Deep local and regional hyperthermia with annular phased array.

33 refractory tumors mainly located in the pelvic cavity after definitive treatment were treated by loco-regional hyperthermia alone (n = 11) or by heat in combination with radiotherapy (n = 22) by annular phased array (APA) manufactured by BSD Corp. Tumors were heated up to more than 42 degrees C in 78% of 347 total heat sessions with induction time 22 +/- 1 (S.D.) minutes during which those of intra-pelvic organs were elevated up to between 41 and 42 degrees C. Tumor response was CR 18%, PR 50% by heat (11.2 +/- 1.5 S.D. fractions) combined with radiotherapy (43.8 +/- 12.5 S.D. Gy) and by heat alone (8.6 +/- 1.3 S.D. fractions) CR 18%, PR 9%. In all heat sessions superficial pain 36%, skin burn (grade 1-2) 12% inside annular array and slight to moderate systemic heat stress 100% were the main adverse reactions we experienced.

Modification of annular phased array heating pattern.

By attaching 8 pairs of opposing Al-flaps, which have an angle of 30 degrees or 60 degrees with E-field direction in annular phased array (APA), on both sides of APA we could enhance the E-field in the pelvic heating center by a factor of 2 to 3 and also could decrease the E-field ratio of neck/pelvic center and ankle/pelvic center more than 50% compared with conventional use without Al-flaps between RF range of 50 and 90 MHz except 70 MHz. But that of pelvic surface/pelvic center was suppressed 30% only when 30 degrees angled 8 Al-flaps used.

Ultrasonographic identification and measurement of the human fetal pancreas in utero.

One hundred seventy ultrasonographic examinations, done to identify and measure the fetal pancreas in utero, were performed on 78 women with regular menstrual cycle, at 20-41 weeks of gestation. The pancreas, which could be identified after 20 weeks of gestation was measured in 38.4% at 36-39 weeks and 80.0% at 20-23 weeks gestation. The length of the fetal pancreas from the head to tail (FP-L) correlated well with the gestational age (r = 0.90, P less than 0.001). The normal sonographic characteristics of the fetal pancreas are discussed.

High-dose rate intracavitary therapy for carcinoma of the uterine cervix: II. Risk factors for rectal complication.

From August 1978 through December 1982, a total of 267 patients with carcinoma of the uterine cervix were treated using remote afterloading high-dose rate intracavitary therapy (RALS) with non-rigid applicator at our department. The data from 199 previously untreated patients with standard application out of 267 were available for this analysis of rectal complication. The incidence of moderate to severe rectal complication (Kottmeier's grade 2 and 3) was 7% (13/199). Cox's regression model was used for the analysis of risk factors for rectal complication in which even minor injuries (grade 1) were included. By using this method, it was clear that the first significant risk factor was z-coordinates of weighted geometric center (WGC-z) (p = 0.0007) and the second corresponding factor was rectal TDF (p = 0.0082), the sum of the rectal dose measured by semiconductor dosimeter (ICD-5) and the dose of external whole pelvic irradiation. From the analysis of application pattern of intracavitary sources, WGC-z indicated the approximation of intracavitary sources to the anterior wall of rectum quantitatively and three-dimensionally. The significance of rectal TDF also implied that the monitoring of rectal dose by ICD-5 had an important role for the prediction and prevention of rectal complication after RALS.

Comparative studies of strains Ictero No. I and RGA as the type strain of Leptospira interrogans: agglutinin absorption test, protein and antigen profiles, and enzyme activities.

Strain Ictero No. I, the first isolate of Leptospira, isolated by Inada and Ido in 1914, was found to be sufficiently qualified to be the type strain of Leptospira interrogans rather than strain RGA. In an agglutinin absorption test, anti-Ictero No. I serum was not absorbed completely with strain RGA, and 25% of the homologous titer remained unabsorbed, while anti-RGA serum was completely absorbed with strain Ictero No. I. Thus, strain Ictero No. I was not serologically identical with strain RGA, and the two strains were considered to be different serovars. A protein band with a molecular weight of approximately 33,000 daltons was detected in strain Ictero No. I but not in strain RGA by SDS-PAGE. By Western blotting, this protein band was detectable with anti-Ictero No. I serum but not with anti-RGA serum. The presence of the 33K protein in strain Ictero No. I, but not in strain RGA, was confirmed by radioimmunoprecipitation using [125I]-labeled antigens, indicating that the protein antigen was surface-exposed. Only 8 of the 89 enzymes activities were different between strains Ictero No. I and RGA (line Sapporo). From the above results, we propose that strain Ictero No. I should be designated as the type strain of L. interrogans instead of strain RGA.

Ultrasonographic evaluation of adrenal involution during antenatal and neonatal periods.

Using real-time ultrasonography, 25 adrenal glands from prenatal to neonatal periods were prospectively examined to assess the process of shrinkage. A similar sonographic structure was seen in both prenatal and neonatal adrenal glands. The area of adrenal gland (AGA) and the rate of decrease were calculated during antenatal and early neonatal periods. The values of AGA were 350 +/- 19 mm2 within a week of delivery, 304 +/- 17 mm2 (87 +/- 2%) just after delivery, 273 +/- 25 mm2 (77 +/- 5%) on the 1st day, 246 +/- 24 mm2 (70 +/- 5%) on the 2nd day, 215 +/- 23 mm2 (61 +/- 5%) on the 3rd day, 196 +/- 22 mm2 (56 +/- 5%) on the 4th day, 173 +/- 18 mm2 (50 +/- 4%) on the 5th day, 154 +/- 14 (44 +/- 4%) on the 6th day, 140 +/- 12 mm2 (40 +/- 2%) on the 7th day, respectively. In conclusion, the postnatal involution of the adrenals was documented by ultrasound.

