Ultrasonographic measurement of the fetal transverse cerebellum in utero.

Ultrasonographic examinations, done to measure the fetal transverse cerebellar diameter (FCD), were performed in 116 women with regular menstrual cycles, at 17-40 weeks of gestation. FCD correlated well with gestational age (r = 0.96, p less than 0.001). The 95% confidence interval of the linear regression was also given. FCD may be a more useful indicator of the accurate gestational age in case of dolichocephaly or brachycephaly, and facilitate antenatal detection of congenital disorders.

Doppler color flow mapping of an invasive mole.

We report here Doppler color flow mapping carried out before and after chemotherapy for an invasive mole. This mapping revealed an abnormal color blood flow within the echo-free space in the uterine myometrium, and pulsed Doppler ultrasound showed a prominent arteriovenous shunt flow. Similar features were obtained by pelvic angiography (PAG). After 4 courses of chemotherapy, the area of abnormal colored flow was reduced and findings on the PAG were supportive. The patient is being closely followed, using Doppler color flow mapping.

Ultrasonographic identification and measurement of the normal ovary in postmenopausal Japanese women.

Real-time ultrasonic scanning examinations were performed on 31 postmenopausal Japanese women with no evidence of pathology. Both ovaries were visualized in 25 patients. Only the left ovary was imaged in 5 and only the right one in 1 patient. Ovarian sectional area (OSA) was 1.45 +/- 0.61 cm2 in the left-sided ovary and 1.36 +/- 0.64 cm2 in the right-sided ovary, with no statistically significant difference. In all 56 ovaries, the OSA was 1.41 +/- 0.63 cm2. These normal values in postmenopausal women provide a useful indicator for detection of postmenopausal palpable ovary syndrome.

Umbilical artery blood flow velocity waveforms and associations with fetal abnormality.

Blood flow velocity waveforms from the umbilical artery of 126 normal fetuses with no evident complications at 20-42 weeks of gestation and 15 fetuses with various abnormalities at 25-42 weeks were recorded using pulsed Doppler ultrasound. The resistance index (RI) was assessed on each blood flow velocity waveform. In 9 fetuses with a slight abnormality, the values of RI were within the mean +/- 2 SD of normal fetuses, and the outcome was good. In 3 of 6 fetuses with a serious abnormality, the values of the RI were high, and 2 of these fetuses died in utero within a few days after the recording. Another fetus with a high RI and 3 with a normal RI died neonatally. Therefore, Doppler ultrasound assessments of the umbilical artery blood flow velocity waveform in utero aid in predicting the outcome for fetuses with major abnormalities.

Transvaginal Doppler color flow mapping.

Transvaginal Doppler color flow mapping was performed on 8 Japanese women (normal, 1; menopause, 2; uterine myoma, 1; endometriosis, 2; pregnant, 2). In all 8, bilateral uterine arteries and branches could be clearly identified in shades of blue and/or red. The color flows were abundant in the pregnant women. Transvaginal Doppler color flow mapping is expected to be an important diagnostic tool for assessing uterine arterial blood flows in physiologic and pathologic conditions of the pelvis.

Brain metastases in patients with limited small cell lung cancer achieving complete remission. Correlation with TNM staging.

We reviewed the brain metastases, after treatment, of 45 patients with limited small cell lung cancer who achieved complete remission by radiochemotherapy or curative operation. No patient received prophylactic cranial irradiation. The incidence of subsequent brain metastases was classified according to pretreatment staging as follows: two of 13 (15 percent) patients in stage I; two of ten (20 percent) in stage II; nine of 17 (53 percent) in stage IIIa; and four of five (80 percent) in stage IIIb. The brain metastases occurred from seven to 29 months after the start of treatment, and the median time of the occurrence was 13 months. Of 17 patients who developed brain metastases and who subsequently received cranial irradiation, there were two in whom relapse had occurred at no other site except the brain and who survived 26 and 79 months after the relapse, respectively. These data indicate that not all patients with limited SCLC achieving CR due to treatment necessarily benefit from PCI.

Fetal cardiac time intervals determined by Doppler echocardiography.

One-hundred and eleven Doppler echocardiographic examinations were performed on 88 normal fetuses, ranging from 16 to 41 weeks of gestation. Aortic and pulmonary maximal velocity (MV), acceleration time (AT), ejection time (ET), MV/AT (acceleration velocity: AV) and AT/ET ratio in systole were assessed on each blood flow velocity waveform. Aortic MV and pulmonary MV correlated well with gestational age, respectively. Good correlations between aortic AT, pulmonary AT and gestational age were evident. There were correlations between aortic AT/ET ratio, pulmonary AT/ET ratio and gestational age, respectively. However, there was no correlation between aortic AV, aortic ET, pulmonary AV, pulmonary ET and gestational age, respectively. These findings may relate to alterations in pulmonary and placental vascular resistance with advance in gestation.

