Clinical application of 201Tl scintigraphy in patients with cold thyroid nodules.

201Tl-chloride scintigraphy was performed in 45 patients with cold thyroid nodules. The 201Tl scintigram was positive in 17 of 18 thyroid patients with cancer (94.4%), 8 of 20 patients with an ademoma (40.0%), 1 of 2 adenomatous goiter patients (50.0%), and all of 5 cases of chronic thyroiditis (100.0%). When the cold nodule was demonstrated to be positive with 201Tl, the statistical chance of the lesion being a cellular one was 100.0% and a risk of its malignancy was 54.8%. On the other hand, the nodule with negative 201Tl concentration had a 14.3% chance of cellularity and a 7.1% risk of malignancy. Thus, 201Tl scintigraphy is of use in the differential diagnosis of the cold thyroid nodule.

Detection of active bleeding from ruptured aortic aneurysm by emergency radionuclide angiography.

Radionuclide angiography of the abdominal aorta provides hemodynamic information of the kidneys, aorta and aneurysm as well as the location and extent of aneurysm. Visualization of early abnormal activity in the pelvic region simultaneously with iliac artery visualization is thought to be a specific finding of active bleeding from a ruptured aneurysm of the abdominal aorta.

Increased concentration of 99mTc-methylene diphosphonate and 67Ga-citrate in extracranial bone metastases from pinealoma.

A rare case of extracranial bone metastases from pinealoma is reported. 99mTc-methylene diphosphonate and 67Ga-citrate scans were of great clinical value in confirming the presence of bone metastases. Bone lesions were positively delineated by both radionuclides, but the concentration of 67Ga-citrate was more extensive than that of 99mTc-methylene diphosphonate.

Clinical evaluation of tumor imaging with 201 TI chloride.

201TI was used as an imaging agent in 173 malignant tumors and 76 benign lesions. The sensitivity, specificity, and accuracy were 0.64, 0.61, and 0.63, respectively. Sensitivity was good in thyroid cancer (0.91) and fair in primary lung cancer (0.70) and primary liver cancer (0.71). Compared with 67Ga, 201TI appears to have a higher sensitivity in thyroid cancer and nearly the same sensitivity in primary lung cancer. 201TI might be useful in distinguishing cold thyroid nodules and in differentiating primary liver cancer from metastases.

[Whole-body retention studies of 167Tm-citrate.--Estimation of radiation dose to human from 167Tm-citrate--(author's transl)].

For the purpose of calculating absorbed dose to humans from 167Tm-citrate, the whole-body retention studies using 5 rats were carried out. Up to 40 days following intravenous injection of 167Tm-citrate, the whole-body counts were monitored with a animal counter. The whole-body retention curve was obtained with three exponentaial components. Namely, the 26% of the injected 167 Tm-citrate had a biological half-time of 3.4 hours, 12.5% had a biological half-time of 99 hours and 61.5% had a biological half-time of 106 days. These results indicate, that three components consist of the rapid clearance from the kidneys, the retention in the liver and other soft tissues with relatively long half-time and the retention in the bones with long half-time. Based on these biological data and the MIRD Committee method, the average dose estimates to the bone and whole-body from intravenous administration of 1 mCi 167Tm-citrate were 7.08 rads and 1.28 rads, respectively.

[99mTc-pertechnetate neck uptake in the evaluation of thyroid function--simple method with gamma camera and pinhole collimator (author's transl)].

The neck uptake ratio of 99mTcO4- was measured using a gamma camera and pinhole collimator. The patient was sit 2 m apart from the collimator surface and radioactivity was counted 20 minutes after injection. The whole body count with lead shield (Cc) and without shield (Cd), and natural background count (CBG) were measured. As the shield was utilized the B filter for conventional thyroidal 131I uptake test (lead plate of 1.3 cmx10 cmx10 cm) and it was placed over the anterior neck to cover the thyroid gland at the measurement of (Cc). Then 99mTcO4- neck uptake ratio was calculated as following: (formula: see text) The mean values were 5.3%, 4.5%, 3.1%, 11.2% and 20.7% in the group of euthyroid, hypothyroid, subacute thyroiditis, hyperthyroid state with chronic thyroiditis and Graves' disease, respectively. This method is very convenient and simple to perform following scintigraphy of the thyroid gland with pinhole collimator.
