesions in the latter group. One basal cell carcinoma each from both groups were faintly visualized. Distribution patterns of these In-111-labeled MoAbs were similar to each other. Except for the first two cases in the former group, labeling efficiencies were 94.3 +/- 0.7% with 96.5 and 92.0 +/- 0.6% with ZME 018. Urinary excretions on the 1st day were 8.3 +/- 0.9% with the former and 9.3 +/- 0.3% with the latter. However, there was no statistical difference. Following the 1st day, these values changed at the level of several per cent from the 2nd to 4th day, showing a subtle and gradual rise and slightly lower values for In-111-ZME 018. But, again, there were no statistical differences. Whole body retention curves in both groups were similar to each other, and blood clearance curves were also found similar in both groups. HAMA titers were checked in all patients and were found elevated after the administration in most of the cases. Although the number of patients tested in this series is small, especially with In-111-ZME 018, both MoAbs labeled with In-111 seem to be equally capable of visualizing malignant melanoma. Possibility of the treatment of malignant melanoma with this kind of technique was also discussed.

[A case of summer type hypersensitivity pneumonitis resulting in chronic respiratory failure and cor pulmonale].

A female case of Japanese summer-type hypersensitivity pneumonitis who was a smoker developed in chronic respiratory failure several years later. Biopsy specimen on first admission showed findings of granulomatous bronchioloalveolitis distributed in the center of secondary lobules. Pulmonary function studies demonstrated restrictive disease with high RV% and low airway conductance. In spite of steroid therapy, dyspnea persisted and the same symptoms were found on next summer. Six years later symptoms of chronic respiratory failure and cor pulmonale developed. Chest X-Ray showed dilated pulmonary artery, cardiomegaly and overinflation without apparent fibrosis. Hypoxemia and hypercapnia were also seen on blood gas analysis. Pulmonary function was unchanged compared to the findings on first admission. Since then long term oxygen therapy was started. It was thought that irreversible small airway disease caused by hypersensitivity pneumonitis was attributable to cor pulmonale and chronic respiratory failure because of her smoking habit and long period of exposure to antigen. As a patient with summer type hypersensitivity pneumonitis always has a possibility of chronic disease developing after long term exposure to antigen, such as a farmer's lung, the cessation of exposure to antigen by complete cleaning up of the patient's environment or moving out were considered to be important.

Clinical, microbiological and immunological studies of post-juvenile periodontitis.

The present study includes the clinical, microbiological and immunological examinations of 2 patients with post-juvenile periodontitis. Bacteroides intermedius was the predominant isolate from periodontal pockets with post-localized juvenile periodontitis. Bacteroides gingivalis, Bacteroides forsythus and Actinobacillus actinomycetemcomitans were detected in samples from periodontal pockets with post-generalized juvenile periodontitis. IgG antibody levels to B. gingivalis were significantly higher in the patients than these of periodontally healthy group. Spirochetes, including Treponema denticola, were found at very high frequencies in all samples from the patients.

[On the behalf of the Japanese Society of Biological Response Modifiers].

BRMs (Biological Response Modifiers) strengthen the host defense mechanism. As a result, therapeutic effects for neoplastic and other diseases can be expected. The first meeting of JBRM (Japanese Society of BRM) was held in Tokyo in 1987. The number of general reports in this meeting was 66, and one special lecture was given. The programme consisted of the special lecture, 42 basic research papers and 23 clinical studies. The main theme of this meeting was "Interferons". The topic of the special lecture was "Treatment of AIDS-associated Kaposi's Sarcoma with Interferons," by Ronald T. Mitsuyasu. Most of general reports were concerned about therapeutic effects against neoplastic diseases and tumor cells by combined administration of BRMs. Some reports dealt with the action, the induction of BRMs and its influences on the human oncogene. Kinds of BRMs reported in this meeting were interferon (natural and recombinant alpha, beta and gamma), interleukin-2 (recombinant), tumor necrosis factor (natural and recombinant), lymphokine-activated killer, tumor infiltrating lymphocyte, OK-432, monoclonal antibody and others. In future, the discovery of new BRMs and therapeutic attempts using them are expected.

[Successful repair of a coarctation complex with an anomalous right subclavian artery in an infant].

A 46-day-old infant weighing 2250 g with the coarctation of the aorta, ventricular septal defect, PDA, the anomalous right subclavian artery, and the persistent left superior vena cava initially underwent by the reversed subclavian flap aortoplasty with the anomalous right subclavian artery and pulmonary arterial banding. The reversed subclavian flap aortoplasty was useful for the coarctation complex with the anomalous right subclavian artery. Two months later he gained weight to 2500 g, and then VSD closure and debanding of PAB was performed successfully. This two-staged operation was recommended for the poor risk coarctation complex with the anomalous subclavian artery.

