We describe a 56-year-old man who had a progressive pseudobulbar palsy, spastic tetraparesis, forced laughing and disturbance of voluntary eyelid closure, and was clinically compatible with chronic progressive spinobulbar spasticity. Magnetic resonance images (MRI) revealed atrophy of the bilateral motor cortices and single photon emission tomography after intravenous injection of N-isopropyl-p-iodoamphetamine iodine-123 (IMP-SPECT) showed hyporadioactivity in the same regions. Electrophysiological studies on supranuclear paralysis of eyelid closure demonstrated that so-called apraxia and motor impersistence coexisted and that in attempts to keep the eyelid closed the inhibition of basal activity of the levator palpebrae superioris muscle and activation of the orbicularis oculi muscle were insufficient, indicating the impaired reciprocity of these ocular muscles. The corresponding lesion of these eyelid symptoms was considered to be the bilateral motor cortices.
Thirteen consecutive Japanese patients with chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) were studied by MRI, evoked potentials, and EEG. We found 3 of these patients exhibited symptoms of CNS disorders. Of these 3, 2 with abnormal MRI and visual evoked potentials, and one with abnormal brainstem auditory evoked potentials were detected. Another case without clinical CNS signs showed abnormal EEG findings. The subclinical CNS abnormalities found in the Japanese patients were considered to be less frequent than in cases from Western countries reported previously.
A sixty-two-year-old man who underwent coronary angiography and received acute thrombolytic and anticoagulant therapy for acute myocardial infarction developed multisystemic injury, including renal insufficiency and cutaneous manifestations. Fundoscopic examination and skin biopsy specimen led to the diagnosis of multiple cholesterol embolization syndrome (MCES). Discontinuation of anticoagulants and administration of hemostatic (carbazochrome, tranexamic acid, reptilase, and vitamin K) and antihyperlipidemic (cholestyramine and probucol) drugs resulted in temporary improvement of cutaneous and renal disorders and extended survival for about one year. Besides severe aortic atherosclerosis, postmortem examination revealed numerous cholesterol emboli to multiple organs. MCES is a rare but serious complication of left heart catheterization and anticoagulant therapy, and the optimal treatment remains to be established. The authors suggest here that the above-mentioned therapy might be effective for management of MCES.
To evaluate the effectiveness of mass screening for uterine cancer in Japan, we compared the changes in the age-adjusted rates of potential years of life lost (PYLL) due to uterine cancer between 1969 and 1972 to 1973 through 1977 between the high coverage-rate (intensively screened) areas and the comparable control areas. The percent reduction in the average age-adjusted rate of PYLL due to uterine cancer and the years of life saved per 100,000 females were greater in the high coverage-rate areas than in the control areas. These results suggested that mass screening programs for uterine cancer contribute to saving years of life.
A 41-year-old woman had Noonan's syndrome. Her heart was complicated by asymmetric septal hypertrophy, hypertrophy of the left ventricular free wall, severe pulmonary stenosis, and right ventricular hypertension. On autopsy, a quantitative histologic analysis of the heart revealed that the area of disarray was limited both to the ventricular septum and the left ventricular free wall as in a normal heart. This is not typical of hypertrophic cardiomyopathy because the extent of disarray is high in most cases of hypertrophic cardiomyopathy. Some form of hypertrophic cardiomyopathy, however, seemed to be present in this patient because right ventricular pressure overload did not affect the left ventricular free wall. To clarify the relation between hypertrophic cardiomyopathy and Noonan's syndrome, quantitative histologic analysis is necessary.
Bacteroides gingivalis (B. gingivalis) is isolated frequently from subgingival plaques of adult periodontal patients. B. gingivalis is a gram-negative anaerobic organism which has fimbriae on its cell surface. In the present study, B. gingivalis fimbriae were examined for their ability to adhere to human gingival fibroblasts (Gin-1), and to stimulate fibroblast-derived thymocyte-activating factor (FTAF) production by Gin-1 cells. The ability of the fimbriae to bind specifically to Gin-1 cells was clearly shown by competition assay between 125I-labeled and unlabeled fimbriae. Significant stimulatory effect of the fimbriae on FTAF production was observed, when the fimbriae were added to Gin-1 cells at a dose of 1 microgram/ml, and this stimulation was observed as early as 24 hr after addition of fimbriae to the cells. It was verified by a spleen cell mitogenic assay for the fimbriae that the stimulatory effect was not due to lipopolysaccharide (LPS) contamination of the fimbriae preparation. The FTAF activity was inhibited about 50% by recombinant human interleukin-1 (IL-1) beta antiserum but not by recombinant human IL-1 alpha antiserum. Therefore, the present study suggests that B. gingivalis fimbriae may play a functional role in the pathogenesis of adult periodontal disease induced by the microorganism.