Antenatal diagnosis of congenital heart disease and fetal arrhythmia by ultrasound: prospective study.

Fetal echocardiographic and Doppler ultrasonographic prospective studies were performed in utero on 299 babies delivered at Hirata Municipal Hospital, Shimane, Japan, from May 1984 to June 1986. Two or three ultrasonographic examinations were performed on each fetus from 20 weeks of gestation to term. Three congenital heart anomalies and 12 fetal arrhythmias were diagnosed antenatally, but 3 heart anomalies (2 small ventricular septal defects and 1 moderate pulmonary stenosis) were not detected in utero. Routine echocardiographic screening appears to be a useful diagnostic tool to detect congenital heart anomalies and fetal arrhythmia antenatally.

Ultrasonographic identification and measurement of the human fetal adrenal gland in utero: clinical application.

The size of the fetal adrenal gland was determined using ultrasonography in 346 fetuses with no complications at 28-40 weeks of gestation and in 12 fetuses of abnormal pregnancies (8 intrauterine growth retardations, 2 anencephalies, 1 intrauterine fetal death and 1 fetus of a mother who had been on steroids for treatment of systemic lupus erythematosus). The fetal adrenal gland area (FAGA), circumference (FAGC) and length (FAGL) were calculated. In 12 abnormal fetuses, FAGA values always fell below the mean +/- 2 SD. Deviations from the normal values were seen in 9 out of 12 cases (75%) in FAGC and in 4 out of 12 cases (33.3%) in FAGL. Of these pregnancies, 4 (33.3%) resulted in intrauterine fetal or neonatal death, and 2 neonates (16.6%) had to be admitted to the neonatal intensive care unit. Measurement of the fetal adrenal gland, especially of the FAGA, should be a pertinent diagnostic tool for perinatologists to manage and control high-risk pregnancies.

Succenturiate placenta diagnosed by ultrasound.

The succenturiate placenta is a morphological abnormality, the antenatal recognition of which is important as vessels connecting the main placenta with the succenturiate placenta may rupture during labor and fetal death may ensue. In addition, retention of the placental material may lead to postpartum hemorrhage. We treated 5 patients with a succenturiate placenta and the antenatal ultrasonograms and related discussions are presented herein.

Analysis of glucose-6-phosphate translocase and hexose-6-phosphate phosphohydrolase, the two obligatory components of microsomal glucose-6-phosphatase system, in rat liver.

A membrane filter procedure developed by Igarashi et al. (1984) for the measurement of glucose 6-phosphate uptake by the microsomes has been demonstrated to be a good method for assaying glucose-6-phosphate translocase, an obligatory component of the microsomal glucose-6-phosphatase system. When glucose-6-phosphate translocase was assayed in developing and diabetic rat livers independently of hexose-6-phosphate phosphohydrolase, another obligatory component of the glucose-6-phosphatase system, the two activities were found to undergo alterations, whose profiles, however, were quite distinct from each other. The profile of the microsomal glucose-6-phosphatase activity resembles the profile of the phosphohydrolase activity rather than that of the translocase activity, suggesting that the phosphohydrolase may be rate-limiting at least under these conditions. AH-109A, a strain of transplantable rat ascites hepatoma, was found to lack both glucose-6-phosphate translocase and hexose-6-phosphate phosphohydrolase activities.

Ultrasonographic measurements of the fetal neck correlated with gestational age.

Ultrasonographic examinations were performed on 121 normal fetuses between 15 to 40 weeks of gestation to measure the fetal neck. The area (FN-A), circumference (FN-C), anteroposterior diameter (FN-APD) and transverse diameter (FN-TD) of the fetal neck correlated well with the gestational age, respectively. Ultrasonographic measurements of the fetal neck in utero are thus new parameters to evaluate the fetal growth and identify the abnormality of the fetal neck in utero.

[Clinical application of monoclonal antibody-drug conjugates in colorectal carcinoma].

Monoclonal antibody, A7, produced from a mouse splenocyte immunized against human colon cancer was used as drug carrier for colon cancer. A7 had not ADCC and ADMC activity but had ACD activity. Anticancer drug, mitomycin C (MMC), and neocarzinostatin (NCS), were covalently bound to A7 to form the conjugates, A7-MMC, and, A7-NCS. In vitro cytotoxic effect of the conjugates on SW1116 was much stronger than that of free MMC or free NCS. The conjugates, A7-NCS, administered in nude mice brought about the highest NCS concentration in tumor, while normal IgG-NCS distributed evenly in all the tissues. The conjugates showed strong antitumor effect on colon cancer transplanted in nude mice. Forty one patients with colorectal cancer including 10 patients with postoperative metastasis were given A7-NCS. The immunoperoxidase and drug concentration studies of the resected specimens revealed that NCS was found to be localized specifically in cancer. There was no serious adverse effect in the patients receiving the conjugate. Of eight patients with postoperative liver metastasis, three showed evidence of tumor reduction on CT scan and three claimed pain relief. The conjugate was of no benefit to the patients with multiple lung metastasis and peritoneal metastasis.
