[Detection of mediastinal lymph node metastasis from lung cancer with positron emission tomography (PET) using 11C-methionine].

Twenty-five patients with primary non-small cell lung cancer underwent the positron emission tomography (PET) using 11C-methionine to detect the mediastinal lymph node metastasis. We introduced the positron angiography to recognize precisely the anatomical orientation of the mediastinal lymph nodes. The 11C-uptake of the lymph node was expressed with distribution absorption ratio (DAR). A total 107 lymph nodes were examined. The average DAR in metastatic lymph nodes (n = 28) was 3.89 while that of non-metastatic nodes (n = 79) was 2.38 indicating a significant difference (p < 0.001). The most adequate threshold for detection of metastasis was 3.3 with sensitivity of 100%, and specificity of 87.3% and overall accuracy of 89.7%. Metastasis of squamous cell carcinoma was diagnosed more accurately than that of adenocarcinoma. Thus, PET using 11C-methionine may offer a new method to detect the mediastinal lymph node metastasis from lung cancer.

Stability of ranitidine hydrochloride with aztreonam, ceftazidime, or piperacillin sodium during simulated Y-site administration.

The stability of ranitidine hydrochloride after being mixed with commonly used i.v. beta-lactam antibiotics and administered by simulated Y-site injection was studied. Solutions of ranitidine 1 mg/mL (as the hydrochloride salt), aztreonam 16.7 mg/mL, ceftazidime 20 mg/mL (with sodium carbonate), and piperacillin 30 mg/mL (as the sodium salt) were prepared by reconstitution in i.v. mini-bags. To simulate Y-site injection, 2 mL of ranitidine hydrochloride was mixed with 2 mL of each antibiotic in glass test tubes. These admixtures were prepared in triplicate and stored at room temperature under fluorescent light. Concentrations of each drug in each admixture were determined by stability-indicating high-performance liquid chromatography immediately and after one, two, and four hours. Aztreonam, ceftazidime, and piperacillin each retained more than 95% of the original concentration for at least four hours when mixed 1:1 with ranitidine. Ranitidine retained more than 90% of its original concentration for at least four hours when combined with each of the other drugs. Ranitidine 1 mg/mL (as the hydrochloride salt) and aztreonam 16.7 mg/mL, ceftazidime 20 mg/mL (with sodium carbonate), or piperacillin 30 mg/mL (as the sodium salt) were stable for at least four hours during simulated Y-site administration.

[Two cases of broncholithiasis removed by bronchofiberscopy].

Two cases of broncholithiasis, removed bronchoscopically, are reported. Case 1 was a 38-year-old female who was admitted with hemoptysis. The chest tomogram showed calcification near the right middle lobe bronchus. Bronchoscopy revealed a broncholith in B4. Component analysis showed that more than 98% of this stone consisted of calcium carbonate. Case 2 was a 75-year-old male who was hospitalized because of continuous cough. The chest radiograph showed calcification and atelectasis in the right upper lobe. Bronchoscopically, right B3 was obstructed by a broncholith. After removal of the stone, the distal part of B3 was noted to be filled with pus. Analysis of the stone's composition revealed calcium phosphate (77%) and calcium carbonate (23%).

Membrane fluidity and lipid hapten structure of liposomes affect calcium signals in antigen-specific B cells.

Antigen-specific B-cell clones directed against a 2,4,6-trinitrophenyl (TNP) hapten have been established [Hamano et al. (1990) J. Immunol. 144, 811-815]. We measured here the cytosolic free calcium ion concentration ([CA2+]i) in these B-cell clones after antigen stimulation. Trinitrophenylated liposomes with different length spacers between TNP and phosphatidylethanolamine (TNP-Cn-PE) increased cytosolic free calcium concentration in TNP-specific B cells (clone TP67.21). The magnitude of calcium signals depended on the length of the spacer. TNP-C6-PE in dipalmitoylphosphatidylcholine (DPPC) liposomes triggered larger calcium signals in B cells than TNP-Cn-PE with n = 0, 4, 8, or 12. The magnitude of the calcium signals was strongly dependent on the fluidity of the liposome membranes. TNP-C6-PE in the solid DPPC liposomes triggered the calcium signals in B cells 50-100 times as efficiently as TNP-C6-PE in the fluid dimyristoylphosphatidylcholine liposomes. The difference between the solid liposomes and the fluid liposomes was more pronounced in triggering calcium signals in B cells than in antibody binding to these liposomes.