[An experimental study of the use of fresh aortic valve homograft for the usage of the vena caval valve. Part 1. The evaluation of hemodynamics and cineangiographic study].

The purpose of this study is to develop a vena caval valve using the fresh aortic valve homograft. A preliminary study was performed in six dogs by interposing a cryopreserved aortic valve homograft with sinus Valsalva into SVC. After making the tricuspid regurgitation, the aortic valve homograft was closed by the regurgitant flow in the systolic phase and opened by the venous return in the diastolic phase. The preliminary study showed that the cryopreserved aortic valve homograft with sinus Valsalva was useful as a vena caval valve immediately after the implantation. Fresh aortic valve homografts were implanted in 11 dogs in the same manner. Eighteen pressure studies and cine-angiographies were performed in six dogs on 8 to 165 days after the implantation. Pressure studies showed minimal/small pressure gradients across the implanted homografts. However, cineangiography revealed appearance of regurgitation through the implanted homograft three months later. Thromboembolism was not happened in all dogs at all. Therefore, we concluded that the fresh aortic homograft with sinus Valsalva might be clinically feasible for use as a vena caval valve.

[Malfunction of the Björk-Shiley valve prosthesis due to disc defacement--report of two cases of successful reoperation].

Derlin-disc model Björk-Shiley valve prosthesis was reoperated due to the disc defacement. One had admitted cerebral embolism which was suggested due to the prosthetic valve malfunction of aortic position and the other had congestive heart failure due to the malfunction of mitral position. These patients had implanted Derlin-disc model Björk-Shiley valve prosthesis in the aortic and mitral position 10 and 11 years ago. One's cineangiocardiography showed mild aortic regurgitation and the other showed mitral regurgitation due to the malfunction of the prosthesis. The episode of cerebral embolism in one patient was suggested due to the malfunction of aortic prosthetic valve, and congestive heart failure in the other patient was due to the malfunction of mitral prosthetic valve. These prostheses which removed at the reoperation were observed with strat shape indentation and distinct of the disc which resulted the malfunction of the prostheses. In patient who replaced with Derlin-disc model Björk-Shiley valve prosthesis should be carefully followed up.

[Pulmonary valve replacement using the right ventricle-pulmonary artery bypass without cardiopulmonary bypass].

We underwent pulmonary valve replacement using the right ventricule pulmonary artery bypass without cardiopulmonary bypass. This right heart bypass was consisted of the vascular graft and the polystyrene tube. The vascular graft was anastomosed to the main pulmonary artery and the polystyrene tube was inserted into the free wall of the right ventricle. These were connected with a cannulating magnetic flow probe. After the bypass was established, the pulmonary artery was clamped and divided. The pulmonary valves were excised and the pulmonary valve replacement was undergone with a pulmonary valve homograft. Hemodynamics during the pulmonary valve replacement was stable and the postoperative course was uneventful. One month later, the cardiac catheterization and cineangiography showed that the implanted homograft was effective sufficiently. We concluded that this right heart bypass was useful for the pulmonary valve replacement.

[A new valved conduit with commissures using a glutaraldehyde preserved equi pericardium].

We developed a new valved conduit with commissures using a glutaraldehyde preserved equine pericardium, and implanted this prosthesis between the right ventricle and the pulmonary artery in the dog to examined the movement of the valve. Our new prosthesis consists of a conduit and three leaflets. Each leaflet is sutured to the inner surface of the conduit to form a valve. The adjacent free edges of the leaflets are sutured together by placing a mattress stitch 2 mm from their attachment to the conduit so that the leaflets form commissures. These commissures are designed to facilitate prompt coaptation of the three leaflets in diastole. A radiopaque marker is anchored to the center of the free edge of each leaflet and to the outer surface of the conduit to investigate its movement. The movement of the radiopaque markers studied by cineangiography revealed good coaptation of three leaflets in diastole and their full opening in systole. These findings demonstrated that the valve in our prosthesis might function well to prevent regurgitation across it.

Histologic studies on the hepatic lesions induced by graft-versus-host reaction in MHC class II disparate hosts compared with primary biliary cirrhosis.

