Transcatheter arterial embolization therapy in cases of recurrent and advanced gynecologic cancer.

Transcatheter internal iliac arterial embolization therapy (TAE) using Gelfoam particles was performed in 24 patients with recurrent gynecologic cancer and ten patients with advanced gynecologic cancer who had previously undergone radiotherapy. The tumor showed complete response (CR) to the therapy in six patients, partial response (PR) in 12, minor response (MR) in three, and no changes (NC) in 13 patients, with the response rate (CR + PR) of 52.9% (18 of 34). No serious or prolonged side effects were encountered except for vesicovaginal fistula in three patients and renal failure in one. The median duration of survival was 299 days, and the 1-year cumulative survival rate was 32.5%. The factors that were associated with favorable outcome after TAE were good general condition, no distant metastases, tumors less than 5 cm in diameter, and responses to the therapy of PR or better. Thus, TAE appears useful for the treatment of recurrent and advanced gynecologic malignancies.

[Esophago-bronchial fistulas in adult].

Esophago-bronchial fistulas in adult are rare and for the most part are due to a malignant tumor. The authors present two cases of benign Esophago-bronchial fistula which were discovered accidentally by esophagography. One case was considered to be congenital. Esophagography is the most useful for diagnosis of this disease.

[The usefulness of 99mTc-PMT cholescintigraphy in the diagnosis of bile leakage or biloma].

5 cases (6 lesions) of bile leakage or bilomas were experienced. All cases showed high accumulative areas in the lesions of bile leakage using 99mTc-PMT cholescintigraphy, while US and CT studies showed no specific findings of fluid-filled areas. Cholescintigraphy demonstrated communications between the injured biliary systems and other spaces very clearly. Even if the needle aspiration failed to get a characteristic bile, which were sometimes seen in bilomas complicated with infection or bleeding, cholescintigraphy was useful for the accurate diagnosis. It is also useful for monitoring the bile drainage.

[MRI of the paranasal sinus mucopyocele].

Seven cases of the paranasal sinus mucopyocele were examined by MRI. All of the mucopyocele in our series were clearly demonstrated on MRI using long SE pulse sequence as high signal intensity. However, the degree of signal intensity was varied based on the contents of the mucopyocele. We can detect the mucopyocele as a three dimensional image by MRI, however, CT was necessary for evaluating the bone destruction caused by mucopyocele.

[Multi-modal treatment of patients with multiple liver metastases caused by sigmoid cancer].

A case of sigmoid cancer with multiple liver metastases (S2PON3 + H3) who was treated by multi-modal treatment was reported. The multi-modal treatment is including intra-arterial administration of anti-cancer drugs as a pre-surgery treatment, intra-arterial infusion chemotherapy lasting for three to five weeks (three times), hyperthermia treatment combined with intra-arterial administration of anti-cancer drugs and intra-arterial expandable metalic stent. The patients lived for 2 years and 4 months in good condition.

[Radiosensitizing effects of perfluorochemicals].

Malignant neoplasms are often refractory to radiotherapy because they contain areas of hypoxic cells that tolerate irradiation, reducing the effect of the treatment. If these areas of hypoxic cells can be oxygenated, the effect of radiotherapy is expected to be enhanced. Hyperbaric oxygen therapy was devised in the 1950s, and the radiosensitizing agent Misonidazole was developed in 1970. However, neither produced satisfactory clinical effects in radiotherapy of tumors. In this study, hypoxic cells in a solid tumor were efficiently oxygenated by the use of perfluorochemicals (PFC) developed as artificial blood with carbogen gas (CG), and the anti-tumor effect of irradiation was enhanced. In C3H mice bearing RIF-1 tumor, the mean oxygen pressure increased to 79.8 mmHg in those treated with PFC and CG as compared with 12.9 mmHg in the controls, and the does modification factor in irradiation of these mice was TCD50 1.47. PFC is currently under clinical trials, and we also noted effective oxygenation of tumors. These findings indicate the usefulness of PFC as a radiosensitizing agent.

[Transcatheter arterial embolization therapy of recurrent pelvic cancer].

