Accounting for nursing costs by DRG.

This study demonstrates the feasibility of determining total nursing costs by DRGs and reinforces the findings of previous studies using the patient classification system methodology. Study implications regarding population trends, inequities of nursing resource use by DRGs, and outliers are discussed also. A final note warns of the potential liability of isolating nursing costs.

Abnormal use of remaining motor units during locomotion in peroneal palsy.

The firing properties of single anterior tibial motor units were studied (during walking) with electromyogram (EMG) techniques in five normal subjects and in 10 patients who had footdrop due to an acute lesion of the common peroneal nerve. In the normal subjects, high threshold units did not fire in the ordinary step cycle and low threshold units fired at 10-25 Hz. In half of the paralyzed subjects, all motor units fired at 40-50 Hz for more than 200 msec in each step cycle, which could be sufficient for maximal force. In the other half of the paralyzed subjects, high threshold units were not recruited in each step cycle and low threshold units fired at 15-35 Hz, indicating that there were force reserves which were not used to prevent footdrop in each stride.

Perspectives on exercise and aging.

This discussion has particular implications for those involved in the care of the older patient with cardiac disease Without increased attention to research that addresses exercise and aging, decisions about the future activity of the elderly person who has sustained a coronary event may be biased by subjective notions. The question remains regarding whether this individual will be given the same options for rehabilitation as one who happens to be middle-aged. Clearly, there is a need to study the responses to exercise of the elderly with and without diagnosed heart disease. There is also need for a greater number of longitudinal studies such as those of Robinson et al. These studies might compare the exercise responses of active, inactive, and athletic individuals from young adulthood to old age.

Borderline states: prognosis and psychotherapy.

In the authors' opinion the borderline concept covers a group of patients difficult to define and to delineate but nevertheless a group different from the conventional diagnostic groups of neurosis and psychosis. During work with a follow-up study of borderline patients it occurred to the authors that the classification of Grinker et al., which makes it possible to divide borderline patients into four groups, can fruitfully be combined with the object relation theory of "the English school" of psychoanalysis. A patient's belonging to one of Grinker's groups is shown to be informative as to how this patient has "solved" what Fairbairn calls "the schizoid dilemma". The conceptual framework thus set up is useful in planning the treatment of borderline patients: each subgroup of borderline patients as described by the authors can be given its own indications and contra-indications as far as working alliance and treatment plan are concerned.

Genetic and enzymatic analysis of a glycerol kinase deficient mutant in Neurospora crassa.

Genetic analysis showed that the glycerol non-utilizing isolate gly-u(234) of Neurospora crassa is derived by mutation in a nuclear gene situated in the right arm of linkage group I, about 2.2 cross-over units distal to ad-9 and 11 units proximal to nit-1. Enzymatic testings using a radiochemical method indicate that the mutant is deficient for the enzyme glycerol kinase. The radiochemical testings further indicate that the mutation has inactivated an inducible glycerol kinase, while a low residual activity may be due to a second, basal and non-inducible glycerol kinase, in accordance with a proposal by North (1973, 1974) that Neurospora has two glycerol kinases with these properties.

Conjunctival goblet cells in patients with cystic fibrosis.

In five patients with cystic fibrosis of the pancreas the mucous glandular system of the conjunctiva was studied, as changes, if any, in the conjunctival goblet cells might be applicable as a diagnostic test in questionable cases. A whole-mount technique was used, specially developed for studying conjunctival goblet cells. In all five cases the qualitative as well as quantitative goblet-cell findings were in accordance with a previously reported normal material. In particular, there were no signs of stagnated secretion.
