We present two cases of subacute thyroiditis in which increased radionuclide activity in the affected areas was observed on 201 Tl thyroid scintigrams. These were taken to evaluate cold areas on the 99m Tc-pertechnetate thyroid scintigrams. This finding suggests that 201 Tl may be a potentially useful radionuclide in visualizing inflammatory lesions.

1) The experimental studies have demonstrated that 99mTc-gluconate reaches a high concentration in the kidneys within 1-2 hours after injection and its concentration in organs adjacent to the kidney is low. 2) The initial images of serial macroautoradiograms visualized the inner medulla as well as the cortex and outex medulla, whereas the delayed images revealed only the outer cortex and outer stripes in the outer medulla. These findings suggest that this renal agent consists of two components and the delayed component deposits in the convoluted and straight segments of proximal tuble. 3) This renal agent visualizes the calyceal system and excretory pathways in the initial study and the renal morphology in the delayed study. Therefore, using this renal agent, it may be possible to evaluate a variety of renal diseases, including obstructive uropathy, space-occupying lesions, congenital deformities, etc,.

During the early arterial phase of a case of moyamoya disease, the radionuclide cerebral angiogram showed abnormal increased perfusion at the surface and the base of the head, but there was no recognizable activity in either middle cerebral artery. Contrast angiography demonstrated bilateral occlusion at the carotid siphon, characteristic net-like vascularities in the base of the brain, many anastomotic connections between the dural and cerebral branches, and collateral transdural circulation. The combination of radionuclide angiography, static brain imaging and CT scanning proved useful in investigating moyamoya disease.

A case of septic arthritis diagnosed by combined 99m Tc-phosphate and 67Ga imaging, which preceded positive cultures of the infecting organism, is presented. A previously healthy 10-year-old girl complained of pain in her left hip with persistent fever. Initial laboratory studies showed a normal white cell count and negative blood cultures. A septic process was not confirmed. Blood flow studies and bone images with 99mTc-phosphate showed markedly increased perfusion in the left hip joint without extension to bone. Subsequent 67Ga images revealed a marked increase in radioactivity in the same region, indicating the presence of a septic process. Serial blood cultures and arthrocentesis showed Staphylococcus aureus. The patient improved on antibiotics.