By light and electron microscopic examinations, histologic changes in the liver of mice with graft-versus-host reaction (GVHR) were analyzed. To induce GVHR, C57BL/6 (B6) spleen cells were injected into (B6Xbm1)F1, (B6Xbm12)F1, and (bm1Xbm12)F1 mice. In (B6Xbm12)F1 recipient mice, bile duct changes resembling chronic non-suppurative destructive cholangitis (CNSDC) and a formation of epithelioid granulomas were observed during the course of GVHR. An epithelioid granuloma in the liver of (B6Xbm1)F1 or (bm1.Xbm12)F1 recipients was not detected. By electron microscopy, the bile duct epithelia were seen to be in close contact with infiltrating cells, and marked alterations of their cytoplasm and microvilli were demonstrated; ie, vacuolation of the cytoplasm, deterioration of microvilli, and bleb formation were frequently observed in the liver of class II-disparate hosts. Concerning the basement membrane, no marked changes characteristic of primary biliary cirrhosis (PBC), such as many-layered basement membranes containing osmium positive substance, were detected. Because the major histocompatibility complex (MHC) class II-disparate system was used in our experimental system in the GVHR, the antigen expressed on the bile duct might be a target and be associated with the formation of the initial hepatic lesions in PBC such as CNSDC and epithelioid granuloma formation. Thus, GVHR across the MHC class II antigen is believed to play an important role in the development of PBC.

[A case of mitral valve replacement with autologous pulmonic valve in congenital mitral stenosis].

A one-year-old infant with congenital mitral stenosis and pulmonary hypertension underwent by mitral valve replacement with his pulmonic valve autograft and pulmonary tract reconstruction with heterogeneous pericardial conduit. His postoperative hemodynamics data showed that left atrial pressure decreased and pulmonary hypertension continued. Echocardiography showed that the implanted autograft functioned properly. On the seventh postoperative day, he died of pulmonary hypertension. In case with congenital mitral stenosis with the small mitral annulus and the small left ventricular cavity, it is difficult to perform mitral valve replacement by commercially available mechanical or tissue valves. Because these valves are not suitable for the small mitral annulus. The mitral valve replacement with pulmonic valve autograft is recommended in such a case with the congenital mitral stenosis.

[The results of surgical treatment of total anomalous pulmonary venous return in neonates].

By the time, the results of surgical treatment for total anomalous pulmonary venous return have been unsatisfactory. From 1981, we changed a surgical procedure fro Gersony-Malm procedure to the posterior approach method, and a surgical technique from the deep hypothermia and the circulatory arrest to the moderate hypothermia and the pump perfusion. Consequently, the results of surgical treatment was improved. From 1981 to 1987, 18 neonates with total anomalous pulmonary venous return underwent corrective operations in our institute. This diagnosis was decided by echocardiography without cardiac catheterization, because the preoperative status of these neonates were poor. Under the cardiopulmonary bypass, we performed the posterior approach method for type I and III, the cut-back method and Van Praagh procedure for II and IV without aortic clamping. According to the posterior approach method, the atrial septal defect was closed through the left atrial incision and the left atrium was anastomosed to the common pulmonary trunk during ventricular fibrillation. The incision was limited within the common pulmonary trunk or the vertical vein and was not extended into the pulmonary veins. According to cut-back method and Van Praagh procedure, the coronary sinus was closed internally so as to avoid the postoperative conduction disturbance. There were four hospital deaths (22.2%). The causes of deaths were pulmonary hypertension in two, low cardiac output in one, and intracranial bleeding in one. From the results of our institution, we concluded that the primary factors determining the outcome were the condition of the patients prior to repair and the severity of pulmonary hypertension.

[Modified Fontan procedure on 106 cases: indication and surgical results].

Since 1974, we have performed modified Fontan procedure on 106 patients, ranging in ages from 1 to 32 years, consisting of 44 cases of tricuspid atresia (TA), 21 with univentricular heart (UVH) of right ventricular type, 18 with UVH of left ventricular type, for which ventricular partition was unfeasible, and 23 with various complex anomalies. Hospital mortality rates for TA and other complex anomalies were 11.4 and 11.3%, respectively. Surgical results have markedly improved recently. Since 1986, 50 cases underwent Fontan procedure with 3 hospital deaths (6.0%). Late death occurred in 4 cases in a mean follow-up period of 49 months. Regarding the indication for operation, majority of patients had 2 to 3 parameters which were out of 10 criteria for Fontan procedure. Regurgitation of atrioventricular valve was repaired by annuloplasty in 19 patients underwent Fontan procedure and 17 survived. Abnormal systemic venous connection was seen in 11 cases and all survived. Association of total anomalous pulmonary venous connection is still a difficult problem and 2 of 5 cases died. Fontan procedure was performed in 8 patients following palliative right ventricular outflow reconstruction for poor development of pulmonary artery and 7 survived. Cumulative mortality rate for the entire series was relatively well at 15.1%.