Mitogenic and colony-stimulating factor-inducing activities of polysaccharide fractions from the fruit bodies of Dictyophora indusiata Fisch.

Biological effects (mitogenic and colony-stimulating factor (CSF)-inducing activities) of five homogeneous polysaccharides and a conjugated polysaccharide fraction isolated from the fruit bodies of Dictyophora indusiata Fisch. were investigated. Fucomammogalactan (T-3-Ad) and conjugated polysaccharide fraction (T-2-A) exhibited significant mitogenic and CSF-inducing activities. Among two beta-(1----6)-branched (1----3)-beta-D-glucans (T-4-N and T-5-N), only T-4-N showed both mitogenic and CSF-inducing effects. Partially O-acetylated (1----3)-alpha-D-mannans (T-2-HN and T-3-M') did not show these effects.

[A case report of cavernostomy for lung aspergilloma].

A 65-year-old male patient with right upper lobe aspergilloma treated surgically due to continuation of hemoptysis was reported. Cavernostomy was indicated in this patient, since he had a low respiratory function. After cavernostomy, cavitry infection with MRSA occurred and additional operation was necessary. We should gave great care to intra-cavitary infection. Loss of lung function after the operation seemed to be minimal.

[Role of surgical treatment in atypical mycobacteriosis of the lung].

During the 15 year period from January, 1976 to September, 1990, we treated 77 patients with atypical mycobacteriosis (AM) of the lung surgically with satisfactory results. There were 56 men and 20 women, a ratio of 2.9 : 1. The age of the patients ranged from 20 to 76, with an average of 50.7 years. The operative rate was 3.7% against the patients admitted with a diagnosis of atypical mycobacterial infection. The number of the patients and the types of bacilli classified according to Runyon's criteria were 4 for Group I (M. kansasii), 63 for Group III (M. avium complex), 3 for Group IV, and 7 unknown. The operative indications we adopted were, 1) the resistance to most antituberculotics, 2) localized lesion, and 3) progressive deteriorations. Sensitivity study showed that over 90% of the patients had bacilli with complete or incomplete resistance against all antituberculotics except cycloserine. The area of major involvement was localized in the upper lobe in 47, but was bilateral in 14 patients. The period of conservative therapy prior to the surgical treatment ranged from 2 to 164 months, with an average of 28 months. As to the operative procedures, 58 had lobectomy, 12 pneumonectomy , 11 segmentectomy or wedge resection, 12 thoracoplasty, and 17 combined procedures. The incidence of post-operative complication was 16.9%. The recurrences were observed in 8 patients (10.4%), who had multiple and bilateral lesions. Complete cure rate was 83.1% and 64 patients were able to resume normal life. Our results indicate that, if properly selected, time required for the treatment may become shorter and the patients with AM may get better results by surgical treatment.

[Intra-pulmonary suture abscess with hemoptysis after partial resection--concerning to the pathogenesis of the suture abscess].

A 53-year-old male with a intra-pulmonary suture abscess was admitted with recurrent hemoptysis. Five year before this admission, he had undergone a partial resection of the left upper lobe for pneumothorax using unabsorbable sutures (braided polyester). Chest CT showed tumor shadow in the left S1(+2) a on admission. Because of recurrent hemoptysis, left upper lobectomy was performed. Hemoptysis was due to the presence of a suture abscess. Pathological and clinical analysis of the intra-pulmonary suture abscess showed that it was caused not only by foreign body reaction but also transbronchial infection.

A human skeleton from the Ohguruwa remains.

In 1941, the Ohguruwa remains were discovered at the Mizuho sports ground site in Nagoya. They date from about 3000 B.C., which is the early Jomon era. When the stadium was reconstructed in 1980, four human skeletons were found. Three of them, however, were in poor condition and moreover, were incomplete. However, the second skeleton was in good condition and could almost be reconstructed, and this skeleton (No. 2) was used for our study. It was found in the classic posture with arms and legs folded. Some pieces of a dog's skeleton were also discovered near the No. 2 skeleton's chest. This particular skeleton generally showed characteristics typical of the Jomon era. It had a stout structure and was judged to be a middle aged male because of the following features. There was considerable attrition of the occlusal surfaces on the remaining teeth. The teeth were worn flat, probably owing to the hard food and to their use as a tool. Both canines and first premolars of the upper and lower arches had been extracted in accordance with the custom of the time. Typical caries and localized periodontal breakdown were not observed, although there was horizontal alveolar bone loss, especially in the anterior regions. The mastoid process was extremely large and prominent. The lateral prominence of the mandible was developed. Analysis of lateral cephalogram revealed that the mandible was in the anterior position. The angles of SNA, SNB and ANB were 89.6 degrees, 89.2 degrees and 0.4 degrees, respectively. The skeletal pattern was definitive Class III. The adaptive changes in the teeth, their supporting tissues, temporomandibular joints and the related muscles--the harmonious masticatory system--were all estimated.