Ultrasonic evaluation of the normal uterus in the neonate.

Using real-time ultrasound, a study was undertaken to evaluate the morphology of normal uterus in the neonate delivered at term. The uterus was visualized in 41 (89.1%) of the 46 neonates. The cervical volume (CE-V) was 3.65 +/- 1.36 cm3, the corporeal volume (CO-V) 1.18 +/- 0.42 cm3, and the CE-V was significantly larger than the CO-V (p less than 0.001). The uterine volume (U-V) was 4.83 +/- 1.57 cm3, and U-V showed a good correlation with birth weight (r = 0.42, p less than 0.01). The endometrial echo was constantly depicted as a mixed echo of a highly echogenic line and/or a myometrial halo. These normal values and the morphology of the neonatal uterus provide useful information for distinguishing a possible pathologic pelvic mass and screening of congenital genital disorders.

Doppler ultrasound assessment of tumor vascularity in gynecologic disorders.

Real-time two-dimensional and pulsed-wave Doppler ultrasonic examinations were performed on 8 normal volunteers and 97 patients with various gynecologic disorders; the objective was to assess uterine and tumor vascularities. Each arterial blood flow velocity wave-form was classified into two types. The resistance indices of normal and abnormal flows were greater than .7 and less than .7, respectively. In normal volunteers, abnormal flows were nil. In 8 of 44 patients with benign tumors (18.2%), abnormal flows were evident and all proved to be cases of leiomyoma or adenomyosis. Doppler signals were not detected in 18 of 36 patients with cervical carcinoma (50%) and abnormal flows were noted in only 6 (16.7%). In all cases of endometrial carcinoma, ovarian carcinoma, and trophoblastic disease, typically abnormal flows were noted. Moreover, in most subjects a decrease in blood flows was observed after chemotherapy by anticancer drugs or irradiation. Therefore, Doppler ultrasound is a pertinent and noninvasive tool that can be used repeatedly for assessing the tumor vascularity in gynecologic disorders.

[Effect on immunological response of host by mitomycin C adsorbed into activated carbon particles (MMC-CH) in mice].

Mitomycin C adsorbed onto activated carbon particles (MMC-CH) has been administered intraperitoneally for C57BL/6 mice. The weight of the spleen and thymus of the mice given MMC-CH was decreased lesser than those of the mice given mitomycin C aqueous solution (MMC-AQ). The number of peritoneal exudate cells (PEC) in the mice given MMC-AQ was decreased remarkably on 1st day after MMC-AQ administration and recovered within normal range on the 7th day. On the other hand, the number of PEC in the mice given MMC-CH was increased remarkably on the 1st day and then gradually decreased to normal range on the 7th day. Reactivity of spleen cells by Con A was inhibited in the spleen cells from the mice given MMC-AQ more than those from the mice given MMC-CH. Fifth percent lethal dose was 8.0mg/kg in the mice given MMC-AQ, and 18.2mg/kg in the mice given MMC-CH.

[Responsiveness and mechanisms of action of steroid hormones in human endometrial adenocarcinoma cells].

The responsiveness and action mechanisms of steroid hormones and epidermal growth factor on human endometrial carcinoma cells are analyzed by using in vitro culture system. 1) The Ishikawa cells, derived from a well differentiated endometrial adenocarcinoma and possess ER and PR, are shown to respond to estrogens by increasing a variety of parameters, viz cell proliferation, PR levels, ALP and DNA polymerase activities. 2) ER and PR of those cells are localized in the nuclei by immunocytochemical staining using the monoclonal antibodies against to ER and PR, confirming the correctness of Gorski and Greene's one step theory involving the action mechanisms of steroid hormones. 3) Progestins reduced the ER level and stimulate E2DH activities and glycogen content, which are completely abolished by anti-progestin (RU486), suggesting that PR of those cells should be functional. 4) These responses to steroid hormones of Ishikawa cells are synergistically enhanced or appeared earlier by addition of EGF. 5) The main metabolite of E2 incubated with Ishikawa cells is E2-3-sulfate instead of E1, indicate that the higher estrogenic status may be persisted in endometrial cancer tissues.

[An attempt of in vitro portal invasion model of hepatocellular carcinoma utilizing permeable collagen membrane].