Embolization of the internal iliac artery with Gelfoam partiles was performed in 14 patients with recurrent pelvic cancer between February 1984 and February 1985. The sites of treatment were the uterus, urinary bladder, sacral mass, vulva and obstrator muscle. Full-dose radiotherapy had been performed as the primary treatment in 10 cases. According to Karnofsky's criteria, the therapeutic effect of embolization was rated as 1-A or better in 71.4% of the patients. Regarding complications of embolization therapy, two cases of vesicovaginal fistula were noted. All these results suggest that embolization is a useful method for the treatment of recurrent pelvic cancer, especially after radiotherapy.

Pancreatic sphincterotomy and pancreatic endoprosthesis.

Recently, endoscopic sphincterotomy (EST), developed as a treatment of bile duct stone or papillary stenosis, has been used for transpapillary biliary drainage in cases of extrahepatic biliary stenosis. For the nonoperative treatment of chronic pancreatitis, we have developed this procedure into a technique for opening the pancreatic duct orifice. Pancreatic sphincterotomy was performed successfully in 10 out of 13 cases with chronic pancreatitis and improved the clinical symptoms in 9 cases. Moreover, in 3 cases we succeeded in inspecting the intrapancreatic duct by peroral pancreatoscopy, and in removing stones from the main pancreatic duct in 2 cases in this series, using the basket. Also through the opened pancreatic orifice, a pancreatic endoprosthesis was placed endoscopically into the main pancreatic duct in 3 cases to improve pancreatic drainage. This report discusses method, evaluation, and complications of pancreatic sphincterotomy in the endoscopic treatment of chronic pancreatitis, and describes successful cases of the basket removal of pancreatic stones and the placement of pancreatic endoprosthesis through the opening of the pancreatic orifice.

New development of peroral cholangioscopy--evaluation of double-angle, oblique viewing cholangioscope.

Seven years have passed since the first report on peroral cholangioscopy by Takekoshi and Takagi (1) in Japan. Recently, several types of peroral cholangioscope with a forceps channel have been made for this treatment. However, the success rate of insertion of these instruments into the common bile duct was less than 50%. Therefore, in cooperation with Fuji-Photo-Optical Co. Ltd., we have made a new scope with the aim of facilitating insertion into the bile duct. Using this new scope, called a double-angle, oblique viewing cholangioscope, we obtain good orientation in the duodenum in en-face view and close inspection of the duodenal papilla, which makes possible easier insertion into the common bile duct. This new scope proved useful in basket removal of bile duct stone, or differential diagnosis between stones and carcinoma of the biliary tract.

[A case of perforated biliary cystadenocarcinoma].

Biliary cystadenocarcinoma is a rare malignancy, and only one preoperatively perforated case was recorded among 36 cases documented in the literature. A 48-year-old female was admitted with chief complaints of right epigastralgia and a right upper abdominal mass. CT scan revealed a well-defined low density area with calcification in the right hepatic lobe. On postcontrast CT, the septa and an internal papillary projection were well enhanced. Angiography demonstrated displacement and stretching of the right hepatic artery encircling a hypovascular lesion with some tumor stain and pooling. At laparotomy, a child-head size multilocular cyst was found in the right hepatic lobe; cyst excision was performed. Pathological analysis showed both benign cystadenoma and cystadenocarcinoma, suggesting a transition from the benign to the malignant state. At 17 months after the procedure, the patient is in good condition.

[Treatment and prevention of infections with cefoxitin sodium in patients with biliary tract infections and obstructive jaundice].

UNLABELLED: A total of 24 patients who was hospitalized in the Internal Medicine Wards of Yamaguchi University attached Hospital and the university's 3 related hospitals were administered with cefoxitin. The breakdown of the patients treated with cefoxitin was 7 with cholecystitis, 7 with choledochitis and the remaining 10 for the prevention of infections with obstructive jaundice. Daily doses of 2-6 g of cefoxitin were administered for 6-40 days by intermittent intravenous drip infusion in divided doses. RESULTS: 1. Of 14 patients with biliary tract infections, 10 (71.4%) responded favorably with cefoxitin. 2. Of 10 patients with obstructive jaundice used for the prevention of infections, 8 (80%) responded favorably with cefoxitin. 3. No untoward side effects were observed. 4. Cefoxitin proved to be a safe and effective antibiotic in the treatment of biliary tract infections and for the prevention of infections in patients with obstructive jaundice.