[A case of mediastinal mature teratoma with elevated of CA 19-9 in tumor fluid].

A 20-year-old female was diagnosed as mediastinal tumor and admitted to our hospital for the operation. Among the preoperative examinations, the serum level of CA 19-9 was within normal limits. The fluid inside the cystic tumor which was removed during the operation showed high levels of CA 19-9. We discussed the relationship between CA 19-9 and cystic lesions. Histopathological study revealed that CA 19-9 was produced in only epithelial cell of the cystic lesion. And we speculated that examination of serum CA 19-9 helps the diagnosis of cystic lesions.

Antibodies against (1R,2R)-cyclohexanediamineplatinum(II)-DNA adduct recognize the conformational differences of isomeric analogues of cyclohexanediamine.

Antibodies reactive to (1R,2R)-cyclohexanediamineplatinum(II)-DNA ((1R,2R)-cyclohexanediamine: 1R,2R-dach) adducts were elicited by immunization of rabbit with calf thymus DNA modified by Pt(1R,2R-dach)Cl2 at a ratio of bound platinum per nucleotide ((D/N)b) of 0.0335. In an enzyme-linked immunosorbent assay (ELISA), the binding of specific antibodies to Pt(1R,2R-dach)-DNA adduct (60 microliters of 1.235 x 10(-7) M Pt in each wells) on the assay plate was competitively inhibited by Pt(1R,2R-dach)-DNA adduct ((D/N)b = 0.0653) in the solution. Almost equal inhibition was observed with Pt(1S,2S-dach)-DNA ((D/N)b = 0.0412), an optical isomer of 1R,2R-dach. Pt(1R,2S-dach)-DNA ((D/N)b = 0.0371) and Pt(1R,3S-dach)-DNA ((D/N)b = 0.0281) in which the cyclohexane ring is stereochemically perpendicular to the platinum chelate plane, also inhibited antibody binding, but these adducts gave only incomplete inhibition at higher Pt-DNA adduct concentrations. Although Pt(1R,2R-dach)-d(GpG) and Pt(1R,2R-dach)(NH3)2 inhibited antibody binding, the affinity of the antibody for Pt(1R,2R-dach)(NH3)2 was lower than with Pt(1R,2R-dach)-DNA, and the inhibition behavior of Pt(1R,2R-dach)-d(GpG) was biphasic, i.e., at the lower concentration the inhibition curve was consistent with that of Pt(1R,2R-dach)-DNA, but at the higher concentration it shifted to that of Pt(1R,2R-dach)(NH3)2. The affinity of the antibody for cis-DDP was markedly lower than with Pt(1R,2R-dach)(NH3)2. These facts suggest that the antibodies may bind to the substituents (the platinum and its surroundings) of the various Pt complexes rather than the DNA structure altered by platinum binding.

Accelerated recovery from gamma-irradiation-induced leukopenia in mice by the biscoclaurine alkaloid, Cepharanthin--comparison with recombinant human granulocyte colony-stimulating factor.

The effect of a biscoclaurine alkaloid drug, Cepharanthin (CE), on recovery from severe leukopenia induced by whole body gamma-irradiation at a dose of 3 Gy, was compared with that of recombinant human granulocyte colony-stimulating factor (rhG-CSF). Daily intraperitoneal administration of 100 micrograms CE into the irradiated mice significantly prevented decrease of leukocyte numbers in the peripheral blood and accelerated recovery from leukopenia to normal. The activity of CE was somewhat weaker than that of 20,000 units of rhG-CSF when administered daily subcutaneously. In cell composition of peripheral leukocytes, CE increased the numbers of polymorphonuclear (PMN) cells and lymphocytes compared with those of the irradiated controls. Stronger change in PMN cell numbers than with CE was induced by administration of rhG-CSF. No significant increment of the activity to form colonies in spleen (colony-forming unit in spleen; CFU-S) was observed in bone marrow cells from irradiated donor mice administered CE, though CE induced CSF production into sera. Administration of rhG-CSF stimulated the CFU-S activity. These results indicate that CE increased recruitment of PMN cells from the marginal pool rather than the stimulation of pluripotent stem cells in the bone marrow.