Hepatocellular carcinoma (HCC) possessed the ability of vascular invasiveness toward hepatic portal vein on the process of progression. This biological character of HCC can influence the patients survival on clinically. In this paper, we tried to establish the in vitro portal invasion model with human materials. The hepatic portal vein endothelial cell (HPVEC) derived from intrahepatic portal veins by surgically, have been propagated, as outgrowth cultures in RPMI-1640 medium with 10% fetal bovine serum, on permeable collagen membranes (KOKEN, Tokyo) containing mainly type I collagen, covered with a solubilized tissue basement membrane (MATRIGEL, Collaborate Res., Inc., Bedford MA) involving type IV collagen, laminin and proteoglycan. The primary cultured HPVEC with polygonal shaped cells forming a pavement stone sheet, were positively stained with Factor VIII related antigen and synthesized both prostacyclin and collagenase inhibitor. Co-culture of primary human HPVEC and HuH-7 (human HCC cell line obtained from Prof. Satoh, Okayama Univ.,) cells were inoculated onto reverse side between collagen membrane and gell formed basement membrane. Morphological alterations on the side of HPVEC can be obtained such as polylayered cells and different cytoplasmic cells among HPVEC. These results indicate that this experimental model can provide an useful in vitro model for the study of HCC portal invasion.

[Radiation therapy of early glottic cancer (T1N0M0); retrospective review of historical control].

From November 1977 through April 1982, a total of 91 patients with early glottic cancer (T1N0M0) were treated with the open field technique of 4 MV X-ray using parallel opposed fields in bandage. Radiation dose administered was 60 Gy in 30 fractions over 6-week periods. Actuarial 5-year disease-free survival rate was 88.7%. Significant prognostic factors of local control were tumor length, tumor width, tumor type, tumor response at 40 Gy and 60 Gy. Ultimate local control rate by salvage surgery was 97% and voice preservation rate was 91%.

Ultrasonographic evaluation of pelvic inflammatory disease.

The relative frequencies of various sonographic patterns and features of pelvic inflammatory disease (PID) were examined in retrospective analyses of sonograms in 36 proven cases. The original sonographic reports correctly predicted PID in 34/36 patients (sensitivity = 94.4%). Two cases were found to be tubo-ovarian abscess although at first they were reported to be ovarian neoplasia. The most frequent finding was dilatation of the fallopian tube (72.2%). String sign within the dilated tube that would reflect increased interface within the endosalpinx was found in 50.0%, fluid collection in the Douglas' pouch in 47.2%, which was confirmed by the culdocentesis and aspiration in 16 cases, and/or tumor formation at the adnexal region in 38.9%. These findings were characteristic but not specific in PID. Careful sonographic scrutinization should improve the diagnostic accuracy of PID in patients with low abdominal pain, high temperature and low back pain.

[Mechanism of action of endocrine therapy of endometrial carcinoma].

The mechanism of action of sex steroid hormones on endometrial carcinoma was analyzed by a cell culture system. 1) Estrogenic actions such as enhancement of ALP activity, PR level and cellular growth were confirmed in Ishikawa endometrial carcinoma cells which possess ER. 2) ER and PR are both localized immunocytochemically in the nuclei of Ishikawa cells, confirming the correctness of Gorsky and Greene's theory in endometrial carcinoma. 3) In Ishikawa cells with PR, progestins stimulated E2DH activity and glycogen synthesis and reduced ER level, thus proving the existence of their hormonal action. We cannot, however, rule out the pharmaceutical action of progestin, because a decrease in nucleic acid syntheses and ALP activity, alteration in electron microscopic observation and finally cellular death were observed in endometrial cancer cells without PR. 4) Conversely, Tamoxifen, anti-estrogen, stimulated the cellular growth in serum-free environment of culture. We must therefore be careful in using it clinically.

[A case of hypertrophic obstructive cardiomyopathy with left atrial giant thrombus during anti-platelet therapy].

A case of a 77-year-old female with hypertrophic obstructive cardiomyopathy having the left atrial "giant" thrombus was reported; the diagnosis of the localization and size of the thrombus was made by employing echocardiography and contrast enhancement computed tomography as well. The clinical record indicated that the thrombus formation, which happened in the course of the antiplatelet therapy, was to be considered due to the complicated condition of the patient, i.e., the combination of atrial fibrillation, low cardiac output, enlargement of left atrium and left ventricular diastolic disfunction, which assumed to predispose the giant thrombus. Avoiding the surgical removal of the thrombus because of the poor conditions of the patient, urokinase treatment was attempted, but not successful, which might be derived from the nature of the thrombus already organized. In general, left atrial giant thrombus is very uncommon status; if any, it is occasionally complicated with mitral stenosis. The case reported deems to be very rare, enough worthy to be reported.
