We report our clinical and pathological observations on four patients with torsion of appendix testis or epididymis, and reviewed 72 cases of torsion of the appendages of intrascrotal organs collected from the Japanese literature; 35 cases of torsion of appendix testis, 36 cases of torsion of appendix epididymis and 1 case of torsion of paradidymis . Case 1 was a 10-year-old boy visiting us because of pain and swelling in his right scrotum continuing for the past ten days. His right scrotum was found to contain a hen-egg sized tender mass. The testis and epididymis could not be differentiated by palpation. The blood count disclosed 10,000 white blood cells. At operation, two appendix epididymis were found in his right scrotum. One of them was twisted 360 degrees clockwise and 10 x 8 x 5 mm in size. Case 2 was a 11-year-old boy with complaint of pain in his left lower abdomen and left scrotum for the past three days. Palpation of his left scrotal contents revealed a slightly hard testis and tender epididymis. At operation, we found his left spermatic cord to be twisted 90 degrees counterclockwise and appendix epididymis 180 degrees counterclockwise. The twisted appendix epididymis was 10 x 8 x 7 mm in size. Case 3 was a 13-year-old boy whose complaint was left scrotal pain. The upper pole of his left testis was found to be swollen and tender by palpation. The exposure of his left scrotum at operation revealed that his left epididymis was abnormally attached to his left testis and had two appendices. One of these appendix epididymis was twisted 180 degrees clockwise and measured 10 x 10 x 8 mm. Case 4 was a 19-year-old male who suffered from right testicular pain for the past seven days. He had a history of three intermittent episodes of similar right testicular pain during the past two years. His right testis was enlarged and palpated slightly hard and tender. We explored his right scrotum surgically and found the appendix testis twisted and enlarged to little-finger's head size. However, it was impossible to determine whether the rotation was clockwise or counterclockwise because the twisted appendix was too severely damaged. The preoperative diagnosis was correct in one case and three were erroneously diagnosed as having torsion of spermatic cord. All four cases were treated by surgery which relieved all patients of discomfort.

Reflex vasoconstriction that occurs in the kidney of the dog can be the result of activation of catecholamine flow reaching the kidney directly from the adrenal medulla via adrenorenal portal vessels. This study was undertaken to learn more about the histologic and fine-structural anatomy of these unusual vessels. We found that they are thin-walled vessels (200-500 micrometers) which are accompanied by many capillaries. These portal vessels have distinctive valves and have sparse, longitudinally arranged musculature which is denser at the base of the valves. The valves in the portal vessels are arranged to permit flow in the direction from the adrenal gland toward the kidney. Smooth muscle at the base of the valve is in close proximity to non-myelinated nerve fibers, suggesting that the opening of these valves may in part be neurally mediated. These unique anatomical features distinguish the adrenorenal portal vessels from other blood vessels and likely have functional importance in control of the adrenorenal circulation.
Changes in the colonic lesions were studied by means of radiographic and colonofiberscopic examination in 7 cases with Crohn's disease treated with total parenteral nutrition (TPN) for an average of 28.6 days. During the treatment, radiographic examination showed that narrowing, irregular contour, and various ulcers of the colon were reversed, at least in part, in all cases, although there was no improvement in the strictured lesions of 2 cases, who finally underwent surgical intervention. The colonic changes were further improved in 4 of the 5 cases who received radiographic examinations of the colon from 18 to 58 months (average 32.2 months) after TPN was discontinued. The present study suggests that treatment with TPN for Crohn's disease is not only beneficial for clinical symptoms but also improves radiographic and endoscopic changes of the colonic lesions except for stricture.

To study in vivo effect of methylcobalamin (CH3-B12) on the peripheral nerve structures, rats with experimental diabetes induced by streptozotocin were administered with daily intramuscular injection of CH3-B12 (500 microgram/kg) for 16 weeks. By isolated nerve fiber studies, CH3-B12-treated diabetic rats showed less incidence of paranodal demyelination as an early sign of segmental demyelination than non-treated diabetic rats. From morphometrical analysis on sural nerves, the reduction in the density of myelinated nerve fibers, nerve fiber size and axon size of myelinated fibers was definitely protected in treated diabetic rats. The results suggested that continuous treatment with CH3-B12 had an ameliorative effect on the peripheral nerve lesions in experimental diabetic neuropathy.

By multivariate analysis using mandible measurements genetic relationships were investigated among 5 inbred strains, DDD, DDK, DSD, DD/Tbr and DDY, derived from the common ancestor, dd mouse. (1) In canonical discriminant analysis (discriminant analysis with reduction of dimensionality), 3 DD strains, DDD, DSD, and DDY, were closer to one another than to any control strain on planes Z1-Z2, Z1-Z3, and Z2-Z3. Only the DDK strain was quite far from those 3 strains. (2) In the discriminant analysis, however, the 5 inbred strains could be classified correctly at the rate of 100%. They were genetically different from one another. (3) The causes for this genetical difference seemed to be the genetic heterogeneity of the ancestor dd mouse and the method of inbreeding which had been independent since the separation.
