[Estimation of I-123 metaiodobenzylguanidine (MIBG) myocardial washout].

A crosstalk from I-123 to Tl-201 (Tl) window was 35 +/- 30% (mean +/- SD) and 30 +/- 10% in a myocardial phantom and the images of 6 patients respectively. However, the crosstalk from Tl to I-123 was approximately 1% in each. I-123 MIBG (MIBG) and Tl myocardial SPECT images were recorded in 3 normal volunteers (N), 10 patients with myocardial infarction (MI), and 4 with dilated cardiomyopathy (DCM). The MIBG and Tl imagings were performed on the other day to avoid the crosstalk. Myocardial washout rates (WR) of Tl and MIBG were derived from 15 min and 4 hour images. WR of Tl was approximately 36% in each group. On the other hand, WR of MIBG in DCM (52 +/- 7%) and MI (41 +/- 14%) groups were statistically higher than in N (24 +/- 7%) group. Thus WR of MIBG would be useful to detect abnormalities in adrenergic nervous system.

[Local hyperthermia using a device made of temperature-sensitive ferrite. The first report: temperature, output characteristics].

The basic study of intracavitary and interstitial hyperthermia was made with the Soft-Heating Method. In this method, a hybrid device composed of non-magnetic metal ring and ferrite rod is used as a heater. Being exposed to external magnetic field, the device produces heat, and its temperature rises to the Curie Temperature of the ferrite rod with self-regulation. The accuracy of the regulation is +/- 0.2 degrees C near the Curie Temperature which can be set up almost freely. In addition to that, the frequency of magnetic field is not so high (20-50 KHz) that there is no damping nor reflection of the field in a human body. Therefore the system seems to be very useful for hyperthermia of deep seated tumor. In this paper, experimental discussion of the possibility of local hyperthermia by the system is made. From the results, it is concluded that the system will be applied to clinical uses.

Micronucleus test with ethyl methanesulfonate administered by intraperitoneal injection and oral gavage.

The effect of route of administration on the outcome of the micronucleus test was studied by administering ethyl methanesulfonate (EMS) by oral gavage (p.o.) and intraperitoneal injection (i.p.) to males of 2 mouse strains, MS/Ae and CD-1. Based on preliminary studies, consisting of a small-scale acute toxicity test and a pilot experiment to determine the optimal sampling time and the appropriate dosages, a micronucleus test was conducted with a 24-h sampling time and doses of 50-400 mg/kg i.p. and p.o. EMS significantly induced micronucleated polychromatic erythrocytes (MNPCEs) with a clear positive dose response by both routes in both strains. Moreover, both routes showed almost the same induction rate of MNPCEs at each dose level tested in both strains.

Acute effect of amitriptyline, phenobarbital or cobaltous chloride on delta-aminolevulinic acid synthetase, heme oxygenase and microsomal heme content and drug metabolism in rat liver.

Cobaltous chloride (CoCl2) caused very marked decreases of cytochrome P-450, b5 and total heme contents and an increase of heme oxygenase activity. On the contrary, phenobarbital (PB) increased hepatic drug-metabolizing enzymes, but the total heme content remained unchanged. On the other hand, amitriptyline (AMT) caused a marked increase of delta-aminolevulinic acid (delta-ALA) synthetase activity at 12 and 24 hr. In addition, the contents of total heme and cytochrome b5 and the activities of aminopyrine (AM) N-demethylase and aniline (AN) hydroxylase at 24 hr were also increased by AMT, whereas cytochrome P-450 content did not change. This may be explained by the fact that AMT would increase hepatic heme synthesis through the prolonged induction of delta-ALA synthetase, but it may not cause an increase in cytochrome P-450 heme because there are increases in the contents of cytochrome b5 and total heme.

[Anesthesia for a patient with HELLP syndrome].

A case of 30 year-old female with HELLP syndrome, who had undergone emergency caesarean section under general anesthesia, was reported. HELLP syndrome is characterized by hemolysis, liver dysfunction and thrombocytopenia, besides symptoms of severe toxemia of pregnancy. After an awake orotracheal intubation, anesthesia was maintained with nitrous oxide, oxygen and muscle relaxant. Blood pressure was controlled with intravenous administration of nitroglycerin. Though the eclampsia was recognized several times during and after the operation, the patient and her baby had no complication nor sequela on their discharge. The key in the anesthetic management of caesarean section in a patient with HELLP syndrome is to control hypertension and eclampsia, to consider the presence of liver and kidney dysfunctions, and to improve anemia and bleeding tendency.