The protein-bound polysaccharide extracted from a fungus, PSK, has been used as a biological response modifier in the treatment of cancer patients in Japan for over ten years. Although the antitumor mechanism of PSK is not fully understood, host-mediated antitumor activity has been claimed to play a significant role. The administration of PSK to tumor-bearing rodents inhibited tumor growth and modulated immune responses. To clarify the potential immunomodulating activities of PSK, we examined the direct effect of PSK on cytokine gene expression and production in human peripheral blood mononuclear cells (PBMC) in vitro. As determined by Northern blotting, PSK was a potent inducer of gene expression for IL-1 alpha, IL-1 beta, IL-6, IL-8, tumor necrosis factor (TNF-alpha) and monocyte chemotactic and activating factor (MCAF), but not for IL-2 and lymphotoxin (LT). Expression of mRNA occurred at 1-3 hr in a dose dependent manner using from 5-400 micrograms/ml of PSK. Furthermore, these cytokines were also produced in response to PSK as detected by ELISA, RIA or bioassays. We speculate that these cytokines may mediate immunoenhancing actions of PSK in vivo.
Facial asymmetry is usually manifested in mandibular prognathism. This is probably because of the intervention of masticatory muscles in some way or another. Functional factors of masticatory muscles are also believed to be responsible for the relapse of facial disharmony even after surgical orthodontic treatment. In the present study, cephalometric analysis was made on a total of 43 patients with mandibular prognathism. The functional activity of their masticatory muscles was measured quantitatively by using electromyography. The data thus obtained were analyzed with respect to their bearings on the morphological points of reference given on posteroanterior cephalogram. Our findings are as follows: 1) At the time of maximal clenching with a bite splint, differences in the activity of left and right masseter muscles had a significantly positive correlation with the deviations from the point Menton on the frontal image, and a significantly negative correlation with the angles of the mandibular ramus to the median line on the frontal view. However, no relation was established between the activity of the masseter muscles and that of the temporal muscles on either side. 2) At the time of maximal clenching in central occlusion, differences in the activities of the masseter muscles on both sides were significantly and positively correlated with the deviations from the point Menton on the frontal image, and significantly and negatively correlated with the angles of the mandibular ramus to the median line. Also a negative correlation was noted between the differences in the activity of temporal muscles on both sides and the deviations from the point Menton. 3) These results suggest that the larger the deflection of the mandible, the larger the activity of the masseter muscle on the deflective side.
Idiopathic interstitial pneumonia (IIP) is known to have a tendency to be associated with lung cancer. Clinical characteristics of lung cancer associated with IIP are discussed in this article. Thirteen cases of lung cancer associated with IIP were examined from 1975 to 1988. A total of 590 cases of lung cancer and 38 cases of IIP were observed during that period. The 13 cases of lung cancer were found to be associated with IIP during the follow-up observation of our patients. Of these 13 patients, 12 men and a woman, the average age was 68.0 years old. All these 13 cases were ex- or present smokers. Five cases of lung cancer had no symptoms, but were detected by abnormal shadows on chest X-ray. Eight cases were detected symptoms. The duration from the onset of IIP to the onset of lung cancer was 36.5 +/- 23.5 months. Primary sites of lung cancer were distributed as follows. Nine cancers were in the left lung (64%) and five cancers were in the right lung (36%). Three cancers were in the hilar region (21%) and 11 cancers were in the peripheral lung field (79%). The numbers of lung cancers were equal in upper and lower lobe. The most common histological type was squamous cell carcinoma, followed by adenocarcinoma. Most cases were in stage III or IV. Three cases were operated, 4 were treated with chemotherapy, 4 were treated with chemotherapy plus irradiation, and 2 cases were treated by BRMs. The frequency of weight loss or finger clubbing in these patients was higher than in IIP patients without lung cancer.(ABSTRACT TRUNCATED AT 250 WORDS)
Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) with central nervous system (CNS) demyelinating lesions has recently been reported to mimic multiple sclerosis (MS). In this paper, a series of patients with CIDP were examined to see if they had CNS involvement. CIDP patients with CNS lesions were then compared to patients with MS with peripheral nervous system (PNS) involvement for similarities. CNS and PNS involvement were detected by clinical symptoms, neurological findings, neuro-otological and neuro-ophthalmological tests, electrophysiological examinations such as electroencephalography, evoked potentials, blink reflex, conventional peripheral nerve conduction studies and electromyography, as well as computed tomography and magnetic resonance imaging (MRI). There were 7 of 17 CIDP patients with CNS involvement, but only 2 of 59 MS patients with PNS lesions were found. The rate of CIDP with CNS involvement (41.2%) was higher than that of MS with PNS lesions (3.4%). The CNS signs and symptoms of 7 CIDP patients were not so constant as their PNS symptoms, and consisted of 1 case with optic neuritis, 4 cases with cerebellar atxia and/or nystagmus, and 3 cases with spinal symptoms. These signs and symptoms are all well known in MS. Prolonged latencies on evoked potentials and high signal white matter lesions on T2 weighted MRI, indicating demyelinating CNS lesions were also similar to those found in MS. The CNS involvement in those patients with CIDP was therefore similar in character to those found in MS, but was far less severe than the PNS finding.(ABSTRACT TRUNCATED AT 250 WORDS)
To promote the comprehensive measure of cancer prevention for future, a hospital-based epidemiological study on a large scale has started at the Aichi Cancer Center Hospital. As the first step of this study by using a common questionnaire, a simultaneous case-control study on the two cancers involving 175 cases with breast cancer, 56 cases with cervical cancer and 231 controls was conducted in 1988. Body weight was positively related to breast cancer only in older (50-69) patients. Young age at first birth (less than or equal to 23) increased the risk of cervical cancer (OR = 4.1). Active and passive smoking increased the risk of cervical cancer (OR = 2.6, 2.3) but only passive smoking increased the risk of older breast cancer (OR = 2.0). Frequent intakes of green vegetables and carrot decreased the risk of younger (30-49) breast cancer (OR = 0.5, 0.5) and cervical cancer (OR = 0.3, 0.5). Some other factors with positive and negative effects on these two cancers were identified in this case-control analysis.
The DNA ploidy pattern was determined by cytofluorometry in 90 cases of surgically resected gastric carcinomas, which were histologically divided into 40 cases of differentiated type and 50 of undifferentiated type. The gastric cancers were classified into two basic ploidy patterns, diploid and aneuploid. Relations among ploidy patterns, histological types and invasion patterns were studied. Diploid patterns were found in 23 cases (58%) of differentiated type and in 37 (74%) of undifferentiated type. In undifferentiated types, aneuploid patterns appeared in 2 (10%) of 19 early cancers, whose rate was significantly lower than that of advanced cancers. 11 (38%) of 29. In differentiated types, however, aneuploids were seen in 10 (48%) of 21 early cancers, not lower than 7 (37%) of 19 advanced ones. Furthermore, in undifferentiated types, aneuploids marked 11 (52%) out of 21 highly invasive carcinomas, while no aneuploids appeared in slightly invasive ones. In differentiated types, aneuploids were found in 3 (38%) out of 8 intramucosal carcinomas and 9 (53%) out of 17 slightly invasive ones. These results suggested that in undifferentiated gastric carcinomas, aneuploid is closely correlated with tumor progression and invasion degree, whereas in differentiated carcinomas such a correlation is not clearly seen.

We have examined polyol pathway kinetics in the lenses of rats made diabetic with streptozotocin. At up to 11 days after diabetes induction, the lenses were isolated and subjected to 'pulse-chase' studies: the lenses were incubated with [13C]glucose and lens metabolism followed by [13C]nuclear magnetic resonance (NMR) spectroscopy. Proton NMR spectroscopy was also performed to measure the hexose monophosphate shunt (HMPS) activity. The results showed that (1) the activity of aldose reductase increased initially and decreased after 11 days of diabetes; (2) the fructose pool increased initially but started to decline after 3 days; (3) the HMPS activity increased nearly 40% immediately after diabetes induction; and (4) the turnover rates of glucose, alpha-glycerophosphate (GP), lactate, sorbitol, and fructose were 80.8 +/- 2.6, 10.1 +/- 1.4, 47.7 +/- 3.7, 7.9 +/- 0.9 and 5.2 +/- 2.2 nmol hr-1 lens-1 (34 mg wet weight lens-1), respectively. Up to 35% of lactate appeared to derive from the polyol pathway. Further, GP was rapidly metabolized, although its fate is currently unknown. These results reveal a far more complex pattern of glucose metabolism in the diabetic lens than that in lenses incubated in high glucose.