Reduction of Cupric Ions with Elemental Sulfur by Thiobacillus ferrooxidans.

In anaerobic or aerobic conditions in the presence of 5 mM sodium cyanide, an inhibitor of iron oxidase, cupric ion (Cu) was reduced enzymatically with elemental sulfur (S) by washed intact cells of Thiobacillus ferrooxidans AP19-3 to give cuprous ion (Cu). The rate of Cu reduction was proportional to the concentrations of S and Cu added to the reaction mixture. The pH optimum for the cupric ion-reducing system was 5.0, and the activity was completely destroyed by 10-min incubation of cells at 70 degrees C. The activity of Cu reduction with S by this strain was strongly inhibited by inhibitors of hydrogen sulfide: ferric ion oxidoreductase (SFORase), such as alpha,alpha'-dipyridyl, 4,5-dihydroxy-m-benzene disulfonic acid disodium salts, and diazine dicarboxylic acid bis-(N, N-dimethylamide). A SFORase purified from this strain, which catalyzes oxidation of both hydrogen sulfide and S with Fe or Mo as an electron acceptor in the presence of glutathione, catalyzed a reduction of Cu by S, and the Michaelis constant of SFORase for Cu was 7.2 mM, indicating that a SFORase catalyzes the reduction of not only Fe and Mo but also Cu.

Cytotoxicity of asymmetric platinum complexes against L-1210 cells. Effect of bulky substituents.

The asymmetric platinum complexes cis-Pt(LL')Cl2 (L = NH3, L' = CH3NH2, (CH3)2NH, C2H5NH2 and (C2H5)2NH and LL' = N,N-dimethylethylenediamine),--one of the NH3 groups of cis-Pt(NH3)2Cl2 was substituted by alkylamine--, were synthesized and their cytotoxic effects have been measured using L-1210 cells. The IC50 values of the asymmetric platinum complexes,--being obtained after 24 h exposure of L-1210 cells to the platinum complexes--, are almost comparable to the corresponding value of cis-Pt(NH3)2Cl2. In 2 h exposure, however, the IC50 values of the platinum complexes were dramatically changed, i.e., a marked difference was observed between those of L' = RNH2 and L' = R2NH. On the other hand, the amounts of platinum taken into the L-1210 cells is little affected by the alkylamino substitution. The results suggest that the bifunctional platinum binding to the target molecule may be responsible for the cytotoxicity.

Vital root resection with advanced periodontitis--case reports.

Furca of multi-rooted teeth are sometimes affected by advanced periodontitis and supporting tissue is almost lost around one of two or two of three roots, while the remaining root or roots are supported by a relatively intact periodontium. In these situations, the procedure of root resection has usually been carried out. Though the necessity of prior or simultaneous endodontic treatment will be considered concomitantly, it is not clear the good periodontal results can be obtained in such cases by these endodontic treatments. If the technique of vital root resection should be indicated to these cases, the vitality of the tooth would be retained and the damage would be minimal. The technique of vital root resection and two periodontally successful clinical cases are presented. This technique appears appropriate in cases in which a multi-rooted tooth is afflicted with a hopeless periodontitis involving one or two roots.

[Orthodontic treatment of malocclusion with periodontitis].

Adults with untreated malocclusions suffer from more periodontal disease than if their malocclusion had been corrected orthodontically. What orthodontists can offer in the management of patients with periodontal disease, how they can help the periodontists and the patients, and how the periodontists can help the orthodontist make treatment safe and purposefully are widely discussed. The purpose of this study is, therefore, to discuss how orthodontic patients with severe periodontitis could be treated and what we should do for these patients during the treatment as well as at their initial visit. One case is presented as an example of treatment for malocclusion with sever periodontitis: The patient was a 23 years 8 months female with a chief complaint of protrusion of upper incisors. Since clinical examination revealed severe periodontitis, periodontal treatment was undertaken for one year prior to orthodontic treatment. From these observations, we are conviced of the importance again of the global approach and the team treatment method in treating malocclusions with periodontitis.