[A clinical study on CA 15-3, CEA, SCC-Ag in patients with squamous cell carcinoma of the lung].

The serum levels of CA 15-3, Carcinoembryonic antigen (CEA) and SCC-Ag were determined in 30 patients with squamous cell carcinoma of the lung. The patients were classified into 2 groups, a peripheral type and a central type, according to bronchoscopic findings. The obtained results were as follows: serum-CA 15-3 and serum-CEA were significantly elevated in patients with peripheral type, and serum-SCC-Ag was elevated in both types of squamous cell carcinoma.

Effects of perphenazine, chlorpromazine or CoCl2 on the activities of delta-aminolevulinic acid synthetase and heme oxygenase and on the content of hemoprotein in rat liver.

Administration of CoCl2 caused a marked decrease of hepatic drug-metabolizing enzymes and the induction of delta-aminolevulinic acid (delta-ALA) synthetase and heme oxygenase. Under the same experimental condition, the inverse relationship between the decrease of hepatic drug-metabolizing enzymes and delta-ALA synthetase activity and the increase of heme oxygenase activity was observed in perphenazine (PPZ)- or chlorpromazine (CPZ)-treated rats. However, the decrease of cytochrome P-450 and aniline hydroxylase by CPZ was later restored or increased over the control level. In addition, CPZ resulted in a marked decrease of total heme content, but this content was not changed by PPZ.

[A case of anaphylactoid reactions to lactated Ringer's solution with 5% maltose].

A 27-years-old man was admitted for correction of nasal fracture. He presented with circulatory collapse and generalised erythema on induction of general anesthesia and during preparation of intradermal test for identifying the offending agent. As both of these responses occurred during intravenous infusion of lactated Ringer's solution with 5% maltose, anaphylactoid responses to this intravenous solution were strongly suspected.

Changes in the motility pattern of human spermatozoa during in vitro incubation.

Suspensions of capacitating human spermatozoa were analyzed for movement characteristics using high-speed videomicrography. The status of the acrosome reaction was also assessed by the zona-free hamster ova penetration test. (1) Movement characteristics of human spermatozoa were classified into 4 types (A, B, C, D). Type A: The movement was active, but its progressive orientation was irregular. Type B: The spermatozoa moved with a wide amplitude of the end of tail. Type C: The amplitude of the tail decreased, and the linear velocity of progression increased. Type D: The whole part of tail showed wavelike rhythmical movements, and the velocity more increased. (2) Movement characteristics of human spermatozoa in vitro gradually changed from Type A to B, C and D. As the spermatozoa classified Type D moved very powerfully and this motility pattern was obviously different from the other types, it was considered hyperactivation of human spermatozoa. The Type D was found from 2 or 3 hr of incubation, and after 3 or 4 hr most of the spermatozoa showed Type D. (3) The motility pattern of human spermatozoa changed to the Type D before the spermatozoa penetrate into zona-free hamster oocytes. It suggested that the hyperactivation occurred before the acrosome reaction takes place.

Effects of perphenazine, diazepam and nitrazepam on amitriptyline metabolism in rat.

Repeated coadministration of amitriptyline (AMT) and perphenazine (PPZ) decreased the AMT-induced increase of hepatic microsomal aminopyrine N-demethylase, aniline hydroxylase, NADPH-cytochrome c reductase and UDP-glucuronyltransferase (UDP-GT) activities in rats. This may be explained by a fact that inhibition of AMT metabolism in hepatic microsomes by PPZ led to increase the non-conjugated formation of AMT and its metabolites in the liver, brain, urine and feces. On the other hand, a slight decrease of drug metabolizing enzyme activity by diazepam (DZP) but not by nitrazepam (NTZ) was enhanced by combined treatment of AMT, whereas the enhancement of UDP-GT activity by DZP or NTZ was suppressed by coadministration of AMT. At the same time there was no clear increase of non-conjugated formation of AMT metabolites in rat urine and feces during combined treatment.
